We investigated the success rates of eyedrop instillation and the distance between the cornea and the dropper tip in 100 volunteers using high-speed digital video recording. Past eyedrop adherence studies assumed that instillation occurred without failure. The ideal distance between the cornea and dropper tip remained unclear, although the general estimate was approximately 2.54 cm (1 inch). This study was approved by the Institutional Review Boards of all participating medical institutions, and all volunteers provided written, informed consent. Successful instillation was defined as when 1 drop fell accurately into the eye on the first attempt. The instillation of ≥2 drops or drops delivered outside the eye was considered a failure. The distance between the eye and dropper tip was measured using still images from a paused digital video camera and a digital ruler. Forty percent of the volunteers instilled eyedrops without instructions from ophthalmologists, pharmacists, or other healthcare workers. When the images were analyzed, the success rate of the first instillation was 70.1%. When the eye was arbitrarily divided into 9 sections, most of the drop sites were the iris or the center of the eye. The distance between the dropper tip and cornea was 2.62 ± 1.75 (median 2.20) cm. These results indicate that the generally recommended distance is usually followed. The successful instillation rate based on the distance from the dropper tip to the cornea was 77% at 1.6 ± 0.88 cm and 54.9% at 4.8 ± 1.25 cm.

これまでの先行研究において,音読時と黙読時における脳活動の空間的分布がfMRIを用いて調べられた.そして,音読時と黙読時に活動する皮質領野はほぼ一致することが明らかとなっている.しかし,これらの皮質領野が音読時と黙読時に同じ機能を果たしているかどうかは曖昧であり,明らかとなっていない.この理由として,これまでの先行研究では,脳活動の空間的分布を調べることを中心としており,音読時と黙読時の皮質領野におけるボクセルパターンの違いを評価してこなかったことが考えられる.本研究では,fMRIを用いて,日本語音節の音読時と黙読時の皮質領野の機能類似性を調査した.機能の類似性を評価する方法として,ボクセルパターン同士の線形相関を計算した.この結果,音読時と黙読時に活動する皮質領野において,ボクセルパターンに類似性が存在することが示唆された.

Mechanochemistry offers sustainable synthesis of the functionalized cellulose nanofiber (CNF). In this study, changes in the microstructure of the CNF aqueous sol by planetary ball milling were investigated in terms of its rheological behavior, crystallinity, and diameter distribution. The surface activity of the CNF was additionally characterized by a pulsed nuclear magnetic resonance (NMR). A decreased thixotropy hysteresis loop observed in the 100 min−1-treated CNFs indicated a weaker interaction among the fibers, but still having a three-dimensional structure. 300 min−1 could collapse them. A decreased x-ray diffraction peak intensity observed in the 500-min−1-treated CNF could indicate a split in the fiber’s bundle as well as shredding. An increase in the wet surface area (SNMR) could indicate surface activity in the 500-min−1 milled CNF sol. Such newly formed hydroxyl groups can serve as effective reaction sites with, for example, the TiO2 precursor and perhaps favorably works to improve the photocatalytic performance.

若年者の乳腺葉状腫瘍はまれであり,線維腺腫,特に若年性線維腺腫との鑑別は困難である.これまでは線維腺腫であれば腫瘤摘出術,葉状腫瘍であればマージンをつけた切除が必要とされてきたが,近年,葉状腫瘍に対する手術においても断端陰性であればマージンは不要であるという報告が増加している.症例は11歳,女児.左乳房に7cm大の腫瘤を自覚し受診.画像検査および針生検では若年性線維腺腫もしくは良性葉状腫瘍が疑われ,腫瘤摘出術を施行した.病理結果は良性葉状腫瘍で断端は陰性であった.自験例では良性葉状腫瘍であったことと,腫瘤摘出術によって断端陰性が得られたことより,追加切除は施行せず,術後6年6カ月無再発で経過している.若年者の葉状腫瘍に対する治療において,根治性とともに整容性や機能温存へ十分配慮した上での方針決定が望まれる.

The hamate bone, one of the carpal (wrist) bones, has a large uncinate process protruding from the palm side. In sports such as golf and tennis, the hamate bone can break if is subjected to a high external force, such as from the handle of a racquet or club. At our hospital we take X-ray images of the hamate bone from two directions: an axial image through the carpal tunnel and an image at the base of the hamate hook (conventional method). While the conventional method makes it easy to create images of the base of the hamate hook, the patient may suffer pain during image-taking because the hamate bone is pulled to cause radial flexion. We therefore investigated a method of imaging that would create three-dimensional computed tomography (3DCT) images of the base of the hamate hook in which the patient would only have to only rotate the wrist externally and elevate the fore-arm without any radial flexion. Our results suggest that it is possible to obtain images of the base of the hamate hook as clear as those acquired using the conventional method with the patient in a comfortable and painless position taking images at an external rotation angle of 50.3° and a forearm elevation angle of 20.3°.
