ハワイでは19世紀半ばからハンセン病の蔓延が始まった。これに対してハワイ王国政府は1865年に公衆衛生政策としての隔離を決定、モロカイ島のカラワオを療養地とし、患者移送を始めた。患者の発見は密告と逮捕により行われ、容赦なくモロカイ島に送られた。政府は当初、カラワオで患者が自給自足する農業コロニーを目指したが、地理的特性や予算不足などが要因となり、農業コロニー構想は失敗し、カラワオは無法地帯となり、多くの混乱が生じた。1873年にはダミアン神父がカラワオに赴き、患者の救済を開始、政府との交渉にも務め、混乱は収拾された。その後、ハンセン病対策費の増加、治安システムの整備などによりカラワオには治安が確保され、療養地は近接するカラウパパへ拡がり、その拡充と制度の整備の過程とアメリカ合衆国の関与の中でハワイでは隔離政策が進展していった。

G. Canguilhem posits in his "The Normal and the Pathological" two judgements: (1) the negation of objectivity of the pathology and the physiology because of the necessary interference of the value judgements between the two phases; (2) the importance of the biological normativity, that's to say, the understanding of the health as something essentially related to the norm and value. The health is not a fact, but a norm to which an organism makes an effort to attain by regulating its conducts. After this affirmation of the normativity in the demarcation between the normal and the pathological, I try to link it to the contemporaneous problematic of bioethics. And situating the health as a norm inside of the world of designing, I give a theoretical permission to the selective abortion in condition of a strict limitation, and to a certain positive interference to the genetic information for giving birth to a perfect baby.

降水メカニズムの解明には、降水量だけでなく、雨滴粒径分布(DSD)の計測による降水物理量の定量的把握が重要である。東西5000kmにわたり多様な地形を持つインドネシア海洋大陸では、インドネシア全体に跨る観測網が不可欠である。本研究では、コトタバン(スマトラ島)・ポンティアナ(カリマンタン島)・マナド(スラウェシ島)・ビアク(ニューギニア島近傍の小島)にディスドロメータを整備し、DSDの連続観測を実施した。経度・降水雲タイプ・MJO・雷活動などとDSD特性との関連について明らかにした。

愛知県において、1963(昭和38)年かららい予防法が廃止になる前年の1995(平成7)年まで33 年間、ハンセン病の外来診療が行われていた。藤楓協会愛知県支部と愛知県衛生部、国立駿河療養所が協力して、延人数で療養所退所者3,877 人、在宅者614 人、患者家族など486 人の計4,977 人の診療が行われた。外来診療で発見された新規患者については、療養所入所をさせることなく外来診療を行った例もあった。 1996(平成8)年からは退所者に対する療養相談を行っており、2010(平成22)年までの15 年間に延349 人が相談に訪れた。

We aimed to elucidate the patterns and trends of autochthonous leprosy in Japan from 1964 to 2009, to compare them with the findings from other studies of leprosy in decline. Data on registered leprosy cases in Japan in the period 1964-2009 were analysed with reference to trends in case detection, geographical distribution, age at diagnosis, sex, classification and family history. A consistent decline in leprosy case detection was observed in all areas of the country over the period 1964-2009. Highest incidence was consistently in Okinawa. Autochthonous leprosy has not been reported in anyone born in Japan since 1980. Increasing average age and a shift towards lower latitudes were demonstrated throughout the period. Analyses of data on autochthonous cases revealed patterns similar to those reported in other countries with declining leprosy.   Okinawa has had the highest incidence of leprosy in all of Japan since the first national survey in 1900. Several possible explanations include the difference of leprosy control history between Okinawa and the rest of Japan, Okinawa's unique geographical condition, large-scale problem of stigma and discrimination against leprosy patients and delayed improvement of socio-economic conditions.

Twenty-six coracohumeral ligaments (C-H hg) were studied by histochemical methods in order to investigate collagen structures and distribution of the nerves. The collagen structure of the C-H hg was different from that of the shoulder capsule in that tere was mainly Type III collagen in it. On the other hand, both Type I and III collagen were in the capsule of the shoulder joints. Regarding the distribution of the nerves, there were a lot of different types. Some C-H hg had many nerves and some had few nerves. Histologically the C-H hg was loose connective tissue which was connected with the periosteum of the coracoid process. We think such a characteristic of the C-H hg may be associated with the cause of frozen shoulder.

本稿は現代の生命倫理学でも重要な位置を占める<人間の尊厳>という概念が、ヨーロッパの歴史の中でどのような文脈の中で生まれ使われてきたのか、その主要な流れを最低限確認することから始めた。その際、インノケンティウス三世の<人間の悲惨>論に重きを置いた記述をした。また、ピコ・デラ・ミランドラの高名な一節が含意する、一種の知性鼓舞論が、純粋に世俗的な位相のみにおいて<人間の尊厳>概念を理解することを困難にしているという事実に注意を喚起した。それらの検討を通してわれわれは、この概念が十全に機能するためには、神のような超越的存在との関係における人間の定位を必要とするということを示した。では現代の世俗的社会の中で、この概念の使命は既に終わったと考えるべきだろうか。いや、そうは考えない。この神的背景を備えた概念は、世俗的微調整の中でその神的含意を解除されながらプラグマティックに使用され続けるという趨勢の中でも、依然として人間存在の超越性を示唆し続けることをやめないだろう。それこそが、この概念の独自な価値なのである。

PurposeThe anatomical characteristics and nerve distribution in the coracohumeral ligament(C-Hligament)and surrounding tissue were studied. The role of the ligament in periarthritis of the shoulder was assessed.Materials and Methods Specimens were obtained from 30 cases,50 joints at autopsy. The cases consisted of 14 males and 16females; with an age range from 29 to 98 years(mean 78 years). The C-H ligament and its surrounding tissues were excised en bloc.Paraffin sections, vibratome sections and whole mount preparations were made and subjected to immunohistochemical staining using NF, PGP9.5, SP and CGRP antibodies in order to the distribution of sensory nerve fibers.

本研究は戦前、日本に唯一存在したハンセン病患者の自由療養地である群馬県吾妻郡草津町湯の沢部落の社会科学的分析の中から、何がハンセン病患者の隔離の二つの側面である「迫害されている患者の社会の圧力からの保護」と「感染源である患者からの社会の防衛」のダイナミズムを後者への優位に導いていったのかを明らかにすることを目的とするものである。その過程は湯の沢部落の実態の解明(「草津湯の沢ハンセン病自由療養地の研究I、II」)、自由療養地議論の展開と消滅の過程の検証と湯の沢部落の関わり(「草津湯の沢ハンセン病自由療養地の研究III」)、湯の沢部落消滅後にその精神が日本の隔離政策に与えた影響(「草津湯の沢ハンセン病自由療養地の研究IV」)などの研究の総体である。<BR>本稿では戦後、栗生楽泉園から始まる患者運動を通して、湯の沢で培われた精神は楽泉園内でも生き続け、患者運動の戦端を開き、「特別病室」を廃止、職員の不正を暴き、やがて多磨全生園と共闘し、「全国癩療養所患者協議会」を生み、絶対隔離政策と対峙する力を形成する様相を描いた。併せて、「特別病室」設置の背景、戦中を中心としての療養所内の混乱の様相とその要因を述べた。

The reliability-based design method was recently introduced to the technical standard for port and harbor facilities. The standard method is partial factors method, however, other methods that evaluate the reliability index directly can also be applied. Taking the design of pile supported wharves as an example, performance function is implicit and the application of the methods that evaluate the reliability index directly is difficult to the practical design. This study aims at clarifying the applicability of response surface method for pile supported wharves. As the performance function can be evaluated explicitly by the method, the calculation of reliability indices is comparatively easy. The applicability of first-order second-moment method is also discussed.
