The purpose of this study was to investigate the trunk muscles causing associated reactions in a hemiplegia patient with cerebrovascular disoders. Clinical evaluation and surface electromyographic based motion analysis were performed to confirm impairment problems in the patient. He showed associated reaction of left elbow flexion and left forearm supination caused by left anterior tilt to right posterior tilt of the trunk in the stance phase shift to the swing phase in gait. Surface electromyographic evaluation was performed while practing a similar gait task. The results indicated that high muscle activity of the affected side biceps brachii muscle was caused by high muscle activity of the unaffected side low back muscles. These finding suggest that not causing hypertonia of unaffected side low back muscles in normal movement of gait is very important.

To analyze the relationship between the problems of impairment and disability is important for the selection of the best therapy. The approach to the problems of impairment and disability is not a special technique, and the basic technique is as follows: expansion of ROM, control of muscle strength and muscle tonus, and control of sensory and motion education. To succeed in this technique it is important that the therapeutic position, the position of the therapist's hand and the end point of each therapy, are appropriate. Recognizing the importance of these processes in providing therapy, should make us the best therapist for the patient.

We report a case of left-side hemiplegia due to subarachnoid hemorrhage. When our patient walked with a T-shaped cane, almost no extension and adduction of the left hip joint from the left loading response to the left mid-stance, poor weight transfer to the left lower limb, and instability from the right mid-stance to the right terminal stance due to hyperabduction of the right hip joint were noted. The patient's right hip joint was externally rotated, and the pelvis was left rotated throughout both standing and walking due to internal rotation weakness of the right hip joint. The left hip joint was poorly flexed in the left swing phase due to hypotonia of the left iliacus muscle, and the left lower leg had been swinging out for many years due to left pelvic elevation along with left lumbar flexion. It was necessary to first address the problems of the right lower limb. Left hip extension, adduction, and internal rotation, left ankle dorsiflexion, and left foot supination were absent. In addition, horizontal movement of the pelvis was difficult. An approach improving the left rotation of the pelvis and external rotation of the left lower leg via external rotation of the right hip joint, resulted in extension and adduction of the left hip joint occurring in the middle stage of the left stance, enabling the patient to transfer weight onto the left lower limb.

There are two types of assessments in physical therapy: top-down and bottom-up. Top-down evaluations, which we recommend, focus on movement observation and analysis. In this article, we introduce some points for analyzing ankle joint problems. When observing movements, determining the details of the movements according to the type of movement is sometimes difficult. In such cases, dynamic alignment evaluation is occasionally performed, an example of which is presented in this article. In addition, patients with ankle joint diseases often have limited range of motion of ankle dorsiflexion. Multiple factors contribute to ankle dorsiflexion limitation, such as flexor hallucis longus and Kager’s fat pad, which we describe in this study using an ultrasound imaging system.

To perform patient treatment in physical therapy, it is necessary to identify the problems correctly. The problems must be narrowed down to the impairments that can be understood from the physical therapy evaluation. To determine the impairment level, it is important that motion observation and motion analysis of the patient’s problems are performed correctly.

We administered physical therapy for a patient who presented with right hemiplegia after cerebral infarction. When lowering clothing with the left hand for toileting, the patient risked falling posteriorly to the right. Therefore, therapy focused on enabling the removal of underclothing. Healthy subjects remove underclothing with the use of left elbow extension and left lateral bending of the trunk, while moving the pelvis in a right lateral direction. However, this patient was unable to adequately bend the trunk to the left and could not lower underclothing on the left side. In addition to not being able to control right lateral movement of the pelvis, she had rearward displacement of the right buttock, and risked falling posteriorly to the right. Physical examination and observation of motion revealed decreased tone of the right external abdominal oblique and the posterior fibers of the gluteus medius; the lower fibers of the gluteus maximus were considered the main problem. Physical therapy enabled the patient to undress for self-toileting by focusing on the impaired muscles.

We encountered 5 patients with cerebrovascular disease, who demonstrated shortening of the trunk muscle. We thought that the shortening was caused by primary low muscle tone. We investigated whether there was an effect on sitting and walking postures by stretching the shortening muscles. So we stretched these muscles at first. But we could not obtain good effect on either static sitting or walking postures. Then we selected one patient, and tried using weight shifting during sitting with sufficient muscle contraction. As a result, we could acquire improvement in both sitting and walking postures. From the above investigation, it was suggested that both stretching the shortened muscles and performing physical therapy based on normal movements were important therapeutic exercise for patients with cerebrovascular disease.

To clarify the function of the vastus medialis obliquus (VMO) and longus (VML), we investigated the electromyographic properties of superficial quadriceps femoris during fatiguing knee extension task.Ten healthy male and 10 female volunteers participated in this study. The subjects performed fatiguing knee extension task at maximal effort (10 times×10 set at 60 deg/sec). Surface electromyography (EMG) was recorded from the VMO, VML, vastus lateralis(VL) and rectus femoris(RF) during isometric knee extension (knee bent at 60 deg and 30% of the maximal voluntary contraction level) with a 90-sec interval between each set. Knee extension peak torque, integrated EMG (IEMG) and median power frequency (MDF) of four muscles during each set were compared using Dunnett's test.Knee extension peak torque decreased gradually and peak torque decreased significantly starting from set 9 in male subjects, while there was no significant decrease in female subjects. IEMG from VMO and VL increased linearly. IEMG increased significantly from set 7 in VMO and set 6 in VL for both male and female subjects. Then, IEMG from both VML and RF increased significantly starting from set 9 in VML and set 8 in RF in male subjects, while there was no significant change in female subjects.MDF of VMO, VL and RF also increased significantly at timepoints similar to those showing increases in IEMG in male subjects. However, MDF of VML increased significantly from set 4 in male subjects. On the other hand, MDF did not change significantly in female subjects.As a result of this study, it was suggested that VMO and VL were facilitated to maintain target knee extension torque during fatigue. Therefore, it was considered that VML and RF facilitated the maintenance of target torque under fatiguing conditions.

Cervical dystonia appeared in March 1998, and it seemed that no treatment was effective. Treatment by means of acupuncture started in March at Kansai College of Oriental Medicine. In the patient with cervical dystonia, EMG at the initial treatment showed only left splenius (SPL). Also shortening of the neck in the left frontal side was found, to treat these symptoms multiple epidermis needles was used. The retaining needles were GV20 for involuntary movement and SI3 for SPL. After 2 months from the start of treatment there was improvement of the neck posture in the sitting position, and smooth neck movement. This case also proves that EMG analysis and correct treatment by means of acupuncture improves the symptoms.

In this study, we measured the dynamics of the infrapatellar fat pad (IFP) associated with knee joint flexionin the postoperative period after total knee arthroplasty (TKA) using ultrasonography over the preoperative topostoperative period. We then investigated at what time point the dynamic decline of the IFP began to occur.The results suggest that the dynamics of the IFP had already decreased at one month postoperatively. Theresults also suggest that for postoperative TKA patients, intervention for the subpatellar fat body should beperformed within the first postoperative month, when the dynamic decline of the IFP begins to occur.

In this study, we investigated the muscle activity that causes spinal and pelvic movements and changes inspinal and pelvic alignment when performing forward reaching in the sitting position at 0.5 seconds, which issimilar to the speed of the flexion phase of standing up movements. We measured the muscle activities of thelongissimus, multifidus and ilicostalis muscles, and the angular changes of the spinal column and hip joint of20 healthy men performing anterior reach motion in the sitting position. The results show that the activity ofall muscles weakened at the time of 20% from the start of movement and tended to increase significantly after40%. In addition, the angle change of the spinal column and hip joint showed flexion of the lower thoracicspine from the time of 20% to 40%, and after 40%, the anterior reach distance increased significantly as theflexion angle of the hip joint increased. The results of this study show that there is a difference in the purposeof exercise between the phase up to 40% of the time from the movement start and the phase that follows.

Patients with shoulder joint dysfunction may perform compensatory motion using the glenohumeral joint,scapula, or trunk to accomplish the intended movement. It is necessary to consider which treatment strategyto pursue, improvement of normal movement or acquisition of compensatory function, depending on theintended goal. In this paper, compensatory movement caused by functional impairment of the glenohumeraljoint and scapula are used as examples. In addition, electromyographic data are presented and details of thekind of compensatory movement occurring are explained.

本研究は,運動観察時に提示される運動の筋収縮強度に対する主観的認識の違いが脊髄運動神経機能の興奮性に及ぼす影響を明らかにすることを目的とした。対象者は健常成人22 名とし,F 波は右母指球上の筋群より導出した。測定の流れは,安静時のF 波を1 分間測定し,4 分間の休息後,運動観察時のF 波を1 分間測定した。観察課題は,母指の内転・外転運動(無負荷映像)と2 種類のセラバンドで母指に負荷を与える(低負荷映像・高負荷映像)とした。また,対象者の認識に基づいて,主観的高負荷,主観的低負荷,主観的無負荷に分類し,各条件間の振幅F/M 比相対値を比較した。振幅F/M 比相対値は,主観的無負荷・低負荷条件と比較し主観的高負荷条件で増加した(p <0.05)。結果より,運動観察において対象者に強い筋収縮が必要な運動であると認識させることで脊髄運動神経機能の興奮性は増加することが示唆された。

Arm elevation movement is used in daily living. We have performed physical therapy for patients with arm elevation difficulty due to problems with the shoulder joint and trunk function. We think self-exercise is important for effective rehabilitation. Therefore, we examined the concepts behind self-exercise for patient with arm elevation difficulty and describe them in this report.

Physiotherapists often have opportunities to perform physical therapy for patients with hemiplegia due to cerebrovascular disorders (among patients with central nervous system diseases), during the acute, recovery and chronic phases of the condition. Physical therapy is performed for patients who have difficulties in maintaining a sitting posture or performing activities while sitting, or who have walking difficulties or difficulties in performing activities while standing, due to trunk or pelvic dysfunctions. The authors consider that physical therapy, focusing on the major causative dysfunction, should be performed for patients at any phase of the disease, and that this requires a proper knowledge of kinesiology and anatomy. In this special topic, we will discuss the main measures for reducing the trunk dysfunction of patients with hemiplegia due to cerebrovascular disorders.

Lateral shift using one hand while sitting on a seat is an important ability in sitting up, for locomotion, to reach the other side. We sometimes control muscle tone of the trunk to change lateral weight bearing on one hand in hemiplegia caused by cerebrovascular diseases in which movements are difficult to perform because of trunk collapse. In this study, we analyzed the activity of the trunk muscles (obliquus abdominis, obliquus externus abdominis, obliquus internus abdominis and lumbar back) with EMG in changing the weight bearing in lateral shift with one hand while sitting on a seat. The results show that the supporting hand is necessary in sitting on the seat. The supported obliquus abdominis muscles, obliquus externus abdominis muscles and lumbar back muscles acted to maintain and fix the trunk as the weight increased. The supported obliquus external abdominis muscles acted to stabilize the upper trunk and rib cage, and the unsupported obliquus abdominis muscles, obliquus internus abdominis muscles and lumbar back muscles elevated pelvis as the weight decreased. Therefore, it is suggested that in the change of weight bearing in lateral shift the supporting hand serves an important purpose in maintaining trunk stability.

〔目的〕立ち上がり動作の殿部離床相における体幹と骨盤に関係する運動を明らかにすることを本研究の目的とした.〔対象と方法〕健常男性15名に立ち上がりを実施させ,画像解析ソフトを用いて各角度の変化を測定した.〔結果〕殿部離床相初期および後期で骨盤前傾,膝関節伸展,下腿前傾が増加した.加えて,初期では体幹前傾,胸腰椎移行部屈曲が増加し,後期では体幹後傾,股関節伸展が増加した.運動パターンは,16パターンに分かれた.〔結語〕殿部離床相初期の体幹前傾には胸腰椎移行部屈曲の関与が大きく,後期の体幹後傾には股関節伸展の関与が大きいことが示唆された.さらに,骨盤前傾には足関節背屈位での膝関節伸展の関与が大きいことが示唆された.

In the physical therapy field, physicians encounter some patients who cannot maintain their posture in the standing position because of decreased activity of the lumbar spine. In such cases, forward weight shifting in the standing position may be performed. The purpose of this study was to examine postural changes and muscle activity during forward weight shifting while standing. We observed that during forward weight shifting, the activity of the multifidus muscle (L4/S1) increased from the initial stage, with the anteversion of the lower legs and pelvis. To maintain the posture of the lower lumbar spine, the muscle activity of multifidus (S1) must start prior to that of multifidus (L4).

Manual muscle testing is used to assess movement during arm elevation. Prior studies of movement during arm elevation have not assessed concurrent elbow joint motion. However, arm elevation in activities of daily living is performed not only with movement of the shoulder joint and shoulder blades, but also with elbow joint movement. Therefore, arm reach movement and arm elevation are different actions. This paper discusses research to date on arm reach movement and describes assessment and exercise therapy methods.
