A reflectionless tube described by Sondhi is utilized to obtain the glottal waveform.The rationale of this method is that the effect of vocal-tract resonances on glottal waveform is considerably reduced by phonating into a reflectionless uniform tube.Measurement of the transmission characteristic (frequency response) of the tube proved minimal attenuation caused by the tube.The obtained glottal waveform in four normal subjects and in nine patients with laryngeal pathologies are reported herein and methods presently available for estimating the glottal waveform are also discussed. The glottal waveform with reflectionless tube proved to be simpler and more practical for clinical use.

Though many different pathologies cause vomiting, they can be divided into three categories. The first is vestibular vomiting such as that caused by motion sickness. The second is vomiting mediated by chemoreceptor trigger zone (CTZ) in the area postrema, such as that seen in poison-induced vomiting. The third is vomiting from the gastro-intestinal tract. Vestibular stimuli, signals from the CTZ, and afferent signals from the gastro-intestinal tract stimulate the vomiting center in the medulla to cause vomiting independently.Histamine H1-blockers (diphenhydramine, promethazine) prevent vestibular vomiting selectively. Dopamine D2-blockers (metoclopramide, domperidone) act on the CTZ to inhibit CTZ-mediated vomiting. Serotonin 5-HT3-blockers block afferent signals from the stomach induced by cisplatin. Diphenidol may inhibit the vomiting center because diphenidol prevents both motion and cisplatin-induced vomiting.

その主な周波数領域が1/3オクターブバンド400Hzから4kHzの胎内音を152名の新生児に聴かせ, その鎮静効果を観察した. そのうちの69名にABRを実施し胎内音の反応と比較した.その結果, 胎内音の鎮静効果とABRの反応とはよく一致した. 胎内音に反応の認められない高度難聴児2名が発見された.これらのことから, 本法は, 新生児聴覚スクリーニング検査として有用であるという確信を深めた.

A 70-year-old male complaining of dizziness and hearing disturbances had sudden onset of meningitis and DIC. Target CT scans revealed latent petrositis with no clinical symptoms for six years. CT scans and MRI were found to be very useful in the early diagnosis of petrositis and the prediction of intracranial complications. This case suggested that asymptomatic petrositis with a high risk of intracranial complications is not rare in the recent antibiotic era. Twenty-three cases of petrositis reported in Japan from 1985 to 1995 were reviewed, and diagnostic and therapeutic problems were discussed.

Intranasal steroids are widely used to treat olfactory disorder, and are known to have fewer side effects than systemic steroids. However, there are some reports that intranasal steroids have induced adrenal insufficiency, even when the steroid dosage is appropriate. We encountered a case of secondary adrenal insufficiency after using intranasal betamethasone for olfactory disorder within the usual dosage. In this case, we were not aware of instructing the patient not to swallow the steroid which had dripped into her pharynx. Thus, a large amount of steroids which had passed through her nasal cavity might have been absorbed in the oropharyngeal mucosa and also ingested. That is possibly the main cause of developing adrenal insufficiency. Recently, Toki-shakuyaku-san, a traditional Chinese medicine, has started to be used for olfactory disorder after a cold. Because of its minimum side effects, it may be safely used for patients with the risk associated with steroid use.

Small cell carcinoma is relatively common in the lung, but very rare in the head and neck region. We describe our experience with two extremely rare cases of small cell carcinoma arising in the nasal cavity and paranasal sinus.(Case 1) The patient was a 54-year-old man who had complained of left eyelid swelling and diplopia. He had a mass in the left maxillary sinus. He was treated with irradiation and chemotherapy. However, during the treatment, distant metastasis appeared in the skin and he died 4 months after the initial treatment.(Case 2) The patient was a 47-year-old woman who had complained of left nasal obstruction. She had a mass in the left nasal cavity. Surgical resection and postoperative radiotherapy were performed. However, after the initial treatment, a metastatic swelling of the left cervical lymph node appeared. Left neck dissection was performed, and she underwent two courses of adjuvant chemotherapy. Following the initial treatment, no evidence of recurrence has been observed for 29 months.

Vogt-Koyanagi-Harada disease has been described as an autoimmune disease which targets melanocytes and causes visual disturbance. In addition, it causes auditory and equilibrium disorders occasionally. In this study, we report a case of Harada's disease with sensorineural hearing loss.A 51-year-old male visited Aichi Medical University complaining of visual and hearing disturbances. An audiogram demonstrated moderate sensorineural hearing loss bilaterally. Visual and auditory acuity improved after administration of steroids. The hearing disturbance in this case may have been caused by an autoimmune reaction to melanocytes in the inner ear, similar to the previously reported phenomenon in the eye.

In performing tonsillectomies, we observe the following precautions: (1) the operation is performed under general anesthesia with fluothane with nitrous oxide and oxygen. (2) local epinephrine injection for the control of postoperative hemorrhage is avoided as there is the risk of evoking postoperative bleeding from reactive vasodilatation after the peripheral vasoconstrictive effect of the drug has worn off. (3) hemostasis is accomplished by ligation of vessels encountered during extirpation.During the past nine years, a total of 284 tonsillectomies among patients ranging from three to thirty-eight years were performed, adhering to these three principles. Gratifying results were obtained in 282 cases. Postoperative hemorrhage occurred in 2 patients. Examination of the surgical wound by direct laryngoscopy revealed the cause of bleeding to be loosening of a ligature. Another complication was dislocation of a central incisor in one case.

It is not easy to treat patients with intractable Meniere's disease. Intratympanic injection of gentamicin or steroid is one option. The other is a surgical procedure like endolymphatic shunt surgery. Middle ear pressure treatment using the Meniett device, which has not been approved by the Japanese government is widely accepted in foreign countries. We had a chance to use this Meniett device under approval of the ethic's committee of Hino Municipal Hospital since September 2004. ‹Methods› Four patients with intractable Meniere's disease who suffered intensive vertigo attack for more than 5 months with conservative medical treatment were employed in the study. Ages ranged from 67 to 72, with 3 females and 1 male. After a ventilation tube was inserted under local anesthesia, treatment with the Meniett was performed 3 times a day at home. ‹Results› Two patients improved and 2 slightly improved concerning vertigo attacks. One showed no change, 1 experienced a worsening, and 2 showed slight improvement concerning the hearing level. ‹Conclusion› The Meniett device is less invasive and may be suitable for elderly patients who prefer conservative treatment. The mechanism involved in the reduction of vertigo attacks is not clear so far, but we speculate that middle ear pressure may suppress serum vasopressin (antidiuretic hormone) secretion and thereby reduce endolymphatic hydrops. It is necessary to accumulate treatment data with the Meniett in Japan to obtain official approval by the government.

Children with unilateral total deafness, indistinct in both etiology and time of onset, usually complain of only unilateral hearing loss. Nandate (1963) suggested terming this type of deafness “juvenile unilateral total deafness of unknown etiology”.On the other hand, our clinical experience has indicated that recurrent true vertigo develops preferentially in adolescence or early adulthood in such patients. The objective of this report was to introduce this new clinical aspect of this deafness.1. During the past 14 years, 89 patients with juvenile unilateral total deafness of unknown etiology were seen at the Gunma University Hospital. All patients conformed to the following criteria: i) Unilateral total deafness of unclear etiology and in which the onset of deafness could not be established to be other than very early in infancy; ii) Normal hearing in the other ear or, if deafness were present there also, normal hearing might be inferred to have existed in infancy; iii) Normal tympanic membranes; iv) Absence of central nervous symptoms. Of these patients, recurrent vertigo was the chief complaint of 27, or 30.3%. The age of onset of vertigo ranged from 9 to 60 years. However, in the majority of patients (19 cases: 70.4%), the onset was from 16 to 30 years. The vestibular symptoms were identical to vestibular symptoms seen in Ménière's disease.2. Among these 27 patients with recurrent vertigo of delayed onset, fluctuating sensory-neural auditory disturbances were evident in the originally non-deaf side in 3 (11.1%). These patients were diagnosed as having Ménière's disease.3. In contrast, vertigo attacks in the other 24 patients (88.9%) were never accompanied by cochlear symptoms. Such were considered to fit the delayed hydrops syndrome, a clinical entity newly postulated by Schuknecht (1976) and which is considered to occur as an aftermath of a labyrinthine injury of sufficient magnitude to destroy hearing but leaves the vestibular function intact.

In order to clarify the role of the amygdaloid nucleus in equilibrium function, the following experiments were carried out: nine electric stimuli, 0.1 msec, 200Hz pulse waves at 3 volts during a 30-second period were applied daily to a rabbit's amygdaloid nucleus (medial or lateral area).Optokinetic nystagmus and the EEG in response to optic stimulation were measured before and after the nine electric stimuli.In addition, the blood pressure was measured in rabbits with repeated electric stimulation of the medial or lateral area of the same amygdaloid nucleus. Stimulation was with 0.1 msec, 200Hz pulse waves at 30 volts for about 23 seconds.(1) Rabbits with repeated electric stimulation of the medial area of the amygdaloid nucleus tended to show increased optokinetic nystagmus and increased EEG activity in response to optic stimulation. Furthermore, their blood pressure increased when the medial area was stimulated.(2) Rabbits with repeated electric stimulation of the lateral area of the amygdaloid nucleus tended to show reduced optokinetic nystagmus and EEG activity and a decrease in blood pressure.The following conclusions were drawn:(1) The amygdaloid nucleus correlates with the brain stem, particularly the ascending activating system, in the maintenance of equilibrium.(2) The medial area of the amygdaloid nucleus actively participates in the maintenance of equilibrium function, promoting activity of the brain stem. The lateral area of the amygdaloid nucleus has an opposite function and seems to inhibit overexcitement of the brain stem, resulting in smooth maintenance of body equilibrium.

There have been many reports of endolymphatic hydrops (EH) in patients with Meniere’s disease (MD) since the first report by Hallpike and Yamakawa in 1938. Mental/physical stress and subsequent increase in release of the stress hormone arginine vasopressin (AVP) supposedly triggers MD. Recently, many lines of evidence have suggested the possibility that AVP is closely linked to the formation of EH in cases of MD. In the present study, we attempted to investigate the relationship between stress and EH in patients with unilateral Meniere’s disease (uMD).We enrolled 113 definite uMD patients from July 2014 to October 2019. All patients underwent 3-T magnetic resonance imaging (MRI) 4 hours after intravenous gadolinium injection. We adopted the criteria for evaluation of EH proposed by Nakashima et al. Stress was evaluated using the depressive self-rating scale (SDS), the psychological stress response scale (SRS), and the dizziness handicap inventory (DHI) modified by Nishiike et al. These stress scores and the blood AVP levels were compared in patients with EH.There was no significant correlation between EH and the stress scores on the affected side, but the anxiety score showed a significant correlation with EH on the sound side (p = 0.04). There was no significant correlation between the EH and AVP in either the affected side or the sound side. We suppose that the formation of EH involves a complex process of stresses.
