〔目的〕スラックラインは綱渡り様のベルト上で歩行等の運動を行うもので,バランス能力向上効果が注目されているが,その要因の検証は不十分である.今回,スラックラインによる運動が体平衡機能に与える影響について検討した.〔対象と方法〕対象は健常な男子9名とし,スラックライン上での歩行等の練習を行う前と練習後に重心動揺を測定し,総軌跡長,外周面積,矩形面積,実効値面積と単位面積軌跡長を比較検討した.〔結果〕面積に関する値は減少していったが,総軌跡長は練習開始後では減少したが中間以降では変化が少なかった.単位面積軌跡長は増加の傾向を示した.〔結語〕単位軌跡長は固有受容器との関係があると考えられている指標であり,スラックラインでの運動は体平衡機能に影響があることが示唆された.

生活機能病の克服や介護予防といった目標を重視する立場から,高齢者の筋力の評価やトレーニングの研究を進める視点について検討した。筋力の評価については,体重比で示された筋力が高齢期における神経・筋の機能低下を反映した指標となる可能性を論じた。日常生活活動能力と筋力との関係については,筋力の水準によって両者の関連の強さが異なる可能性を述べた。筋力トレーニングについては,筋力増強の方法について自験例の検討結果を紹介するとともに,その目標やリスクの重要性についても言及した。

〔目的〕経験の浅い理学療法士のキャリア発達を促す支援策としての基礎資料を得ることを目的に,キャリア・アダプタビリティの現状を調査し,かつキャリア・アダプタビリティとメンターとの関係性について検討した.〔対象と方法〕対象は,経験年数が1年から5年までの理学療法士149名とした.質問紙にて,基本属性,キャリア・アダプタビリティ尺度,メンターの有無を調査した.〔結果〕メンターを持つものは,持たないものと比べ,キャリア・アダプタビリティの関心因子,コントロール因子,自信因子の得点が有意に高かった.〔結語〕理学療法士のキャリア発達にメンターが関与する可能性が示唆された.

〔目的〕日常生活動作は概ね自立しているも在宅復帰に対する不安を強く訴えた症例の要因について明らかにすること.〔対象と方法〕対象は橋延髄梗塞によりWallenberg症候群を呈した患者 1 名.一般的な理学療法評価に合わせて,入院時から退院までの約150日間,1週間ごとに①10 m歩行試験,②自己効力感,③不安と抑うつ,④状態-特性不安の評価を実施した.〔結果〕10 m歩行試験と不安・状態不安,自己効力感と③・④との間に負の相関を認め,10 m歩行試験と自己効力感の間に正の相関を認めた.また,片脚立位時間の延長に伴いできる日常生活動作も拡大した.〔結語〕退院に向けて心身能力の改善が不十分であった症例に対して,前庭機能に着目した介入や心理面への考慮が必要であった可能性が示唆された.

[Purpose] This study aimed to evaluate the effects of taping and hot packs on premenstrual syndrome, in an attempt to generate basic data for physical therapy intervention for premenstrual syndrome. [Subjects and Methods] Thirty-two females in their 20s with premenstrual syndrome were randomly assigned to a taping group (n=10), hot pack group (n=11), and taping with hot pack group (n=11). Premenstrual syndrome was assessed using the Menstrual Distress Questionnaire in each participant prior to intervention and was re-assessed after applying kinesio taping and/or hot pack from 10 days before the estimated date of menstruation until the first day of menstruation. [Results] Data revealed that the taping and taping with hot pack groups showed significantly reduced premenstrual syndrome following intervention. In terms of the differences in the Menstrual Distress Questionnaire total score among the groups, the taping with hot pack and hot pack groups showed a significant difference. [Conclusion] These findings indicate that kinesio taping is an easy, non-drug intervention for female college students with premenstrual syndrome.

[Purpose] The soleus H-reflex is depressed at stimulation rates greater than 0.1 Hz. This reflex depression is referred to as postactivation depression. Postactivation depression reflects the reduced efficacy of the Ia-motoneurons synapses when they are evaluated after a previous activation. The aim of this study was to determine whether the recovery of motor functions in the lower extremities affects the PAD of the soleus H-reflex in patients with subacute stroke undergoing rehabilitation. [Subjects and Methods] Eight patients with subacute stroke patients were recruited. Postactivation depression, Fugl-Meyer score (lower-limb portion), walking velocity, the Modified Ashworth Scale, and center of pressure sway during standing were measured within three days of admission to rehabilitation and 50 days later. [Results] After rehabilitation, Fugl-Meyer scores, center of pressure path length, and walking velocity were significantly improved, and postactivation depression had significantly increased. There was a significant positive correlation between the rates of change of postactivation depression and center of pressure path length. [Conclusion] The results demonstrated that postactivation depression is partially normalized after rehabilitation in patients with subacute stroke, and suggested that the recovery in lower extremity function after stroke particularly standing stability is affected by spinal synaptic plasticity.

[Purpose] Electromyography biofeedback therapy is applied to various diseases during physical therapy for motor learning. Our aim was to develop a low-cost electromyography biofeedback device kit that students could build by themselves in class and to evaluate whether this kit was an adequate educational tool for physical therapy students. [Subjects and Methods] This study included 34 volunteers from a 3 year physical therapy vocational school and a 4 year university course. All students were tasked with: 1) completing a pre-questionnaire; 2) attending basic lectures on electromyography biofeedback therapy and how to build the device; 3) building the device; and 4) completing a post-questionnaire. [Results] Mean time required to build the device was 50.8 ± 34.1 minutes for the vocational course students and 27.4 ± 8.8 minutes for university students. For both groups, there was a significant increase in the number of students stating “have an interest in,” “be involved with,” and “have knowledge of electromyography biofeedback” when pre- and post-questionnaires were compared. In the post questionnaire, the frequency of the “knowledge increased” response was higher. [Conclusion] Physical therapy students can easily build our new device in a classroom, and this kit is a useful educational tool to motivate learning in electromyography biofeedback.

[Purpose] The aim of this study was to compare immediate effects between new medical stretching (NMS) and conventional medical stretching (CMS) techniques on Hamstrings flexibility. [Subjects and Methods] Thirteen healthy adult males, with finger floor distance (FFD) less than zero centimeter, without known musculoskeletal and neurological impairment in spine or lower extremities, were included. The subjects were randomly allocated to two groups. The subjects were instructed to perform NMS and CMS (hold for 30 seconds once, twice for each side of lower extremity) for both sides (total two minutes, only one session for one day). The interval between the two techniques was one week. FFD was measured with digital standing trunk flexion meter at the pre-intervention and post-intervention of both techniques. [Results] The mean values of FFD improved at the post-interventions of both techniques. The tests of within subject effects indicated that the main effect of treatment was not significant but the main effect of time was significant and the interaction of treatment and time was also significant. [Conclusion] The results of this study indicated that both medical stretching techniques were effective on Hamstrings flexibility immediately after the intervention and NMS technique was more effective on improving flexibility.

[Purpose] The aim of this double-blind, randomized and placebo-controlled study is to investigate the effects of Transcutaneous Electrical Nerve Stimulation for reducing the side effects of Chemotherapy-induced Peripheral Neuropathy in cancer patients undergoing chemotherapy with oxaloplatin or paclitaxel. [Subjects and Methods] Twenty-four patients were randomly allocated into two groups: active or placebo stimulation. All patients were assessed for pain, numbness/tingiling, frequency of symptoms, and quality of life. The transcutaneous Electrical Nerve Stimulation device was applied daily with modulating frequencies ranging between 7 Hz and 65 Hz in distal limb regions during three cycles of chemotherapy (45 days). The other stimulation parameters were: pulse duration of 200 μsec, intensity at the highest tolerable level, and increases in intensity when it diminished. [Results] The data showed no difference between active or placebo groups in terms of pain, numbness/tingling, frequency of symptoms or impact on daily life activities. [Conclusion] These results suggest that Transcutaneous Electrical Nerve Stimulation applied in the frequency variation mode was not proven to be effective to improve the symptoms of Chemotherapy-induced Peripheral Neuropathy during chemotherapy cycles. There was no worsening of symptoms in subsequent cycles of the onset of symptoms of the disease.

[Purpose] Untreated early-onset scoliosis may eventually progress to more than 90° after growth, cause severe health problems, and increase chance of mortality. Therefore, surgical intervention is often indicated prior to the development of a life-threatening deformity. This case report aims to reveal how a 76-year-old male patient with curves exceeding 110° is functioning with minimal difficulty. [Subject and Methods] The patient, who has never had surgical intervention for scoliosis, can perform his everyday activities. His curves were 111° thoracic and 118° lumbar when he presented at the author’s office in January 2015. [Results] The patient reported that he rarely needs a physician and participates in endurance sports like jogging. Despite this, the patient recognizes his restrictive ventilation disorder (shortness of breath) when he is inactive. The patient complained of shortness of breath and cosmetic concerns. [Conclusion] The current guidelines indicate that early-onset scoliosis should be operated at a young age; however, this protocol is not supported by high-quality evidence. Notably, patients with curvatures exceeding 100° after puberty may have a reasonable quality of life when they lead an active life with regular participation in endurance sports and physical rehabilitation.

[Purpose] The aim of this study was to examine the influence of Structural Integration and Fascial Fitness, a new form of physical exercise, on body image and the perception of back pain. [Subjects and Methods] In total, 33 participants with non-specific back pain were split into two groups and performed three sessions of Structural Integration or Fascial Fitness within a 3-week period. Before and after the interventions, perception of back pain and body image were evaluated using standardized questionnaires. [Results] Structural Integration significantly decreased non-specified back pain and improved both “negative body image” and “vital body dynamics”. Fascial Fitness led to a significant improvement on the “negative body image” subscale. Benefits of Structural Integration did not significantly vary in magnitude from those for fascial fitness. [Conclusion] Both Structural Integration and Fascial Fitness can lead to a more positive body image after only three sessions. Moreover, the therapeutic technique of Structural Integration can reduce back pain.

[Purpose] This study aimed to examine the effects of an 8-week modified Pilates program on the variability of inter-joint coordination in the elderly during walking. [Subjects and Methods] Twenty elderly participants with no recent history of orthopedic abnormalities (age, 67.9 ± 2.7 years; height, 163.7 ± 8.9 cm; weight, 67.1 ± 11.6 kg) were recruited for this study and randomly allocated to a modified Pilates exercise group or a control group. Three-dimensional motion analysis was performed on both groups to evaluate the effects of the Pilates exercise. [Results] There was no significant difference in the joint variability of the ankle, knee, and hip joints between the groups, both before training and after training. However, there was a significant increase in the hip-knee deviation phase value in the exercise group after the program was completed, and this increase was also significant when compared with that in the control group. [Conclusion] This study has demonstrated that an 8-week modified Pilates exercise program can have a positive impact on the gait of elderly participants, potentially by enhancing neuromuscular adjustment, which may have positive implications for reducing their fall risk.

[Purpose] The purpose of this study was to evaluate the effect of modified cervical exercise and determine whether such exercise improves the range of motion of the cervical movement in smartphone users with forward head posture. [Subjects and Methods] Some 32 subjects with forward head posture participated in this study. They were randomly allocated to three groups, and the modified cervical exercises were performed either once, twice, or three times per day. The exercise program was followed for four weeks and then the joint range of motion of the participants was measured. [Results] A significantly increased range of motion was seen in all three groups that performed the modified cervical exercises. The analysis of the effects among the three groups indicated that the greatest effect was seen in Group C, members of which performed the modified exercises three times per day. In addition, a significant difference was found between Group A and Group C in terms of the inter-group results. [Conclusion] According to the results of this study, although the modified cervical exercises were performed for only a relatively short duration (four weeks), the exercises brought about an improvement in the forward head posture that was induced by using a smartphone.

〔目的〕PBLテュートリアル教育の効果と問題点および学生の学習行動の変化を検討した.〔対象と方法〕1~4年生を対象に3年間自由記載のアンケートを実施し,計量テキスト分析した.〔結果〕クラスター分析より,【PBL授業への積極的参加】,【主体的な学習姿勢の修得】などの因子が形成された.学年別コードの分析より,4年次では能動的な姿勢や自己学習のコードの増加を示した.〔結語〕PBLの効果として,主体的学習や自己学習の増加などが確認できた.PBLの経験を積んだ高学年で主体的学習行動への変化が見られ,4年間のカリキュラムを通して学生の学習行動の変化を促すプログラムの構築の必要性が示唆された.

[Purpose] This study aimed to compare the effects of two different mobilization techniques in the management of patients with adhesive capsulitis. [Subjects and Methods] Thirty non-diabetic men and women with adhesive capsulitis were randomly allocated to the reverse distraction group (n=15) or Kaltenborn group (n=15). The reverse distraction technique and Kaltenborn’s caudal and posterior glides (grades III and IV) were applied 10–15 times along with conventional physical therapy for 18 treatment sessions in 6 weeks. Pain was measured with a visual analog scale, abduction and external rotation range of motion with goniometry, hand behind back reach with inch tape, and functional disability with the Flexilevel scale of shoulder function before and after the treatment. [Results] Although all the variables improved significantly in both groups after 18 intervention sessions, reverse distraction was significantly better than Kaltenborn’s caudal and posterior glides in decreasing pain and improving abduction range of motion and functional scores. [Conclusion] This study supports the clinical use of reverse distraction as an alternative to conventional mobilization techniques to decrease pain and improve range of motion and functional scores in patients with adhesive capsulitis.

〔目的〕末梢性および心因性めまい患者に対する前庭リハビリテーション(Vestibular Rehabilitation: VR)の効果を比較した.〔対象〕めまい患者25名(心因性:PS群9名,末梢性:PE群16名).めまい重症度,Berg Balance ScaleおよびDizziness Handicap Inventory日本語版(DHI-J)を用いて,めまい症状と障害の程度を評価し,介入前後の比較から効果量を求めた.〔結果〕VRの効果量は,PE群ではすべての評価で高い効果,PS群では自律神経評価,DHI-Fが中等度,DHI-Eが低い効果であった.〔結語〕VRは,PE群のめまい重症度と障害の改善,PS群ではめまい症状および機能障害の改善に有効な可能性がある.

[Purpose] To estimate the effect of Klapp method on idiopathic scoliosis in school students. [Subjects and Methods] A single-blind randomized clinical trial with 22 students randomly divided into intervention group (n=12) and inactive control group (n=10). Exercise protocol consisted of Klapp method, 20 sessions, three times a week for intervention group, and inactivity for control group. Dorsal muscle strength was measured by dynamometer; body asymmetries and gibbosity angles were measured by biophotogrammetry. Data were obtained by Generalized Estimated Equation, with 5% significance level. Clinical impact for dependent variables was estimated by “d” Cohen. [Results] There was no change in intragroup analysis and intergroup for all postural symmetry variables. However, it was detected intergroup difference in extensor muscle strength and intergroup difference with marginal significance of gibbosity angles. Regarding extensor muscle strength, intervention group produced average improvement of 7.0 kgf compared to control group. Gibbosity angles progressed less in intervention group, with 5.71° average delay compared to control group. [Conclusion] Klapp method was effective for gibbosity stabilization and it improves spine extensor muscle strength.

老人保健施設や地域で活躍する理学療法士が増えるなかで,理学療法士には直接的に対処できない医学的問題を抱える患者に遭遇する機会が増加している。諸外国ではすでに理学療法士による独立診療が行われている。このような状況の中で,リスクを把握し,回避するために骨関節系疾患と同一症状を呈するほかの医学的疾患とを鑑別する能力が求められている。患者の病歴,主観的訴え,客観的所見が筋骨格系以外の問題の存在を示唆し,医学的フォローアップが必要であることを同定する能力を理学療法鑑別診断という。理学療法士は患者の訴えを包括的にとらえ,理学療法の禁忌となる症状や理学療法士の知識の範囲を超える病態を示唆する症状がみられる場合には医師に報告しなければならない。本稿では整形外科領域でのリスクを回避するための理学療法鑑別診断について記載する。
