Epadel S®,a seamless capsule form of ethyl icosapentate (EPA-E),was made into a suspension in 20 ml of hot water (55°C) and stirred with a polystyrene (PS) disposable spoon.When left in the suspension,the surface of the spoon started to dissolve after 45 minutes,and was completely dissolved after 12 hours.We also investigated the dissolution of PS utensils in suspensions made from teprenone (Selbex®) capsules or powder,or gefarnate (Gefanil®) capsules.Up till now,there have been no reports of toxicity due to PS or the non-reactants and/or by-products produced in the PS production process.However,as the dissolution of spoons,cups or other common utensils made of PS in drug suspensions could cause concern among patients,caregivers,and medical staff,it is important to instruct them to avoid the use of such utensils when preparing drug suspensions.

The purpose of this study was to evaluate the potential benefits arising from pharmacists accompanying doctors during their rounds in long-term care insured facilities for the elderly. Based on the medical records, the pharmaceutical content provided to physicians, medication history of residents, and residents’ level of care were analyzed. There were 157 interventions related to drug reduction that contributed to polypharmacy decrease. These interventions were maintained for six months and led to a cost reduction of approximately 1.67 million yen. Furthermore, one year after these interventions, a trend toward positive correlation was recorded between the improvement in residents’ level of care and the decrease in the number of medications taken. Overall, this study highlighted that pharmacists attending consultant-led rounds at long-term care insured facilities for the elderly was advantageous with regards to reducing polypharmacy and pharmaceutical costs.

To administer oral anticancer drugs safely, the simple suspension method has been introduced in many hospitals. Therefore, concerning drugs for which it is unclear whether or not this method is applicable, testing must be able to be conducted at any time. In this study, we investigated 20 oral anticancer drugs to expand information on the application of the simple suspension method. Disintegration/suspension and permeability tests were conducted, as described in the 3rd version of the Tube Administration Handbook for Oral Drugs. All products were disintegrated/suspended after 10 minutes. On permeability tests, there was no residue in any tube for tubal feeding. On the final evaluation, the products were regarded as suitable (grade 1). Bicalutamide tablets (80 mg, TCK and KN), which were analyzed in this study, were regarded as suitable (grade 1) on the final evaluation. On the other hand, the simple suspension method is not applicable for a brand-name drug, Casodex® tablets (80 mg). This may be related to the different additives. Furthermore, the results suggest that, even when the simple suspension method is not applicable for a brand-name drug, it may become applicable for generic drugs. This may provide a new merit for promoting the use of generic drugs.

Patients with reduced swallowing function have difficulty taking tablets in their original form. In such cases, tablets are crushed (crushing method) and administered through feeding tubes, but this practice may affect efficacy. A simple suspension method (SSM) has been widely used to address issues of administering crushed tablets through feeding tubes. Most of all angiotensin-converting enzyme (ACE) inhibitors are ester prodrugs. When they are suspended with magnesium oxide, ester prodrugs are hydrolyzed, and the active metabolite increases before absorption. In this study, we investigated the effects of SSM and crushing methods on the pharmacokinetics of the ACE inhibitor using temocapril and magnesium oxide. A human clinical trial was conducted to compare the pharmacokinetics of temocapril and magnesium oxide in three groups in six healthy adult men (period 1, tablet group; period 2, SSM group; and period 3, crushing-and-mixed group). The pharmacokinetics of temocaprilat, which is the active metabolite, were examined. The AUC0-24 and Cmax of the SSM group were 89.3% and 86.1% compared with those of the tablet group, whereas those of the crushing-and-mixed group decreased to 73.0% and 78.9%, respectively. In the crushing-and-mixed group, the concentration of temocapril decreased during crushing and long-term storage. Findings in the present study suggest that the crushing method may decrease in AUC0-24 and Cmax and that the expected drug effect may not be achieved. For ester prodrugs, the use of SSM might be considered rather than the crushing method.

In cancer chemotherapy, it is very important to take into account the patient’s background. In recent years, a simple suspension method has attracted increased attention as a method that prevents changes in the stability and safety of various drugs. However, of 135 oral anticancer drugs, only 28 have been examined using this method, as of April 2013. In this study, we carefully investigated whether 53 oral anticancer drugs could be adapted to the simple suspension method, except for the 28 drugs that had already been previously reported. The results showed that most of these oral anticancer drugs could be adapted to the simple suspension method. Of seven drugs that were not adapted, six were generic drugs. In addition, it was clear that the evaluation of bicalutamide tablets was significantly different from our expected results. In conclusion, we were able to qualitatively assess all 53 oral anticancer drugs. This is equivalent to half of 107 untested drugs. These results provide useful information to cancer patients using oral anticancer drugs prepared using the simple suspension method.

【目的】従来の薬剤経管投与法である粉砕法は薬効の減少につながる薬剤量の損失が指摘されている。そこで粉砕法による薬剤量損失に対する簡易懸濁法の有用性について検討した。【方法】頻繁に粉砕指示がなされる5種類の薬剤を用いて粉砕・分包による薬物含量減少、薬剤調製時の懸濁性および実際の経管投与を想定した薬物含量について2つの方法を比較した。【結果】薬剤を粉砕・分包するとそれぞれの薬物含量は減少した。またワーファリン®錠を粉砕して水に溶解すると完全には懸濁せず、小さな塊が生じたが、簡易懸濁法では均一に懸濁した。ワーファリン®錠の経管投与を想定した実験において粉砕法では薬物含量が大幅に減少したが、簡易懸濁法では、ほとんど損失が認められなかった。【結論】簡易懸濁法は粉砕法に比べて薬剤損失の面で有用性が高いことが示唆され、ワーファリン®錠のように安定性が悪い薬剤では特に適正な薬物投与に貢献出来ると考えられる。

Objective: We conducted a survey on drug information accumulated by pharmaceutical companies about the adequacy of administration of crushed or simply suspended internal medicines through enteral feeding tube, examination methods to confirm adequacy and inquiries from medical institutions to pharmaceutical companies about the adequacy of these methods.Methods: We sent a questionnaire to 162 pharmaceutical companies that sell internal medicines to collect information. The survey was conducted from May 1, 2016 to March 31, 2017.Results: The questionnaire response rate was 61% (99 companies responded). Eighty and 90 percent of the companies possessed information about the drug crushing methods and simple suspension methods used for administration of internal medicines,respectively. The type of information and examination methods used varied among the companies, was very limited, and was often limited to new drugs. The information acquisition rate about crushing methods was 69.3% in original examination methods of pharmaceutical companies. On the other hand, 90.3% of the information about simple suspension methods was obtained by the unified method of Hand Book of Simple Suspension Method.Conclusions: In the future, medical practice and patients will benefit if examination methods to confirm the adequacy of crushing and administration through feeding tubes are commonly and consistently obtained by pharmaceutical companies. Furthermore, it would be very useful for information of crushing methods and simple suspension methods to be included in package inserts and interview forms.

The role of pharmacists in the US that have advanced functions compared to the traditional pharmacists is based on the collaborative drug therapy management (CDTM). Recently in Japan, the importance of enhancing the team approach whose concept is to make the best use of the each health providers' expertise has been discussed in the health care society. As regards pharmacists, pharmacists' pivotal role in medication therapy management is in great demand in the discussion. On the other hand, pharmacists are more exposed to a litigation risk as pharmacists sometimes are sued for their medical negligence these days.In this manuscript, we conducted a literature and case review for pharmacist responsibility conducting the CDTM in the US. However, we could not find reports and cases, though a litigation risk for pharmacists has been enhanced with the pharmacist role expanding in the CDTM. Furthermore, we conducted a literature and case review for nurse practitioner (NP) as well. As a result, in some cases, NP's negligence was judged independently by the action of NP was whether appropriate or not with no consideration for physician's supervise, and the supervising or collaborating physician's negligence was judged according to the physician's action in the consultation is appropriate or not. As the number of litigation for NP increased, nurse societies raised so many opinions on how to judge appropriately such as which the standard of care (SOC) for physicians or for NPs was applicable and as whether a physician might have an expert witness qualification.

1976年昭和大学薬学部卒業後、同大学病院に就職、1996年同大学藤が丘リハビリテーション病院に異動、2006年同大学薬学部教員になった。1986年結婚、2016年高速道路で自損事故により危うく天国(地獄?)へ。振り返えると西暦末尾6の年が転換期であった。30年間の臨床薬剤師の後、16年間教員として教育・研究に携わり、現在も客員教授として研究を続ける日々が続いている。忘れてかけている過去を思い出し、思いつくまま最終講義として綴ってみたい。

Drugs prepared by pharmacist may sometimes clog the nasogastric tubes, gastrostomy tubes or jejunostomy tubes during drug administration. As a result, such drugs were examined regarding their possibility of clogging. Consequently, 11% of the capsulated drugs (89 drugs) could not be used with tubes due to the fact that they did not come in a dispenser that can be used with tubes. In addition, 15% of the drugs clogged the thin tubes (8Fr.). 7% of the fine granules and granules (94 drugs) could not be used with tubes, 10% of them clogged the thin tubes.On the other hand, in order to decrease the amount of loss due to crushing the tablets, as well as to prevent health hazards, the tablets were dispersed in water. Experiments were performed to obtain data on the disintegration of the tablets in the water. As a result, 74% of 514 tablets were found to be appropriate for administration using this method, and this rate was higher than for crushable tablets (53%).The results were complied and the list of the drugs that could be successfully administered by feeding tubes was made. Accordingly, the problems in crushing were solved and tube clogging could thus be successfully prevented thus resulting in an improved QOL for the patients.
