A 68-year-old woman was admitted to the hospital with lower abdominal pain. Abdominal CT revealed a long linear high-density foreign object measuring about 3 cm long, which had penetrated the wall of the sigmoid colon. Emergency endoscopy was performed, because the patient was diagnosed as having possible penetration of the sigmoid colon by a fish bone. The emergency endoscopic examination revealed a fish bone penetrating the sigmoid colon, which was removed with grasping forceps. The removed bone measured 3 cm in length and was believed to belong to a sea bream. The patient was treated conservatively without any complications. For intestinal perforation and penetration caused by a fish bone, such as in our case, endoscopic treatment may be considered as the treatment procedure of first choice.

Eosinophilic esophagitis (EoE) is rarely reported in Japan. We experienced two cases of EoE in one year. Case 1 : A 55-year-old woman with an allergic rhinitis complained of heart burn. Endoscopy revealed that esophageal mucosa was so thick as to be unable to observe the vessels, and white exudates and longitudinal furrows were found in the esophagus. Its caliber was slightly narrow. Case 2 : A 40-year-old man complained of nausea with no history of allergic disease. About one-third of the circumference in the middle esophagus, mucosal thickening and longitudinal furrows were observed during endoscopy, where the vessels were obscured. In both cases, histological examination of biopsy specimens revealed marked eosinophil infiltration in the esophageal mucosa, more than 20 per high-power field (HPF) . These patients were diagnosed with EoE based on the diagnostic guideline, but their histories and endoscopic findings were considerably different. It will be necessary to elucidate what brought about the differences.

胆汁酸は食道炎の原因の1つとされている。筆者は,胃液カラーカードによる胆汁の簡便測定法を開発して,逆流性食道炎と胃内胆汁酸との関係を検討してきた。 当クリニックを受診し,内視鏡検査を施行した727例を対象として検討し,胃内胆汁色(-)では55.9%,胃内胆汁色高度群では63.8%のgrade Mの食道炎が認められ,後者に食道炎が有意(P<0.05)に高かった。また,逆流性食道炎(-)で胃内胆汁色(-)群(A群)23例と逆流性食道炎(+)で胃内胆汁色(+)群(B群)15例を対比し,胆汁酸はA群B群の全例で検査されたがA群の11.5±4.3μg/mlよりB群は217.0±249.0μg/mlと有意に高かった。トリプシン活性やpHには差が認められなかった。また,コール酸(CA)およびケノデオキシコール酸(CDCA)等の一次胆汁酸はA群55.3%に対しB群78.5%とB群に多かった。逆流性食道炎の発生に胆汁酸が関与していると考えられた。

We report a relatively rare case of malignant lymphoma in a patient with ulcerative colitis. A 31-year-old woman visited our hospital for right cervical lymphadenopathy associated with common cold-like symptoms. She had been diagnosed as having ulcerative colitis at the age of 19 years and received treatment with salazosulfapyridine and prednisolone intermittently. CT examination revealed enlarged lymph nodes in the cervical, supraclavicular, and mediastinal lymph node regions. Histopathological examination yielded a diagnosis of non-Hodgkin lymphoma (peripheral T-cell lymphoma) , Stage IIB. She was treated with six cycles of combination chemotherapy with the CHOP regimen (cyclophosphamide, doxorubicin, vincristine, and prednisolone) , followed subsequently by autologous peripheral blood stem cell transplantation. No evidence of relapse of ulcerative colitis was observed during the period of chemotherapy and autologous peripheral blood stem cell transplantation.

内視鏡的大腸ポリープ切除術について,後出血の危険因子および後出血予防処置としてのクリッピングの有効性について検討を行った。2004年から2007年の4年間に,当院にて施行した1049病変について解析を行った。出血例は26病変に認められ,出血率は2.5%であった。病変部位,病変の大きさ,病変形態,切除方法,組織型,クリップ施行の有無について検討を行った。後出血の要因として,病変部位としては左側結腸,大きさは15mm以上,病変形態は広基性または10mm以上の有茎性,切除方法は分割切除,組織型は腺癌であった。クリッピングの有効性については,施行例・非施行例での背景が異なっており,有効性を示すためには施行規準を設けた前向き研究が必要と考えられた。

A 34-years-old woman, who had received Kasai operation due to congenital bile duct atresia at the age of 0, was pointed out esophageal varices and liver cirrhosis at one year and a half before pregnancy. At 25 weeks pregnancy, the esophageal varices was ruptured and she received endoscopic variceal ligation after emergent admission. Although the variceal bleeding was well controlled, the fetus grew up slowly and its growth arrest was recognized by ultrasound examination at 29 weeks pregnancy. Caesarean section was performed and a boy of 842g was delivered. Genital bleeding, however, continued postoperatively because of poor uterine contraction, and a large amount of blood transfusion was necessary. Both of uterine arterial embolization was performed by interventional radiology and she was discharged on the postoperative 15th day. The newborn grew up and was also discharged on the 92nd day of his birth.

A 42-year-old woman was consulted to our hospital due to abdominal pain, diarrhea, and hematochezia. Laboratory tests showed leukocytosis, elevated CRP level, and Escheria coli (serum type O-166) was detected. First we diagnosed infectious colitis, so prescribed antibiotics. But her symptoms still continued. Colonoscopy and barium enema study revealed the total colitis type of ulcerative colitis, and it was comfirmed pathologically. We continued medical therapy with total parenteral nutrition, salazosulfapyridine, and prednisolone. After these treatment the patient condition improved. Follow up study by colonoscopy showed that the erosion, and pseudopolyposis remained, but inflammatory findings were improved markedly. The special type of Escheria coli caused infectious enterocolitis including diarrhea and gastroenteritis. It is concluded that emergent colonoscopy and stool culture are important for the diagnosis and treatment of ulceraive colitis.

A 37-year-old male visited a nearby medical clinic with a history of recurrent vomiting after taking a commercial calcium supplement (calcium tablets) and drinking water. Since fiberoptic laryngoscopy revealed no abnormalities, the patient was referred to our hospital for further examination. Emergency endoscopy revealed a supplement-like white substance incarcerated in the upper esophagus. We crushed it with forceps, followed by washing. A proton pump inhibitor (PPI) and alginic acid were prescribed. At the second endoscopy performed on June, 2013, transnasal endoscopy was needed and insertion of the endoscope was found to be difficult due to ulceration and stenosis of the upper esophagus. When we performed the third endoscopy on June, there was no improvement in the stenosis and the ulceration was cicatrized, and minor bleeding was caused by the transnasal endoscope insertion. The patient was prescribed oral PPI therapy for the following month. The subjective symptoms disappeared and food intake became possible. Recently, a large number of subjects have begun to take a variety of supplements available in the market, and various types of complications related to the use of these supplements have been reported. We report this rare case of esophageal ulceration/cicatricial stenosis caused by oral administration of a commercial calcium supplement, with a discussion of the relevant literature.
