「桂」に類する生薬(以下「桂類生薬」)は,古典医書中,多くの薬名で記述されており,それらの薬名と基原の対応は現在も未解決の問題である。我々は『本草経集注』「序録」の「桂」の長さと重さに関する記述に,先に明らかにした同書の度量衡の換算値を適用することにより,当時「桂」として流通していた生薬は,現在のシナモンスティックに相当するCinnamomum cassia の枝皮(カシア枝皮)であったと強く推測した。推測の妥当性を確認するためカシア枝皮に含まれるケイアルデヒドとクマリンを生薬市場で入手した種々の桂類生薬と定量比較したところ,カシア枝皮中の2成分の含有量は,市場で上品とされる日本薬局方適合ベトナム産ケイヒ(C. cassia の幹皮)と近い値を示し,この推測を支持する結果を得た。C. cassia の幹皮の代わりに枝皮を医薬品として応用できる可能性があると考えられた。

医療安全委員会では,安全に漢方方剤を使用するための啓発活動を行っており,前回,日本医療機能評価機構の薬局から登録されたヒヤリ・ハット事例を分析した。今回は,同機構の医療機関から登録された医療事故とヒヤリ・ハット事例を分析した。漢方製剤が関係する事例は626件であった。医療事故には,薬剤性肝障害事例があった。 ヒヤリ・ハット事例に関しては,処方時では漢方エキス製剤の1包の内容量の勘違いによる用法用量の誤り,調剤時では製剤番号・外観の類似や漢方処方名の類似による調剤の誤り,投薬時では漢方処方名まで確認せずに,漢字表記やメーカー名だけで判断することによる投薬の誤りがあった。ヒヤリ・ハット事例は当事者本人や同職種者に限らず,他職種者や患者本人から発見される事例も多かったことから,ヒヤリ・ハット事例は同職種者間での共有に留まらず,他職種者とも共有することが,医療安全の推進につながると考えられた。

シアノヒドリン〔2〕がブルシンと錯体をつくり,一方の光学活性体へ変換されることを見いだした。たとえば,(±)-1-シアノ-2,2-ジメチル-1-フェニル-1-プロパノール〔2a〕をメタノール中ブルシンで処理すると,94%eeの(+)-〔2a〕へ定量的に変換された。同様な方法で,(+)-〔2b〕,(+)-〔2c〕,(-)-〔2d〕,(-)-〔2e〕,(+)-〔2p〕および(-)-〔2u〕も得られた。

神経変性疾患の連続74例について気血水スコアを用い気虚,気鬱,気逆,血虚,瘀血,水滞の有無(証の病態6項目)を評価し,同年代のその他神経疾患の連続149例を比較対照として比較検討した。年齢,性別,重症度も共変量とした多変量解析の結果,神経変性疾患ではその他の神経疾患と比較して血虚,水滞,気鬱の順で関連性が高く,調整済みオッズ比(95%信頼区間)はそれぞれ3.02(1.43-6.48),2.37(1.13-5.11),2.33(1.01-5.44)だった。神経変性疾患と最も関連性が高い証は血虚であった。四物湯類(四物湯加減)の処方を考慮することは,患者の苦痛軽減に寄与できる可能性がある。自覚症状に加え脈候,舌候,腹候など東洋医学的な尺度を用いた治療効果の判定が必要である。

感冒147例の東洋医学的病態像を検討した。問診票と東洋医学的診察により,以下の三群に分類して証に合わせた漢方方剤を投与し,発病時期・主訴・自他覚症状・体質などを比較検討した。(1)風寒感冒は42.9%で葛根湯・桂枝湯が多用された。冬季に好発し夏季に少なく,胃弱者や冷え性体質者が多かった。悪寒(主訴に多い)・四肢冷感などの表寒証が特徴的症状で,浮脈以外の他覚症状は少なかった。(2)風熱感冒は46.3%で,銀翹散が多用された。春季に好発し秋季にもみられ,暑がりや壮健体質者がやや多かった。咽頭痛(主訴に多い)・咽頭発赤・熱感・熱感触知・冷水を好む口渇などの表熱証,裏熱証,乾燥症状さらに少数の寒証症状など多彩な症状がみられた。(3)その他は10.9%で駆風解毒湯が多用され寒熱錯雑証と考えられた。

盗汗治療代表的方剤の当帰六黄湯(李東垣『蘭室秘蔵』)の成立過程を構成生薬と各時代の盗汗病態理論より検討した。黄耆の配合は主に陽虚で出現という漢隋代盗汗理論,補血滋陰薬の生地黄・熟地黄・当帰の配合は陰虚という宋代盗汗理論,清熱薬の黄連・黄芩・黄柏の配合は陰虚の熱により津液が押し出されるという宋代盗汗理論に基く。後者は特に劉完素『黄帝素問宣明論方』中の盗汗治療方剤大金花丸の影響が大きい。本剤は各時代の盗汗学説の集大成のために高い有用性を備えたのであろう。医書の諸説より,本剤の適応病態や問題点を検討した。弱い熱証(『丹渓心法』),強い気虚証(『張氏医通』『丹渓心法』),強い陰虚証(『医学心悟』)を呈する盗汗病態には不適当との説。脾胃を損傷しやすい(『医方切用』)との指摘。自汗への応用も可能との説(『医学正伝』『景岳全書』)などがある。これらより本剤の適応は陰虚証と熱証がほぼ同様程度,気虚証はより軽度な病態であり,このような病態では自汗にも使用可能といえる。

Paulo Coelho is one of the most widely read authors in the world today.The main aim of his works is to bring things out of hiding. He considers the world as the place where people make their dreams come true, but they seem to have been kept away from power to do so. In his writings, something inevitable happens and the protagonists are forced to face with their lives. Examining themselves thoroughly, they find out false stories which have been told over and over since their childhood and that finally they accepted. On the other hand, they start to search for truths. Through their eyes, we can notice the world is much richer than we think. Coelho tries to let us know that we have power and possibility so that we can live as we really want.

The disability movement has struggled hard to gain acceptance for the 'big idea' that disability can be considered as a form of institutional discrimination or social oppression. As these 'social model' have gained political currency so they have engendered a profound reexamination of British social policy. Disability movements has been emphasized rights of disabled people, futhermore, they has been proposed that self-assessment and self-management in human services. Disability movements promoted a sustained campaign for direct payments legislation. Community Care (Direct Payments) Bill was published in November 1995. The legislation came into force the following year, permitting local authorities to make direct payments to people assessed as needing community care services. Consumerism formed the major basis of this new legislation. But Important value conflicts were apparent in discussion about direct payments in disability mevement. 誘電体結晶にパルス面制御された高強度の超短光パルスを照射すると,100kV/cmを越える最大電場のピコ秒モノサイクルパルスが発生する.これを用いて物質中の電子や分子をコヒーレントに大振幅で運動させることで,新規な物性の発現が期待される.また準DC的強電場印加の手段として物質の内部電場を直接支配できる可能性がある.
