A case of electrical burn injury was reported. After coming into contact with high tension electrical current (20, 000 volt), severe electrical burns of bilateral forearms and lower extremity were evoked. Immediately, débridement of the wounds was made, however, circulation distrubance of the blood vessels arose, which enhanced degeneration and necrosis of the soft tissue. Within a month, three limbs amputations were done, and he was survived and recovered fortunately. The patient is doing training of prosthesis exercise at present.

脳卒中片麻痺に処方される短下肢装具の機能を調査する目的で,全国の回復期リハビリテーション病院にアンケート調査を行った.集計した 226 病院の 2,260個の短下肢装具の機能について検討した結果,シューホーン AFO やオルトップ AFO など足関節の底屈,背屈に制動と補助の機能があるタイプが 56.62%と最も多く,底屈,背屈に固定,遊動,制限の設定ができるタイプが 16.64%で第2位であった.全国を 8つの地域区分で検討すると,ほぼ類似の処方傾向が見られたが,各県別での検討ではばらつきも少なくなかった.

健康成人,男性946名,女性842名の両下肢の下肢伸展挙上角度(SLR角度)を調査し,SLRテストを評価する上に参考となる事項について述べた.即ち,1.日本人成人におけるSLR角度の正常値は,男性が右下肢78.7度±9.70度,左下肢79.0度±9.94度であり,女性が右下肢85.7度±8.48度,左下肢87.3度±8.38度であった.これらSLR角度の臨界値は男性65度,女性75度と考えられた.2.SLRテストを評価するにあたっては,正常者のSLR角度には男女差があり女性の角度が大きいこと,並びに左右差はなく左下肢と右下肢との間に高い相関があることに配慮する必要がある.3.また正常者におけるtight hamstringsの頻度は,男性が12.5%,女性が13.2%であった.

脳卒中に対してのシューホーンブレースとその類似装具を含めたシューホーン型短下肢装具は,わが国での処方頻度が高いが,その形状と適応病態との関連についての検討はまだ十分とは言えない.本装具の形状によるプラスチックのたわみの程度と完成時の背屈角度,靴を履いて起立した時の下腿の前傾角度(SVA)などの違いによって,適応となる足関節部の筋力,膝関節の安定性,下腿三頭筋の痙縮の程度などが変わることを強調した.さらに本装具を装着しての歩行で問題が見られた時の対処法について述べた.

The general of Tokugawa shogunate gave daimyos "Ryochi-hannmotsu", the certificate of the governance on the region and "Ryochi-mokuroku", which describes the contents of the region. However, Ryochi-hannmotsu from the first general Ieyasu Tokugawa to Daimyo Satake in 1602 did not contain "Kokudaka", the size of the region. Daimyo Satake got Ryochi-hannmotsu and Ryochi-mokuroku for the first time from the fourth general Ietsuna Tokugawa in 1664. Although the kokudaka in the Ryochi-hannmotsu was 200000-koku, the sumu of the kokudaka at each district described in the Ryochi-mokuroku was 199320-koku. This paper focused on this discrepancy for the first time and provided the premise to investigate the 200000-koku region of Daimyo Satake assured by the general.

We have performed 22 lower extremity amputations with immediate post-surgical prosthetic fitting since November 1966.Among the causes of these amputations, 9 were of malignant bone tumors, 8 of gangrens due to peripheral vascular disease and 5 of other causes such as traumas and congenital malformation.In general, very early ambulation after surgery, early completion of final prosthetic fitting, and marked shortning of hospitalization have resulted from this technique. The authors however want to emphasize that very precise cautions will be needed in order to be successful in primary wound healing in the case of the amputation due to peripheral vascular disease.

数種類のα-アミラーゼ阻害剤の効果, α-アミラーゼ作用を抑制した場合の発芽処理麦の特性について実験を行い次の結果を得た. 1) 従来の色素でん粉法に若干の改良を加えた簡便方法によりα-アミラーゼ活性の測定を行い,阻害剤の影響を比較した.硝酸銀,硝酸銅, EDTA,硫酸ヒドラジンは, 0.1Mでα-アミラーゼに対し高い阻害効果を示した.特にEDTAは, α-アミラーゼそのものに対する阻害剤で取り扱いも容易なことから,利点が認められた. 2) 発芽処理麦から調製した小麦粉,小麦でん粉のアミログラム特性,フォーリングナンバー値は阻害剤を添加することにより健全麦からのものに類似したものとなったことから,発芽処理麦ではでん粉そのものはそれ程障害を受けていないが生地等になって初めて活性化したα-アミラーゼの作用を受け糊化特性の劣化を呈することが確認された. 3) EDTA等の阻害剤を使用することにより,発芽小麦の潜在的なビスコグラム最高粘度,フォーリングナンバー値をでん粉の単離,トルエン処理等を行うことなく測定する可能性が認められた.

There are two possible causes that developed the restriction of joint range of motion after intramuscular injection.The first category is peripheral nerve injury after injection which fregently occurred in the radial nerve and the sciatic nerve. The others is degeneration or scar of the muscle tissue which encounter often in rectus femoris muscle in children.The contracture of shoulder joint due to deltoid muscle shortening secondary to the injection is very rare.This 34 year-old-female patient came to our clinic with pain and stiffness feeling on her right shoulder joint. We could get the history that she received repeated intramuscular injections to her right deltoid muscle for two years.She was operated and was found a whitish cord like scarred band, 1cm wide, in the posterior part of the deltoid which was removed subsequently.The histology showed marked atrophy, degeneration, and hyalinization of the muscle tissue.She got fairly good range of motion in the horizontal adduction at the shoulder joint after the surgery.Generally it is difficult to diagnosed this kind of disease on its early stage.This adult case was not dropery diagnosed for about two years.

1.出羽国絵図・陸奥国絵図の所在調査と写真撮影東北地方の所蔵機関のみならず,現存の可能性がある機関でも調査を行い,出羽国絵図160点,陸奥国絵図164点の所在を確認した。このうち今年度は弘前市立図書館所蔵の津軽領国絵図の撮影を実施したほか,東北歴史博物館,宮城県図書館,国立公文書館所蔵の国絵図の写真版なども収集した。2.現存国絵図の年代推定とデータベースの作成国絵図の原本調査を行い,環境・景観復原を行うために不可欠な絵図の作成年代を推定し,出羽・陸奥国絵図の所在データベースを作成した。さらに,本研究の目的には含めていなかったが,撮影した国絵図の画像データベースの作成も進め,一部完成している。3.環境・景観変化の復原作業画像処理によって正保・元禄国絵図の図形の歪みを修正し,さらに現況の図形に変形した上で,東北地方の平野部について,明治後期測量の旧5万分の1地形図をベースにした正保または元禄期の環境・景観の復原作業を実施した。その一部を報告書に掲載したが,いずれ何らかの方法で公開していきたい。

Tenosynovitis in hand and finger is exudative or proliferative inflammation of tenosynovium. Clinically, it shows the symptom of local swelling, palpable tumors, motion disturbance of fingers in spite of its different causes. And it sometimes causes tendon rupture or nerve palsy. Diagnosis is made by clinical symptoms, laboratory data, bacterial and fungal culture and histopathology.<br>We presented three cases of <i>tenosynovitis</i> associated with rupture of extensor pollicis longus tendon (Case No. 1) and carpal tunnel syndrome (Case No. 2). One case out of three was caused by tuberculosis. But in other two cases, clear diagnosis was not estabilshed even after such thorough examinations. In this paper, we discussed some problems of diagnostic process, and emphasized that tenosynovitis in hand fingers had possibility of causing tendon rupture and in carpal canal causing carpal tunnel syndrome regardless of its etiology.
