〔目的〕内的リズム形成を目的とした運動療法による,パーキンソン病患者のすくみ足とタッピング能力への改善効果を検討する事とした.〔対象〕一定頻度でのタッピングの持続が困難で,視覚・聴覚刺激を用いた外的手がかりによるすくみ足の制御が困難であった進行期パーキンソン病患者1名とした.〔方法〕内的リズム形成能力の向上を目的とした1回20分の運動療法を2回/週の頻度で8週間実施し,タッピング課題による内的リズム形成能力の評価,および歩行評価から,その治療効果を検証した.〔結果〕介入によって一定頻度でのタッピング持続回数の増加,すくみ足歩行の軽減が認められた.〔結語〕外的手がかりに代り,内的リズム形成を促す運動療法による治療介入は,すくみ足症状を軽減できる可能性がある.

[Purpose] The aim of the present study was described the kinematic characteristics of gait in stroke patients with two different arteries involved. [Subjects and Methods] Two patients who had suffered a basilar (A) or middle (B) cerebral artery ischemic stroke were compared with a control (C). Seventeen inertial sensors were used with acquisition rate of 120 Hz. The participants walked 3 times on a 10 meter walkway. From the raw data, the three gait cycles from the middle of each trial were chosen and analyzed. [Results] During the stance phase, patients A and B had a lower hip angle at initial contact and maximum flexion angle during load response than the control. Patient A and the control subject had similar knee angle values at initial contact, and patient B presented a flexed position in the initial phase of the gait cycle. The maximum flexion angles during loading response were also higher for patient B. The sagittal plane excursion for the ankle joint was lower for patient B in comparison with the other subjects. [Conclusion] Differences during walking between patients who had stroke in different arteries may be related to an alternative compensatory strategy. Patient A and the control subject had similar gait cycle curves at all joints, while patient B showed a rigid synergic pattern.

[Purpose] The purpose of this study was to investigate the effect of muscle vibration in the lower extremities in patients with Parkinson’s disease (PD) during walking. [Subjects] Nine patients with PD participated in this study and were tested with and without vibration (vibration at 60 Hz). [Methods] Eight oscillators of vibration were attached to the muscle bellies (tibialis anterior, gastrocnemius, biceps femoris, and rectus femoris) on both sides of the lower extremities with adhesive tape in this study. Spatiotemporal gait parameters were measured using a motion analysis system. [Results] Stride length and walking speed with vibration were significantly increased compared with those without vibration in PD patients. [Conclusion] These results suggest that the application of vibration to lower extremity muscles in patients with PD may improve the parkinsonian gait pattern.

[Purpose] The purpose of this study was to examine the effects of running in place while using the abdominal drawing-in method on healthy adults’ lumbar stability. [Subjects] A total of 30 subjects were divided into a training group of 15 subjects and a control group of 15 subjects. [Methods] The training group ran in place using the abdominal drawing-in method for 30 minutes per session, three sessions per week, for a total of six weeks. For both the training group and the control group, static lumbar stability (SLS) and dynamic lumbar stability (DLS) were measured before and after the experiment using a pressure biofeedback unit. [Results] Pre- and post-intervention measurements were compared within the training group and the control group. According to the results, the training group showed statistically significant differences in DLS. [Conclusion] Running in place, which can be performed easily regardless of time and location, can be recommended as an exercise that will improve the dynamic lumbar stability of students or office workers.

[Purpose] The purpose of this study was to compare the effects of shoulder stabilization exercise and shoulder isometric resistance exercise on maximum voluntary isometric contraction (MVIC) rate of change of shoulder stabilization muscles and hand function. [Method] The subjects of this study were nineteen healthy adults who had no problems with their musculoskeletal system. Group I performed shoulder stabilization exercise with a ball for 30 minutes. Group II performed shoulder isometric resistance exercise without a ball for 30 minutes. Electromyography recorded the MVIC rate of change of the upper trapezius, lower trapezius and serratus anterior in a manual muscle testing position after each exercise. All subjects were assessed for hand function (power and dexterity) using a dynamometer, pinch gauge, grooved pegboard and Purdue pegboard. The EMG data was compared using the independent t-test. Hand function was compared using the independent t-test and the paired t-test. [Results] The results of study were as follows: the MVIC rate of change showed significant difference between the shoulder stabilization exercise group and the shoulder isometric resistance exercise group for the upper trapezius and serratus anterior. In the shoulder stabilization exercise group, power grip, pinch, grooved pegboard test and Purdue pegboard test results were significantly increased post-intervention. In the shoulder isometric resistance exercise group, pinch was significantly increased post-intervention, but the other test results showed no significant differences. [Conclusion] We conclude that shoulder stabilization exercise is more effective than shoulder isometric resistance exercise for shoulder stability and hand function.

本研究の目的はFRT距離に影響を与えていると考えられている項目を抽出し,身長,年齢,左右のCOPの前後長,体幹前傾角度,歩行速度のそれぞれの関係を検証した。重回帰分析によるFRT距離の予測検定では,標準化重回帰係数 βにて身長と体幹前傾角度のみ有意な変数を得られた。また,左右足によるCOP前後長の比較では有意な相関がみられた。よってバランス能力の評価として実施する場合,身長による正規化の必要性を示唆している。加えて前方リーチ動作に伴う運動戦略において股関節の運動である体幹の前傾能力が重要であることも判明した。また,そのCOPの結果から利き足等左右側の影響をうかがうことはできなかった。

[Purpose] This study examined the relationship between core stability and exercise intensity during a pulley-based shoulder exercise (PBSE) on an unstable support surface. [Subjects] Twenty healthy college students enrolled in this study. [Methods] Surface EMG was carried out in twenty healthy adult men. The electromyographic activities of the rectus abdominis (RA), erector spinae (ES), exercises with 14 kg or 26 kg of resistance and external oblique (EO) muscles during pulley-based shoulder on an unstable support surface (USS) were compared. [Results] The EMG signals of the RA, ES, and EO did not increase with increasing exercise resistance. [Conclusion] Increasing the exercise intensity to increase the core stability during PBSE on a USS may be ineffective.

[Purpose] The present study longitudinally investigated injury occurrences and the risk factors for muscle tightness, core stability, and dynamic standing balance among junior high school student baseball players. [Subjects] Thirty-nine male students, belonging to baseball clubs at 2 junior high schools, participated in this study. [Methods] Study measurements were obtained twice, once in the early stage of the baseball season (March) and once at the end of the season (July). All subjects underwent muscle tightness testing, the Star Excursion Balance Test (SEBT), and trunk endurance testing during each measurement session. [Results] Fifteen players experienced episodes of elbow or shoulder pain while throwing. Players in the pain group demonstrated a significant increase in the tightness of their shoulder internal rotators, axis-leg quadriceps, and axis-leg hamstrings. There was no clear evidence of differences of changes in core stability and dynamic standing balance between the groups. [Conclusion] The results of this study suggest that lower extremity muscle tightness early in a season and the subsequent decline in the flexibility of the axis-leg quadriceps and hamstrings during the season may be due to an increased upper extremity load while throwing, thus producing shoulder and elbow pain.

[Purpose] To clarify the influence of nonspecific low back pain (NSLBP) on force fluctuation and the myoelectric data of back muscles during isometric trunk extension at low to high force levels. [Subjects] Fourteen male subjects with NSLBP and 14 healthy male control subjects participated in this study. [Methods] All participants extended their trunk isometrically maintaining 10 levels of target force [2, 5, 10, 15, 20, 30, 50, 70, 80 and 90% of maximal voluntary contraction (MVC) in a random order] for about 4 seconds with visual feedback. A force transducer and tri-axis force sensor were positioned at the 7th thoracic vertebra to measure force output and the direction of force. Myoelectric activities of the back muscles (longissimus thoracis, L2 level; multifidus, S1 level) were recorded by surface electromyography. [Results] Force output of NSLBP subjects fluctuated more than that of healthy subjects at 30% and 50%MVC. Higher median power frequency in the multifidus was observed in NSLBP subjects at moderate to high force levels. [Conclusion] These results show that the properties of force output in NSLBP subjects differ from those in healthy subjects, suggesting that the assessment of force fluctuation of back muscles at moderate force levels is a useful index for evaluating and discriminating NSLBP.

[Purpose] The purpose of this study was to analyze the activity engagement of the retiree population in South Korea. [Methods] The Korean-Activity Card Sort (K-ACS) was used to collect research data. A One-way ANOVA and post-hoc comparisons showed significant group effects among three age groups. The independent t-tests was used to analyze the differences in mean retained level of activity (MRA) between men and women. [Results] The one-way ANOVA showed statistically significant differences in MRA among different age groups. Scheffé’s test revealed a statistically significant decrease in MRA in Group A, aged more than 75 years, as compared to the other two age groups. When participants were divided by gender, MRA of instrumental activities showed a statistically significant difference between the 65–74 years group and the 55–65 years group, but no difference in females of the leisure activities among the age groups. The independent t-tests demonstrated significant gender differences in MRA of activity of the 55–64 years group. [Conclusion] These findings suggest that health professionals should monitor the changes in retained level of activity after age 75, to maintain their engagement, and the importance of age-, gender- and activity-specific analyses in order to identifying patterns of activity engagement.

私が滞在したマレイシアとマラウイでは身体障害者が使用している補装具にも,また支給方法にも違いがみられた。特にマラウイにおいては経済的にも物的にも決して豊かではない状況で,現地で入手可能な材料を用いて補装具が作製されており,そのアイディアには驚かされた。今回,この2国における補装具・車椅子の現状と支給方法を紹介する。

〔目的〕簡便な方法で無酸素性作業閾値(Anaerobic threshold; AT)を測定することが求められている。本研究の第一の目的は,心拍数二乗(HR)2法によってATの検出を試み,基礎的データを提供することとした。さらに,第二の目的はV-slope法ならびに二重積屈曲点(Double Product Break Point; DPBP)法を用いて検出されたATを比較し,(HR)2法の有用性を検討した。〔対象〕健常成人男性10名(23.8±2.5歳)を対象とした。〔方法〕自転車エルゴメーター上にて,5分間の安静座位の後,0 wattでのウォーミングアップを3分間行い,続いて30 wattsから開始した。運動負荷は2分ごとに10 wattずつ負荷を増加させる多段階運動負荷試験を用いた。〔結果〕(HR)2法,DPBP法,V-slope法によるAT検出時のVO2は,それぞれ有意差を認めなかった。また,AT検出時のwattsもそれぞれ有意差を認めなかった。〔結論〕(HR)2法はAT検出に関わる心拍数ならびに負荷量を簡便に知ることが可能な方法の一つであると考えられた。

〔目的〕本研究の目的は,咳嗽力の指標としてPEFを測定し,非脳卒中者や嚥下障害の有無で比較するとともに,脳卒中患者のPEFに関連する因子を明らかにすることである。〔対象〕対象は,発症後6ヶ月以内の脳卒中患者46名(嚥下障害群22名,非嚥下障害群24名)と対照群として非脳卒中者24名であった。〔方法〕PEFの測定は電子ピークフローメータを用いた。本研究では脳卒中患者の咳嗽力に関連すると考えられる因子として年齢,身長,体重,BMI,Brunnstrom recovery stage,Barthel Index,血清アルブミン値,摂食・嚥下能力グレードを取り上げた。〔結果〕PEFは, 対照群,非嚥下障害群, 嚥下障害群の順に低値を認めた。体重,BMI,PEF値,血清アルブミン値,Barthel Indexは非嚥下障害群より嚥下障害群の方が有意に低い値を示した。全脳卒中患者のPEF値を目的変数にしたステップワイズ重回帰式に取り込まれた因子は,血清アルブミン値,身長,摂食・嚥下能力グレードであった。〔結語〕脳卒中患者の咳嗽力は非脳卒中者より低下しており,その原因には嚥下障害による低栄養状態が関連していることが示唆された。

[Purpose] This study investigated the effects of resistance exercise using Thera-band on balance of elderly adults. [Methods] Subjects (age range, 60–70 years) were randomly assigned to an experimental (n=12) or control group (n=12). The experimental group performed stretching and resistance exercises, and the control group performed stretching exercises only. Before and after the 5-week intervention, the participants’ static and dynamic balance were evaluated using the Berg Balance Scale, the Timed Up & Go Test, and the Tetrax Portable Multiple System (Tetrax Ltd., Ramat Gan, Israel) after 5 weeks. [Results] After the intervention, the values of the Tetrax in the weight distribution index with eyes open and that with eyes closed and the stability test index with eyes open were significantly lower in the resistance exercise group than in the control group, and the pre-test values were was significantly higher than the post-test values. However, there were no significant differences between groups in the values of the Berg Balance Scale, the Timed Up & Go Test, and the Tetrax stability test index with the eyes closed. [Conclusion] The findings of this study indicate that resistance exercise using the Thera-band is possible to improve the static and dynamic balance of elderly adults.

[Purpose] The purpose of this study was to investigate the effect of 8 weeks aquatic exercise on the gait stability of the elderly using dynamic factors: center of mass (COM), velocity of center of mass (COMV), and center of pressure (COP). [Subjects] Eleven elderly participants (age: 77.18 ± 4.96 yrs, height: 149.48 ± 3.61 cm, body mass: 56.94 ± 6.62 kg, and leg length: 82.36 ± 2.98 cm), participated in this study. [Methods] To identify the 8-week aquatic training effect, 3-D motion analysis with 7 infrared cameras and one force plate, was performed. [Results] For the COM-COP inclination angles, significantly decreased medial inclination angles were shown in both the posterior and anterior swing phases. For the COMV-COP inclination angles, decreased medial inclination angles were shown in both the posterior and anterior swing phases, but significant difference was found only in the posterior phase. [Conclusion] The results suggest that 8 weeks aquatic exercise is effective at improving the gait stability of the elderly. Further studies should extend the training period to gain statistically significant results for the effect of aquatic exercise in the anterior-posterior direction.

[Purpose] The purpose of this study was to investigate the effect of progressive resistance training (PRT) integrated with foot and ankle compression on the gait ability of post-stroke patients. [Subjects and Methods] Participants were randomly allocated to two groups: the PRT group (n=14) and the control group (n=14). Subjects in the PRT group received training for 30 minutes, five days per week, for a period of six weeks. Gait ability was evaluated using the RsScan system. [Results] Use of PRT integrated with foot and ankle compression resulted in significant improvements in temporal parameters (gait velocity, step time, and double limb support) and spatial parameters (step length, stride length, and heel-to-heel base of support). [Conclusion] Progressive resistance training integrated with foot and ankle compression improved the gait ability of stroke patients. These results suggest the feasibility and suitability of integration of PRT with foot and ankle compression for individuals with stroke.

[Purpose] To examine changes in the knee joint’s isokinetic muscle functions following systematic and gradual rehabilitation exercises lasting for 12 weeks for male and female patients who underwent anterior cruciate ligament (ACL) reconstruction. Differences in muscle functions between the uninvolved side (US) and the involved side (IS) before surgery, differences in muscle functions between US and IS after rehabilitation exercises lasting for 12 weeks, and changes in muscle functions on US and IS between before and after surgery were analyzed to examine the effects of accelerated rehabilitation exercises after ACL reconstruction. [Subjects] The study subjects were 10 patients, five females and five males, who underwent ACL reconstruction performed by the same surgeon. [Methods] As a measuring tool, a Biodex Multi-joint system 3pro (USA), which is an isokinetic measuring device, was used to examine the flexion and extension forces of the knee joint. During isokinetic muscle strength evaluation, the ROM of US was set to be the same as that of IS for consistency of measurement. [Results] At 60°/s, the isokinetic muscle functions of the females did not show any significant change between before and after surgery in any of the variables on both US and IS. At 60°/s, the isokinetic muscle functions of the males did not show any significant change between before and after surgery in the peak torque, average power, and entire work done on US. In extension, peak torque on IS did not show any significant change.

[Purpose] This study aimed to examine the effects of kinesio tape applied to chronic low back pain (CLBP) patients on anticipatory postural control and cerebral cortex potential. [Subjects and Methods] Twenty patients whose low back pain had continued for more than 12 weeks were selected and assigned to a control group (n=10) to which ordinary physical therapy was applied and an experimental group (n=10) to which kinesio tape was applied. Anticipatory postural control was evaluated using electromyography, and movement-related cortical potential (MRCP) was assessed using electroencephalography. Clinical evaluation was performed using a visual analogue scale and the Oswestry disability index. [Results] According to the analysis results for anticipatory postural control, there were significant decreases in the transversus abdominis (TrA) muscle and the external oblique muscle in both groups. Among them, the TrA of the experimental group exhibited the greatest differences. According to the results of a between-group comparison, there was significant difference in the TrA between the two groups. There was also a significant decrease in the MRCP of both groups. In particular, changes in the movement monitoring potential (MMP) of the experimental group were greatest at Fz, C3, Cz, and C4. According to the between-group comparison, there were significant differences in MMP at F3, C3, and Cz. Both groups saw VAS and ODI significantly decrease. Among them, the ODI of the experimental group underwent the greatest change. [Conclusion] Kinesio tape applied to CLBP patients reduced their pain and positively affected their anticipatory postural control and MRCP.
