The activity of bathing involves multiple motions that consist of basic abilities and functions of the upper limbs. This paper aims to describe "the motion of stepping over the bathtub" that is a necessary activity of bathing. We describe these motions, and simulate the motion which can reproduced in the rehabilitation room, from sitting to long sitting on a platform. We divided these motions into three phases and explain the movement of normal adults and adults with hemiplegia, as well as the points of treatment about each phase.<br>

In physical therapy for patients with central nervous system disorders, the goal is often the acquisition of walking movement. Physical therapy is required to always consider the risk of falls in hospitalized life and to judge the degree of independence for each individual. The authors believe that it is necessary not only to rely solely on the determination of gait independence, but also to supplement it by observing movement, which is a necessary clinical ability required of a physical therapist. In this paper, we focus on the movement of initiating gait and present approaches for improving lower limb dysfunction while deepening the understanding of normal movement.

Clinically, strengthening exercises for scapular abductor muscles such as serratus anterior is usually prescribed for patients with scapular winging. However, the effect of such exercises on shoulder function is still unclear. The purpose of this study was to examine the changes of muscle strength on the shoulder joint after exercise for scapular abductor muscles. Thirty-six healthy male subjects participated in this study. All subjects were randomly assigned to either the exercise group (EX group) or control group (CO group). Initially, the muscle strengths of scapular protraction (PR), shoulder flexion (FL), abduction (AB), external rotation (ER), and internal rotation (IR) were measured in both groups. The EX group was instructed to perform the modified "elbow push up plus". The CO group did not perform any exercise. The muscle strengths were measured again after the training period. The paired t-test was used to compare the muscle strengths between before and after the training period. The muscle strengths of scapular PR, shoulder FL, AB and IR were significantly increased in the EX group (p<0.05). On the other hand, no significant increase was found in the CO group. Since the movement of shoulder FL, AB and IR involve scapular posterior tilt, upward rotation, and protraction movement, respectively, the increase of scapular abduction strength might have caused the increase in muscle strengths of shoulder FL, AB and IR. The muscle strength of the shoulder complex may depend on the function of the scapulothoracic joint.

A difference of alignment of rear foot between lying down and standing in patients who have sports disorders is often observed. The purpose of this study was to clarify the relationship between the alignment of the rear foot and activities of ankle flexor muscles during the calf-raise. We identified an alteration of the alignment of the rear foot between the prone position to standing by measuring leg heel angle under each condition, and classified subjects into three groups (varus, neutral, valgus) according to angle alteration. In addition, we measured activities of the tibialis posterior muscle, soleus muscle, medial head of the gastrocnemius muscle and lateral head of the gastrocnemius muscle during the calf-raise with surface electromyography. The varus group showed significantly greater activation of the tibialis posterior muscle than the valgus group. Activation of the medial head of the gastrocnemius muscle displayed similar findings with an increase from the valgus group to the neutral and varus groups. These results suggest that if alignment of the rear foot in standing is more varus than in lying prone, sports disorders are potentially more likely in therapeutic calf-raise exercises and in running during training.

During 45°-flexion of the hip joint, actions on the pelvis and integral values of abdominal muscle groups were measured by maintaining SLR at 3 different angles of hip joint abduction (0°, 15°, and 30°) to evaluate the relationship between the two. The results of this study suggest that rotating and anteverting actions occur on the pelvic side ipsilateral of SLR when the SLR is held at 30°-abduction of the hip joint in healthy volunteers. In addition, integral values of the abdominal scalenus ipsi- and contralateral of SLR and those of the rectus muscle of the abdomen ipsilateral of SLR obtained by maintaining SLR at 30°-abduction of the hip joint were significantly larger than those obtained at 0°-abduction of the hip joint (p<0.05). Integral values of the rectus muscle of the abdomen contralateral of SLR increase with the angle of hip joint abduction. These results suggest that attention should be focused on hip joint abduction angles during SLR exercise to increase the activity of the entire abdominal muscle groups.

The important points for improvement of functional impairment of the upper limb are clarification of the impairment level and identification of the tissue related to the impairment. Also, we have to estimate the impairment not only from the perspective of the upper limb, but also from a whole-body viewpoint including the trunk and lower limbs. This paper presents the author's approaches for improving functional impairment.

Lateral movement in the sitting position is a task which is used in evaluation and treatment in clinical practice. Also lateral movement in the sitting position can be seen in many activities of daily living. However, patients with dysfunction of the hip muscles have more difficulty performing lateral movement in the sitting position than healthy person, because, they cannot raise the opposite side buttock from the weight-bearing surface. To do lateral movement in the sitting position is reported to require lateral tilt of the pelvis. The trunk is displaced to the moving side by this lateral tilt of the pelvis. Furthermore, lateral movement in the sitting position requires muscle activity of the hip muscles that hold the lateral tilt position of the pelvis. Lateral tilt of the pelvis in lateral movement in the sitting position is a movement of the hip joint. TO promote smooth lateral movement in the sitting position, it is necessary to know the kinematics of the hip joint. However, the kinematics of the hip joint are different between sitting and standing postures. For this reason, interpretation of the kinematics of the hip joint is difficult in lateral movement in the sitting position. In this paper, we measured COP, and surface EMG of the hip girdle muscles during lateral movement in the sitting position following the procedures of a previous study, and carried out a video analysis of the displacement of the pelvis. We also examined the factors involved in successful lateral tilt of the pelvis in lateral movement in the sitting position.

The purpose of this study was to determine the optimal time of stretching to increase flexibility of the hamstring muscles, as defined by measuring the degree of SLR. Nineteen subjects (ten males and nine females), ranging in age from 21 to 25 years, without known musculoskeletal impairments of their spine or lower extremities, participated. A goniometer was used to measure the range of hip flexion with knee extension (the range of SLR) during the 120-second static stretching. The degree of SLR significantly increased (p<0.05) until 75 seconds under sustained hamstring muscle stretching. No significant increase in the degree of SLR occurred during from 75 to 120 seconds. The results of this study suggest that 75-second duration is the effective amount of time to sustain hamstring muscle stretching in order to increase flexibility of the hamstring muscles.

This study examined the influence of posture change of hemiplegic stroke patients in their forward stepping motion. The results suggest that forward stepping motions differ in terms of joint motion, electromyogram pattern, and center of pressure owing to hip joint and tibial external rotation. This study introduces an intervention based on evaluation of the forward stepping motion.

Scapular movements and activation should be assessed in patients with shoulder disorders. In the evaluation and treatment of scapular motion, the focus must be on scapular muscle activity and scapulothoracic, sternoclavicular, and acromioclavicular joint movements. Evaluation of the relationships between the scapula and other adjacent joints such as the glenohumeral joint and trunk will also be helpful for shoulder rehabilitation. This paper presents an assessment of scapular function based on electromyographic data and a case report.

In rehabilitation, progress in the order of passive, automatic assistance, automatic, and resistance exercises is common. Automatic assistance exercise occupies an important position as preparation for automatic exercise acquisition, but it is easily influenced by the experience and skill of the therapist. Therefore, in this study, we focused on the automatic assistance movement necessary for the handling technique of the upper extremity. The relationship between the amount of upper limb assistance and shoulder muscle activity and the muscle activity pattern between automatic and automatic assistance exercises were examined. The points in the handling technique of the upper limb are described.

Central nervous system disease is a disease that occurs in the central nervous system from the brain to the spinal cord due to various reasons. The pathology is diverse, and the symptoms are different even for the same disease. In order to develop physical therapy, it is important to evaluate the posture and movements of actual subjects. Physical therapy technique is considered to be whether the therapist can respond well to changes in the subject in a clinical setting. Here, we describe our own approaches for physical therapy for central nervous system diseases from the viewpoint of imitation, guidance, and voluntary movements.

The foot region plays an important role in postural control. It is often observed that improvement of foot and foot joint functions leads to improvement of movements of the lower limbs, pelvis, trunk, and eventually the entire body. We generally use the position of plantar pressure as a parameter for the evaluation of the foot region and foot joints, because the center of plantar pressure (COP) provides important information. In patients with dysfunction in the foot region, COP is often deviated to the big or little toe side during movements, or induction of COP from the little toe side to the big toe side is often difficult. In therapeutic exercise for control of COP in such patients, closed kinetic chain exercise is considered effective. In such exercises, monitoring of the necessary activities of the peripedal muscles caused by changes in COP is important. In this study, we examined the electromyographic integral of the peripedal muscles caused by changes in COP using a ground reaction force plate and electromyography. In the measurement, a surface electromyograph, Mlyosystem 1200 (NORAXON Co.), was used for recording electromyography. For electromyography analysis, MyoResearch (NORAXON Co.) software was used for calculation of the integrated values of the electromyography. A force plate (Anima Co.) was synchronized with the surface electromyograph. Weight loading on the forefoot and hind foot induced characteristic muscular activity patterns of muscles around the foot. For changing COP, muscular activity of muscles around the foot is necessary, and it is important to know that the muscular activity pattern varies depending on the position of COP.

The authors use top-down evaluation to determine impairments and perform physical therapy regardless of musculoskeletal or central nervous system disorders. One of the abnormal postures in patients with musculoskeletal disorders is a hunched posture with trunk flexion. In addition to the influence of the trunk, hunched posture tends to become more prominent with the influence of the lower limbs, lifestyle, and aging,. In this paper, we explain how to weigh the relevance of the problems considering the effects of not only the trunk but also the lower limbs, when considering the problems of trunk flexion posture in musculoskeletal disorders. In addition, we also explain the main points for performing examinations and physical therapy for problems of the trunk, while presenting posture and motion analysis of cases and electromyogram data of healthy subjects.

This study examined the motor imagery effect using F-waves in order to verify a report that performance and body function were improved by motor imagery. To improve motor accuracy, motor practice for 30 seconds or 1 minutes before motor imagery was important. For the assumed scenarios in activities of daily living, motor imagery of compound movements must include the shoulder, elbow, wrist, and finger. Under these circumstances, it is desirable to set the task difficulty according to individual motor imagery ability. In the future, thorough investigation of F-wave forms will be necessary for the interpretation of the excitability of the spinal neural function during motor imagery.

The purpose of this study was to investigate whether ROM on pronation of the transverse tarsal joint affected both pelvic movement and EMG activity of gluteus medius muscle on hemi-standing. We researched two experiments about ROM limitation on pronation of the transverse tarsal joint. One was the inherent limitation: it was classified as the hypo-mobility side (ROM on pronation of the transverse tarsal joint <10°) or normal-mobility side (ROM on pronation of the transverse tarsal joint ≥10). Another was artificial limitation by meanes of taping. It discriminated between the non-taping group and the taping group (ROM on pronation of the transverse tarsal joint=0°). In each experiment, subjects were instructed to maintain hemi-standing with 90 degree hip flexion on the unsupported side. We compared the degree of eversion of the subtalar joint, pelvic movement and EMG activity of the gluteus medius muscle on the supported side on hemi-standing on both the hypo-mobility side and the normal-mobility side; and taping group and non-taping group in two experiments, respectively. In both of the two experiments on ROM limitation on pronation ofthe transverse tarsal joint, pelvic movement tended to increase pelvic elevating to the opposite side on hemi-standing or the degree of eversion of the subtalar joint on the supported side significantly increased on hemi-standing, and EMG activity of the gluteus medius muscle significantly increased on hemi-standing. We would like to emphasize that pelvic movement was altered by ROM limitation on pronation of the transverse tarsal joint. The main reason was that ROM limitation on pronation of the transverse tarsal joint on the supported side caused excessive eversion of the subtalar joint on hemi-standing. And excessive eversion of the subtalar joint caused the obstruction to shift the pelvis in the direction of the supported side for hemi-standing. We suggest that the pelvic elevation to the opposite side was caused to compensate for obstruction when shifting the pelvis in the direction of the supported side.

The first edition of this book was published in 1946 and it has now reached the seventh edition. The principal change made in the sixth edition was the information on the testing of infants and young children. And also the hip extension test in the supine position has been added. If for any reason a patient cannot lie prone, the new method is used.

We conducted physical therapy for a patient who experienced pain in the right gluteal and lateral femoral regions during walking following right hip replacement. Pain appeared due to flexion, adduction, and internal rotation of the right hip joint accompanied by exaggerated anterior inclination and right rotation of the pelvis on right heel contact through to right mid-stance. Physical therapy was conducted for the limited range of right hip joint motion and muscle weakness around this joint, which was assumed to be the cause of the abnormal gait and pain. Following this, standing step practice (Physical Therapy A) was conduced. After 6 weeks of Physical Therapy A, the limited range of right hip joint motion and muscle weakness around the joint had improved; however, the gait and pain had not improved. Thus, the therapy was re-evaluated, and bridge exercise was added to Physical Therapy A (Physical Therapy B). Physical Therapy B enabled induction of muscle activity and contractile patterns similar to those occurring during walking with respect to the muscle activity around the right hip joint that is necessary for right heel contact through to right mid-stance. A single session of Physical Therapy B improved the gait and reduced pain. Therefore, we concluded that the promotion of complex muscle activity, not in a single direction, and improvement of the muscle strength and contractile pattern are necessary in physical therapy for problematic muscle weakness.

In this study, we examined the patterns of activity of the trunk muscles during weight shifting to one side of the lower limbs in the standing position in five patients with Parkinson’s disease and trunk malalignment. In the movement task, the patients first shifted their weight to one side of the lower limbs and attempted to move the maximum distance in which they could shift their weight to the moving limb in 1 s. Then, the patients held the posture with the maximum weight shift for 1 s and returned to the starting position in the next 1 s. The patterns of activity of the bilateral multifidus, iliocostalis, and longissimus muscles as well as that of the internal oblique and gluteus medius muscles on the weight-bearing side were analyzed. Compared with that of the starting position, we observed an increase in thoracic kyphosis during the movement task, and this was common to all of the patients. In addition, although we observed coronal plane inclination of the pelvis associated with hip joint movement during the movement task, 4 of the 5 patients did not show any change in trunk alignment compared with that in the starting position. The patterns of activation had the following characteristic tendencies: activation of the bilateral longissimus muscles was observed in the starting position in all patients, activation of the longissimus and iliocostalis muscles on the nonweight-bearing side was increased in 4 of the 5 patients, and activation was weak in the internal oblique and gluteus medius muscles on the weight-bearing side in 4 of the 5 patients. We observed a significant difference between the muscle activation patterns reported by previous studies of healthy subjects and those of our present study. When using weight shifting to one side of the lower limbs in the standing position as an exercise therapy for patients with Parkinson’s disease, activation of the internal oblique, gluteus medius, and multifidus muscles on the weight-bearing side increases. Therefore, it is necessary to implement exercises with the intention of performing the movements, with the pelvis in a horizontal position and the trunk in a vertical position. Moreover, it is necessary to ensure that activation of the bilateral longissimus muscles or that of the iliocostal muscle on the nonweight-bearing side does not differ from the activation of the starting position.

Quadriceps setting is widely used as a muscle strengthening exercise for the quadriceps femoris muscle. It is frequently employed from the early post-operative stage for patients receiving knee joint replacement surgery, such as total knee arthroplasty, at our hospital. Quadriceps setting is frequently performed with ankle joint dorsiflexion in the supine position. Patients performing quadriceps setting often say “I can’t understand where to flex the muscle in the leg.” Therefore, we analyzed this exercise in patients receiving knee joint treatments such as total knee replacement surgery. The patients were explained in a simple and easy-to-understand manner the correct way of performing quadriceps setting for efficient restoration of the quadriceps femoris muscle activity. In addition, we devised a quadriceps setting exercise involving knee extension and ankle dorsiflexion with the heel pressed down, encouraging simultaneous contraction of the quadriceps and gluteus maximus muscles, and analyzed the effectiveness of this exercise on quadriceps femoris muscle activity.
