[Purpose] The purpose of this preliminary study was to assess the trade-off relationship between the hip and ankle joints after plantarflexor training in children with spastic hemiplegic cerebral palsy (CP). [Subjects and Methods] Three boys aged 9, 10, and 13 years with spastic hemiplegic CP participated in the study. Gait analysis was performed using a three-dimensional motion analysis device and a floor reaction force detection device before and after plantarflexor training. Data on gait speed and stride length for both sides were collected. Peak hip and ankle powers in the sagittal plane and ankle-to-hip power ratio (A2/H3 ratio) were calculated. Plantarflexor training comprised heel raises and exercise band resistance at the participant’s home (3 times/week for 12 weeks). [Results] The A2/H3 ratio increased significantly on both sides in two of three subjects after training. Peak A2 power increased significantly on both sides in subject 3 and on the affected side of subject 2. Peak H3 power decreased significantly on the non-affected side of subjects 1 and 2. [Conclusion] This study confirmed that two of three subjects demonstrated a trade-off relationship between the hip and ankle joints during gait after plantarflexor training.

[Purpose] This study aimed to determine the impact of aerobic exercise on aerobic capacity, balance, and treadmill time in patients with thermal burn injury. [Subjects and Methods] Burned adult patients, aged 20–40 years (n=30), from both sexes, with second degree thermal burn injuries covering 20–40% of the total body surface area (TBSA), were enrolled in this trial for 3 months. Patients were randomly divided into; group A (n=15), which performed an aerobic exercise program 3 days/week for 60 min and participated in a traditional physical therapy program, and group B (n=15), which only participated in a traditional exercise program 3 days/week. Maximal aerobic capacity, treadmill time, and Berg balance scale were measured before and after the study. [Results] In both groups, the results revealed significant improvements after treatment in all measurements; however, the improvement in group A was superior to that in group B. [Conclusion] The results provide evidence that aerobic exercises for adults with healed burn injuries improve aerobic physical fitness and balance.

[Purpose] The purpose of this study was to investigate the therapeutic effects of virtual reality-based ankle exercise on the dynamic balance, muscle tone, and gait ability of stroke subjects. [Subjects and Methods] Twenty persons who were in the chronic stroke subjects of this study and they were included and assigned to two groups: experimental (VRAE; Virtual Reality-based Ankle Exercise group) (n=10) and control groups (n=10). The VRAE group performed virtual environment system ankle exercise, and the control group watched a video. Both groups performed their respective interventions for 30 minutes per day, 5 times per week over a 6-week period. To confirm the effects of the intervention, dynamic balance, muscle tone, and spatiotemporal gait were evaluated. [Results] The results showed that the dynamic balance and muscle tone was significantly more improved after the intervention compared to before in the VRAE group (dynamic balance: 5.50±2.57; muscle tone: 0.90±0.39), and the improvements were more significant than those in the control (dynamic balance: 1.22±2.05; muscle tone: 0.10±0.21). Spatiotemporal gait measures were significantly more increased in the paretic limb after the intervention compared to before in the VRAE group and the improvements were more significant than those in the control group. [Conclusion] This study demonstrated that virtual reality-based ankle exercise effectively improves the dynamic balance, muscle tone, and gait ability of stroke patients.

[Purpose] This study sought to determine the effects of brain-computer interface-based functional electrical stimulation (BCI-FES) on brain activation in patients with stroke. [Subjects] The subjects were randomized to in a BCI-FES group (n=5) and a functional electrical stimulation (FES) group (n=5). [Methods] Patients in the BCI-FES group received ankle dorsiflexion training with FES for 30 minutes per day, 5 times under the brain-computer interface-based program. The FES group received ankle dorsiflexion training with FES for the same amount of time. [Results] The BCI-FES group demonstrated significant differences in the frontopolar regions 1 and 2 attention indexes, and frontopolar 1 activation index. The FES group demonstrated no significant differences. There were significant differences in the frontopolar 1 region activation index between the two groups after the interventions. [Conclusion] The results of this study suggest that BCI-FES training may be more effective in stimulating brain activation than only FES training in patients recovering from stroke.

[Purpose] To examine how the following factors would influence discharge to home: the family's view about the discharge destination at the time of the stroke patient's admission, the social background including the presence and sex of spouse and the number of family members, and the age of the patient. [Subjects] A total of 189 subjects with first time stroke were divided into 3 groups according to their families' views about discharge destination as follows: discharge to home group (96 subjects), discharge to facilities group (52 subjects), and undecided group (41 subjects). Each group was further divided into 2 sub-groups: discharge to home and discharge to facilities. [Methods] A multivariate logistic regression analysis was performed, considering the discharge destination as the response variables, and admission and discharge FIM scores of 18 items as explanatory variable. Also, in order to observe the influence of social background, comparisons by the presence and sex of spouse, by the number of family members, and by age were conducted. [Results] In a multivariate logistic regression analysis for all subjects at discharge, "eating" and "problem solving" were selected. In a comparison by age, a significant difference was recognized between those aged up to 79 years and those aged 80 years and older on the probability of discharge home. [Conclusion] The result suggests that eating and cognitive function play important roles in determining the discharge destination of stroke patients.

[Purpose] To assess the effect of hamstring and quadriceps strengthening exercises on pain intensity, gait velocity, maximum isometric strength, and activities of daily living of patients with knee osteoarthritis (OA). [Subjects and Methods] A total of 20 patients with knee OA, 50 to 65 years of age (57.65 ± 4.78 years), received hot packs, strengthening exercises for the quadriceps and the hamstring muscles and stretching exercises for hamstring muscles. Outcome measures included: the Western Ontario and McMaster Universities OA index questionnaire (WOMAC) scores for assessing health status and health outcomes of knee OA; self-reported pain intensity scores, measured using a visual analogue scale; the 50 ft walk test (a measure of gait velocity and function); and handheld dynamometry (a tool used to measure maximum isometric strength of knee extension and flexion). [Results] There was a significant difference between pre- and post-intervention measures of pain intensity, 50 ft walk times, hamstring strength, and quadriceps strength. Significant differences in WOMAC measures were also observed in the subscales of pain, stiffness and physical function, as well as WOMAC total scores. [Conclusion] Strengthening the hamstring muscles in addition to strengthening the quadriceps muscles proved to be beneficial for perceived knee pain, range of motion, and decreasing the limitation of functional performance of patients with knee OA.

[Purpose] Osteoarthritis (OA) of the knee is the most common form of joint disease. It is one of the major causes of impaired function that reduces quality of life (QOL) worldwide. The purpose of this study was to compare exercise treatments for hamstring and quadriceps strength in the management of knee osteoarthritis. [Subjects and Methods] Forty patients with OA knee, aged 50–65 years were divided into 2 groups. The first group (57.65±4.78 years) received hot packs and performed strengthening exercises for the quadriceps and hamstring, and stretching exercises for the hamstring. The second group (58.15±5.11 years) received hot packs and performed strengthening exercises for only the quadriceps, and stretching exercise for the hamstring. Outcome measures were the WOMAC (Western Ontario and McMaster Universities OA index questionnaire), Visual Analogue Scale (VAS) assessment of pain, the Fifty-Foot Walk Test (FWS), and Handheld dynamometry. [Results] There was a significant difference between the groups. The first group showed a more significant result than the second group. [Conclusion] Strengthening of the hamstrings in addition to strengthening of the quadriceps was shown to be beneficial for improving subjective knee pain, range of motion and decreasing the limitation of functional performance of patients with knee osteoarthritis.

[Purpose] The main purpose of our study was the evaluation of the effects of long-term volleyball practice on knee joint proprioception and balance of young female athletes. [Subjects and Methods] An observational case-control study was performed. The study enrolled 19 female volleyball players in the experimental group and 19 sedentary counterparts as controls. A Biodex balance system and dynamometer were used for the evaluations. The paired t-test was used to determine the significance of differences between the performance of athletes and controls. [Results] The knee proprioception analysis showed a significant difference at 60° joint position in active and passive tests. A similar trend, but without significance, was found for the 20° joint position. In the postural stability tests both groups showed similar results with no significant differences between them. [Conclusion] In conclusion, the results indicate a significant influence on joint proprioception is elicited by long-term exposure to a team sport like volleyball. However, the postural stability indexes showed similar trends in both groups, highlighting the analogous ontogenesis of the subjects investigated and the low influence of volleyball practice on postural stability.

[Purpose] The aim of this study was to determine the effects of Thai massage on physical fitness in soccer players. [Subjects and Methods] Thirty-four soccer players were randomly assigned to receive either rest (the control group) or three 30-minute sessions of Thai massage over a period of 10 days. Seven physical fitness tests consisting of sit and reach, hand grip strength, 40 yards technical agility, 50-meter sprint, sit-ups, push-ups, and VO2, max were measured before and after Thai massage or rest. [Results] All the physical fitness tests were significantly improved after a single session of Thai massage, whereas only the sit and reach, and the sit-ups tests were improved in the control group. [Conclusion] Thai massage could provide an improvement in physical performance in soccer players.

[Purpose] The purpose of this study was to determine the effects of brain-computer interface (BCI)-based functional electrical stimulation (FES) on balance and gait function in patients with stroke. [Subjects] Subjects were randomly allocated to a BCI-FES group (n=5) and a FES group (n=5). [Methods] The BCI-FES group received ankle dorsiflexion training with FES according to a BCI-based program for 30 minutes per day for 5 days. The FES group received ankle dorsiflexion training with FES for the same duration. [Results] Following the intervention, the BCI-FES group showed significant differences in Timed Up and Go test value, cadence, and step length on the affected side. The FES group showed no significant differences after the intervention. However, there were no significant differences between the 2 groups after the intervention. [Conclusion] The results of this study suggest that BCI-based FES training is a more effective exercise for balance and gait function than FES training alone in patients with stroke.

[Purpose] Smartphone use reportedly changes posture. However, how neck posture is altered in smartphone users with neck pain is unknown. This study examined changes in the posture of young adults with and without mild neck pain (MNP) when using a smartphone. [Subjects] Thirteen control subjects and 14 subjects with MNP who used smartphones were recruited. [Methods] The upper cervical (UC) and lower cervical (LC) angles in the sagittal plane were measured using an ultrasound-based motion analysis system while the seated subjects used a smartphone for 5 min. [Results] During smartphone use, the MNP group exhibited greater UC and LC flexion angles than the control group. [Conclusion] These findings suggest that young adults with MNP are more careful and more frequently utilize a neutral neck posture than young adults without MNP when using a smartphone while sitting.

[Purpose] The aim of this study was to describe the importance of patient-related factors in rehabilitation. We focused on the type A behavior pattern. If individuals with the type A behavior pattern have better compliance, they would have a shorter length of hospital stay than those with non-type A behavior. We compared the length of stay of patients with the type A behavior pattern with that of patients with a non-type A behavior pattern. [Subjects and Methods] Fifty-seven patients staying in a comprehensive rehabilitation unit participated in this study. Type A behavior pattern, length of stay, and Barthel Index were assessed. We use the Student’s t-test to examine the statistical differences in length of stay and Barthel Index at discharge between subjects with type A behavior and those without type A behavior. [Results] Age and Barthel Index at discharge were not significantly different between the two groups. However, length of stay was significantly higher in the non-type A group compared with the type A group. [Conclusion] Patients with the type A behavior pattern had a shorter length of hospital stay than patients with a non-type A behavior pattern. In conclusion, our results suggest that the type A behavior pattern shortens the length of hospital stay. Those data show that we should consider the patient’s characteristics in rehabilitation to protect the patient and for financial benefit.

手指対立運動時の感覚運動野(sensorimotor cortex: SMC)の賦活の左右差を,機能的MRIを用いて検討した。対象は右利きの健常成人12名で,1秒間に1回の速度で連続的に手指対立運動を自発的に行う課題を右手,左手各々において行った。右手,左手運動時の対側SMCの平均賦活信号強度は,右手運動より左手運動で有意に高かった(右手,左手,各々11.68,16.82, p<0.05)。さらに,右手運動時は対側SMCのみに賦活が見られたのに対して,左手運動時は両側SMCに賦活がみられた。以上の結果は,巧緻性の低い左手は右手に比べ、多くの神経活動を必要とすることを示唆している。

〔目的〕本研究は足底知覚トレーニングが後期高齢者の立位姿勢バランスの安定化に対しての有効性を検証した.〔対象〕介護老人保健施設に入所する後期高齢者19名とし,トレーニング群9名,コントロール群10名にそれぞれ無作為に振り分けた.〔方法〕トレーニング群には硬度の異なるスポンジマットを弁別させ,コントロール群には一定の硬度のスポンジマットにて立位姿勢を保持させ,10日間実施した.〔結果〕トレーニング群に立位重心動揺値の有意な減少が認められた.〔結語〕これらの結果より生理的に感覚機能が低下する後期高齢者においても,立位姿勢バランスの安定化に対して足底知覚課題が有効的に作用することが示唆された.

[Purpose] The aim of this study was to evaluate the changes in function and mental state after thoracic mobilization and manipulation in patients with chronic lower back pain (LBP). [Subjects and Methods] Thirty-six subjects were randomly divided into mobilization group (group A), manipulation group (group B) and control group (group C). The Oswestry disability index (ODI) was used to measure the functional impairment of patients with LBP. A multiple spinal diagnosis was used to measure the range of motion (ROM) of vertebra segments. The Fear-avoidance beliefs questionnaire (FABQ) was used to investigate the mental state of LBP patients. [Results] Group A and group B were significantly different from group C in terms of the ODI. Between groups, there was no difference in ROM during trunk flexion. Group A and group B were also significantly different from the control group in extension ROM. The FABQ of group B was significantly different from that of group A. [Conclusion] Application of mobilization or manipulation to thoracic lumbar vertebrae has a positive effect on function, mental state, and ROM in patients with lower back pain.

〔目的〕虚弱高齢者用10秒椅子立ち上がりテスト(Frail CS-10)がパーキンソン病患者に応用可能か否かを検討した.〔対象〕パーキンソン病患者21名とした.〔方法〕Frail CS-10と従来から下肢筋力の代表値として用いられている大腿四頭筋筋力を測定し,下肢機能の指標(重心動揺,TUG,5 m最速歩行,10 m障害物歩行,FIM-M)との関連を検討した.〔結果〕Frail CS-10はTUG,10 m障害物歩行およびFIM-Mとの間に有意な相関を示した.一方,大腿四頭筋筋力は,すべての身体機能との間に有意な相関を示さなかった.〔結語〕従来から下肢筋力の代表値として用いられている大腿四頭筋筋力よりもFrail CS-10の方が,パーキンソン病患者の下肢機能を推測するための簡便な評価法である.

〔目的〕パーキンソン病患者に有用なバランステストを検証するために,パーキンソン病患者を対象に臨床で使用頻度の高いバランステストを数種類測定し,どのバランステストがADLと関連するのかについて検討した.〔対象〕パーキンソン病患者20名(平均年齢72.4歳)を対象とした.〔方法〕TUG,片脚立ちテスト,FRTおよび重心動揺(外周面積,総軌跡長)を測定し, FIM-MおよびFIM-M下位項目との関係をピアソンの相関係数を用いて検討した.〔結果〕TUGはFIM-Mおよび下位項目である移乗,移動動作との間に有意な相関が認められ,セルフケアと排泄とは有意な相関は認められなかった.その他の片脚立ちテスト,FRT,外周面積,総軌跡長とFIM-MおよびすべてのFIM-M下位項目との間には有意な相関は認められなかった.〔結語〕得られた知見から,今回測定を行ったバランステストの中では,TUGのみがパーキンソン病患者のADLと関連していることが示唆された.

本研究の目的は,閉眼および開眼片足立ち測定,Multi-Directional Reach Testの3評価結果における転倒群と非転倒群の比較,転倒回数との関連性および3評価結果間の相関について調べ,高齢者の転倒要因とされるバランス能力をどのような視点から捉えるべきか検討した。結果,いずれの評価結果においても転倒群が有意に少ない値を示し,また,転倒回数が多い者ほど値が少なかった。転倒群では,各評価結果間すべてにおいて有意な相関が認められた。以上より,転倒要因として圧中心保持能力と圧中心偏移能力の両方の視点からバランス能力を捉える必要のあることが示唆された。

〔目的〕平成19年度と20年度前期定期試験において,定期試験成績向上に学習意欲が影響を及ぼしたかを検討した。〔対象〕某A専門学校理学療法学科学生(101名)とした。〔方法〕平成19年度および20年度前期定期試験を基に各学生の偏差値を算出した。また,定期試験の直前に学習意欲に関するアンケートも実施した。ここで,定期試験において成績が向上した学生と低下した学生とについて,学習意欲アンケート結果との関係を多重ロジスティック回帰分析にて検討した。〔結果〕外発的動機づけと精神的健康度が有意な説明変数として抽出された。〔結語〕今後,学習意欲を向上させることの出来るような対応の検討を行っていきたいと考えた。

[Purpose, Case, and Methods] A female bottlenose dolphin (Tursiops truncatus) in captivity developed necrosis of the tail flukes. Although the diseased site healed after surgical resection, the loss of approximately 75% of the tail greatly affected her swimming performance. To restore swimming ability, we developed artificial tail flukes as a prosthetic swimming aid and provided physical therapy that included swimming training from postoperative day 1 to day 1427. [Results] The prosthetic enabled the dolphin to recover swimming ability almost to the level prior to disease onset, but even acquire applied movement, and reestablish social relationships, thus greatly improving the animal's quality of life. [Conclusion] The results clearly demonstrate that, as in postoperative rehabilitation in humans, the use of prosthetic devices in physical therapy can be beneficial for marine animals such as dolphins.
