A mechanics-of-materials deflection analysis of wooden baseball bats at impact is developed. Consider the bat which is simply supported a point of grip and elsewhere free to rotate, and the bat is subjected to the impact force with ball and the inertia force due to the sudden change of the angular velocity in swing. The static force balance gives the relation between the impact force and the inertia force, but the magnitude is left undetermined. By assuming that the impact duration is 1 ms, we get the solutions of the energy conservation equation as a function of hitting point. The present model provides the evidence for the COP (center of percussion) to be located very close to but not exactly in the sweet spot where the kinetic energy of bats can be most effectively transferred to the outgoing velocity of batted ball. Additionally, the deflection analysis determines the bending stress imposed at given velocities of bat and ball. It is shown that the largest bending stress is always located at the top of grip length irrespectively to hitting point. One hundred fifty broken bats in Hokkaido University play fields were collected and fracture surfaces were examined.

Dakekanba is the most popular broad leaf tree in Hokkaido. This study proposed the wood for manufacturing baseball bat. In order to compare the dakekanba wood with the others manufactured historically; sugar maple, aodamo, yellow birch, and white ash, full baseball bats with the same dimension were prepared from these woods. The performance of bat was studied by the impact test with fast pitched ball. The flexural vibration was measured by means of strain gauge. Results of the modal analysis showed that the first natural frequency was around 35 Hz and the second, 233 Hz, which were almost equal among these woods. The amplitudes of these modes increased with increasing the speed of pitched ball. As a result, the maple bat was the hardest in deflection and slowest in damping, and aodamo was the softest in deflection and the fastest in damping. It was concluded that these properties of dakekanba bat were close to those of yellow birch and white ash, which have served as intermediate between maple and aodamo.

症例は47歳,男性。3型胃癌の診断で紹介されたが,上部消化管内視鏡検査で幽門前庭部から胃体部にかけて易出血性の不整な多発潰瘍を認め,胃梅毒を疑った。生検組織でTreponema pallidumを確認し,Benzylpenicillin benzathineによる駆梅療法を施行した。症状は速やかに改善し,潰瘍も瘢痕化,治癒した。 胃梅毒の内視鏡所見は特徴的で,本症の存在を念頭において内視鏡検査を行うことが重要と考えられる。

【背景と目的】医学部医学科1 年生に対し、入学後のモチベーションの低下を防ぎ、医師のプロフェッショナリズムを意識させながら、能動的な学習姿勢を涵養する教育方法は、未だ確立されたものがない。本学では1 年次に「臨床医学入門」の授業を行なって、この問題への対応に努めているが、その一環として行われたワークショップ授業と教育効果について報告する。【対象と方法】1 年生を対象とし、平成21年度より開講している科目「臨床医学入門」の一環として6 月にワークショップを実施した。学生を小グループに分け、まず附属病院内の七夕飾りとして、患者・家族が願い事を書いた短冊を見せた。その後「患者さんの願いと医師が果たすべき役割」をテーマとして、KJ 法を用いてプロダクトを作成し、全員の前で発表した。更に自由記載形式のアンケートで、ワークショップの感想を記載してもらった。【結果】プロダクトにまとめられた学生たちの意見は、患者・家族の願いは想像以上に多様であり、また医師や医療に対する期待は大きく、切実な思いを痛感したとするものが多かった。アンケートでは、患者・家族の期待に応えるための努力の必要性、高い目的を持ち真剣に学ぼうとしている仲間への尊敬、グループワーク自体が将来のチーム医療の練習であるなど、医師を目指す上での認識を新たにしているものが多かった。【結論】1 年生に患者・家族の医療に対する思いを情報として伝え、かつ同級生同士討論することは、医師の社会的役割を改めて認識させると同時に、学習に対する有力な動機付けとなりうる

「精神保健福祉資料」(以下、630調査と表記する)によると精神科病院における身体拘束実施者数は2018年度630調査において全国で11,362人に上り、630調査で身体拘束実施者数の調査を開始した2003年度と比べ約2.2倍に増えている。身体拘束実施の状況および身体拘束実施に関連する要因を明らかにするため630調査の各年度の都道府県別データ等を用いて分析を行った。その結果、身体拘束実施率には地域差が存在した。有意に高い都道府県、有意に低い都道府県が存在し、2017年度・2018年度でほぼ同様の傾向が見られた。最高と最低の開きは、2017年度11.6倍、2018年度は20.5倍にも上った。この身体拘束実施率の都道府県による違い、「東高西低」ともとれる現象はなぜ生じるのか、その要因を明らかにすることが、身体拘束の縮減につながる可能性があることが考えられた。患者の年齢は75歳以上が身体拘束実施率が有意に高かった。診断名では「F7 精神遅滞(知的障害)」「F0 症状性を含む器質性精神障害」の身体拘束実施率が有意に高く、診断名の細項目では「精神遅滞(知的障害)」「アルツハイマー病型認知症」「血管性認知症」などで有意に高かった。精神科病院における、75歳以上の患者、認知症患者や精神遅滞(知的障害)患者に対して身体拘束に代わる対応方法を見つけていく必要性は高いと考えられた。

【背景と目的】医学部1 年生に対し、入学後のモチベーションの低下を防ぎ、能動的な学習姿勢を修得させ、さらに医師のプロフェッショナリズムを涵養する教育方法は、未だ確立されたものがあるとは言い難い。本学では1 年次に「臨床医学入門」の授業を通年で行なって、この問題への対応に努めており、本稿では本科目の全体像について報告する。【対象と方法】1 年次学生を対象とし、平成21年度より開講している科目「臨床医学入門」は毎週水曜の午後行われる。教育方法は講義、実習、演習(ワークショップ)など多岐にわたる。授業内容としては、(1)「こんな医師になりたい」をテーマに作文を書き、同級生全員の前で自己紹介を兼ねて発表、(2)「患者さんの願いと医師が果たすべき役割」をテーマとしたワークショップ、(3)「弘前大学医学部の歩みとこれから」の講義、(4)アーリーエクスポージャー、(5)地域医療の最前線の医師による講義「現場の医療を知ろう」、(6)コミュニケーション実習「模擬患者さんと話してみよう」、(7)地元について知る講義「津軽学」、(8)まとめのワークショップ、などから成っている。これらの教育内容はいずれも、Intrapersonal professionalism、Interpersonalprofessionalism、Public professionalismの涵養に通じるものである。これらは医師になる者の基本であり、特に本学学生の約半数を占める地域枠入学者のPublic professionalismの修得は重視されている。【結論】プロフェッショナリズムの涵養に焦点を当てた初年次教育は、長期的な効果が期待できる。

A case of drug-induced fulminant hepatitis is reported. A 48-year-old female was transferred to Saga Medical School Hospital because of fever, general fatigue, jaundice and marked liver dysfunction after being treated by her private physician with several drugs containing antibiotics and aspirin DL-lysine. On admission, prothrombin time was prolonged (13%), but the patient's consciousness was clear. Viral markers for hepatitis A, B and C were negative. On the fourth hospital day, consciousness disturbance (hepatic coma: grade IV) developed, and despite intensive treatment including glucagon-insulin therapy and plasma exchange, the patients died of hepatic coma with multiple organ failure on the eleventh hospital day. The liver weighed 710g, and exhibited features of massive necrosis histologically. The lymphocyte stimulating test was positive for cefmetazol and aspirin DL-lysine. A review of literature showed that in Japan about 5% of cases of fulminant hepatitis are due to drugs other than halothane. In most cases reported recently, the lymphocyte stimulating test has been positive for the causative drugs.

1.ボラ土培地を用いたバラの循環型養液栽培では、見かけの吸収濃度から作成した循環処方を使用し、給液量と設定ECはそれぞれ夏期(6〜8月)は2000mL/株、EC1.2dS/m、冬期(12〜3月)は1200mL/株、EC1.5dS/m、それ以外の期間は1600mL/株、EC1.5dS/mでの養液管理が適する。2.循環栽培で1年経過後はボラ土のリン酸吸収が低下するため循環処方のリン酸濃度を下げた処方に切り替える。また、pHが低くなる場合はアンモニア濃度を低くし、カルシウム濃度を高くし、pHが高くなる場合はアンモニア濃度を高くし、カルシウム濃度を低くすることで養液の安定化を図る。
