本研究では,台湾における農用地土壌汚染の社会的背景と,汚染及び汚染拡大の分布特性を明らかにすると共に,汚染対策の課題を日本の農用地土壌汚染対策の取組との対比により明らかにした.台湾の農用地土壌汚染は都市計画農業区やその周辺地域に多く,農村地域にある違法操業を含む工場排水の農業用排水路への放流が主因である.台湾の農用地土壌汚染対策による汚染の除去は対症療法的であり,工場排水への対策や未処理の底質による汚染の拡散や蓄積の防止を考慮した対策は取られない.本稿では,このような事態への対応方策として,①現在の点的な対症療法的対策から日本の農用地土壌汚染対策のような面的かつ抜本的な対策への改善,②工場の立地規制や排水基準の見直し,違法操業の取り締まり,③都市計画区域外の農業区については圃場整備事業等を契機とした土地利用の整序,を提案した.

This study focused on Subjective Well-Being(SWB) of residents in rural area, and investigated how SWB of residents and its determinants differ between areas with different features. We employed the capital approach in this study to analyze how combinations of different capitals would influence on residents' SWB. The city of Nantan was chosen as a case study area, which was divided into three parts (urban/suburban/rural area) using statistical data. Then we compared the mean SWB of residents, and conducted multiple regression analysis to identify which factor had significant effect on SWB of residents in each area. Our analysis clarified that personal factors such as income and health status had similar effect on SWB in all areas while factors related to living circumstances such as road conditions and trust in neighbors had different effect among areas.

This study addresses pertinent factors in sustaining small-scale water supply cooperatives (SWSCs). Based on our survey results, we identified three important areas to be addressed prior to the construction of water-supply infrastructure: (1) Identify the characteristics of communities to be involved in an SWSC, so that the SWSC can understand the shared interests of prospective members, (2) Identify the experiences of local communities in rule-making, for example, those based on neighborhood associations, and (3) For those communities that cannot fulfill (1) and (2), capacity-building among member residents is necessary, if they are to achieve the public or common interests of members through the enactment of SWSCs.

農山村地域では,過疎化・高齢化に加えて混住化や就業機会の多様化により,地域コミュニティが弱体化し,従来コミュニティが担ってきた様々な地域環境の維持・管理が困難となる地域が増加している。地縁型の町内会や各種団体だけでは解決しきれない問題に対応することを目指して新たな自治組織が設立される例が増えており,例としてまちづくり協議会等が挙げられる。また,複数集落による集落機能の相互補完や自治範囲の見直し等,新たなコミュニティの将来像を策定することを目指した「集落機能再編促進事業」(農林水産省)や,農業者だけでなく地域住民や自治会・関係団体等の多様な主体が幅広く連携して新たな活動組織を形成することを要件とした施策,「農地・水・環境保全向上対策事業」(農林水産省)など,組織変革を伴う集落機能の再編は国の事業にも取り入れられている。このように,住民自治組織の再編や新設が実施され,事業の受け皿となる事例が増えており,受け皿組織から地域住民へのスムーズな情報共有は,特に住民参加型の事業・取組における住民の理解・参加の促進のための重要な課題である。地域内での情報や知識の共有には回覧板や広報紙など様々なメディアが利用されているが,対面での情報伝達である口コミも依然として重要な手段の一つであり,その際に活用されるのが地域内のネットワークである。ネットワークは,コミュニティの持つ機能を表現する概念として多くの分野で注目が集まっている社会関係資本(ソーシャル・キャピタル)に関する多様な定義においても,中心的な要素とされている。2000年以降,人や組織等の間のネットワークを定量的に評価する社会ネットワーク分析が,様々な分野で導入されつつある。社会ネットワーク分析の特徴は,社会現象を,人や組織の持つ属性や規範から説明するのではなく,人のふるまいや思考・行動を規定する要因として関係性やその強度といった社会構造から説明を試みる点にある。農山村地域において幅広く住民参加が求められる事業における情報共有の実態を明らかにすることにより,事業の受け皿組織の選定や構成等のための重要な知見を得られることが期待される。そこで本研究では,集落・旧村範囲の農山村地域においてSNS(Social Networking Service)の活用に取り組んだプロジェクト(III章にて詳述)を事例として,受け皿組織から他の住民への情報共有の実態と促進に向けた課題について,特に地域内ネットワークが活用される口コミに着目し,社会ネットワーク分析により,個人や組織で構成される社会構造から定量的に明らかにすることを目的とする。

症例は53歳の男性.工事現場のコンクリートから突出した鉄筋の上に座り受傷した.その後肛門痛があったが軽度であったため3週間放置し,症状悪化後近医を受診,外傷性直腸損傷による直腸周囲膿瘍の診断にて当院紹介入院となった.入院時は肛門痛著明で歩行困難であり,脱水状態であった.直腸指診では直腸後壁側に半周性の直腸壁欠損があり,外傷性直腸穿孔の診断で,双口式人工肛門を造設し,直腸周囲膿瘍のドレナージ術を施行した.未治療の糖尿病があり,膿瘍の改善が不良で治療に難渋したが,直腸穿孔部が閉鎖していることを確認の上,術後7カ月後人工肛門を閉鎖した.杙創性直腸穿孔は通常受傷後直ちに治療される場合が多いが,経肛門的な直腸損傷の場合,症状が乏しい場合があり,本症例のように受診が遅れることがある.受診の遅延は治療の難渋につながり,合併症の頻度を高めるため,早期の診断,治療が予後の改善には重要である.

An availability of the Internet in rural area is now close to that in urban area in Japan. But generally, it is still considered that it's difficult for inhabitants in rural area, who are mostly elderly, to use the Internet. But communication via the Internet is very important for rural inhabitants to recognize what matters in their areas and how to solve it themselves, cooperating with various kinds of stakeholders. In this research, we identified the current condition of digital divide in 3 small regions in Kyoto prefecture and discussed what the causes of it are and how to improve such condition from now on.

農村地域においてもインターネット利用環境の整備が進んでおり,SNS 等のソーシャル・メディアには地域のための新たな交流の場としての役割が期待される。一方で,そのようなツールの利用が個人的なものに偏ると,逆に地域意識が低下し,地域コミュニティが一層弱体化することも懸念される。本研究では,SNS の利用タイプ間における地域意識の差について比較分析を行った。その結果,1)地域意識は外向き・内向きで異なること,2)実名SNS の利用者では,特に外向きの地域意識が高いこと,3)いずれの地域意識についても農村部の方が都市部より高いこと,4)将来,実名SNS の利用により地域外との交流の活発化が期待できることなどが明らかとなった。

In recent years, foreign tourists have increased in Japan. This research carried out a questionnaire survey for residents in Ine-town, Kyoto Prefecture, with focuses on the local resident's awareness, to determine factors affecting acceptance of foreign visitors and the potential impact of ‘over-tourism’. As a result, "positive evaluation for inbound tourism" and "inter-cultural acceptance" directly had a positive impact on acceptance of foreign visitors. The higher the evaluation on the economic and social effects of inbound tourism is, the more foreign tourists are accepted. The more the local residents accept the foreign culture, the more favorable it is for foreign tourists and the more they are welcomed. The evaluation on ‘over-tourism’ has indirectly and negatively influenced the "acceptance of visitors" through "positive effects of inbound tourism" and "inter-cultural acceptance". It is necessary to consider the attributes of local residents when planning and implementing efforts to improve the local residents' acceptance of visitors and lower the negative evaluation for ‘over-tourism’. This study has a significant contribution to establishment of the social system for inbound tourism in rural areas, from the viewpoint of understanding of local residents' awareness. Sustainable inbound tourism can be expected by clarifying the factors contributing to acceptance of visiting guests and potential impacts of ‘over-tourism’.

C/CA系のHD療法においてHD開始15-30分前後に補体活性化により患者白血球数が一過性に減少することが報告されてきた。一方、補体活性化作用のない抗OKT3モノクローナル抗体投与後も一過性の白血球減少を生じ、抗体結合後の荷電の変化によることも推察された。このため陽(陰)イオン交換樹脂を用いて白血球に対する吸着率を検討した結果、HD患者白血球では陰イオン交換樹脂に対する吸着率が高いだけではなく、正常白血球ではみられなかった陽イオン交換樹脂にたいしても80%以上の吸着率を示した。またHD患者白血球はHD開始15分前後に陰イオン交換樹脂に対する吸着率が低下し、この時期の患者血漿を正常白血球に添加、incubateすると正常白血球の陰イオン交換樹脂に対する吸着率が増加した。

In this study, we conducted a qualitative research to investigate how regional living conditions affect residents' subjective well-being. We did interviews with 21 people who lived in Suzu city, a rural area located at the northern edge of the Noto peninsula, Japan. A series of interviews was recorded and was transcribed after the interviews for coding and analysis. By coding the transcribed scripts, we found some common regional factors which affect residents' well-being. In general, factors related to natural capital and time usage have positive affect to well-being. On the other hand, factors related to social capital have both positive and negative influences to well-being while factors related to physical capital have mainly negative influences on well-being. These results suggest that we should add some regional factors reflecting characteristics of rural life when we conduct questionnaire survey to investigate residents' well-being in rural area.

韓国は1990年代の後半から社会的問題として台頭した,基盤施設が伴わない無秩序的な開発行為(以下,乱開発)を改善し,計画的な土地利用・開発を誘導するため,「国土基本法」と「国土の計画および利用に関する法律」(以下,国土計画法)を2002年に制定,2003年に施行した。しかし農村地域の乱開発は深化し続けており,上記法律に基づいた国土利用管理体系による農村の計画的開発及び保全の有効性に疑問が提起されている。ソンミリヨンらは,国土計画法の施行に際して,新たに導入される制度の特徴と予想される問題点を分析した。また,ソンジュインらは,国土計画法の導入以後の小規模分散開発など農村土地利用の問題点を全国レベルの視点から全般的に分析した。本研究は国土計画法における国土利用計画及び土地利用管理体系の特徴を明らかにすると共に,特に具体的事例から,土地利用管理の根幹になる都市(郡)管理計画(以下,都市管理計画)の農村土地利用管理手段としての限界と,農地法などその他の農村地域の土地利用に関する個別法制度の問題点を明らかにすることを目的とする。

To revitalize rural areas, information originating from rural areas has been highly demanded. Social media such as SNS has been getting essential for information communication, however, because of the aging and depopulation, rural areas lack actors undertaking that role. In this study, we focused on information communication activities using Facebook pages by Community-Reactivating Cooperator Squad and clarified the actual situation and challenges of SNS use. As a result, the utilization ratio of the Facebook pages was low, and a few differences were observed in ways of using it between effectively using areas and not effectively using areas.

In this research, we tried to verify the role of the administration to invigoration of a less-favored area anew. We selected the Sugiyama district in Maizuru-city as study area, and analyzed the process which the area invigorated paying attention to the action of administration. Result from listening survey uncovered that suitable care of administration was functioning as a important factor behind the Sugiyama district accomplished endogenous development. Although "the new public" which consists of various subjects as a bearer of an area is expected in recent years, we made it clear that the role of administration is still important.

肛門部尖圭コンジローマ症例をHPVタイプにより分類し,疫学的,臨床的違いの有無について検討した.液相ハイブリダイゼーション法によりHPVのハイリスク型,ローリスク型の有無を調べえた166症例をHPVタイプ(ハイリスク:H ローリスク:L 陽性+ 陰性-)で分類すると,H+L+型39例,H+L-型2例,H-L+型97例,H-L-型28例であった.H+L+型は男性が多く,H-L-型は女性が多く年齢層が高いという傾向があった.居住地では,千葉北西部でH-症例,東京都内でH+症例が多く,疫学的違いがある可能性が考えられた.肉眼型では,H-L-型で散在型が多く臨床的な違いのある可能性も示唆された.HIV陽性例はH+L+型7例,H+L-型1例,H-L+型2例にみられH+症例で高率であった.術後の再発率は全体として33%であるが,HIV陽性例では67%と高く,免疫抑制状態が再発に関連すると考えられた.

There is a growing attention to the concept of the coupled social-ecological system (SES) where human and biophysical systems are intricately linked. This recognition calls for a more integrated approach for the sustainable use and conservation of ecosystems and biodiversity. In this paper, we identified hotspot of social-ecological production landscapes (SEPLs) of the Noto Peninsula, Ishikawa, where both human-perceived social and physically measured ecological values overlap. Questionnaire survey was conducted targeting at residents of and (potential) visitors to the Noto Peninsula to explore human-perceived landscape values of the study area. The result of questionnaire survey was geo-referenced to develop value surfaces of twelve landscape values with a Geographic Information System (GIS). In addition, the ecological value map was developed employing Satoyama Index as an indicator for agricultural landscape heterogeneity, which was then overlaid with the maps of landscape values to identify spatial distribution of hotspots of social-ecological production landscapes. Our analysis identified that hotspots existed across the study area, especially in coastal plains of suburban areas where diverse land use with multiple landscape values were observed. We concluded the paper by discussing the limitations and potential managerial implications of hotspots mapping of social-ecological production landscapes for sustainable landscape management.

Nowadays, community-based organizations in rural areas are restructured to complement the weakened functionalities of communities. Such newly created organizations often undertake some projects purposes of which are to vitalize their own areas. It's important to share information and knowledge of the projects requiring citizen participation. In this study, we clarified the actual situation and problems of information sharing in rural areas by utilizing social network analysis. Through a questionnaire survey conducted at 3 districts in Kyoto prefecture, we made 2-mode graphs and analyzed network indicators obtained from the graphs. We found that information is mainly conveyed in person or through regional media and residents with information are located in advantageous positions in the regional social networks.

症例は78歳男性,高度便秘のために肛門痛が持続し来院。尿閉,便失禁状態であり,CT上直腸壁の著明な拡張と間膜組織の炎症像があり切迫破裂状態と判断した。保存的に加療し,排尿障害,排便障害ともに1週間程度で改善し退院した。退院後,自己判断で緩下剤の服薬を中止したところ症状の再燃を認めた。再度保存的加療で改善し,その後は内服を継続的に使用することで経過良好となった。便秘による特発性巨大直腸症は時に穿孔を起こして,重篤な状態を招くことがある。高度便秘で肛門痛,排尿困難,便失禁を呈する症例では,宿便による直腸切迫破裂というべき症例の存在に留意し,慎重に診療をすべきである。
