症例は33歳男性,ロケット花火が口腔内で爆発し受傷,頸部腫張を自覚し近医受診,頸部皮下気腫,縦隔気腫,咽頭熱傷の診断にて救急搬送された。当初呼吸状態も安定していたため保存的に治療を行っていたが,左側下咽頭側壁から後壁にかけての粘膜の浮腫が著明となり,また壊死が加わり,進行した。画像診断では,咽頭後間隙を中心とした深頸部膿瘍の形成が認められ,さらに縦隔内にまで進展していることが指摘されたため,頸部外切開によるドレナージを行ったところ,花火の爆発片を咽頭後間隙内に認めたため,これを除去した上,周囲の壊死組織の切除,咽頭裂傷部の縫合と共に,縦隔及び胸腔ドレナージを行った。術後,局所処置,抗生剤投与を継続して行い,改善した。<br>本症例は救命し得たが,開胸による縦隔ドレナージを要する結果となり,受傷当日の頸部外切開等の早期対応が必要な症例であったと考えられた。

器質的な難聴を伴わない聴こえの問題をもつ聴覚処理問障害(APD: Auditory Processing Disorder)の小児に対し、医師と臨床心理士が心身両面からの評価と支援を試みた。多くの事例で問題は多軸にわたっており、心身にわたる多軸的評価と、多職種協働による支援システムの必要性が考察された。聴覚処理障害事例では補聴システムの利用や学校や家庭との連携による環境調整が有効であった。また多くは二次的に機能性難聴を併発しており、心理療法が有効であった。欧米におけるAPD評価支援システムの標準的確立をモデルに、日本においても特別支援教育への位置づけを踏まえた評価、支援の充実が望まれる。

甲状腺全摘後の甲状腺分化癌高リスク症例に対するI-131の外来内用療法が認可され,われわれの施設でも本法の適応と考えられた22例中19例で実施できた。当初,東日本大震災の福島原発放射能汚染事故に関連し,本法に躊躇される例があった。甲状腺ホルモン休薬法に比しrhTSH(Recombinant human thyroid stimulation hormone,ヒト遺伝子組み換え甲状腺刺激ホルモン)法では合併症が少なかった。19例中3例に局所再発と1例に遠隔転移が認められた。これらの症例はいずれも頸部再発歴があり,再手術のあとにI-131内用療法を受けた症例であった。したがって,今回のI-131内用療法は,甲状腺癌再発症例の再々発予防に対する効果は十分でない印象であったが,さらに十分な症例数と観察期間をもって判断すべきと思われた。

The innervation of cholinergic nerves in the Eustachian tube was light- and electron-microscopically observed using a histochemical technique. Acetylcholinesterase -positive nerves were seen just beneath the epithelial layer all along the tubal wall, around the blood vessels of the subepithelial space, and especially around the tubal glands. It is suggested from these findings that the cholinergic nerve plays an important role in controlling the blood flow in the subepithelial space and also in regulating the secretion of the tubal gland.

自然再生鼓膜を持つ側頭骨26耳の観察を行った. 従来, 耳鏡所見より自然再生鼓膜は皮膚層と粘膜層の2層のみで中間層はないとされていたが, そのほとんどの例において中間層の存在する事が確認され, 従来の考えの誤まりを指摘した.又, 移行部の観察より穿孔閉鎖機序には皮膚層が先行するとの印象を持った.一方保存的又は手術的閉鎖法によって再生された鼓膜の電顕的像を含めた組織学的観察により, 保存的閉鎖法により再生された鼓膜は, 正常鼓膜に近い構造を持つ事が判明された.

Thyroid fine-needle aspiration biopsy (FNAB) is a simple, cost-effective and minimally traumatic technique for the diagnosis of thyroid nodules. We report on two cases of acute thyroid swelling after FNAB. In both cases, within two hours after completion of the procedure, the entire thyroid gland enlarged rapidly. Ultrasound showed diffuse swelling of the entire thyroid gland with a heterogeneous appearance. There were no signs of bleeding or airway obstruction. The next morning a repeat US showed that the thyroid had returned to its normal size.It has been demonstrated that nerve fibers storing neuropeptides such as vasoactive intestinal peptide, substance P, cholecystokinin, neuropeptide Y, peptide histidine isoleucine, galanin, and calcitonin gene-­related peptide (CGRP) exist in the thyroid.The clinical and ultrasound data support the hypothesis of capillary leakage as the underlying mechanism, possibly evoked by neuropeptide release.

スギ花粉症の患者に対して第 2 世代抗ヒスタミン薬を問診表を用いて選択させた報告では患者満足度はのきなみ高く、患者参加型医療は満足度を向上させ、ひいてはアドヒアランス向上につながることを示唆している。今回われわれは、スギ花粉症の診断から薬剤選択までの作業に患者が参加できるような問診表を作成し、2011 年のスギ花粉症に対して使用し、アンケート調査も行った。実際に鼻アレルギー診療ガイドラインどおりに薬剤が処方されたのは約 55%の患者であった。41.2%の患者はガイドラインの重症度分類を認知しており、35.3%の患者は重症度に応じて推奨薬剤が異なることを知っていた。 64.7%の患者は自分で薬剤を選択できてよかったとした。総合的な満足度では「やや満足」、「満足」以上で 100%という評価であり、このような薬剤自己選択式問診表はスギ花粉症患者の満足度を向上できることが示唆された。

A video-endoscope is a popular device in the ENT clinic. We usually use it only in observation. In case of a hemostasis in the nasal cavity, we use a rigid endoscope, however all clinics do not possess a rigid type because of its high price. In this background, we try to use the video-endoscope using a hard sheath of Endoscrub®. This application makes us easier in stopping the epistaxis in our daily clinic. Also in a treatment after endoscopic sinus surgery and a biopsy from deep lesion of a nasal cavity.

高度肥満(BMI 52)の甲状腺癌手術症例を経験した。術後出血の診断や,出血に対する気管切開術の容易化のため,甲状腺手術の際に前頸部脂肪組織切除術を併用した。今後の高度肥満症例の増加が考えられるが,肥満症例でも積極的に甲状腺手術が行えるよう,前頸部脂肪組織切除術併用などの手術法の工夫が必要と思われる。

The vagus nerve is one of the 12 cranial nerves, which is considered as the 10th nerve. The vagus nerve extended from the medulla oblongata to the second segment of the transverse colon. This nerve innervates the majority of the muscle of the pharynx, soft palate and larynx. In cases of vagus nerve paralysis, palatal arch movement is restricted. The arches cannot move up and back in order to close off the nasopharynx. This causes hoarseness, nasal speech, aspiration and regurgitation of liquid through the nose. In this article, we report 10 cases of the vagus nerve paralysis. The patient’ mean age is 59.4 (range 34-83). There were 5 male and 5 female patients, among which there were 6 cases of right and 4 cases of left side paralysis. In six cases the chief complaint was vocal disorders. The causes included 5 cases of Varicella-Zoster virus infection (all confirmed) and 4 cases of central paralysis. In 9 cases of the 10 patients, nasopharyngeal incompetence, elevation or constriction of the paralyzed side and saliva pooling in the pyriform sinus were observed. In 4 cases of the 10 cases, the vagus nerve paralysis improved after treatment within 6 months. In cases of vagus nerve paralysis immediate and accurate diagnosis and treatment are crucial. It is important to observe the palatal arches as they contract and the soft palatal as it swings up and back in order to close off the nasopharynx.

Various surgery supported equipments have recently advanced remarkably. The possibility of the navigation system as the tool for surgical skill improvement was evaluated in 30 cases between January, 2006 and September, 2008. The 28 cases was assessed as “Useful” of the navigation system, and the 8 cases was assessed as “Necessary” of the navigation system. It was concluded that the usefulness of navigation system had being changed by the surgeon's experience from the first stage to the present and it will play an important role in the surgical skill improvement and safe operation.

Forestier病は全身の靱帯・腱付着部の硬直・骨化を示す強直性脊椎骨増殖症 (diffuse idiopathic skeletal hyperosteosis) の1つである. 頸椎で生じたものでは喉頭や咽頭・食道を圧迫して咽喉頭違和感や呼吸障害・嚥下障害をきたす. 進行例には切除術が選択されるが, 十分な病変切除がされたにもかかわらず, 手術効果が即座に出ない例も認められ, 症状発生には, 病変の物理的な喉頭・食道圧迫だけでなく, 局所での浮腫や繊維化を含めた炎症の機序が考えられている.我々は今回, 本疾患の5例に対し手術的治療を行った. 病変部の正確な切除と上・下喉頭神経などの温存を図ったが, 症例1・2では不十分な手術成績であった. そこで, 手術に際し骨病変切除で生じる椎骨と骨膜間の死腔を減少させるため骨膜を寄せて縫合する処置を追加し良好な成績を得た. こうしたことから, Forestier病の手術に際し, 椎前部の死腔縮小処置が局所の炎症を低下させ, 術後の症状回復を早めるのではないかと考える.

感音難聴350耳を対象に67-S語表を用いて語音弁別検査を行い, その結果をもとに, 聴覚域値別 (10dBステップ) に, 域値上各レベルで語音明瞭度がその耳の最高明瞭度であった割合 (最高明瞭度達成率) を求めた。 最高明瞭度達成率が最大であったレベルは, 感覚レベル (本研究では語音聴力レベルと純音検査の平均聴覚域値レベルとの差) で表すと, 平均聴覚域値レベルが10dB台では50dB, これから平均聴覚域値レベルが10dB上昇するごとにほぼ5dBずつ低下し, 70dB台では15-20dBとなった。 これらの結果は, 一般の感音難聴に語音弁別検査, 補聴器適合判定を行う場合に, 明瞭度が最大となる語音レベル (最適語音レベル) を推定する一つの目安となるものと考えられた。
