<p>若年者に対するインプラント治療の適応や埋入方法は,個々の成長と発育に左右されるため,慎重な診断と成長予測に基づいた治療計画が必須となる.今回,16歳女性の上顎前歯部にインプラント治療を行い,20年以上の長期に渡って機能的・審美的に満足な結果が得られたので報告する.</p><p>本症例では,20年の間にインプラント上部構造と隣在する中切歯切端との間に約2.1mmの差が生じた.このような垂直的位置変化には,顎骨の成長,第三大臼歯の萌出,永久歯列の経年的変化など種々の要因が影響するものと思われた.確かに,成長が終了するのを待ってからインプラント治療を行う方が望ましいと思われるが,先天性欠損や外傷による少数歯欠損などの理由から若年者にインプラント治療を求められることもある.そのような場合は,各々の患者の成長曲線,骨年齢,第三大臼歯の萌出力などによる影響やセファログラムによる矯正的分析を考慮しなければならない.また,隣在歯の位置を確認した後,インプラント体を口蓋側寄りとし,深くなりすぎないよう慎重に埋入することも重要である.</p>

A questionnaire survey was conducted to determine the information required by patients prior to providing informed consent for dental implant surgery. Questionnaires were distributed to 1,585 patients who visited the Tokyo Medical and Dental University Hospital for oral implants between January 2012 and December 2012, of which 1,159 (73%) valid questionnaires were collected. The results showed that preservation of an adjacent tooth was the reason that 32% of participants chose implant therapy. Over 70% of participants indicated that their main concerns regarding implant therapy were aftereffects of surgery, the number of years the implants would be in place, and treatment cost. Regarding knowledge about implant therapy, patients' most frequent reply was“ I do not know” when asked about postoperative pain, peri-implantitis, the importance of implant maintenance, and the relationship between general health and implant therapy. Fifty-nine percent of the patients recognized smoking as a risk factor for periodontal disease, of which 23% recognized smoking as a risk factor for implant therapy. Sixty-one percent of patients replied“ more than 20 years” when asked about the number of years that they believed the implants would be in place. This research identified the information required by patients in order to provide informed consent. Comprehensive information is required before informed consent is sought, as the majority of patients did not possess sufficient knowledge regarding implant therapy.

長期症例のインプラントに対して,しばしばインプラントの残存率が一つの成功基準として使用される.しかし,患者の実態を知るにはそれだけでは十分といえない.この研究の目的はインプラント治療を受けた患者の長期経過の実態を知ることである.我々は,インプラント治療後20年以上経過した患者に対しアンケート調査を行った.患者は九州インプラント研究会に所属する歯科医師によって治療された.アンケートは1,168名に送付し509名からの回答を得た(回答率44%).回答者の内,78%がインプラントに何も問題ないと答えた.また,歯の経過については68%が何もないと回答した.食事については84%が何でもよく噛めると回答した.また93%がインプラント治療に満足していると回答した.

2010年にわが国の5学会の顎骨壊死検討委員会から出されたポジションペーパーが2016年に改訂され,骨吸収抑制薬関連顎骨壊死(ARONJ)の新しい診断基準が示された.ビスフォスフォネートやデノスマブのような骨吸収抑制薬は,骨転移を有する悪性腫瘍患者や骨粗鬆症に投与されており,ARONJは骨吸収抑制薬で治療を受ける患者におけるまれな合併症である.ポジションペーパーには,医師と歯科医師との間の相互連携と協力がARONJを管理するために重要,と提唱されている.まず,骨吸収抑制薬による治療開始までに,抜歯などの骨への侵襲を伴う歯科治療は終えておくことが望ましい.医師から歯科医師への依頼は少ないのが現状で,これを増やすことが急務である.そして,治療開始までに口腔管理が開始されたとしても,骨吸収抑制薬を投与中に歯性感染症を生じることはあり得る.抜歯はARONJ発症のリスク因子の1つとされるが,抜歯を避けることによる感染の持続もまたリスクであり,必要に応じて抜歯する.ARONJ予防のための骨吸収抑制薬の休薬の有効性は証明されていないため,医師と歯科医師との間で慎重に協議,検討する.

Purpose : This study was performed to investigate and compare the mechanical influence of attachment types on the implant and mucosal tissue under the posterior part of implant overdentures (IOD) with a combination of mechanical reaction analysis and structural analysis using the finite element method (FEM) .Methods : Implant overdentures with ball attachments (B-IOD), implant overdentures with locator attachments (L-IOD), and complete dentures (CD) were established in the experiment model. A 50 N load was applied onto the right first molar position of the overdenture, and the bending moment on each implant and the overdenture displacement were measured. Additionally, the FEM model was prepared based on DICOM data of the in vitro model. The interface between the denture base and mucosal tissue was assumed to be a contact interface, not a continuous interface. Displacement data measured by the experiment model analysis were inputted into the FEM model, and the stress distribution in the mucosal tissue under the posterior part of the denture was evaluated.Results : In the L-IOD, the bending moment in the loaded-side implant was greater than that in the nonloaded side. In the B-IOD, the stress on both the loaded and non-loaded sides was smaller and almost equal. Mucosal stress in the loaded posterior part using B-IOD was the highest among the B-IOD, L-OD, and CD. The mucosal stress in the L-IOD was lower than that in the B-IOD and CD on both the loaded and non-loaded sides.Conclusion : Stress in the mucosal tissue under the denture base was able to be evaluated using a combination of mechanical reaction analysis and FEM analysis. Differences in the mucosal stress distribution were detected among the CD, B-IOD, and L-IOD. The highest mucosal stress value was shown in the B-IOD.

In recent years, implants have been widely used in dental and oral fields. Many investigators have reported the indication of implants. Most of these cases progressed well, but the others did not. To investigate the patient's awareness of dental treatments for removal of dental implants, we mailed a questionnaire to 33 patients. 1. The rate of collected questionnaires was 81.8%. 2. 92.6% of the patients underwent treatments such as partial dentures after removal of the implants, but only 40% of the patients were satisfied with these treatments. 3. This investigation clarified that informed consent between the dentists and the patients was insufficient.

The purpose of this study was to examine the applicability of the newly-established method to analyze distribution of occlusal force among implant superstructure and natural teeth at different clench levels. Dental Prescale (Fuji Film Co. Tokyo) was used to record occlusal contacts and the force loaded on each of them. Four clench levels, which were 25, 50, 75, and 100%MVC, were regulated by visual biofeedback of masseter muscle EMG. This method was applied to five patients who received osseointegrated implants at the prosthodontic clinic of Kyushu University Dental Hospital attached to The Faculty of Dentistry and the applicability of the method was discussed. The results indicated that the Implant Support Ratio, which was defined in this study, can be used for the quantitative evaluation of the occlusal force distribution among implant superstructure and natural teeth at different clench levels. It was suggested that the distribution of occlusal force depends not only on the patterns of missing teeth but also the occlusal scheme and the conditions of the residual natural teeth.

In the severely atrophic maxilla, prosthetic rehabilitation is difficult. However, rehabilitation using osseointegrated implants with bone grafting can resolve this problem. Among the various methods of bone graft to the maxilla, sinus lift is commonly used. This report presents two cases of oral rehabilitation using osseointegrated implants with the sinus lift. In case 1, grafted bone was harvested from the mental region of the mandible, and implantation was performed at the same time as the autogenous corticocancellous bone block graft. In case 2, grafted bone was harvested from the alveolar bone in the anterior region of the mandible, and implantation was performed secondarily after the autogenous particulate bone graft. Bone biopsy for histological analysis was performed in the above two cases. In both cases, new trabecular bone was observed in the augmented sinus floor. However, the trabeculae were more dense on the alveolar side than in the central area. In the case with splintered bone, empty bone lacunae were seen in the grafted region.

In recent years, implants have been widely used in dental and oral fields. Many cases have been reported, and the indication of implants has been investigated. Many cases progressed well, but a few cases did not. The unfavorable cases of dental implant were reexamined by clinical observation. The results were as follows 1. The number of unfavorable cases of dental implant increased until 1992 but decreased after 1993. 2. The patients were 20 males and 29 females, and 83.7% of them were 40-69 years of age. 3. Pain was the most common chief complaint of the patients, followed by swelling. 4. Systemic diseases, including hypertension and diabetes mellitus, were noted in 63.3% of the patients. 5. Only 24.5% of the patients who visited our department were referred to us by the dentists who performed the implantations. 6. Right after implantation, 26.5% of the patients showed symptoms. 7. Treatment at our department was removal of most of the implants.

The bone augmentation method for dental implants has become a main means of controlling the prognosis of dental implants in clinical treatment. Platelet-rich plasma (PRP) has been accepted as a safe material which can be extracted from the blood of patients. Furthermore, PRP is used in many treatments to enhance bone formation. The aim of this study was to identify what kind of growth factor was affected by PRP on bone formation in an animal experiment. Six adult dogs were selected, and four implants were placed in the lower jaw of each dog; two implants were located on the right side and the other two on the left side, to obtain control and experimental sides respectively. After that, an artificial bony defect was made between two implants on both sides, and then the defect on the experimental side was filled up with PRP extracted from the same dog. Radiographic, histological and immunohistochemical methods were used for a comparative analysis. The results showed new bone formation due to PRP at the first week after the operation, and it was identified by both radiographic and histological methods. In addition, positive reactions of PDGF in immunohistochemical qualitative analysis and quantitative search were confirmed. From the data it was concluded that PDGF was the main growth factor for the bone formation when PRP was applied to the artificial bony defects of the studied dogs.

The recent advent of improved low-level laser irradiation (LLLI) therapy has promoted interest in clinical implantology. It has been reported that LLLI on bony implant sites might have positive effects on the integration of implants. The biostimulatory effect of cell proliferation and bone formation by LLLI has been investigated, but little is known about the molecular basis of biostimulatory mechanisms. Since LLLI will be useful to support implant therapy, it is important to elucidate the mechanism of the biostimulatory effect of LLLI on bone formation. We previously constructed the cDNA library of mouse osteoblastic cells (MC 3 T 3-E 1), which enhanced gene expression by LLLI using a subtracted gene cloning technology. In the present study, we further analyzed the DNA nucleotide sequence of gene clones, and focused on a gene clone designated MCL-174. The nucleotide sequence of MCL-174 insert was determined and assessed in the standard nucleotide-nucleotide BLAST (blastn) homology-search using NCBI DNA databases. DNA nucleotide sequences of clone MCL-174 inserted DNA exhibited 99% homology with Mus musculus annexin Ⅲ gene. Reverse-transcription PCR analysis showed that the mRNA level was enhanced by LLLI. These findings suggest that LLLI may enhance mRNA transcription and play a role in stimulating proliferation of osteoblasts through the enhancement of annexin Ⅲ gene expression. Annexin Ⅲ was detected in secretory ameloblasts and odontoblasts, and it was thought to be involved in the regulation of cell calcium. These findings suggest that the biostimulatory effect of LLLI on bone formation may relate through gene expression of annexin Ⅲ.

To clarify the effects of clinical factors on the failure (early failure) of osseointegration establishment of titanium implants, we statistically carried out a clinical examination on 373 fixtures of Brånemark implants. Fifteen fixtures (4.0%) failed to establish osseointegration before the fitting of superstructures. Results are summarized as follows: 1.Age and sex did not affect early failure. 2.Although there was no significant difference in the rate of early failure between the maxilla and the mandible, the prognosis of the maxilla, in women, was poorer than that of the mandible. No difference among regions in the maxilla or mandible was detected. 3.Variations of diameters and kinds of fixtures did not affect early failure. 4.In the length of fixture, the prognosis of 10 mm or less of the fixtures was poor, and the prognosis of fixtures of 13 mm or less was poorer in the maxilla. 5.The initial stability of fixtures at installation was deeply relevant to early failure. 6.The healing period between fixture installation and abutment connection did not affect early failure. 7.There was no significant difference in the rate of early failure between smokers and non smokers.

Evaluation of jawbone quality is very important for successful implant treatment; however, a method to evaluate bone quality of the jaw has not been established. Furthermore, at present, bone classifications are relatively rough and subjective methods for pre-operative assessment. This study compared the buccal and the lingual cortical bone mineral densities (BMD), and correlation trabecular BNID and cortical BMD by quantitative computed tomography (QCT). The subjects were 56 patients with mandibular distal extension defect (20 males, 17 pre-menopausal and 19 post-menopausal women;age range 30 to 79 years). The buccal and lingual cortical BMD and the trabecular BMD were measured by QCT. The difference and the correlation among the three groups were estimated. The results were as follows. 1. The buccal cortical BMD was significantly higher than the lingual cortical BMD in the male and pre-menopausal group. However, no significant difference was seen between the buccal and the lingual cortical BMD in the post-menopausal group. 2. The buccal cortical BMD correlated well with the trabecular BMD in the male group. 3. The buccal and the lingual cortical BMD did not correlate with the trabecular BMD in the pre-menopausal group. 4. The buccal cortical BMD correlated significantly with the trabecular BMD in the post-menopausal group. In conclusion, it is important to consider these tendencies of mandibular BMD in dental implant treatment.

Globular titanium with a diameter of 212~250 μm was prepared using the rotating electrode method. It was then sintered under vacuum conditions to produce solid titanium material, to make the implant. We investigated the titanium implant material by evaluating the bond strength of the titanium particles to the titanium implant, the void area between titanium particles, the polarization resistance of titanium particles to the globular titanium implant, and alternating-current (AC) impedance properties. The following findings were obtained. 1. A shear force of about 5N was shown between the globular titanium implant and the titanium particles, although measurement was difficult. 2. The void area between globular titanium particles with a diameter of 212~250 μm ranged from 1.84×10-3 mm2 to 2.56×10-3mm2. 3. The Ecorr range of the titanium and globular titanium implant in 0.9% NaCl and artificial saliva aolution was -201~ -347 mV, and Icorr was 0.04×10-5~1.3×10-5mA/cm2. For titanium particles, Ecorr was -346 mV, and Icorr was 1.04×10-5mA/cm2. 4. The alternating current impedance measurement demonstrated increased corrosion resistance of the titanium material and globular titanium implant, which formed a passive oxide film in 0.9% NaCl and artificial saliva.

We prepared two types of bone defect fillers (BDFs) which were made from a mixture of α-tricalcium phosphate (α-TCP) with chitosan or alginate in solution. A solidifier was added to the mixtures. These solutions were then kneaded to be solidified. The solid substances, named chitosan-BDF and alginate-BDF, were implanted in femurs of guinea pig to study the effects of the BDFs on the processes of bone formation by EPMA. The alginate-BDF was surrounded with growing bone from all directions at 6 months after the operation and seemed to have an affinity to the tissues. However, no degraded or absorbed form of the BDF was observed. In contrast, most of the chitosan-BDF was replaced by growing bone at 6 months. The adjacent part of the BDF to surrounding cortical bone was especially well-absorbed, probably due to being involved in a reabsorption process after ossification. According to the data, both BDFs were shown to have excellent mechanical properties for filling bone defects. Especially, the chitosan-BDF would be useful as bone defect filler because of its induction of ossification and its smooth absorption.

The purpose of this study was to determine whether clinical success can be achieved with immediate loading in the edentulous maxilla and mandible with Brånemark implants (Nobel Biocare, Goteborg, Sweden) at one year after placement of the implants. The study sample consisted of 18 patients who were edentulous or about to lose all remaining teeth. A total of 118 fixtures were placed in 7 maxillae and 12 mandibles, including one bimaxillary patient. Ninety seven of the 118 fixtures were immediately loaded and 21 fixtures were submerged. Five to 7 fixtures (mean of 5.9 fixtures) supported the prostheses in the maxilla and 4 or 5 fixtures (mean of 4.7 fixtures) in the mandible. All fixtures immediately loaded were placed in the incisor and premolar regions of both maxilla and mandible. The mean lengths of the fixtures in the maxillae and mandibles were 14.8±1.8 mm and,15.6±2.6 mm, respectively.All of 41 immediate loading fixtures in the maxillae showed a placement torque of more than 35 Ncm. Although 5 of 56 fixtures in the mandibles showed a placement torque of 30 Ncm, remaining fixtures showed more than 35 Ncm. Provisional implant-supported fixed prostheses were fabricated in a laboratory from an impression and were placed on the next day after surgery. After a 4-to 7-month healing period, a definitive prosthesis was fabricated and placed. One implant, although still osseointegrated, was removed owing to bone resorption. In the patient, the provisional prosthesis was supported by 4 remaining fixtures;thus the cumulative survival rates for fixtures and prostheses were 99.0%and 100%, respectively, after one year. This clinical report suggests that immediate loading of implant-supported fixed prostheses in the edentulous maxilla and mandible can be a safe and successful treatment as long as patients are carefully and strictly selected.

We performed dental implant treatment following autogenous bone graft from the iliac bone to the sinus floor for a 68-year-old male patient with severe atrophy of the posterior maxilla. After bone grafting, the sinus floor was lifted up 10 mm to 15 mm. On the 14th day after the operation, fracture of the iliac crest occurred accidentally. An avulsion fracture of the anterior superior iliac spin at the lower part of the donor site was diagnosed. We treated the fracture conservatively, and after four weeks, the patient recovered to be able to go up and down the stairs. Four months later, four implants were inserted in the bone grafted area. For patients with severe atrophy of the posterior maxilla, sinus elevation with iliac bone harvesting is considered to be an effective method. However, there are possible complications associated with iliac bone grafting, such as in this case. Therefore, careful management after such an operation is necessary.
