大腸憩室出血の部位同定 大腸憩室出血の内視鏡診断には,SRH(stigmata of recent hemorrhage)を捉えることが必要である(Fig.1).SRHとは活動性出血,非出血性露出血管,除去によって活動性出血もしくは露出血管を伴う凝血塊付着などの所見を指す.憩室出血は間欠的な動脈出血を来すという疾患特性から,特に自然止血後に膨大な数の憩室の中から責任憩室を見つけ出すのは容易ではない.SRH同定率を上げるためには,内視鏡検査を行うタイミングや検査時の工夫,経口洗浄剤による前処置,造影CT併用など個々のケースに見合った選択をする必要がある.

Today's project management of mechatronics development is difficult to manage, because mechatronics product is becoming highly complicated. This paper proposes an integrated model of the project information based on representation of mechatronics product and product development process. Based on this project model, this paper also addresses a management method of the project. The project model is represented as a process of design tasks and resources. The product model is represented as a design parameter, product function, and product structure. The dependencies between/inside the product and process are modeled as a MDM (Multiple Domain Matrix) matrix and DMM (Domain Mapping Matrix). The system finds and suggests the gap between designer's recognition and derived dependency. This suggestion can help the designer to elaborate and improve the project model. A proto-type system is implemented and applied to the design example of an electric solar boat. The result shows the elaboration process of the project information.

 The 2011 off the Pacific coast of Tohoku Earthquake brought severe damage caused by wide-ranging soil liquefaction across the Tohoku and Kanto districts. In Abiko City, Chiba Prefecture, heavy liquefaction damage occurred in a small area, which was not accurately predicted by the existing liquefaction hazard map. The authors consider the reason to be land history, such as landform development and artificial landfill, which was not taken into account in the assessment of liquefaction risk. Accordingly, Abiko City has revised the hazard map to take account of micro-landform classifications using land condition maps, old edition maps, and aerial photographs with the assistance of the Geospatial Information Authority's support team.

Mean annual air temperature (MAAT), mean annual surface temperature (MAST), bottom temperature of winter snow cover (BTS), and morphometric parameters of rock glaciers indicate the states of the rock glacier activities in the Yari-Hotaka mountain range, Northern Japanese Alps. The air temperature conditions at the Mountain Hut Minamidake suggest that this monitoring site belongs to the discontinuous permafrost zone. The BTS and MAST values at the North and South Okiretto rock glaciers indicate the occurrence of permafrost. BTS and MAST at the Tenguppara I rock glacier indicate the absence of permafrost. At the North Minamisawa rock glacier, MAST was slightly above 0 °C, accordingly degrading permafrost that possibly exists in this rock glacier. These rock glaciers are not moving at present, as inferred from the morphologic and vegetational characteristics of these rock glaciers. These results suggest that the North Minamisawa, North Okiretto, and South Okiretto rock glaciers are of the inactive type, and Tenguppara I rock glacier is of the fossil type. The other rock glaciers in this mountain range are of the fossil type, as inferred from collapse and subdued forms, and extensive vegetation cover.

An omnidirectional mobile vehicle created with Mecanum wheels does not easily negotiate stepped terrain, because the small slanted freely moving wheels in the Mecanum wheel rotate at the step edge and cannot obtain a grip sufficient for climbing. We developed two types of driving wheel mechanisms based on the Mecanum wheel. One is a hybrid that combines a larger grouser wheel with a smaller Mecanum wheel with a synchronized peripheral velocity and vertically adjustable central shaft. The other comprises a split Mecanum wheel where the halves shift back and forth. The radius of the grouser wheel hybrid must be carefully designed so that the grouser can produce an adequate grip at a low position for reliable transition to the frictional grip of the Mecanum wheel. Similarly, for the split Mecanum wheel, the horizontal shift distance must be carefully determined so that the grip of the edge of one half is maintained until an adequate grip can be obtained by the other half of the wheel. Experimental robots were built with Mecanum wheels 152mm in diameter to demonstrate both wheel types. These robots showed good performance by negotiating a 100mm step (grouser type) and a 70mm step (split type).

目的 東日本大震災後の栃木県における自殺の動向を明らかにする。方法 2008年1月から2011年6月までの間に栃木県内で発生したすべての自殺者のデータ(警察データ)と,警察庁が公表している自殺統計を用いて記述疫学研究を行った。結果 2008-2010年の総自殺者数は栃木県で1,795人,全国で96,784人であり,2011年上半期では栃木県で281人,全国で15,906人であった。2011年上半期における栃木県の月別自殺率3月から5月にかけて増加していた。栃木県では,4月は45-64歳と65歳以上の男女で身体の病気の悩みによる自殺が多く,5月は20-44歳と45-64歳の男,20-44歳の女でうつ病の悩みによる自殺が多かった。結論 東日本大震災の後,全国と同様に栃木県においても自殺率の増加がみられた。被災地の周辺地域においても地域資源を活用した自殺予防を早期に開始するべきなのかもしれない。

要旨:今回,筆者らは生活行為向上マネジメント(以下,MTDLP)を用いて,統合失調症の40代男性(以下,A氏)の地域生活移行支援を経験した.MTDLPの補助ツールとして,運動技能とプロセス技能評価(AMPS)を用いた.我々は,多職種連携支援をマネジメントし,A氏が退院後に必要で大切な食事の準備に焦点をあてた介入や,退院後の再発予防に対する心理教育などを行った.その結果,生活に対する有能性や認知機能障害が改善し,地域生活にスムーズにつながった.MTDLPは,統合失調症者の地域生活移行支援として有用なマネジメントツールだと考えられた.

高血圧症, 骨粗鬆症の既往がある83歳女性. 入院10日程前から食思不振があった. 入院4日前から食思不振が増悪し, ふらつきや1分程続く胸部圧迫感が出現した. 入院当日から動悸が出現したため, 当院を受診した. 心電図は洞調律で多形性心室性期外収縮が頻発し, 580msと著明なQT延長を認めた. 胸部レントゲンでは軽度心拡大を認めた. 採血では低カリウム血症 (2.3mEq/L), 低マグネシウム血症 (1.6mg/dL) を認めた. 検査終了後に突然強直性痙攣が出現し, 心肺停止となった. 無脈性多形性心室頻拍が確認され, 除細動150J 1回で洞調律に復帰した. 入院後は電解質を補正し, QT延長はやや遷延したものの, 心室性不整脈は著減した. また, 経過中たこつぼ様の壁運動を伴ったが, 第4病日で意識はほぼ清明にまで改善した. しかし第13病日に頭蓋内出血を発症し, 急変, 死亡退院となった. QT延長, 多形性心室頻拍に低カリウム血症, 低マグネシウム血症を伴った症例の報告は少ない. 本症例に関して, 低マグネシウム血症と心室性不整脈の観点から文献的考察を混じえ, 考察する.

Industrial sightseeing is noteworthy in diversification of sightseeing needs. It is defined as a sightseeing course to include at least one industrial heritage. Muroran City had grown up as a heavy industrial city and a port city until 1971. The present situation and issues of industrial sightseeing in Muroran City were analyzed through the questionnaire to participants of factories sightseeing event. Experience to make something in young generation, combination of industrial heritages and sightseeing boat to see natural coastal viewpoints, watching whales or dolphins and restaurants were requested and should be considered to make program industrial sightseeing.
