The dynamics of an atom in a strong infrared laser field (1013 W/cm2) result in substantial changes to the field-free electronic energy levels, which can be probed on time scales shorter than the laser cycle using isolated attosecond pulses. Here, we measure the transient absorption of an isolated attosecond pulse by laser-dressed bound states of neon near the first ionization threshold. The observed subcycle changes in the absorption spectrum result from both laser-induced ac Stark shifts and from quantum interferences between different multiphoton excitation pathways. We further demonstrate the ability to experimentally turn off the quantum interference mechanism by eliminating one of the interfering pathways.

PurposeTo evaluate the efficacy of proton beam therapy (PBT) for pediatric patients with advanced neuroblastoma.MethodsPBT was conducted at 21 sites in 14 patients with neuroblastoma from 1984 to 2010. Most patients were difficult to treat with photon radiotherapy. Two and 6 patients were classified into stages 3 and 4, respectively, and 6 patients had recurrent disease. Seven of the 8 patients who received PBT as the initial treatment were classified as the high risk group. Twelve patients had gross residual disease before PBT and 2 had undergone intraoperative radiotherapy before PBT. Five patients received PBT for multiple sites, including remote metastases. Photon radiotherapy was used in combination with PBT for 3 patients. The PBT doses ranged from 19.8 to 45.5 GyE (median: 30.6 GyE).ResultsSeven patients are alive with no evidence of disease, 1 is alive with disease progression, and 6 died due to the tumor. Recurrence in the treatment field was not observed and the 3-year locoregional control rate was 82%. Severe acute radiotoxicity was not observed, but 1 patient had narrowing of the aorta and asymptomatic vertebral compression fracture at 28 years after PBT, and hair loss was prolonged in one patient.ConclusionPBT may be a better alternative to photon radiotherapy for children with advanced neuroblastoma, and may be conducted safely for patients with neuroblastoma that is difficult to manage using photon beams.

The Agreement between the Government of Thailand and the Government of the United States of America for Financing Certain Educational Exchange Programs, or what is known as the Fulbright Agreement, was signed on July 1, 1950, after more than two years of negotiation. By examining the process of those negotiations between the two countries against the background of the Cold War, this paper looks into the meaning of the agreement to both Thailand and the United States at its inception. It is revealed that Thailand was initially not very interested in the conclusion of the agreement as it saw little benefit in it mainly due to its institutional limitation that for the Thai citizens only transportation cost to study in the U.S. could be funded. The U.S., on the other hand, was eager to strengthen its "friendship" ties with Thailand, which the U.S. came to regard as an important supporter of the U.S. policies against Communist forces in Asia, particularly after 1949. The U.S. therefore wished to promote the exchange of students and experts in order to help the Thai public understand the U.S. policies. In the end, Thailand accepted the agreement as they wished for the smooth realization of other military and economic assistant programs that the U.S. was planning to extend to Thailand, which were important for the Phibun regime to strengthen its position within the domestic political situation. Such situations conditioned the early operation of the exchanges initiated by the program.

ObjectiveTo investigate the feasibility of rehabilitation training with a new wearable robot.DesignBefore-after clinical intervention.SettingUniversity hospital and private rehabilitation facilities.ParticipantsA convenience sample of patients (N=38) with limited mobility. The underlying diseases were stroke (n=12), spinal cord injuries (n=8), musculoskeletal diseases (n=4), and other diseases (n=14).InterventionsThe patients received 90-minute training with a wearable robot twice per week for 8 weeks (16 sessions).Main Outcome MeasuresFunctional ambulation was assessed with the 10-m walk test (10MWT) and the Timed Up & Go (TUG) test, and balance ability was assessed with the Berg Balance Scale (BBS). Both assessments were performed at baseline and after rehabilitation.ResultsThirty-two patients completed 16 sessions of training with the wearable robot. The results of the 10MWT included significant improvements in gait speed, number of steps, and cadence. Although improvements were observed, as measured with the TUG test and BBS, the results were not statistically significant. No serious adverse events were observed during the training.ConclusionsEight weeks of rehabilitative training with the wearable robot (16 sessions of 90min) could be performed safely and effectively, even many years after the subjects received their diagnosis.
