Background:It has been 7 years since tolvaptan was approved in Japan for the indication of heart failure in patients with volume overload; the drug can be used in patients with normonatremia. Hypernatremia was identified as a significant adverse event to be prevented.Methods and Results:We compiled and analyzed data from 3,349 patients over 5 years to identify patients at high risk of hypernatremia with tolvaptan treatment. The incidence of hypernatremia, defined as serum sodium ≥150 mEq/L, was 3.65%. Baseline serum sodium concentrations, serum potassium concentrations, blood urea nitrogen : creatinine ratio, initial tolvaptan dose, and age were identified as risk factors for hypernatremia. A hypernatremia risk score was developed using the odds ratios for these factors. The high-risk population was defined as patients with a risk score ≥17.80.Conclusions:To prevent the occurrence of hypernatremic events in patients taking tolvaptan, we recommend a very low starting dose (i.e., 3.75 mg/day) in patients identified as being at high risk of hypernatremia using our new scoring process.

The horizontal movement vectors of the maximum rainfall area associated with local heavy rainfall around Tokyo in the afternoon of high-temperature summer days were analyzed using a numerical algorithm in order to investigate their relationship to the wind vectors in the free atmosphere. First, the movement vectors were objectively identified every 10 minutes from radar echo intensity data, and their time average from the onset to the termination of a heavy rainfall event was calculated. The results show that the maximum areas of localized heavy rainfall around Tokyo most frequently move to the east-southeast and southeast. Moreover, the direction of movement is shifted to the right relative to the mean direction of the free atmospheric winds in most cases. It is also implied that water vapor supply from the south in the boundary layer plays a role in the rightward movement.

Clostridium butyricum TO-A, Enterococcus faecium T-110, and Bacillus subtilis TO-A are sold as oral probiotic preparations and reportedly exhibit many beneficial effects on the health of hosts, including humans and livestock. In this study, we compared the ability of these clinically applied probiotic bacteria with Escherichia coli OP50 in extending the lifespan of Caenorhabditis elegans. To compare the C. elegans lifespan-extending effects of the three bacteria, experiments were performed using a nematode growth medium containing a small amount of trypticase soy agar. The maximum lifespans of worms fed C. butyricum TO-A, E. faecium T-110, or B. subtilis TO-A increased by 11, 12, and 26%, respectively, compared with worms fed E. coli OP50. In addition, we conducted a metabolomic analysis of methanol extracts of B. subtilis TO-A cells, which exhibited the strongest lifespan-extending effect on C. elegans among the probiotic bacteria tested in this study. As a result, 59 candidate substances involved in extending the lifespan of C. elegans were identified in B. subtilis TO-A cells.

Background: The Impella®percutaneous left ventricular assist device has been available in Japan since 2017. This is the first large-scale registry study to analyze the efficacy and safety of Impella in Japanese patients with acute myocardial infarction with cardiogenic shock (AMICS).Methods and Results: The Japanese registry for Percutaneous Ventricular Assist Device (J-PVAD) has registered all consecutive Japanese patients treated with Impella. We extracted data for 593 AMICS patients from J-PVAD and analyzed 30-day survival and safety profiles. Overall 30-day survival was 63.1%. The 30-day survival of the Impella alone and Impella plus venoarterial extracorporeal membrane oxygenation (ECPELLA) groups was 80.9% and 45.7%, respectively. The Impella alone group was older and had a lower rate of cardiac arrest, milder consciousness disturbance, less inotrope use, lower serum lactate concentrations, higher B-type natriuretic peptide concentrations, and higher left ventricular ejection fraction (LVEF) than the ECPELLA group. Cox regression analysis revealed that older age and comorbid renal disturbance were common risk factors affecting 30-day mortality in both groups. Major adverse events were hemolysis (10.8%), hemorrhage/hematoma (7.6%), peripheral ischemia (4.4%), stroke (1.3%), and thrombosis (0.7%). LVEF improved in both groups during support.Conclusions: AMICS treatment with Impella showed favorable 30-day survival and safety profiles. The survival rate of patients treated with Impella alone was particularly high. Further studies are needed to improve outcomes of patients with ECPELLA support.

Objective: Most large-vessel occlusions (LVOs) amenable to acute recanalization occur in the internal carotid or middle cerebral artery. However, few LVOs with a multivessel disease can be difficult to treat. This study aimed to determine the outcomes of mechanical thrombectomy in patients with both anterior and middle cerebral artery occlusions.Methods: We retrospectively collected data for patients who had undergone mechanical thrombectomy since January 2016 at Fukushima Medical University and its affiliated institutions (10 institutions). Patients with occluded vessels in the anterior and middle cerebral arteries were selected, and patient background, treatment course, and outcomes were reviewed.Results: A total of 341 mechanical thrombectomies were performed during the study period. Seven patients had occlusions involving both anterior and middle cerebral arteries. In these seven patients, the median time from onset to imaging, imaging to puncture, and puncture to recanalization was 106, 60, and 74 min, respectively. Only one patient (14%) had a modified Rankin Scale of 0–2 at 90 days.Conclusion: Comorbid anterior cerebral artery occlusion may worsen the outcome of patients with middle cerebral artery occlusion.

Objective: Central venous disease, defined as ≥50% stenosis or obstruction of central veins, is one of many life-threatening complications faced by patients on hemodialysis. It often presents as upper limb edema to the arteriovenous (AV) shunt for hemodialysis, although neurological symptoms are rare. We report a case of central venous disease with neurological symptoms associated with endovascular therapy.Case Presentation: A 79-year-old man presented with status epilepticus. His past medical history included rectal carcinoma when he was 69 years old and indication for hemodialysis when he was 79 years old. However, he had no history of neurological disease or epilepsy. On arrival at our facility, CT perfusion revealed venous circulation dysfunction on the left cerebral hemisphere. DSA demonstrated regurgitation from the AV shunt on left upper limb to the cerebral veins and obstruction of the left subclavian vein. Ligation of the causal AV shunt was deemed difficult due to surrounding edema; therefore, endovascular transarterial coil embolization was performed. After completely occluding the AV shunt, patient’s condition improved significantly. The patient was discharged 3 days later without neurologic symptoms, with no recurrence of epilepsy was observed to date.Conclusion: Coil embolization of causal AV shunt significantly improved the neurological symptoms of central venous disease.

Background:Although diuretic resistance leading to residual congestion is a known predictor of a poorer heart failure (HF) prognosis, better therapeutic strategies for effective and safe decongestion have not been established.Methods and Results:In this study, 81 HF patients with fluid retention (despite taking ≥40 mg/day furosemide (FUR)), with an estimated glomerular filtration rate <45 mL/min/1.73 m2, were randomized into 2 groups and administered either ≤15 mg/day additive tolvaptan (TLV) or ≤40 mg/day increased FUR for 7 days. Changes in urine volume between baseline and mean urine volume during treatment were significantly higher in the TLV than FUR group (P=0.0003). Although there was no significant decrease in body weight or improved signs and symptoms of congestion between the 2 groups, the increase in serum creatinine on Day 7 from baseline was significantly smaller in the TLV than FUR group (P=0.038). Multiple logistic regression analysis revealed that additive TLV (odds ratio 0.157, 95% confidence interval 0.043–0.605, P=0.001) was an independent clinical factor for improved renal function during treatment compared with increased FUR.Conclusions:In HF patients with residual congestion and renal dysfunction refractory to standard therapy, additive TLV increased urine volume without further renal impairment compared with patients who received an increased dose of FUR.

Climate changes around Japan associated with upper troposphere and stratosphere responses of global warming during December-January-February were investigated using the storyline approach and the Coupled Model Intercomparison Project 5 dataset. Climate change was calculated by subtracting the 1959-1990 mean in historical simulations from the 2068-2099 mean in the Representative Concentration Pathways 8.5. Four storylines in plausible future climates were discussed by considering two remote indices representing tropical amplification (tropical upper tropospheric temperature changes) and stratospheric vortex strength. Stratosphere-troposphere connections in terms of zonal wind responses are most pronounced in the storyline of high tropical amplification with strong stratospheric vortex in which the subtropical jet shifts substantially northward. The multimodel mean displays more (less) precipitation in the northern (southern) parts of Japan, while the storyline of high (low) tropical amplification with strong (weak) stratospheric vortex shows increasing (decreasing) precipitation in most parts of Japan. Projected precipitation changes around Japan depend heavily on the storyline adopted and the degree of global warming in these two storylines. Alternatively, precipitation changes depend mostly on the overall strength of global warming with minor influences from storylines in the case of low (high) tropical amplification with strong (weak) stratospheric vortex.

Large-scale trends related to the precipitation in central Japan were investigated in the Coupled Model Intercomparison Project 5 dataset. In the Representative Concentration Pathways 4.5 projections, surface temperature over central Japan increases by 1 to 4 K during the 21st century in most models. Focusing on the trend of surface-air temperature and precipitation in central Japan, these models were categorized into two groups: The precipitation significantly increases in 11 models (group A). On the other hand, the trends of precipitation are small in the remaining 20 models (group B). Analyses of high-frequency components based on daily data revealed that the difference in the precipitation prediction is associated with that in the storm activity around Japan. Relatively enhanced meridional surface-air temperature gradient in the subpolar region may contribute to reinforced storm activity. On the other hand, the zonal surface pressure gradient associated with the Siberian high and the Aleutian low is not strengthened in group A. The climatological wintertime monsoon does not appear to contribute to the difference of precipitation trend in central Japan between the two groups. Moreover, the influence of any other planetary-scale variations is not implied.

Oceanic responses to relatively strong Madden-Julian Oscillations (MJOs) and background winds controlled by El Niño-Southern Oscillation (ENSO) are examined. The MJO’s arrival excites dominant downwelling and upwelling Kelvin waves during El Niño developing (pre-El Niño: PEN) and other (non-PEN) phases, respectively. These opposite signals come from background wind directions under different ENSO phases and exert opposite impacts on SST. In addition, MJO convection itself develops accompanied by larger surface wind variations during PEN phases, which can be related to the interactive amplifications of synoptic- and planetary-scale disturbances when westerly wind bursts occur. Consequently, the strength of westerly forcing and its oceanic response during PEN phases are larger than that of the corresponding easterly forcing and its response during non-PEN phases. These results suggest that modulations of MJO amplitude and structure under the background westerly and easterly winds associated with ENSO phases exert opposite but asymmetric impacts on the ocean.

Background:In Japan, tolvaptan is indicated for patients with heart failure and volume overload who have inadequate response to other diuretics. In contrast to the USA and Europe, tolvaptan can be used in Japan in patients with normal sodium levels.Methods and Results:In this multicenter, non-interventional, post-marketing surveillance study, prospective data from 3,349 patients treated with tolvaptan over a 5-year period were analyzed to identify benefits and risks. By Day 2 of treatment, 76.9% of evaluable patients had an increase in baseline 24-h urine volume (tolvaptan responders). Mean change in body weight was similar between 7.5 mg and 15 mg dosage groups (−3.6±3.9 kg and −3.7±4.0 kg, respectively). Improvement or disappearance rates for congestive symptoms from baseline to Day 14 ranged from 77.7% for lower limb edema to 51.1% for 3rd sound. Adverse drug reactions were reported in 18.1% of patients, most frequently thirst (8.4%). No case of central pontine myelinolysis was reported. All-cause mortality was significantly lower in patients with improved sodium concentration and increased 24-h urine volume.Conclusions:The effectiveness and safety of tolvaptan in real-world clinical settings was confirmed in this large-scale analysis. The 7.5-mg dose was equally as effective as the 15-mg dose and had a better safety profile. Improvements in all-cause mortality were suggested in tolvaptan responders.

Background:Serelaxin, a recombinant form of human relaxin-2, is in development for treating acute heart failure (AHF) and a Phase II study in Japanese AHF patients was conducted.Methods and Results:A randomized, double-blind, placebo-controlled study of serelaxin at 10 and 30 µg·kg–1·day–1continuous intravenous infusion for up to 48 h, added to standard care for Japanese AHF patients. Primary endpoints were adverse events (AEs) through Day 5, serious AEs (SAEs) through Day 14, and serelaxin pharmacokinetics. Secondary endpoints included changes in systolic blood pressure (SBP) and cardiorenal biomarkers. A total of 46 patients received the study drug and were followed for 60 days. The observed AE profile was comparable between the groups, with no AEs of concern. Dose-dependent increase in the serum concentration of serelaxin was observed across the 2 dose rates of serelaxin. A greater reduction in SBP was observed with serelaxin 30 µg·kg–1·day–1vs. placebo (–7.7 [–16.4, 1.0] mmHg). A greater reduction in NT-proBNP was noted with serelaxin (–50.8% and –54.9% for 10 and 30 µg·kg–1·day–1, respectively at Day 2).Conclusions:Serelaxin was well tolerated in this study with Japanese AHF patients, with no AEs of concern and favorable beneficial trends on efficacy. These findings support further evaluation of serelaxin 30 µg·kg–1·day–1in this patient population.
