We herein report two cases of drug-induced liver injury (DILI) due to mosapride. Case 1: A 78-year-old man was admitted with elevated transaminase levels. The cessation of mosapride led to the improvement of elevated liver enzyme levels. Case 2: A 54-year-old man was admitted with jaundice. Mosapride was discontinued immediately, and methylprednisolone was administered for acute liver failure. The patient's data showed improvement, and he was discharged on Day 32. In both cases, mosapride gave a positive response to a drug-induced lymphocyte stimulation test (DLST), and the patient's score based on the criteria for DILI was "highly probable".

肺性心はその病態から,肺血管の解剖学的閉塞によって起こる肺血管障害型と,低酸素血症およびアシドーシスによって起こる換気障害型とに分けることができ,それぞれ異なった進展様式をとる.また,臨床症状については,肺血栓塞栓による急性肺性心では呼吸困難,胸痛,頻脈,右心系心雑音などがよく見られるのに対し,慢性肺性心ではその上に浮腫,肝うっ血が見られるのが特徴である.

活性酸素は脂質,蛋白,核酸など生体の多くの分子を標的とし反応する.それゆえ多彩な活性を有し,組織障害性因子として炎症,虚血,癌といった病態生理に関与し,また,バイオシグナルとして種々の生理機能の調節も行っている.活性酸素は,環境因子のみならず多くの生体内酵素系,ミトコンドリアなどより生じるが,炎症,虚血疾患においては好中球の関与が極めて重要である.スーパーオキシドとnitric oxide(NO)との相互反応も重要であり,それらが同時に生じるような場では,その微小循環系への影響,細胞障害性の二面より検討する必要がある,このような活性酸素の動態を生体における抗酸化防御機構との関連で検討することは,疾患の病態解明,治療につながる可能性がある.

近年,診察室血圧が正常で,診察室以外の血圧が高値を示す「仮面高血圧」が注目されている.仮面高血圧は未治療者の10~15%,治療中高血圧患者の20~25%に存在する.その心血管リスクは正常血圧より2~3倍高く,診察室と診察室以外の血圧レベルが共に高い持続性高血圧と同程度かそれ以上である.仮面高血圧の表現型には,早朝血圧,ストレス性高血圧,夜間高血圧があり,その治療には個々の背景病態を把握する必要がある.早朝高血圧をターゲットにした降圧療法が,仮面高血圧治療の最初の第一歩である.

尿細管性アシドーシスとは糸球体濾過量が正常か軽度低下にもかかわらず代謝性アシドーシスがみられる場合で尿細管からの酸排泄の低下が原因である.障害部位により,近位尿細管性アシドーシスと遠位尿細管性アシドーシスに分けられる.近位尿細管性アシドーシスは低K血症を呈するが,遠位尿細管性アシドーシスには低K血症を呈するものと高K血症を呈するものとがある.炭酸水素ナトリウム負荷試験,酸負荷試験,フロセミド負荷試験,硫酸ナトリウム負荷試験,中性リン酸ナトリウム負荷試験などにより障害部位および機序を鑑別する.

症例は49歳,女性.胆石症の精査のため入院した際, CEAの異常高値と甲状腺の硬い腫瘤に気付き,姉が甲状腺癌で死亡していることから,髄様癌,さらに多発性内分泌腫瘍症2型を疑い精査をすすめた.血中カルシトニンが高値で,穿刺吸引細胞診で甲状腺髄様癌と診断.腹部CTでは左右副腎の腫大を認め,尿中カテコラミンも高値で褐色細胞腫と診断.遺伝子検査でret癌選伝子に点突然変異を認め, MEN2Aと最終的に診断した.遺伝性腫瘍を念頭にいれた家族歴聴取の重要性を示唆する症例であった.

A 58-year-old man was referred to our institution for an evaluation of nephrotic range proteinuria. Renal biopsy showed a marked expansion of the mesangial matrix and thickening of glomerular basement membrane (GBM) in periodic acid-silver methenamine (PAM). Immunofluorescence (IF) revealed strong staining for the monoclonal kappa light chain. EM demonstrated massive subendothelial and mesangial dense deposits. As a result, light chain deposition disease (LCDD) was diagnosed. Melphalan and prednisolone (MP) therapy was started, which was continued for 10 years with minimal complications. Serial evaluations of renal histology revealed the resolution of nodular lesions and the glomeruli became nearly normal. MP therapy can therefore be an effective therapeutic option for LCDD if it is continued over the long term.

全身性エリテマトーデス(systemic lupus erythematosus:SLE)は多様な病態を示し,高頻度に再燃や持続性の活動性病変を呈する難治性の自己免疫性疾患である.最近,新たなSLE分類基準,ループス腎炎の治療やTreat to Targetのための診療のリコメンデーションなどが相次いで提唱されている.また,抗マラリア薬やミコフェノール酸モフェチル(mycophenolate mofetil:MMF)は国際的にSLEの主要な治療薬となっており,国内でも早期の適応承認が期待されているほか,新たな分子標的治療薬も開発されている.

Objective It has been increasingly recognized in various clinical areas that self-efficacy promotes the level of competence in patients. The validity, applicability and potential usefulness of a new, simple model for assessing self-efficacy in the elderly with special reference to frailty were investigated for improving elderly patients' accomplishments. Methods The subjects of the present study comprised 257 elderly people who were members of the New Elder Citizen Movement in Japan and their mean age was 82.3±3.8 years. Interview materials including self-efficacy questionnaires were sent to all participants in advance and all other physical examinations were performed at the Life Planning Center Clinic. Results The internal consistency and close relation among a set of items used as a measure of self-efficacy were evaluated by Cronbach's alpha index, which was 0.79. Although no age-dependent difference was identified in either sex, gender-related differences in some factors were noted. Regarding several parametric parameters, Beck's inventory alone revealed a significant relationship to self-efficacy in both sexes. Additionally, non-parametric items such as stamina, power and memory were strongly correlated with self-efficacy in both sexes. Frailty showed a significant independent relationship with self-efficacy in a multiple linear regression model analysis and using Beck's inventory, stamina, power and memory were identified to be independent factors for self-efficacy. Conclusion The simple assessment of self-efficacy described in this study may be a useful tool for successful aging of elderly people.

結核は世界人口の1/3が感染しており,年間880万人が発病し,145万人が死亡している.わが国でも年間2万人以上が発病し,結核の中まん延国といわれている.わが国の結核発病の特徴は,高齢者結核の増加,地域格差の拡大,ハイリスク者への集中などがあげられる.結核の確定診断は結核菌の証明であり,最近は核酸増幅法検査が普及し,迅速に診断確定されるようになった.治療はイソニアジド,リファンピシンを主軸に,ピラジナミドを含む初期強化短期化学療法が推奨される.結核予防法は2007年に感染症法に統合され,初感染結核に対する予防投与は,化学予防から潜在性結核感染症の治療と名称が変わり,年齢制限が廃止された.また発病リスクの高い既感染者も治療の対象とした.結核感染の診断には5歳以下の小児を除き,ツベルクリン反応検査にかわり,既往のBCG接種の影響を受けない結核感染診断キットのクォンティフェロンTBゴールドが用いられている.

A 41-year-old man developed sustained monomorphic ventricular tachycardia (VT) with a left bundle branch block and inferior axis pattern during treadmill exercise concomitantly with unmasking of the typical Brugada electrocardiography (ECG) pattern. The typical ECG phenotype was provoked by a class IC drug. VT was not inducible with programmed electrical stimulation, but premature ventricular beat and non-sustained VT with the same morphology increased in frequency with isoproterenol treatment. Additionally, atrioventricular nodal reentrant tachycardia (AVNRT) was induced by electrical stimulation and VT and AVNRT were treated by radiofrequency catheter ablation.

We herein report an elderly patient with a history of restless legs syndrome (RLS) who developed unpleasant sensations in the lower abdomen and perineum. The patient' s symptoms occurred during the evening and at rest and forced the patient to void, resulting in some symptom relief. Urological examinations and spinal magnetic resonance imaging findings were unremarkable. The adjunctive use of rotigotine resolved her symptoms. The symptoms were considered to be "restless bladder". Further studies are required to elucidate whether restless bladder is an identical condition to the clinical entities of other RLS variants or whether it represents an unusual feature of RLS.

Crohn病の病因はいまだ不明で,遺伝的素因,感染,食事因子,血流障害,免疫学的異常といった観点から研究がなされてきた.特に,単球・マクロファージ系細胞の機能異常および管腔より侵入した外来抗原に対する免疫反応の異常が,肉芽腫や潰瘍,瘻孔などの病変を形成するという考えが有力となっている.初期治療として栄養療法が有用であるが,再燃や進展を防ぐには,免疫学的異常をふまえた適切な免疫統御療法が必要となる.

腎前性急性腎障害(acute kidney injury:AKI),特に敗血症性AKIではエピネフリンとバゾプレッシンが腎機能保護に有用であることが指摘されている.心腎連関に伴うAKI,急性心不全に伴うAKIでは,トルバブタン,hANPが腎機能保護に有効であることが報告されている.しかし,そのエビデンスは少なく,現在トルバブタンでは大規模介入試験が進行中である.造影剤誘発性AKIも含む腎性AKIにおいては,残念ながらAKI発症後の治療薬として有効なものはない.AKIの治療薬として,抗酸化薬などを中心に新たな治療薬の可能性が研究されつつある.

敗血症の臨床像は患者の基礎疾患で異なり,ショックや播種性血管内凝固症候群(DIC)を合併した症例は予後が不良で,敗血症治療時には合併症治療も行なう必要がある.敗血症性ショックの病態は末梢血管拡張と血管透過性亢進とによる循環障害で,治療の基本は体液補正である.敗血症によるDICは線溶抑制型で,より臓器障害を起こし易いので積極的な治療が求められる.したがって感染症治療時は原因治療とともに全身管理が重要な課題となる.
