We report the clinical study of 9 Judo players who had painful finger joints and complaints of difficulty in performing. There were 18 joints in all 16 of which (88.9%) had a history of trauma (ex. sprain and collateral ligament injury), but no history of fractures or dislocations. Instability of the joints was not found on clinical examination. Almost all players complained of a limitation in ROM and pain when they gripped a rival's sleeve with finger full flexion. X-ray examination of the finger joints revealed osteoarthritic changes in 15 joints (83.3%) and fragments in 7 joints (38.9%). No statistical differences were found in the techniques and experience between the 9 players and the other 11 players who had no finger complaints. The 2 joints with fragments had no history of trauma, therefore suggesting that they developed due to the distinctive overuse reaction in Judo.

We allowed early weight-bearing and ankle movement after operation for ankle fractures of supination-external rotation injury type. As this method gave better results in a follow-up, we think that this method can be applied to another fracture types too.

Thirteen cases of EDC tendon dislocation at the MP joint were treated and studied clinically. Nine after were treated conservatively by splinting, while 4 cases were treated operatively using Wheeidon's method.Six cases of subluxation of EDC healed rapidly after 2-3 weeks, but in 3 dislocation cases it was difficult to achieve optimal healing and the patients complained of disturbances in activity of daily living during treatment due to the splinting. On the other hand, 4 cases treated by surgery all healed.I believe that operative treatment is the first choice for dislocation of EDC tendon at the MP joint.

Sound transmission of the femur was recorded by holding a microphone on the pubic symphysis and tapping the patella with a autohammer.The sound transmissions in fractures and pseudarthroses of the femur were low in pitch and long in duration, and they returned to the normal one, delaying a few week after X-ray findings of facture healing.

In thirty-six cases with lumlar diseases, CT studies of the shape of the lateral recess (L5) were performed. A series of CT scans at L5 lateral recess showed the narrowest level of it and the L5 neve root condition in it. Three types of the shape were classified, which were triangular type, trefoil type and flat trefoil type. Especially the flat trefoil type was associated with LCS. We concluded that the narrowest level of the lateral recess of LCS was the entrance of the nerve root, which was trapped there by the configuration of the recess with the degenerative soft tissue, on the other hand, the lateral recess of OA was not narrowest at the entrance level, but narrowest at the pedicle level. The difference in the shape contributed to the pathogenesis of symptoms of the LCS.

A 55-year-old man fell down while inebriated and sustained a right clavicular fracture. He complained of paresthesia, paleness, coldness and parlalysis of his right arm. An expanding subcutaneous hematoma in the supraclavicular region was noted as well as radial artery pulsation at the right wrist.A rupture of the Subclavian artery was diagnosed without an angiogram and the middle part of the clavicle arteriogrhaphy was performed. Pulse deficit distal to the injury is one sign of Subclavian artery injury. However this is not altogether reliable because of good collateral circulation about the shoulder. There is no clear consensus as to which approach to the subclavian artery would give the best exposure and in particular control the proximal segment. For succesful management, proper surgical exposure and revascularization techniques are essential.

The relationship between the abnormalities of the posterior elements and anterior elements in lumbosacral transitional vertebrae, and then the relationship between disc degeneration and lumbosacral transitional vertebrae were investigated. The modified Jinnaka's classification for the abnormalities of posterior elements and the modified igh's classification for the abnormalities of anterior elements were employed for this purpose. Of 597 patients with low back pain, 82 patients (14%) presented the posterior abnormalities and 90 patients (15%) presented the anterior abnormalities. Ninety% of type I of posterior abnormalities showed normal disc in anterior, whereas only 4% of type II-IV showed normal disc. Ninety-four% of type III of posterior abnormalities had type I and type II (and III) of anterior abnormalities which were thought typical fixed transitional vertebrae. It presents a greater than normal incidence of the disc degeneration, especially posterior slip at the level of just above the lumbo-sacral transitional vertebrae.

A 37 years old female ruptured the plantar fascia of the foot without associated injury to the achilles tendon while she was under a low posture with the knee flexed, ankle plantar flexed and MP joint dorsiflexed during badminton play.It can be considered that the rupture was brought out by the Windlass Action of the plantar fascia due to extreme dorsiflexion of the MP joint. A surgical primary suture was performed with an excellent result.

We experienced a case of dysphasia and dysphonia due to Ankylosing spinal hyperostosis (ASH) of the cervical spine.The patient was a 78-year-old man who was admitted to the department of internal medicine in our hospital for the examination and treatment of dysphasia and Mendelson syndrome. Dysphonia appeared later. X-rays of the cervical spine showed ossification of the anterior longitudinal ligament (OALL) at the C2 to C7 regions. The patient received surgical treatment.After the removal of OALL, dysphasia and dysphonia improved. The causes of these ASH symptom are examined in this study.

Many authors have reported that disc herniation can be resorbed by monocytic phagocytosis. However, how the recruitment of monocytes is triggered is still unknown.We investigated monocyte chemoattractant protein-1 (MCP-1) by using the experimental rat model in vivo and rabbit model in vitro to ascertain the precise mechanism of macrophage recruitment in the early phase of disc resorption.The autologous intervertebral discs from tails of Wister rat were subcutaneously implanted into the abdomen. These discs were used for immunohistochemical study. Additionally, we employed tissue culture models of rabbit's discs, and the quantification of MCP-1 in these media was performed.MCP-1 positive disc chondrocytes were observed in the nucleus pulposus and the inner annulus fibrosis on the third day. Monocyte derived macrophages were considerably observed around the intervertebral disc by the seventh day. The MCP-1 induced by rrlL-1β or rrTNFα indicated its peak volume at 12 hours and at 18 hours after incubation.Our study suggests that intervertebral disc chondrocytes have chemotactic properties and play an active role in the recruitment of monocytes involved in disc resorption. Moreover, inflammatory cytokines induce and regulate the production of MCP-1 in the intervertebral disc cells.

To study the several changes of the disc just above the lumbo-sacral transitional vertebrae, the features of slipping at this level were examined in 216 patients who had lumbo-sacral transitional vertebrae in both anterior and posterior sides.Type II and type III of modified Custivelli's classification for the posterior element and the modified Wigh's Classification for anterior element were employed for this purpose. The results were compared with 597 patients with low back pain who visited our clinic.Isthmus spondylolisthesis (8 cases, 3.7%) was almost same as the frequency in our low back pain patients. The slight increase was shown in degenerative spondylolisthesis (30 cases, 14%). The greater incidence was proved in posterior slip (115 cases, 52.3%) in this series compared to the our low back pain group (19%). The almost same incidence was shown in young group and in old group. The slight increase was found in type I and type II in the anterior classication and in type IIb and type IIIb in the posterior classification. In conclusion, posterior slip at the spine just above the lumbo-sacral transitional vertebrae was one of the special features.

The purpose of this paper is to report morphological features of the cells in the human posterior longitudinal ligament, including its attachment to the vertebral bodies, and of the cultured cells derived from PLL. We used the transmission electron microscopy to observe the cells. Far from its vertebral attachment, the main constituent cells in PLL are fibroblasts. The closer to the vertebral bodies fibroblasts become degenerative, and chondrocytes are observed in the irregular collagen fibers. The closer still more, calcium deposits on the collagen fibers, and osteocytes become the dominant cells. Compared with these three types of cells fibroblast, chondrocyte, and osteocyte in PLL, the cultured cells resemble fibroblasts.

We sent questionnaires to 1, 416 registered hospitals of The Japanese Orthopaedic Association and made inquiries about the use of bupivacaine for spinal anesthesia. We received answers from 870 hospitals (reply rate of 61.4%). In 85.7% of these hospitals, orthopaedic surgeons anesthetize their patients, performing surgical operations of their hips or lower extremities. In 41.4% of these hospitals, they use Marcain® for spinal anesthesia. During the 6 month period between January through June of 1998, 39, 690 patients were anesthetized by orthopaedic surgeons in those 870 hospitals. 27, 287 cases were for spinal anesthesia and Marcain spinal were used in 7, 321 cases. The most common side-effects were hypotension, but its degree was mild and controllable. No neurologic side-effects were reported. 90.5% of the surgeons who had experienced the Marcain spinal anesthesia reply that they need preservative-free bupivacaine for spinal anesthesia strongly.

In 128 patients diagnosed cervical spondylosis except myelopathy tyep during 1971.All patients were given the treatment of cervical traction. 61 patients were investigated, male 24 patients, female 37 patients.The results were obtained as followed.1) 18 patients were excellent results, 22 patients were good.2) In 17 patients out of excellent and good results, we found that the symptoms relieved in two weeks after cervical traction.3) In excellent patients, there were effective for shoulder pain, neck pain and sensory disterbance of fingers.4) On the other hand, 21 patients were noted for no effect of cervical traction, and out of these 21 patients, 12 patients were no change on the symptoms, in 2 patients the symptoms had progressive worse.5) It was noted that there was no relationships on the effection and cervical X-ray findings.6) Out of excellent and good results, 93% of the cases were improved during three months by the treatment of cervical traction.

We observed a nine-year-old boy who suffered from a so-called Brodie's abscess involving the adjacent metaphysis and epiphysis of the distal tibia, communicating through the growth cartilage of the epiphyseal plate. The patient presented himself with the mild symptoms and subtle clinical findings that are characteristic of Brodie's abscess, which is usually confined to the metaphyseal, and occasionally to the epiphyseal, bone. Initially, he was treated by surgical evacuation of the abscess and with intravenous antibiotics, and then by oral andtibiotics. The pathogenic organism was Staphylococcus aureus. At follow-up 10 months after treatment, he had normal results without any evidence of a growth disturbance. As far as we know, there have been 27 previously reported cases of Brodie's abscess traversing the epiphyseal plate. We believe that surgical evacuation of the lesion, followed by antibiotics, is an appropriate treatment for some children presenting themselves with this condition.

We examined 26 cases with suspected tumor mass with ultrasonography between 1998 to 2000. Seventeen cases were diagnosed. Diagnosis was 8 ganglion, 3 lipoma, 3 atheroma, 3 Baker's cysts, and 1 malignant fibrous histiocytoma. Ultrasonography is readily available, inexpensive, and noninvasive. Generally ultrasonography assists in determining the size and consistency of a soft tissue mass. It is apparently useful for the diagnosis of the lipoma, atheroma, and ganglion.

We experienced a case of a broken intramedually nail in the tibia g months after operation for a tibial shaft fracture using the intremedually nail. The proximal portion of the nail was removed easily by the usual method, while the distal portion of the nail was removed using an extra extraction device. This extraction device is useful for the removal of broken intramedually nails, and is also less invasive than some of the methods reported before.Considering that there is no character of metal fatigue from the analysis of the end of broken nail using SEM, the reason for the broken nail lies in the nail strength.

The results of marginal excision and irradiation (3, 000-5, 000cGy) in 9 patients with liposarcoma of soft tissue were analyzed to determine the effectiveness of radiation therapy. Only one out of 9 patients, who received 3000cGy, developed a local recurrence at 3 years and 3 months after surgery. The 5-year actuarial local control rate was 84.7%, which was similar to that by radical and/or curative wide resection in soft tissue sarcomas. In addition, histological examination disclosed that a liposarcoma with preoperative radiation fell into almost necrosis.It suggests that the combination of conservative surgery and radiation therapy in the treatment of liposarcomas is of use to maintain a functional limb.

We have performed 22 lower extremity amputations with immediate post-surgical prosthetic fitting since November 1966.Among the causes of these amputations, 9 were of malignant bone tumors, 8 of gangrens due to peripheral vascular disease and 5 of other causes such as traumas and congenital malformation.In general, very early ambulation after surgery, early completion of final prosthetic fitting, and marked shortning of hospitalization have resulted from this technique. The authors however want to emphasize that very precise cautions will be needed in order to be successful in primary wound healing in the case of the amputation due to peripheral vascular disease.

Sternal fractures without external injury are exceedingly rare.A 79-yr-old woman has been troubled with ‘asthma’ which she appeared to be wheezing and having a cough since her mid fifties.A bulging of the center part to the sternum was noted without external injury on arising the morning on January 18, 1989.An X-ray film showed a sternal fracture and blood chemistry results did not indicate a malignant tumor.We suggested that weakness of the sternum by osteoporosis, the transmission of forces to the sternum by compression vertebral fracture and chronic pressure to the thorax with ‘asthma’ were important factors in the onset of a non-traumatic sternal fracture.
