本邦における痙攣性発声障害患者数や臨床像を明らかにすることを目的として,アンケート方式による疫学調査を実施した.本疾患の音声所見を収録したサンプルCDを作成したうえで,全国の主要な耳鼻咽喉科医療機関655施設に調査表を送付した.その結果,過去2年間にこれらの医療機関を受診した患者は1,534例あり,このうち新規患者は887例(0.70人/10万人)いることが確認できた.これにより,有病率は3.5~7.0人/10万人以上になることが推測された.臨床像としては,年齢は20および30歳代が59.0%を占め,男女比は1:4.1と女性が多かった.病型は内転型が約93.2%を占め,症状は内転型では声のつまりや努力性発声,外転型では失声や声が抜けるなどが特徴的であった.症状発現から医療機関受診までの期間の中央値は3.0年であった.治療はA型ボツリヌス毒素の内喉頭筋内注入療法や甲状軟骨形成術Ⅱ型などが,一部の医療機関で集約的に行われていた.今回の調査を通して,痙攣性発声障害は耳鼻咽喉科医においてもまだ十分に認識されていないことが推測された.調査結果などを基にして,診断基準の作成や治療指針の確立が望まれる.

One hundred two patients with parotid tumor were treated in the ENT Department, Kyoto Pref ectural University of Medicine between 1989 and 1994. Twenty-four of the 102 parotid tumors were Warthin's tumor (adenolymphoma). This incidence of Warthin's tumor was higher than that in any previous reports. This and other recent reports show that the incidence of Warthin's tumor has tended to increase recently, but the reason for this increase is not clear. In general, it is said that Warthin's tumor is found much more frequntly in males than in females, so we reviewed and analyzed 9 clinical cases of Warthin's tumor found in women. The average age of these women was approximately 10 years younger than that of male patients. No substantial differences were seen between male and female cases of Warthin's tumor on CT and Echo findings.

A 60-year-old male suffered from left temporalgia and developed mastoiditis. A jugular foramen syndrome (cranial nerve IX, X and XII deficit) that occurred in spite of mastoidectomy was improved by means of dissection of petrosal part.One month later. at the contra-lateral side of the jugular foramen syndrome, Gradenigo syndrome originated from mastoiditis occurred. Mastoidectomy resulted in improvement of clinical symptoms.Five months after the surgery, the patient developed left superior orbital fissure syndrome (cranial nerve II, III and V palsy) with thickening of the dura mater which was demonstrated by Gd-DTPA MRI.Corticosteroid therapy was effective except for blindness.These clinical findings of cranial nerve palsies were suspected to be associated with the migrating disseminated multiple cranial neuropathy.

We report a rare case of a foreign body, a large snail, in the esophagus. A 20-year-old male, a freshman in college, was invited to a party at a music club and was requested by senior students to do a comical performance. He decided to swallow a large snail in its shell. Severe throat pain was immediately noted. He tried to induce vomiting, but was unable to and came to our hospital for emergency case. No foreign body was found in the hypopharynx. Plain X-ray examination showed a circular shadow in the neck, 3 centimeters in diameter. We identified the foreign body in the esophagus in the neck. We tried to remove the object with an endoscope under general anesthesia. The shell was found at the inlet of the esophagus and was spherical and smooth, which made it impossible to grip with forceps. Therefore, we removed the foreign body via external incision of the neck.
