目的: 従来, 水音, 冷水, 特定空間 (たとえば電車の中) などの, 聴覚, 触覚, 視覚刺激により, 尿意切迫感が起こることがあることは経験的に知られていた. 本研究では, 過活動膀胱 (overactive bladder: OBA) 患者を対象に, 聴覚刺激 (水音) が尿意に及ぼす影響を, 自覚症状と膀胱内圧測定により明らかにすることを目的とした. 対象: 2009年6月から2010年12月までの間に順天堂医院を受診した患者で, 尿意切迫感を訴え, OBAの症状診断基準を満たす57例を対象とした. 方法: 被験群50例では刺激のない状態と聴覚刺激下の2回, 対照群7例では刺激のない状態で2回, それぞれ膀胱内圧測定を施行し, 初発尿意時膀胱容量 (volume at FDV without auditory stimulation: vFDVa-), 最大尿意時膀胱容量 (volume at MDV without auditory stimulation: vMDVa-), 初発尿意時から最大尿意時までの時間 (time to urgency: tURG) を測定した. 聴覚, 触覚, 視覚刺激による症状誘発の有無について問診し, 過活動膀胱症状スコア (Overactive bladder symptomscore: OABSS), 国際前立腺症状スコア (International prostate symptom score: IPSS) は, 質問票により回答を得た. 結果: 被験群の26%が水音による尿意切迫感を覚えると回答した. また膀胱内圧測定では, 被験群で, 聴覚刺激 (水音) 下でのvFDVa-, vMDVa-, tUGRが, 刺激のない状態と比べて有意に減少していた. 水音によるtURGの変化率は, 被験群の74%で減少し, vFDVa-およびIPSSと強い相関を認めた. 結語: OABでは, 聴覚刺激 (水音) により尿意切迫感を覚える例が, 一般人口を対象とした報告に比べて多くみられた. また膀胱内圧測定で, 聴覚刺激 (水音) によりtURGの減少を認めることが, OABの有用なsurrogate markerとなるか, 今後検討が必要と考えられた.

近代京都の都市形成に果たした土地区画整理事業の意義について地域論的な観点から考察した。この事業は,1920年代の人口急増と都市発展に対して,市街地を取り囲む形で計画・実施され,スプロール的拡大を防ぎ,整然とした街区からなる郊外地域を創出していった。都市計画事業として実施されたが,高燥地の北部では組合による居住環境整備の下,良好な住宅地が形成され,今日に継承されている。他方,南西部では市街地化の進行,地権者の多さから組合結成には至らず,多くの地区が市代執行で実施された。区画整備の進行とともに地価上昇をみ,工場のほか商店,住宅などの立地する混在地域へと変容していった。事業の実施においては,当該地区の自然環境や交通整備など地域的条件,地区住民(組合員)の取り組む姿勢が大きく影響した。平安京以来の歴史の継承性(格子状街区割など)を強く意識しかつ近代的思想も取り入れ作成された「敷地割報告書」が事業計画の基盤をなした。

本稿は3つの部分よりなる。1節では、調査のあらましについて述べる。2節では、コンサルタ ントより伺った人類学的な情報を示す。3節では短いウイルタ語のテキストを1つ示す。短期間で の調査であり、拙い報告となってしまったが、他にない情報が少しでも得られていれば幸いである。 誤謬等の御指摘を乞う。 The northern dialect of Uilta is spoken on northern Sakhalin. This paper consists of three parts:§1.a brief report on the fieldwork;§2.ethnological information by elicitation;§3.a small text. The topics of ethnological information are about reindeer breeding, fishing, stockfish and so on. The text is told by E.A.Bibikova (born in Daagi, 1940). The content of the text is a memory of her childhood.

Dactylogyrus petruschewskyi Gussev, 1955 (Monogenea: Dactylogyridae) was found infecting the gills of Megalobrama amblycephala Yih, 1955 (Cypriniformes: Cyprinidae: Cultrinae) that was sampled from the Ono River flowing into Lake Kasumigaura in Ibaraki Prefecture, central Japan. This is the first record of this monogenean from Japan, and this parasite was most likely introduced and established along with M. amblycephala from China into Japan. The 28S rDNA sequence of D. petruschewskyi is available in the database but not based on a published study. We analyzed molecular data based on newly collected specimens, and the obtained sequence corresponded to the available sequence.

This paper tries to analyse that dealed with some accounts depicted about Radheya or Karna from Mahabharata. Its experiment is very important prosess to investigate the mainly theme of Karnabhara, a Sanskrit drama by Bhasa for us. As a result, there is depicted a brave man Radheya in our Epic. Exactly sometime he came across so tragic affair. But the writer of our Epic answers in the affirmative whole things that were acted by him. So perhaps there is an element of the brave, the theme of our Drama.

The microbiota-gut-brain axis transmits bidirectional communication between the gut and the central nervous system and links the emotional and cognitive centers of the brain with peripheral gut functions. This communication occurs along the axis via local, paracrine, and endocrine mechanisms involving a variety of gut-derived peptide/amine produced by enteroendocrine cells. Neural networks, such as the enteric nervous system, and the central nervous system, including the autonomic nervous system, also transmit information through the microbiota-gut-brain axis. Recent advances in research have described the importance of the gut microbiota in influencing normal physiology and contributing to disease. We are only beginning to understand this bidirectional communication system. In this review, we summarize the available data supporting the existence of these interactions, highlighting data related to the contribution of enteroendocrine cells and the enteric nervous system as an interface between the gut microbiota and brain.

症例は57歳,女性.主訴は口渇.1995(平成7)年より高血圧,高脂血症で外来通院中.2002(平成14)年10月の健診でFPG 220 mg/dlを指摘され,精査目的で受診した.普段から毎朝4単位程度の果物を摂取していたが,加えてみかんを10個から15個程度連日摂取していた.外来受診時には空腹時血糖値が198 mg/dlであったが,尿中ケトン体は陽性で,ケトーシスを伴った2型糖尿病と診断した.果物の大量摂取がケトーシスを伴う糖尿病の原因となった報告は極めて少ない.みかんはショ糖が多いことや水分の含有量が多いなどの特徴があるため,ソフトドリンクケトーシスと類似した機序でケトーシスを呈したものと考えられる.果物過剰摂取によって発症したケトーシスを合併した2型糖尿病の特徴を明らかにすることは非常に重要であると考えて報告する.
