More than sixty years has passed since the outbreak of Minamata disease, and high-level methylmercury contaminations now seem nonexistent in Japan. However, mercury has been continuously discharged from natural sources and industrial activities, and the health effects on children susceptible to methylmercury exposure at low levels, in addition to mercury contamination from mercury or gold mining areas in developing countries, become a worldwide concern. In this article, we provide a recent overview of epidemiological studies regarding methylmercury and mercury. The following findings were obtained. (1) Many papers on exposure assessment of methylmercury/mercury have been published since the Minamata Convention on Mercury was adopted in 2013. (2) The most crucial problem is child developmental neurotoxicity resulting from prenatal exposure to methylmercury, but its precise assessment seems to be difficult because most of such effects are neither severe nor specific. (3) Several problems raised in birth cohort studies (e.g., whether IQ deficits due to prenatal methylmercury exposure remain when the children become adults, or whether the postnatal exposure at low levels also causes such adverse effects in children) remain unsolved. (4) Concurrent exposure models of methylmercury, lead, polychlorinated biphenyls, aresenic, and organochlorine pesticides, as well as possible antagonists such as polyunsaturated fatty acids and selenium, should be considered in the study design because the exposure levels of methylmercury are extremely low in developed countries. (5) Further animal experiments and molecular biological studies, in addition to human studies, are required to clarify the mechanism of methylmercury toxicity.

Methylmercury, the causative agent of Minamata disease, can easily penetrate the brain, and adult-type Minamata disease patients showed neurological symptoms according to the brain regions where the neurons, mainly in the cerebrum and cerebellum, were damaged. In addition, fetuses are exposed to methylmercury via the placenta from maternal fish consumption, and high-level exposure to methylmercury causes damage to the brains of infants. Typical patients with fetal-type Minamata disease (i.e., serious poisoning caused by in utero exposure to methylmercury) were born during the period of severe methylmercury pollution in 1955–1959, although they showed no abnormality during gestation nor at delivery. However, they showed difficulties in head control, sitting, and walking, and showed disturbances in mental development, these symptoms that are similar to those of cerebral palsy, during the growth periods after birth. The impaired development of fetal-type Minamata disease patients was one of the most tragic and characteristic feature of Minamata disease. In this review, we first summarize 1) the effects of prenatal methylmercury exposure in Minamata disease. Then, we introduce the studies that were conducted mainly by Sakamoto et al. as follows: 2) a retrospective study on temporal and regional variations of methylmercury pollution in Minamata area using preserved umbilical cord methylmercury, 3) decline in male sex ratio observed in Minamata area, 4) characteristics of hand tremor and postural sway in fetal-type Minamata disease patients, 5) methylmercury transfer from mothers to infants during gestation and lactation (the role of placenta), 6) extrapolation studies using rat models on the effects of prenatal methylmercury exposure on the human brain, and 7) risks and benefits of fish consumption.

A workshop on the Scientific Issues Relevant to Assessment of Health Effects from Exposure to Methylmercury was held in Raleigh, North Carolina, November 18-20, 1998. At that time, most discussions focused on two of the major epidemiologic studies, e. g., Seychelles child development study and Faroese birth cohort study, associating methylmercury exposure with an array of developmental measures in children. These two studies seemed to provide different conclusions on the potential health effects of methylmercury and significant uncertainties remained because of issues related to exposure, neurobehavioral endpoints, confounders and statistics, and design. Since then, each group researching the Seychellois or Faroes cohort has reported some new findings on the risk assessment of methylmercury. This article is intended to present an overview of the above research, as well as benchmark dose calculations. One implication is that neuropsychological measures may depend on social and cultural factors including race and language, and another is that a key to resolve whether the exposure has harmed the fetus appears to lie in neurophysiological measures such as brainstem auditory evoked potentials and electrocardiographic R-R interval variability. In addition, it is likely that the findings published tend to underestimate the neurotoxic effects of developmental methylmercury exposure. In light of the precautionary principle, a conclusion on health effects of low-level dietary exposures to methylmercury needs to be drawn from all available data including the New Zealand study.

現在,殺虫剤の化学物質曝露が及ぼす健康への影響について国内外で関心を集めており,尿中バイオマーカーを用いた曝露レベル等を評価する試みがアメリカやドイツ等で多く実施されている。合成ピレスロイド系殺虫剤(PYR)は農業用あるいは家庭用殺虫剤として日本人にも馴染のある化学物質であるが,PYR曝露マーカーである尿中クリサンテマムジカルボン酸(CDCA)および3フェノキシ安息香酸(3PBA)排泄量に関する日本人のデータは少ない。本研究では日本人成人の尿中に排泄されるCDCAおよび3PBA量をモニタリングし,それらの季節変動,職域間差および曝露源について検討した。調査対象は食品配送小売業者(FD, n=92),リンゴ農家(AF, n=144)および殺虫剤撒布職域従事者(PCO, n=24)とし,それぞれ夏季および冬季に採尿とアンケート調査を行った。ガスクロマトグラフ質量分析計で定量された尿中3PBAとCDCAは非正規分布を示していたため,対数変換値(正規化)を用いて季節変動はpaired t-検定,職域間差は一元配置分散分析,その後の検定にはScheffeの方法を用いて有意差を検出した。全対象者における尿中3PBAとCDCAの検出率は92%以上であり,ほとんどの日本人が日常的にPYRに曝露していることが明らかとなった。夏と冬における3PBA濃度の幾何平均値(GM)はそれぞれ0.7および 0.5 (FD),0.9および0.4 (AF),2.6および1.8 (PCO) (μg/g creatinine)であった。また,CDCAのGMは0.33および0.13 (FD), 0.30および0.21 (AF), and 0.56および0.26 (PCO)であり,PCOの3PBAを除き,代謝物量は冬に比べて夏で有意に高い値を示した(p<0.05)。すなわち,冬に比べて夏におけるPYRの曝露レベルが高いことが示唆された。PCOの3PBA量は他群のそれに比べて高い値を示した。一方,CDCAにはその傾向が見られなかったことから,CDCAに代謝されるPYRの職業的曝露は多くないことが推察される。FDのみを対象とした予備的解析において,夏における蚊やハエ防除のための家庭用殺虫剤使用者(n=12)の尿中CDCA量は,殺虫剤非使用者(n=80)に比べて有意に高いことが明らかとなった(GM 0.70 v.s. 0.29 mg/g creatinine, p<0.05)。すなわち,室内で使用したPYR殺虫剤がPYR曝露源の一部であることが示唆された。

フェロー出生コホート研究(Faroese Birth Cohort Study)は,メチル水銀の小児神経発達影響に関する研究として世界的に有名である。この理由は,米国科学アカデミーが米国環境保護庁(EPA)のメチル水銀に関する基準摂取量(RfD)を再検討する際に参照すべき重要な研究であると結論したことにある。本稿は,北大西洋に浮かぶ18の島々からなるフェロー諸島が環境科学領域の研究対象集団として何故選択されたのか,そこで検査された7歳児および14歳児からどのような研究成果が得られたのか,そしてリスク評価の経過でフェロー諸島にどのような変化がもたらされたのかについて概説した。また,この出生コホート研究の解析過程で吟味された"リスクの過小評価"についても触れた。

3歳児および小学生における睡眠時間と生活習慣の関連とともに睡眠時間と自律神経機能との関連を調査した。その結果、3歳児においては夜間睡眠と昼寝を合計した総睡眠時間630分未満の児童において交感神経機能および副交感神経機能の低下が認められた。生活習慣と保護者の生活習慣との関係をみると、保護者の帰宅時間が遅い児童に睡眠時間の短縮が認められた。保護者の意識や生活習慣を見直すことが必要と考えられ、生徒児童の健康を考えるときには、睡眠時間の確保が重要であることが示唆された。
