Birth trauma, particularly femoral fracture, have been rarely found and few are followed up for a long time.We had three babies with femoral fractures treated by Bryant's traction and they showed good results.Perfect anatomical reduction was not essential for perfect function, because deformities in babies have corrected themselves spontaneously.

Accelerometric measurement of gait ananlysis is a simple, inexpensive and effective way to evaluate gait and was first introduced by Liberson and colleagues. Smidt applied accelerometry to the normal and abnormal patterns of gait. Ogata used accelerometry to quantitatively vevaluate the lateral thrust of the knee and investigated the dynamic effects of a wedged insole on the thrust of OA knees.In the present study, we used an accelerometer, strain gauge and goniometer to inveshgate the effects of accelerometry on heel strike in gait analysis. The acceleration placed on the calcaneal region on heel strike appeared faster than any other measurement. Our results suggest that an accelerometer placed on the calcaneal region provides the most useful information on the exact moment when the heel strikes the ground.

【目的】当院では橈骨遠位端骨折において遠位関節面の骨片が小さな症例に対して,掌側ロッキングプレートであるAcu-Loc distal radius plate system(Acumed社;以下,Acu-Loc)を使用した内固定を行っている.その治療成績を検討し報告する.【方法】2010年1月から2012年12月までの間,本骨折に対して掌側ロッキングプレート固定を行った274症例のうち,Acu-Locを使用した63症例を対象とした.骨折型はAO分類でA-type19例,B-type3例,C-type41例であった.治療成績についてはCooneyの評価基準を用いた.【結果】Excellent:43例,Good:19例,Fair:1例と概ね良好な術後成績を示した.【結論】Acu-Locはプレートをより遠位に設置でき,遠位スクリューを関節面近傍に刺入できるため,遠位骨片が小さい症例で良好な成績を得る事ができた.しかし,いわゆるWatershed Lineを超えるプレートであるためFPL損傷などの報告もあり,慎重な手術適応が必要となる.

日本では,猟,クレー射撃などでの散弾銃の所持が許可されているため,事件,事故により散弾銃創を治療する可能性はある.今回我々は2007年12月に佐世保市のスポーツジムで発生した散弾銃乱射事件の被害者の治療を経験した.当院に搬送された被弾者は4例で,1例は死亡,3例は待機的に散弾摘出を行った.1例はすべて摘出できたが,2例は全摘困難であり一部残存した.散弾銃創における治療,合併症について報告する.

25歳女性.妊娠34週突然左腰殿部痛出現.仙骨硬膜外ブロック施行されるも疼痛軽減せず当院産科に緊急搬送となる.入院時疼痛強く仰臥位不能,左仙腸関節部に圧痛を認めた.発熱,炎症所見(白血球数9400/μl,CRP 11.91)があり,骨盤部MRI上左仙腸関節周囲の信号変化,また血液培養,カテーテル尿培養よりメチシリン感受性黄色ブドウ球菌を認め,尿路感染に伴う菌血症により発生した化膿性仙腸関節炎と診断し,セファゾリンによる抗菌化学療法を開始.症状改善し妊娠39週にて帝王切開により出産.母子ともに経過良好であり産後3週自宅退院となる.仙腸関節は菌血症性関節炎の好発部位の一つであり,妊娠にともなう子宮の増大等が誘因と考えられた.胎児への影響を考慮する必要があるため診断,治療に難渋するが,妊婦の腰殿部痛の原因として化膿性仙腸関節炎も鑑別する必要がある.

We reported end-results of 19 cases (13 males and 6 females) with patellar fracture, who entered our clinic. These cases were 17 to 78 years old, and followed for 1 years and 8 months to 7 years and 10 months (mean, 4 years and 6 months). They were evaluated for the range of motion of the knee joint, Pain, limping, muscle atrophy of quadriceps and grade of satisfaction. The clinical results were excellent in 11 cases, good in 5, fair in 3 and poor in none. Their main complaint was fatigue of knee, and those patients had muscle atrophy of quadriceps. Comminuted fracture and the step-off of articular surface after treatment contributed to poor results.

Large curves between 50 and 80 degrees progress after skeletal maturity in untrated idiopathic scoliosis. In patients treated surgically for idiopathic scoliosis, curves sometime progress after removal of spinal instrumentation. Twenty-seven patients were evaluated to quantitate curve progression after removal of spinal instrumentation and identify factors leading to curve progression. Curve progressed in nine out of the twenty-seven patients. Curve progression is related to only curve magnitude, and not to correction rate, interval between operation and removal of spinal instrumentation, age, Rissers sign, type of scoliosis (adolescent, juvenile, infantile) , and spinal balance.

我々は高度肥満が原因で腰椎硬膜外脂肪腫症を発症したと考えられる1例を経験したので報告する.症例は71歳女性,身長150cm体重91kg BMI 40.4 肥満4度 糖尿病以外の合併症はなくステロイド投与歴はなかった.2カ月前から腰痛,両下肢の痺れ,間欠性跛行をきたし当院初診.MRIにてL4/5椎間にて硬膜外脂肪の増生による著明な硬膜管の狭小化を認めた.脊髄造影でも同様の所見であったため硬膜外脂肪腫症と診断し脊椎除圧手術を施行した結果,術後症状は軽快した.脊椎硬膜外脂肪腫症は外因性ステロイド長期投与や肥満に伴い脊柱管内に増生した脂肪組織により硬膜管が圧迫されることにより発症するとされる比較的稀な疾患である.日本人は欧米に比べれば高度肥満者の割合が少ないため肥満のみが原因で神経症状を有する硬膜外脂肪腫症を発症するのは比較的稀である.

This study reviewed an over one-year follow-up results of 18 cases who underwent posterior lumbar interbody fusion with Brantigan I/F cage. The clinical success was defined according to Hirabayashi's scale. 9 cases were categorized as excellent, 4 cases as good, 2 cases as fair, and the remaining 3 cases as poor. Plane X-ray radiographs revealed bone union in 18 out of 20 levels (union rate 90.0%), and non-union in only one level. The remaining cases could be concluded as neither union nor non-union because of interrueted bone trabeculae. Assessment results of X-ray radiographs were most or less satisfactory and correction loss was not observed.<br>X-rays of some patients treated for degenerative spondylolisthesis showed carbon fiber cages buried deep in the upper vertebral bodies. A reaming device could be slipped onto the edges of the lower vertebral bodies, so pre-shaving them would be important. It is important that the autologous bone graft harvested from the lamina by laminectomy be unrelated with any donor site pain from the ilium. However it should be borne in mind that of MOB patients have poor bone quality after the autologous bone graft, the possibility of non-union is high. We concluded that the Brantigan I/F cage is an useful implant, but certain characteristics should be clarified for clinical and union success.

A case of the failure of the ACE Retrograde Trauma (ART) Femoral Nail System (9mm, Titanium alloy) is presented. A 55-year-old female was injured in a traffic accident, and X-ray films showed right femoral supracondylar and shaft fracture. She was treated using an ACE Retrograde Trauma (ART) Femoral Nail System (9mm, Titanium alloy) at our hospital. Partial weight-bearing gait was started 4 weeks after operation, and full weight-bearing 6 weeks. The patient complained of right femoral pain 11 weeks after operation. The nail was broken at the proximal screw hole and femoral shaft fracture was displaced again. The patient was reoperated on using the Osteo IC-Nail System (11mm). The surface of the broken nail was analyzed by Scanning Electron Microscopy (SEM). It was shown that the cause of nail breakage was fatigue destruction. In conclusion, we should use larger nails in this case.

We experienced a case of a 34 year-old man who ruptured his pectoralis major due to bench press exercise. Operation was performed three months after injury resulting in satisfactory functional repair but the patient was not satisfied with the cosmetic. Rupture of the pectoralis major is a rare illness with only about 100 reports found so far. In many of these reports, surgery achieved good results for complete rupture cases.However, there were fewer reports on old cases, but many of those available showed that the operation achieved good functional results, but there were not so many good cosmetic results.

In our clinic, there were eighteen non-union cases of the upper or lower extremities since 1983. Fourteen cases were operated: twelve cases were operated with the conventional method consisting of refreshment of the non-union sites, fixation with plates and screws, and bone grafts. All cases showed good results.

We recently had two rare cases of a stress fracture in the lower extremity. The first case was a 14-year-old sprinter who suffered a stress fracture of the medial malleolus. The second case was a 15-year-old baseball player who had an initial stress fracture in the femur and later stress fracture in the ipsilateral tibia. We shortly discussed about these cases.

We review the clinical results of 19 cases with intramuscular hemangioma. Subjects included 6 males and 13 females ranging in age from 5 to 58 years (median, 23 years). The tumor was located in the upper extremities in 3 patients, the trunk in 3, and the lower extremities in 13. Pain on motion was found in 17 patients (89.5%) and limitation in motion range was reported in 60%. Surgical planning was done using MRI and a wide surgical margin was used in regard to the affected muscle and a marginal margin for surrounding tissues. The excision was performed in all patients and no recurrence was found in the duration of follow-up ranging from 7 to 101 months (median, 57 months). Furthermore, the limitation of motion was completely improved and the postoperative disability such as gait disturbance was not noted. Thus, the wide surgical excision of intramuscular hemangioma achieved satisfactory results.

There are rarely few reports on the clavicular motion during elevation of the extremity. This is may due to the complicated shape of the clavicle and its three-dimensional movement. The purpose of this study is to analyze the movements of clavicle by using X-ray fluoroscope.The study was performed on a 21 years old female who had clavicular fracture a year ago and had fixation with plate and screws. She had no difference on both shoulder ROM when examined. In front of X-ray fluoroscope, she was asked to abduct her arm to full in 10 seconds, and the movements were recorded on videotape. From the location of plate and screws we analyzed the clavicular motion.The clavicle rotated clockwise in its long axis when viewing from the left sagittal plane. This movement increased constantly during the elevation of arm and reached 50 degrees at maximum abduction. In Frontal view, 25 degrees of upward movement was observed, where the majority of the movement was seen within 90 degrees of elevation.

比較的稀な外傷である上腕三頭筋腱皮下断裂を経験したので報告する.症例は69歳女性.二分脊椎に伴う両下肢不全麻痺のため,両松葉杖歩行中,転倒し右肘痛を生じた.近医を受診し,上腕三頭筋腱断裂の診断で当科紹介された.初診時,右上腕遠位に腫脹・圧痛・皮下出血,肘頭近位に陥凹を触知.可動域制限,筋力低下はあるも,肘伸展は可能であった.MRIで三頭筋腱の連続性消失,周囲にT2高信号の浮腫,出血像を認めた.腱縫合術を施行し,肘頭に骨孔を作成し断端を縫着した.術後3週間のシーネ固定を予定したが安静を保てず,術後17日目に再断裂を確認.再度肘頭に縫着し,人工靭帯で補強を行った.再手術後は3週よりROM訓練開始し,術後8週にはROM 20~120°となり,筋力も回復した.上腕三頭筋腱皮下断裂の治療では腱縫合が推奨され,一般に予後も良好である.再断裂の報告は少ないが,慎重な後療法を行う必要がある.

Treatment regimens for frozen shoulder vary. These include joint distension which is performed in conjunction with the arthrography of the shoulder. Distension arthrography was performed in 19 cases from April 1997 to March 1998, of which nine cases (4 men, 5 women) were available for this study. One case underwent surgical manipulation after joint distension. One case was admitted for surgery but joint distension on the day of admission was so effective that she discharged without surgery. The average JOA score was 55.4 before this procedure and 84.1 at the time of follow-up. As the treatment for frozen shoulder, we recommend this procedure before surgery.

両膝完全型内側円板状半月板の1例を経験したので報告する.【症例】13歳,男性【主訴】右膝痛【現病歴】1年ほど前から特に誘因なく右膝の内側に運動時痛を認めた.徐々に疼痛が増強したため,当科を受診した.【現症】両膝共に腫脹,膝蓋跳動,可動域制限は認めなかった.圧痛を右膝の内側関節裂隙に認めた.【画像】単純X線像では,特に変化は認められなかった.MRIで内部に水平断裂を伴う完全型内側円板状半月板を認めた.左膝にも同様の所見を認めた.【経過】右膝に対し関節鏡下半月板形成術を行った.現在術後7カ月であるが疼痛は消失し,スポーツへ復帰している.

Spontaneous rupture of the tendon during long-term systemic administration of steroids is rare and it is found only sporadically at the Achilles tendon.We report two cases of spontaneous rupture of the tendon during long-term systemic steroid therapy.One of the patients who was administered steroids for systemic lupus erythematosus developed rupture of the Achilles tendon, and the other treated for polyneuritis showed rupture of the tendon of the long head of the biceps brachii muscle.These two cases are considered to show that rupture of the tendon occurred due to degeneration and weakness of tendon tissue during long-term systemic steroid therapy.

【目的】H19年からH21年に行ったゆうきプログラムでは,統計学的に有意な改善は見られなかったことを第120回西日本整形外科学会で報告した.今回,九州臨床研究支援センターに同じデータの分析を依頼し異なった結果を得たので報告する.【方法】H19年10月からH21年11月まで8の字ゆらしとCKC中殿筋調整訓練によるゆうきプログラムを行った片側性変形性股関節症99例99関節を対象とした.男性17例(67±8歳),女性82例(63±12歳)であった.治療開始時と3ヵ月後でのJOAスコアー,NRSを比較した.前回は,Wilcoxonの符号順位検定を行い,両側P<0.05を有意とした.今回,九州臨床研究支援センターに当時と同じエクセルデータの分析を依頼した.【結果】今回は,1標本t-検定での分析が行われJOAスコアー,NRSともに統計学的に有意な改善が見られていた.【考察】診療機関ではデータの入力のみを行い,データの抽出・解析は,専門の第3機関に依頼する必要があると思われた.
