症例は43歳の女性.鉄欠乏性貧血の治療を受け改善したが,突然死を来した.剖検で,両肺動脈主幹部に遊離血栓が証明され,肺塞栓症が突然死の原因と考えられ,塞栓源は左総腸骨静脈に形成された血栓であることが示唆された.子宮腺筋症による圧排が,血栓形成の原因となったと考えられた.子宮の良性腫瘍では経過観察されている例も多く,内科診療上,肺塞栓症の危険性を念頭に置いた管理が必要と考えられたので報告した.

第一に、紛争解決の理論・手法同士の関係性について「紛争を構成する要素」「要素に対応した解決理論」「解決を実践するための手法」という観点から整理し、多様な紛争事例に対応できる「戦略的紛争解決」という枠組みを創出した。第二に、欧米で発展した紛争解決の手法をアジアや日本で活用する場合の適用可能性に関して日中韓等の方に質問紙調査を行い、知見を得た。第三に、原発災害被災者の家族や地域内での人間関係の葛藤のケアと変容支援のためアクション・リサーチを行った。「福島の若手らの水俣訪問ツアー」等を実施し、環境災害からの人生と地域再生に向けた日本型の修復的正義プロジェクトの一つのモデルが形成された。

未曾有の原発災害における人間経験を生活変化と健康影響に焦点を当て長期的に追跡し、実態解明を行うとともに、社会的亀裂を修復するために次の二つの取り組みを行った。第1に、福島県中通り9市町村の2008年度出生児とその母親を対象に、原発事故が与える影響を生活と健康に焦点を当て継続的に記録するための大規模の調査票調査を行った。第2に、上記の福島県内の調査対象者への半構造化面接を行った。これは、当事者の語り部活動を行うための準備作業である。放射能の健康影響についての不安の度合いやリスク対処行動において温度差がある母親が、原発事故後の経験を広く社会と共有できることを目指している。

We report a 49-year-old man who was a human T-cell leukemia virus type 1 (HTLV-1) carrier, born in Okinawa prefecture where both strongyloidiasis and HTLV-1 are endemic. He presented with fever, headache and urinary retention. On the basis of CSF examination and MRI findings, his condition was diagnosed as myelitis. He received methylprednisolone pulse therapy. He was transferred to our hospital due to severe paralytic ileus. Strongyloides stercoralis (S. stercoralis) was found in the duodenal stained tissue of a biopsy specimen. Ivermectin applied both orally and through enema were ineffective because of severe ileus and intestinal bleeding. Nine mg (200μg/kg) of ivermectin solution was administered subcutaneously every other day for five days (total amount 45mg). The S. stercoralis burden in the stool decreased and paralytic ileus gradually resolved. Three weeks after the resolution of S. stercoralis infection, purulent meningitis developed and acute obstructive hydrocephalus appeared. The hydrocephalus improved by ventricular drainage. Approximately three months after drainage, he died of incidental aspiratory pneumonia. Autopsy showed neither eggs nor larvae of S. stercoralis in the organs. In this case, the fourth reported case in the world, subcutaneous ivermectin injection was dramatically effective. We should consider a diagnosis of strongyloidiasis for any patient from Okinawa prefecture who was an HTLV-1 carrier presenting with unknown origin ileus after treatment of steroid therapy.

慶応4年(1868)春,政権を天皇に返した徳川将軍の処置に,不平をとなえる旧幕府の人たちは反乱の気配をみせた。そこで官軍がさしむけられ,有栖川宮(ありすがわのみや)を総督とする征討軍は江戸に向い,横浜を基地とするため神奈川奉行の意向をただした。奉行は世界大勢に通じていたので,幕府役人ではあるが無条件で協力することを約した。その結果,病院を設置するため洲千弁天(しゆうかんべんてん)境内の語学所と,野毛山の寺小屋修文館の建物を提供した。4月17日に開設した横浜軍陣病院は英国軍医のウイルスが主任となり,女の看護人を雇い入れて傷病兵を看護した。日本最初の職業看護婦である。病院のあとは国電桜木町駅前と,野毛山動物園入口の老松中学校にあたる。老松中学の地は明治5年に官民合同の近代的病院となり関東大震災まで存続した。今の横浜市大医学部病院の前身である。

In the areas affected by the TEPCO First Fukushima Nuclear Accident, lots of conflicts among community members and family members and the destruction of many human relationships have taken place. They also happened in the areas affected by the Minimata Mercury Polution. There are four reasons why conflict and destruction of human relationshipoccur under serious environmental pollution : 1) high distresses in the society under pollution or disaster cause conflicts, 2) urgent needs of social change to respond to the pollution and disaster underscore anew the worldview differences among the stakeholders, 3) the pollution and disastere reveals hidden structural violence in the society, 4) polution and contamination of natural environment such as water, air, and soils threaten basic human needs, and the lack of basic human needs increases risk of conflict. In order to resolve conflicts in the communities affected by the TEPCO nuclear disaster, Curle's conflict transformation model for asymmetric conflict would be useful.

A demand of quality improvement for flatness and edge-drop has been recently increasing in cold rolling of steel strip. The accurate control systems of strip shape and edge-drop have been developed for the new tandem cold rolling mill in Yawata Works, which has 5 stands of 6 Hi-UC・WR shift mill. The control system of strip shape consists of the initial set-up for roll benders, intermediate roll shifts of all stands, a feedback control for roll benders and roll levelling of No.5 stand by using signals of the shape meter which is installed at the exit of No.5 stand. The control system of edge-drop consists of the initial set-up for work roll shifts of No.1∿4 stands and the feedforward control for work roll shifts of No.1∿4 stands by using signals of the crown meter which is installed at the entry of No.1 stand. Application of these control systems to the new tandem cold mill shows the good controllability of flatness and edge-drop in cold rolling of steel strip. The strip flatness is within ±0.7% of inclination in an overall length of strip, and the accuracy of edge-drop is 1.2μm in average and within ±3μm in an overall length of strip.
