A patient was diagnosed with pagophagia, a type of pica, during dental treatment and, on suspicion of anemia, was referred to an internal medicine clinic where she was diagnosed with severe anemia. We give a brief review of the literature concerning anemia and pagophagia.The patient was a 46-year-old woman who visited our clinic for dental treatment. Her medical history included noncontributory systemic findings, and she showed no abnormalities in facial features or otherwise.Intraoral findings: Several missing molars, but good overall oral health. Discolored gums presenting with anemic coloring. No abnormalities in the lips or tongue, but dental attrition visible throughout the jaw.Verbal examination revealed pagophagia of about 1kg per day, and so the patient was referred to a local internal medicine clinic on suspicion of anemia. A blood sample revealed Hb 4.0g/dl and a diagnosis of severe anemia was made. The patient was referred to a general hospital in the city for detailed full-body examinations and iron preparation drugs that corrected both the anemia and the pagophagia.The reported rate of anemia associated with pagophagia varies from 0.4 to 24.7%. However, it is clear that there is a link between pagophagia and iron deficiency anemia.

この研究の目的は漢方薬 (麦門冬湯) の口腔乾燥に対する臨床的効果を検討することである。対象患者は口腔乾燥症で自治医科大学付属病院を受診した男性1名, 女性15名である。年齢は54歳から82歳で, 平均67歳であった。対象16名中15名の患者は他の疾患に罹患し何らかの投薬をうけていた。われわれは, 対象患者に対して, 麦門冬湯を1ヶ月間投与した。13例 (81%) は漢方薬の麦門冬湯投与により, 自覚症状として口腔乾燥の改善を認めた。8例 (50%) において, 5分間安静時唾液量は, 0.5ml/5分以上であった。薬剤の副作用は認めなかった。

歯磨剤, 洗口剤の安全性を検討する目的で, 市販されている歯磨剤, 洗口剤の多くに含まれているラウリル硫酸ナトリウム (SLS) とglycerineの各種ヒト細胞に及ぼす影響を検討した。ヒト歯肉上皮細胞に各種濃度のSLSを作用させて細胞の形態を観察した結果, 細胞は, SLSの濃度依存的に変化した。0.01% SLSを作用させた場合, 作用直後細胞が小型化し, 1分後にはほとんどの細胞が溶解した。0.001% SLSを作用させた場合,作用後3分で細胞が小さくなり始めたがその程度は弱かった。ヒト赤血球に各種濃度のSLSを作用させると,赤血球は,SLSの濃度依存的に形態が正常の円盤型から外方突出型の金平糖状に変化し, 0.004%以上では作用後1分で溶血した。ヒト大動脈血管内皮細胞(HAEC)に各種濃度のSLSを作用させると, 0.0022%以上では作用後1分で変化が起こり, 5分ではほとんどの細胞が溶解した。諸細胞にglycerineを作用させた場合, glycerineによる細胞の障害は低濃度, 短時間では大きいものではなかった。ヒト赤血球を用いてSLSの作用点を観察した結果, 膜蛋白質の関与が観察された。これらの結果から, SLSやglycerine, 特にSLSは, 微量で, 短時間の作用でも諸細胞に障害を与えるため,使用に際しては十分な注意が必要と考える。

65歳の男性。繰り返す口内炎を主訴に,近医から当科を紹介され受診した。初診時,口腔内には多数のアフタを認めたが,偽膜形成は認めなかった。細菌培養検査にてカンジダ陽性であったため抗真菌薬を使用したが症状を繰り返した。外陰部潰瘍を認めていたことから内科に対診したところ,HIV感染が判明した。その後,HIV治療が開始され,現在は口腔内症状も改善し経過観察を行っている。

A case of orbital abscess with gas gangrene due to odontogenic infection is reported. Although orbital abscesses are rare, they may result in fatal complications. A 68-year-old woman presented with pain and swelling of the left cheek and lower eyelid. She was unable to open her left eye. Oral examination revealed alveolar gingival swelling due to a periapical lesion of the left upper anterior tooth. Computed tomographic scans confirmed abscess formation in the left orbit and the presence of a gas bubble. The ocular tension was 50 mmHg. On admission to our hospital, the patient started to receive antibiotics (PAPM/BP and CLDM), the left lower eyelid was incised, and the abscess was successfully drained. <i>Alpha-streptococcus</i>, <i>coagulase-negative staphylococcus</i>, and <i>Prevotella melaniogenica</i> were detected in the suppuration. The patient was given a diagnosis of orbital abscess with gas gangrene due to odontogenic infection extending by way of the orbit.

乾燥剤の誤飲・誤嚥は,高齢者や小児で多く認められる。酸化カルシウムは,食品乾燥剤として多用されており,シリカゲルと比較して誤飲した場合に発熱とアルカリによる粘膜腐食作用により症状が重症化しやすい。今回,われわれは乾燥剤の誤食により広範囲な口腔粘膜化学損傷を生じた1例を経験した。患者は74歳,女性で酸化カルシウムを誤食し当院を受診した。広範囲に口腔粘膜の潰瘍と舌の腫脹,口腔乾燥を認めた。入院の上,口腔衛生管理と栄養管理を行い症状は改善し15日目に退院となった。乾燥剤の種類と摂取量に基づいた適切な処置を行うことが肝要である。

口腔扁平苔癬と肝機能障害との関連性を明らかにするため, 病理組織学的に扁平苔癬と診断された68症例について, 肝機能との関連を臨床統計的に検討した。(1) 肝機能異常群 (Group 1) は12例 (17. 6%), 正常群 (Group 2) は56例 (82.4%) であった。(2) 性差は Group 2で男性が多く, 平均年齢は差はなかった。(3) 主訴は Group 2に比べ Group 1でしみるが多かった。(4) 既往歴は Group 1で肝疾患, 次いで糖尿病の順で, Group 2では高血圧, 次いで消化器潰瘍であった。(5) 発症部位は両群とも頬粘膜, 舌, 歯肉の順であったが1人平均病変部位数は Group 1は4.8部位, Group 2は2.8部位と明らかに Group 1の方が多かった。(6) 臨床型は Group 1ではびらん型が60.7%と最も多く, Group 2では網状型が多かった。すなわち肝機能異常群はびらん型が多く, 病変が広範囲であることが明らかになった。以上のことから肝機能異常が扁平苔癬の発症・増悪因子として関与している可能性が示唆された。

A 55-year-old male with myelodysplastic syndrome had been treated at the Hematology Department at our hospital. Swelling of the left cheek developed in the patient, which led to his referral to our department. Since blisters and distinct dark red plaques were noted on the right forearm as well as the lower limbs, a dermatological examination was requested. Biopsy of the skin rash was performed; the biopsy result and clinical course led to the diagnosis of cheek phlegmon associated with Sweet's disease. We provided treatment for the infection and Sweet's disease. The patient received transfusions of peripheral stem cells. He has had a good outcome for 3 years and 6 months.

Electrotome is frequently used for surgical treatments in the field of dentistry and oral surgery. We report a rare case of gingival and alveolar bone necrosis caused after gingivectomy with electrotome. The patient was a 30-year-old female complained of gingival discolor. She noticed the gingival discoloration 2 days after the gingivectomy with electrotome by her family dentist. The gingiva around the left upper central incisor was changed to white in color. Nine days after the electrosurgery, the left upper central incisor fell off spontaneously, and 7 weeks after the electrosurgery, the sequester was removed. Epithelization was completed 2 weeks after the sequestrectomy, and she was successufully treated by the fixed bridge and removable artificial gingiva.

We report a patient with jaw trismus demonstrating Square-Mandible face.<BR>Clinically, the patient had a square face with prominent masseter muscles and mandibular angles. The interincisal opening was 26mm, with limited lateral and anteroposterior mobility.<BR>The clinical diagnosis was restricted mouth opening due to bilateral hyperplasia of the masseter muscle aponeuorses. Masseter muscle myotomy with aponeurectomy was performed bilaterally by an intraoral approach. Mouth opening improved to 40mm after incision of the aponeuroses.<BR>An examination of the patient one year after surgery demonstrated an interincisal opening of 37mm without strain.

Psoriasis is a chronic dermatitis of undetermined etiology. The patient was a 60-year-old male who complained of pus discharge from maxillary frontal gingiva. The patient had been suffered from psoriasis vulgaris for 8 years, and there were many reddish papules in breast, abdomen, limbs and facial skin. Radiological examination showed a lowered maxillary left canine, periapical radiolucency of the maxillary right first premolar and horizontal loss of alveolar bone in maxilla and mandibular bone. After treatment of marginal periodontitis, the maxillary right first premolar and the impacted maxillary left canine were extracted. After these treatments, reddish papules rapidly improved and disappeared completely within 2 weeks after extraction. This clinical course suggests that psoriasis was caused by dental focal infection.
