Falls are common in elderly patients and are often serious. Several drugs have been associated with an increased risk of fall. Older adults often take multiple drugs for chronic diseases, and thus may be at increased risk from drugs associated with fall. We investigated the association between drug use and falling in hospitalized older people, with the goal of identifying medications that may increase the risk of a fall. A retrospective case control study was performed at the National Hospital Organization Kumamoto Saishunso Hospital in Japan. Medications taken by patients who fell (n=57) were compared with those taken by patients who did not fall (n=63). The median age (interquartile range; IQR) of the fall and non-fall groups were 75.0 (67.0-83.0) and 80.0 (70.3-84.5) years, respectively. The characteristics of the two groups were similar, with no significant differences in age, sex, or body weight. The probability of falling increased when the patients used zolpidem [odds ratio (OR)=2.47; 95%CI: 1.09-5.63; p<0.05] and calcium channel antagonists (OR=0.299; 95%CI: 0.13-0.68; p<0.01), and was also related to physical factors (OR=2.27; 95%CI: 1.01-5.09; p<0.05). Elderly patients taking zolpidem may fall due to sleepiness, and blood pressure control may be important to prevent orthostatic high blood pressure. In the treatment of elderly people, medical staff should try to choose drugs that prevent fall or are not associated with falling.

The research subject is the former Tetsuma Akaboshi House, designed by Antonin Raymond. The viewpoints based on the conservation succession process are following.</p><p>1. Institutional status such as rights related to the owner</p><p> The system of rights related to the owner effectively responded to changes in the social situation.</p><p>2. Daily use by the owner and resident</p><p> It was effective that the owner and resident lived with the philosophy of supporting the building's maintenance and inheriting.</p>

<p>This paper aims to clarify how the living space of the townhouses is used during float festival, the characteristics of the space for watching it, and their transformations. Findings are as follows. First, the living space is open to the street, which has become more prominent at present. Hospitality has not changed, but the visitor type has been unified and fixed. Second, basically, the watching space faces the street. In addition, view of the street, daily use and floor finishing affect its selection. At present, a new watching space appeared with the change of living space.</p>

軍港都市の都市計画や都市形成に関して、従来は専ら海軍の存在・役割やその画一的なグリッドプランが注目・強調されてきたが、近年では主体性を発揮する地域の姿や空間の多様性が見出されつつある。こうした状況を踏まえ、本研究では、軍港都市呉における市街地形成過程そのものが分節的な性質を有していたことを確認したうえで、その性質を発現要因とともに体系的、構造的に整理することを目的とする。具体的には、地図資料により都市形成の空間的な実態を把握したのちに、文献資料により個別の開発事業やその背景を調査し、呉の都市建設期における市街地形成の地理的・空間的な展開とその背景を日本海軍の策定した呉の都市建設計画との関連性に着目しながら整理した。その結果、呉の市街地形成過程の分節性は、①事業手法の選択性、②建設過程の段階性、➂街区形状の適応性という3つの要素に整理できること、こうした分節性の発現要因は建設計画・制度と地域の社会的条件に大別できることが明らかになった。

Urbanism Places Exhibition 2018 was held at 55HIROBA of the Shinjuku Mitsui Building in September 2018. The central concept raised by the exhibition on urban-planning-based public spaces was “planning heritage as real place”. The exhibition consisted of 4 thematic programs, which had a common principle not to disturb day-to-day activities in the planning heritage place. On the other hand, the exhibition itself was expected to derive public space functions and possibilities. The visitors’ evaluation made clear challenges on the balance between the conceptual pursuit and the visitors’ satisfaction as well as the differences of the evaluation between experts and non-experts.

Over a past few years, we have determined crystal structures of several metal-containing oxygenases including indoleamin-2, 3-dioxygenase and some P450s which have interesting chemistry and play important roles in metabolism. Molecular mechanisms of these oxygenases are discussed based on the crystal structures.

The micro-organisms in 35 samples obtained from inside moats of the Imperial Palace of Japan in Tokyo during the years 2000-2004 were studied, and only the additional species unrecorded in the previous survey during the years 1996-1999 are enumerated. They are 49 species of 35 genera, viz. 10 species of 8 genera in the Protozoa (8 species of Rhizopoda, and 2 species of Chiliatea), 22 species (including variety and forma) of 12 genera in the Rotatoria, 1 species of the Gastrotricha, 1 species of the Nematoda, 15 species of 13 genera in the Arthropoda (13 species of Crustacea, and 2 species of Insecta). In 20 samples obtained in the previous survey, 81 species of 40 genera were recorded (Tanaka & Takeda, 2000), although the nematod worms and insect larvae were not examined. As a result, at present, the micro-organisms are known by a total of 130 species of 63 genera from four inside moats, Kami-dokan-bori, Naka-dokan-bori, Shimo-dokan-bori and Hasuike-bori, and from a small pond, Hisago-ike. Comparing the limnological fauna of the inside moats of the Imperial Palace of Japan with that of some ponds in the Akasaka Imperial Gardens in Tokyo recorded by Tanaka et al. (2005), it is definitely said that the inside moats of the Imperial Palace of Japan are still kept in good condition due to its large scale, though the Imperial Palace of Japan is surrounded by artificial buildings in Metropolitan Tokyo.

全身疾患を有する患者に対して,緩和精神安定材を用いる静脈内鎮静法は有効であるが,薬剤静脈内投与の場合,過量投与や覚醒遅延などの問題が生ずることがある.最近では,緩和精神安定剤の拮抗薬フルマゼニルが市販されるようになった.これを有効に利用することは,静脈内鎮静法のリスクを低くするばかりではなく,患者の帰宅可能時間の短縮,帰宅時のさまざまな問題の解決にもつながる.フルマゼニルについては多くの研究があるが,実際の応用方法は,歯科麻酔領域での基準確立には未だ至っていない.また,フルマゼニルの副作用として,頭痛,興奮などの精神神経症状や血圧上昇,頻脈などの循環器系症状が知られている.本研究では,静脈内鎮静法をより安全に行なうためのフルマゼニル投与法の確立を目的とし,今回は虚血性心疾患を有する患者に対するフルマゼニルの投与を行なって投与後の循環器系副作用について検索した.虚血性心疾患を有する患者のうちミダゾラム(ドルミカム^<(8)>)による静脈内鎮静法を行う患者のうち,本研究に対しての充分な理解と承諾を得られた症例を対象とした.ドルミカム^<(8)>による静脈内鎮静法下治療終了後フルマゼニル(アニキセート^<(8)>)0.5mgを静脈内投与した.術中および術後2時間は,BP,ECGなどをモニタリングし,バイタルサインを記録,帰宅許可時にホルターECGを装着し,術後24時間のECG変化を記録した.なお,帰宅許可にあたっては,平衡機能計(1G06SP)を用いた重心動揺を測定、平衡機能回復をもって帰宅許可した.以上より,虚血性心疾患患者に対するフルマゼニル(アネキセート^<(8)>)0.5mg静脈内投与は,術後24時間心電図上では影響を及ぼさなかった.虚血性心疾患患者に対するフルマゼニル使用は,術後平衡機能回復時間を短縮させ帰宅時間を早くするのみならず,安全に使用できると考えられた.今後は,症例数を増やし,安全な投与方法についてさらに多方向から研究する予定である.
