〔目的〕膝前十字靱帯(ACL)再建術後1年のHop test回復状況を調査し,回復基準に至りにくいHop testを明らかにすること.〔対象と方法〕ACL再建術を施行した218症例を対象とし,術後1年のVertical Hop(VH),Single Leg Hop(SLH),Triple Hop(TH)が健患比90%以上に回復した割合と3種のHop test全項目が健患比90%以上に回復した割合を調査した.〔結果〕Hop testの回復割合はVH 67.9%,SLH 78.4%,TH 78.9%であり,VHの回復割合が最も低かった.Hop test全項目が健患比90%以上に回復した割合は59.2%であった.〔結語〕約4割の症例においてHop testが健患比90%未満であり,特にVHの回復率が低かった.

症例は39歳,男性.亜急性に進行する下肢筋力低下で入院した.末梢神経障害を認め,ステロイドで改善したが,減量開始から1年で症状が再燃した.再入院時の胸部単純CT(computed tomography)にて乳腺腫大を認め,女性化乳房がPOEMS(polyneuropathy,organomegaly,endocrinopathy,M-protein, skin changes)症候群(Crow-Fukase症候群)を疑う契機となり,血管内皮増殖因子(vascular endothelial growth factor:VEGF)の高値や骨硬化性病変等から診断に至った.胸腹水や浮腫はなく,M蛋白は陰性であった.骨病変に対する放射線治療のみで良好な経過を得た.本例は女性化乳房の検出に胸部CTが有用であった.

Spinocerebellar ataxia is a group of neurodegenerative disorders clinically presenting adult onset cerebellar ataxia. To date, 21 different genes (SCA1, 2, 3, 5, 6, 7, 8, 10, 11, 12, 13, 14, 15, 17, 23, 27, 28, 31, 35, 36 and DRPLA) and additionally 10 different gene loci (SCA4, 18, 19, 20, 21, 25, 26, 29, 30 and 32) are identified. Among these, SCA6 and SCA31 are the two common diseases clinically presenting as a relatively predominant cerebellar syndrome, whereas Machado-Joseph disease/SCA3, DRPLA, SCA1 and SCA2 are SCAs often associated with extra-cerebellar manifestations. SCA31 is a late-onset purely cerebellar ataxia caused by a complex pentanucleotide repeat containing (TGGAA)n lying in an intronic region shared by two genes, BEAN (brain expressed, associated with NEDD4) and TK2 (thymidine kinase 2). In situ hybridization analysis in patients' Purkinje cells demonstrated that pentanucleotide repeats transcribed in BEAN direction form RNA aggregates ("RNA foci"), and essential splicing factors, SFRS1 and SFRS9, bind to (UGGAA)n, the transcript of (TGGAA)nin vitro. Our preliminary data also demonstrated that (UGGAA)n is toxic when expressed in cultured cells. These findings may imply that RNA-mediated pathogenesis is involved in SCA31. Further studies are needed to explore precise mechanism of this disease.

室温のゆらぎ条件 (28~30℃: 1/fゆらぎに基づくもので, 平均45分の短周期ゆらぎと平均90分の長周期ゆらぎ) と28℃で一定の定常条件で温度制御を行い, 終夜睡眠ポリグラフおよび体表面皮膚温を測定した. さらに, 起床時に主観的睡眠感と温熱感の調査を行った. 被験者は健康な男子大学生12名で, 適応夜3夜の後, 3条件の実験を無作為の順序で行った. 睡眠時間は8時間 (11時30分就床, 7時30分起床) とし, 睡眠中はトランクスのみの裸の状態で寝具の使用も禁止した. 有効な10名についての結果では, 定常条件において stage IIの出現率が長周期ゆらぎ条件よりも多いことが示された. また, REM+徐波睡眠の全就床時間に対する割合を睡眠質の指標とした場合, ゆらぎ条件と定常条件間で有意差が認められ, ゆらぎ条件のほうがやや良い睡眠であることが示唆された. その他の各種睡眠パラメータおよび主観的睡眠感では条件間で有意差は認められなかった.

Upon completion of the workshop, a survey was conducted to measure attitude changes in participants' views and images of human rights. The results revealed that, on attending the workshop, students showed an improvement in their understanding of human rights. They learned that the term "human rights" is not just an abstract concept related to philosophical principles such as virtue and justice, and became aware that human rights affect and permeate their daily lives. Finally, students recognized the need to give more importance to the practice of human rights. Based on these results, possible directions for human rights education were discussed.
