There is the increasing concern of the society to prevent the tumbling of the aged. The study of the static, as well as dynamic aspects, such as the muscular strength of the lower-limb and the postural stability, should be developed, especially from the viewpoint of the aged. This paper focuses on the external observation of the foot and toenail, as being correlated to the physical functions of the lower-limb against tumbling. The lower-limb functions are evaluated in terms of the 10 m walking time, the toe-gap force and single-foot standing period. The correlation to the personal tumbling experiences is also examined. It is seen that the groups, which exhibit external abnormalities in the foot and the toenail, generally decline in the muscular strength and postural stability. They also have more frequent tumbling experiences and express in their concern of the danger of tumbling. It seems that those shapes abnormalities can indicate, to some extent, the tumbling danger of the aged.

We began to print the laboratory data of each patient on their in-hospital prescription in April, 2010 and further introduced a prescription checking system associated with laboratory data in December, 2014. This study evaluated the changed prescriptions based on prescription queries and the usefulness of using prescriptions with laboratory data as well as the prescription checking system. We examined the number of changed in-hospital prescriptions after the prescription queries in three periods: before and after using prescriptions with laboratory data (from June, 2009 to November, 2009 and from June, 2010 to November, 2010, respectively), and after the introduction of the prescription checking system (from September, 2016 to August, 2017). Although there were no changed prescriptions based on the laboratory data before using prescriptions with laboratory data, 6.6% of changed prescriptions were based on laboratory data after using prescriptions with laboratory data. In addition, the changed prescription ratio based on laboratory data significantly increased after the introduction of the prescription checking system compared with that after using prescriptions with laboratory data (8.9% versus 6.6%, P = 0.015). In conclusion, using prescriptions with laboratory data and the prescription checking system associated with laboratory data were useful for checking prescriptions effectively in order to avoid adverse drug reactions.

This study was the examination of the sustained effects of the autonomic nerve response due to the difference in the posture of the supine and the standing after the acupuncture method of the "skin / subcutaneous tissue, exhalation phase, sitting position". The subjects were 11 healthy male college students without medication and smoking habits (age 19.6 ± 1.0 years old, height 172.1 ± 4.1 cm, body weight 70.4 ± 4.3 kg). Cardiac autonomic nerve function was measured by measuring electrocardiogram and analyzing by spectral analysis of heart rate variability. In the evaluation by spectral analysis, the high frequency component HF (0.15 - 0.40 Hz) and the low frequency component LF (0.04 - 0.15 Hz) were measured, and HF component was used as a cardiac parasympathetic function and LF / HF component was used as an index of cardiac sympathetic function. HR and HF components showed no significant change at standing posture with passage of time after the acupuncture stimulation. However, HR and HF components showed significant change at supine posture with passage of time after the acupuncture stimulation, and enhanced the cardiac parasympathetic function. Furthermore, significant increase of LF / HF component was observed between 5 minutes and 25 minutes after the stimulation in the supine position. From these results, it was obvious that the sustained effects of acupuncture stimulation in the enhancement of cardiac parasympathetic function was caused by supine posture. Moreover, it was suggested that this acupuncture method may be enhance both cardiac parasympathetic and sympathetic nerves.

【目的】体性感覚の低下は高齢者の転倒にも深く関与する。本研究は高齢者の足底触覚閾値を測定し,転倒事象との関連を明らかにする。【方法】通所介護施設の利用者110 名を対象に,高い刺激強度再現性を有する足底感覚計で足底触覚閾値を測定した。併せて下肢筋力など9 項目の心身機能を測定し,転倒群と非転倒群の間で比較した。さらに転倒を目的変数としたロジスティック回帰分析にて各変数のオッズ比を算出し,過去1 年間の転倒歴に対する足底触覚閾値の関連を検討した。【結果】転倒群と非転倒群との比較で足底触覚閾値,足関節背屈角度に有意差が認められた。性別比と疾患の有無で調整したオッズ比は足底触覚閾値と足関節背屈角度が有意であった。【結論】足底触覚閾値は高齢者の転倒を説明する変数として強い関連が認められた。要介護認定者の集団では転倒リスクの評価に従来の指標に加えて足底触覚閾値の測定が有効である可能性が示唆された。

Staphylococcus aureus(S. aureus,黄色ブドウ球菌)はアトピー性皮膚炎の炎症部で顕著に増加して炎症を増悪化する。一方,健常者の皮膚に多いStaphylococcus epidermidis(S. epidermidis,表皮ブドウ球菌)はS. aureusの生育抑制などの有用機能を保持する。多数の脂肪酸のS. aureusとS. epidermidisに対する抗菌活性を調べたところ,6-cis-C16:1,7-cis-C16:1,および9-cis-C16:1はS. aureusの生育を抑制し,S. epidermidisの生育を抑制しない選択的抗菌活性を保持し,アトピー性皮膚炎の炎症緩和に寄与することが期待される。皮脂中に存在する6-cis-C16:1は天然油脂中に存在しないことから,7-cis-C16:1と9-cis-C16:1を微生物法で生産することを試みた。Aeromonas hydrophilaおよびAcinetobacter sp.は,9-cis-C18:1を含む植物油を基質としたとき,9-cis-C18:1の炭素数を小さくした7-cis-C16:1に変換した。種々の遺伝子組換えを施したSaccharomyces cerevisiaeは高含量の9-cis-C16:1を含む油脂を生産した。さらに精製条件の検討により9-cis-C16:1含量を98%まで高めることができた。また,消費者の認知度の高いBifidobacterium sp. JCM 7042株は菌体内に7-cis-C16:1を生産した。

当院では整形外科領域の炎症を伴った症例について霊仙除痛飲の方意で越婢加朮湯と大黄牡丹皮湯を併用して治療を行っている。代表症例を提示して原文による文献的考察を行った。また,整形外科疾患に於いて非ステロイド性消炎鎮痛剤(NSAIDs : non-steroidal anti-inflammatory drugs) が使用しにくいまたは無効であった症例に対しても同処方は有効であり,文献的考察を加えて報告する。

Web ホスティングサービスにて管理者がテナントごとのコンテンツを制御できないような高集積マルチテナント Web サーバ環境では,ホスト間のリソース競合を減らすことが安定運用にとって不可欠である.しかしホスト数が増えるにつれ,サーバ内の原因となるホストの監視や制御のコストも増加するため運用は難しくなる.本論文ではリソースの各指標の時系列データの変化点検出,ならびにサーバ内ホストやプログラムの各指標の重みづけによって,システムリソース逼迫状況下で多量のリソースを消費するリクエストを同定し隔離する自律的アーキテクチャを提案する.

急性腰痛症に対して非ステロイド性消炎鎮痛剤(NSAIDs, non-steroidal anti-inflammatory drugs)を用いたが効果が得られないため,大黄牡丹皮湯と四物湯を併用して用いて効果が得られた3症例を経験したので報告する。 症例1は86歳の男性で,症例2は56歳女性でありいずれも急性腰痛症と診断した。症例3は69歳男性で急性腰痛症,軽度の左根性坐骨神経痛と診断して治療した。全症例とも1から2週間の投薬で腰痛等の症状が軽快した。『万病回春』の調栄活絡湯の方意に準じて,大黄牡丹皮湯と四物湯を併用した。急性腰痛症に対する漢方治療において大黄牡丹皮湯と四物湯の併用による治療は有用であると考えられる。

The aim of this study was to evaluate the postural control ability using stabilogram-diffusion analysis (SDA). Any parameters of SDA were compared with several parameters of body sway derived from recordings of the center of pressure (COP) with the aid of a static force platform. The subjects were 38 elderly healthy volunteers. They quietly stood on a static force plat form with open and closing eyes. As the results, there was a significant correlation between the diffusion constant derived from the parameters of SDA and the parameters of COP with eyes open and closed. The correlation was found to be greater in the closing eye condition than that in the open eye condition. The correlations between diffusion coefficients and the range and mean of anterior-posterior velocity were greater than the correlations between the diffusion coefficients and the range and mean of the lateral velocity. Thus, the muscle of the tibialis anterior and the planter muscles was suggested.

高齢社会に伴い, 寝たきりや座りきりといった新しい社会問題が起きている. 2025年には, 虚弱·要介護の痴呆高齢者·寝たきり高齢者を合わせた推計は520万人になると予測され, 脳血管障害や転倒骨折などを経験した高齢者の生活の質(Quality of Life:QOL)の確保や自立支援など早急に解決しなくてはならない課題を抱えている. より効果的にリハビリを行うために, タキザワ式リハビリテーションプログラムが開発され, その中で創動運動が実施されている. 下服運動は下肢リハビリのため, 対象者自ら患側の足を健側の足で動作させるものである. しかしその定量的評価や効果は解析されていない.<BR>そこで本研究では, 創動運動による効果を総合的に検討するために, 筋電図による下肢の筋活動と, 運動力学的側面から対象者の動作を定量的に解析するシステムを開発した.
