HLA領域の遺伝的多型は医学分野のみならず,人類の移住,混合,自然選択や遺伝的適応といった進化過程を推測する集団遺伝学においても重要なツールとして用いられている。日本人の起源については諸説あるが,約30,000年前の後期旧石器時代に日本列島へと移住してきた狩猟採集民である縄文人と,紀元前1,000年から西暦300年頃に朝鮮半島から日本列島へと移住してきた弥生人の祖先が混血して,現在の日本人集団の起源になったとする“混合モデル”が有力であると考えられている。本稿では,日本人集団の混合的起源を支持する遺伝的知見について概説するとともに,HLA遺伝子多型から日本人の祖先集団を推測する遺伝的痕跡の探索について,最新の研究を紹介したい。

社会基盤施設に関連した事故に関する判例などが報道されると,工学で考えられている設計問題の枠組みと,法律で規定されている枠組みに差があることを感じることがある.本研究はこのような法律の考え方と,設計の考え方の関係を明確にし,社会基盤施設に求められる安全性や技術者の責任を明らかにすることを目的とする.判例を検討していると,瑕疵に関する考え方も,徐々に安全性が相対的なものであるという認識に変化していると思われる.判例は依然通説である「客観説」をとっているが,その判断にあたり,予見可能性,回避可能性など技術的見識を加え,結果の重大性や構造物の重要性などを多面的に勘案するようになってきていると思われる.

〔目的〕外気功を含めたCAMを積極的に併用することによって、腫瘍の縮小を見た2例について報告する。〔症例I〕A.F.76才♂歯科医師結腸腫瘍自覚的症状無くB.D.0-ring Testにて下行結腸に腫瘍の共鳴あり。内視鏡検査、組織検査にて同部に2.5cm程度のTubular Adenoma(Group3)を確認。その後、約2ヶ月外気功を含めたCAMの併用後、腫瘍が約1cmに縮小し、内視鏡的切除を可能にした。〔症例II〕M.Y.59才♂医師転移性肝内腫瘍排便異常、体重減少を主訴。内視鏡検査にて、大腸悪性腫瘍(ClassV)、画像診断により肝への転移を認める。大腸腫瘍切除手術後、約9ヶ月間、化学療法を施行。その後、外気功を併用し、画像診断により、肝転移性癌腫瘍の縮小傾向良性化が認められた。〔症例III〕T.S.42才♂会社員脳動脈瘤1999年8月2日交通事故の後遺症にてMRI・脳血管造影により動脈瘤2ヶ所を認めた外気功の併用にて動脈瘤の縮小、血流の改善をみた。〔結論〕以上、外気功を併用し、経過良好の3症例について報告する。

Acute or chronic graft rejection is the primary threat to transplant recipients. The golden standard for rejection diagnosis is invasive biopsy. Measurement of donor derived cell free DNA (ddcfDNA) in the blood have been described as a non-invasive biomarker for organ transplantations. To develop a diagnostic system to detect graft injury before diagnosis by biopsy, we established capture-based sequencing procedure on extracted cf DNA which targets 1000 selected single nucleotide polymorphism sites. The ddcfDNA percentages were highly elevated on the first days after transplantation, but ddcfDNA percentages were decreased gradually within 7-14 days in stable patients with no sign of rejection. In 80% of cases having rejection episodes and all cases having biopsy proven rejection, ddcfDNA levels showed greater expressive signals on days of rejection episode. Additionally, we found evidences showing that ddcfDNA level sensitively corresponded with immunosuppressant dosages which were administered on recipients. This non-invasive method enables us close follow up the graft injury and prevent graft rejection.

我が国でも、統合医療が徐々に浸透してきているが、その反面、エビデンスの不足から代替医療に対する批判がいまだ多いことも事実である。特に目に見えない情報を扱うエネルギー医学・波動医学においては、科学的アプローチが困難であり、従来のEBMの手法ではない新しい評価方法も必要である。過去、気の世界や潜在能力について様々な研究成果を上げられてきたISLISの頭脳に、今後さらに代替・統合医療分野での科学的評価方法を追及していただくことを期待する。

「波動」という新しい概念が提唱され、波動測定器というものが健康度や病気の進行度、物品の品質などを計測できる機器であるとされている。しかし、この種の機器での分析にあたり、計測担当者の操作技術、心理状態が計測結果に影響する要因として考えられており、研究者によっても得られた計測値の評価についての解釈は様々である。本実験では、熟練度の異なるオペレーターが、11種類の歯に装着する各種歯科材料を対象として、磁気波動共鳴分析器「MIRS」を用いて計測を行い、その計測値の差について評価した。またこれらの歯科材料の材料相互間の評価でなく、各被験者との相性を評価することができるかどうかを、同様に「MIRS」を用いて、十分な経験のあるオペレーターにより計測し、評価・検討を行った。

A 19-year-old girl was admitted to our hospital with nausea, vomiting, hiccups, constipation and syncope. After hiccups or vomiting sinus arrest developed and lasted more than 5-8 seconds. She lost consciousness every one hour. Based on an electrocardiographic diagnosis of sick sinus syndrome (SSS), a temporary pacemaker was implanted. The next day, although her syncope and bradycardia disappeared, she had orthostatic tachycardia of over 120 beats/minute and swelling of the legs, which led to a diagnosis of postural orthostatic tachycardia syndrome (POTS). Neurologically, she showed the right-sided tongue deviation and parasympathetic system disorders revealed by coefficient of variation of R-R interval (CVR-R), the Achner eye-ball pressure test, the valsalva ratio, and the head-up-tilt test. Brain MRI disclosed a small hyperintense lesion on a T2-weighted image with gadolinium enhancement in the right dorsal medulla including the hypoglottis nucleus and the posterior nucleus of vagus. After steroid pulse therapy (methyl prednisolone 1 g/day×3 days, 5 times) was administered, this lesion became smaller and finally disappeared. Before the lesion disappeared, she was able to begin rehabilitation by wearing elastic stockings and treatment with midodrine hydrochloride. The following year, she developed other MRI-proven brain lesions, suggestive of demyelination. Such a spinal and temporal distribution of lesions led to a diagnosis of multiple sclerosis (MS). A case of POTS caused by MS has not been reported previously, however, MS often affects the medullary paraventricular regions associated with autonomic failures. Autonomic failures often prevent patients from experiencing early rehabilitations. We should promptly give symptomatic treatment against autonomic failures, which leads to good patient recovery not only in patient vitality but also functionality.

The Bi-Digital O-ring test is a diagnostic method in which the patient's physical condition is judged by his/her finger stregth. The patient is made to form a Bi-Digital ring using the thumb and any other finger on the same hand. The doctor then forms two of the same rings with his/her own hands, interlocking them with the patient's ring and then, While the patient using his own finger strength resists, the doctor attempts to pull open the ring from both sides using his own bi-digital o-rings. Diagnosis is made based on the patient's ability to maintain closure of the ring.We apply this test to make diagnosis in the areas of conservative dentistry, prosthetic dentistry and oral surgery. It is a very useful method for occlusal diagnosis, determination of mandibular position and teatment and diagnosis of temporomandibular joint porblems. We would like to report on our objective observations and clinical cases.

全国的な種牛評価の可能性を遺伝的結合度および遺伝率推定値の点から検討し,その結果をふまえて全国的な種牛評価のための最適な数学モデルに関して検討した.1997∼2002年までに全国に分布する(有)安愚楽共済牧場傘下の肥育農家から出荷された黒毛和種肥育牛のフィールド記録を用いた.数学モデルとして,母数効果の取り上げ方が異なる6つのモデルを設定し,産肉性形質に関する遺伝率を推定し,育種価を予測した.肥育農家を単位とする分集団間の遺伝的結合度が確認できたことおよび遺伝率の推定値が中程度であったことから,全国規模での種牛評価が可能であると推察される.また,予測育種価の予測誤差分散(PEV),赤池の情報量規準(AIC)および自由度調整済寄与率を指標としてモデルの選択を行った結果,全ての母数効果を組み合わせ効果として取り上げたモデルもしくは性の効果のみを組み合わせ効果から外したモデルが最適なモデルであると推察される.種牛評価を全国規模に拡大することで,より効率的な育種改良を行うことができると考えられる.
