睡眠時ブラキシズムは睡眠中に行われる歯ぎしりとくいしばりの総称で,咀嚼筋活動を主体とした非機能的運動である.睡眠時ブラキシズム関連の歯科的トラブルには枚挙にいとまがなく,特にインプラント治療を行う患者のブラキシズムには注意を要する.例えばオッセオインテグレーションが獲得される前にブラキシズムの力が特定のインプラント体に集中すると脱落のリスクが増す.また,オッセオインテグレーションが獲得された後であっても,ブラキシズムによると考えられる上部構造の破損やスクリューのゆるみなどの問題が頻繁に報告されている.歯根膜をもたないインプラントの上部構造,特に最後臼歯の補綴装置咬合面には力が集中するため特別な配慮が必要となる.高度な知識と技術を駆使して渾身のインプラント治療を行っても,ブラキシズムの評価と対応を誤ると不幸な結果を招くことになる.睡眠時ブラキシズムを合理的かつ確実に抑制する方法はないが,臨床的には想定されるリスク因子への対応とブラキシズムの力から顎口腔系を護ることを主眼としたオクルーザルスプリント療法が併用される.ここで,ブラキシズムによって顎口腔系に生じる力の分布は咬合接触関係によって規定されるため,スプリントに付与する咬合接触関係には注意を要する.つまり,睡眠時ブラキシズムに関連したバイオメカニクスを理解し,合理的にオクルーザルスプリントをデザインする必要がある.本稿では,睡眠時ブラキシズムに関連する睡眠生理学とバイオメカニクスを解説し,インプラント患者の睡眠時ブラキシズムへの対応について解説する.

The displacement of dental implants into the maxillary sinus is a common complication of maxillary implant surgery, and it is therefore important to investigate how such displacements occur and methods for removing the implants. We describe 11 cases in which dental implants were accidentally displaced into the maxillary sinus from 2005 to 2015. All cases were referred to our clinic from private dental clinics. Five males and 6 females, whose ages ranged from 32 to 81 years, participated in the study. The implants were parallel in design in eight cases, and tapered in design in three cases. In seven of the 11 cases, the implant had been placed in combination with a socket lift in the maxilla. Bone height at the displaced implant sites was 3.6±1.7 mm as measured by computed tomography. One dental implant was displaced into the maxillary sinus during implant placement. In three cases, the implants were displaced into the maxillary sinus in the post-first stage surgery period, and in four cases, during the second stage surgery. Two cases were sleeping implants. In one case, displacement occurred while the implant was being extracted because of peri-implantitis. Removal of implants from the maxillary sinus was achieved using three different approaches: access through the bone crestal defect in two cases, fenestration of the anterior wall of the maxillary sinus in seven cases, and endoscopic sinus surgery in one case. Fenestration of the anterior wall of the maxillary sinus is most commonly used because it is a simple technique that allows good surgical access. Endoscopic sinus surgery is the preferred choice for patients with chronic sinusitis. A crestal approach should only be used when a large defect is present on the alveolar ridge. Displaced dental implants in the maxillary sinus should be removed as soon as possible to avoid the development of maxillary sinusitis. Careful clinical and radiographic evaluation and appropriate treatment planning should take place before removal is undertaken.

PRP (platelet rich plasma) has a beneficial effect on wound healing due to growth factors.When the bone graft was performed combination to PRP, the outcomes brought about a favorable condition in the graft area soon after operation. Early reports about PRP, the preparation of PRP dissociated from 400~500 whole blood as autologous blood. The procedure was complex and must return to transfusion of remaining blood after surgery. Recently, the importance of PRP is recognizing in bone graft and the opportunity to use PRP is increasing. Therefore, it is required to obtain a little quantity of PRP more easily. Thus, a method of a little PRP preparation was examined, and the optimal centrifugation method was selected.In this time, twice centrifuging separated 1 ml PRP from 8 ml whole blood was used for 20 volunteers, and the centrifugation method was prepared for four centrifugation groups. As for the results, the group which contained the highest concentration of platelets was 2,400 rpm 7minutes/3,000rpm-10minutes(1st/2nd). The mean of platelet level was 119.3(±29.7)×104/μl and magnification was 4.9 in the group. It was found that the 1 ml PRP which separated from 8 ml whole blood contained either equivalent or more platelets compared with another techniques. Therefore, it was suspected that the 1 ml PRP also has a beneficial effect on wound and bone healing.

審美領域においてインプラント治療が患者の満足を得られるためには,歯冠修復と同様に辺縁歯肉と歯間乳頭の位置,およびその長期的安定が求められる.これらを確立するためにはインプラント埋入部位に必要な骨(硬組織)と軟組織を確保することが重要である.生体には固有差があるが,一般的にはインプラントとアバットメントが同径の場合インプラントの頰舌(唇口蓋)側2mm以上,プラットフォーム・スイッチングの場合1.5mm以上の骨を確保し,どちらのデザインのものでもインプラントと天然歯間に1.5mm以上,インプラントとインプラント間に3mm以上の骨を確保すべきであろう.インプラント周囲の軟組織の高さは4mm程度必要と思われるため,インプラント埋入の深さはインプラントとアバットメントが同径の場合辺縁歯肉より3mm程度根尖側方向,プラットフォーム・スイッチングの場合4mm程度根尖側方向に埋入すべきではないかと多くの文献から示唆される.頰舌(唇口蓋)的軟組織の厚みは2mm程度必要だと思われる.上部構造の形態は頰舌(唇口蓋)軟組織の厚みを確保するため,歯肉縁下で凹状に,隣接形態は歯の修復と同様にハーフ・ポンティック・テクニックが応用できる.組織が不足する場合はその造成を行わなければならないが,審美性の長期予後を考えると骨の造成には遅延吸収性または,非吸収性の骨補塡材を用い,軟組織に関しては自家移植による造成が長く維持される.

The first requirement of implant treatment is to obtain sufficient bone quantity in the implant placement area. We have developed a method to obtain new bone and bone apposition nonsurgically using minor tooth movement (MTM) before insertion of implants. Here, we report two cases treated using this approach that both had satisfactory outcomes. Case 1 was a 46-year-old woman with insufficient vertical bone quantity in the area where the maxillary right first molar was lost. There was some distance between the root apex of the second premolar and maxillary sinus. In this case, the maxillary second premolar was distally moved to the area of tooth loss using MTM. The period of orthodontic treatment was 13 months. An implant was inserted in the original area in which the second premolar had been present. The superstructure was then placed on the implant. Four years have passed since this treatment and the outcome is satisfactory. Case 2 was a 29-year-old man with insufficient vertical bone quantity in the area where the maxillary right first molar had existed. The root apex of the second premolar touched the maxillary sinus. The maxillary second premolar was distally moved using MTM. The period of orthodontic treatment was 10 months. An implant was inserted into the newly made space and the superstructure was placed on the implant. Metamorphosis was observed in the remodeled bone wall of the maxillary sinus attached to the distal part of the moved maxillary second premolar. After three years and two months, the outcome is favorable. These two cases show that the MTM approach makes it possible to obtain new bone and bone apposition and to insert an implant in the newly made space in the original bone.

<p>若年者に対するインプラント治療の適応や埋入方法は,個々の成長と発育に左右されるため,慎重な診断と成長予測に基づいた治療計画が必須となる.今回,16歳女性の上顎前歯部にインプラント治療を行い,20年以上の長期に渡って機能的・審美的に満足な結果が得られたので報告する.</p><p>本症例では,20年の間にインプラント上部構造と隣在する中切歯切端との間に約2.1mmの差が生じた.このような垂直的位置変化には,顎骨の成長,第三大臼歯の萌出,永久歯列の経年的変化など種々の要因が影響するものと思われた.確かに,成長が終了するのを待ってからインプラント治療を行う方が望ましいと思われるが,先天性欠損や外傷による少数歯欠損などの理由から若年者にインプラント治療を求められることもある.そのような場合は,各々の患者の成長曲線,骨年齢,第三大臼歯の萌出力などによる影響やセファログラムによる矯正的分析を考慮しなければならない.また,隣在歯の位置を確認した後,インプラント体を口蓋側寄りとし,深くなりすぎないよう慎重に埋入することも重要である.</p>

An alveolar bone graft, followed by the insertion of threaded titanium endosseous (Brnemark System) implants, was performed in two patients with cleft lip and palate.<br/> One patient, with bilateral cleft lip and palate, underwent an iliac bone graft after orthodontic treatment. An oronasal fistula was simultaneously closed by bilateral labial flaps and a tongue flap. Brnemark System implants were placed for treatment of congenital missing lateral incisors, 17 months after the grafting. The other patient, with unilateral cleft lip and alveolus, underwent a chin bone graft. Although necrotic bone resulting from exposure of the palatal side of the grafted bone was removed, most of the grafted bone survived. Six months later, occlusal reconstruction using Brnemark System implants was performed. The postoperative course was uneventful in both cases. Moreover, these patients were satisfied with both esthetic and functional results, which improved the patient's QOL.<br/> These results indicate that endosseous implants combined with bone grafting are more reliable than conventional procedures for occlusal reconstruction in patients with alveolar cleft.<br/>

A questionnaire survey was conducted to determine the information required by patients prior to providing informed consent for dental implant surgery. Questionnaires were distributed to 1,585 patients who visited the Tokyo Medical and Dental University Hospital for oral implants between January 2012 and December 2012, of which 1,159 (73%) valid questionnaires were collected. The results showed that preservation of an adjacent tooth was the reason that 32% of participants chose implant therapy. Over 70% of participants indicated that their main concerns regarding implant therapy were aftereffects of surgery, the number of years the implants would be in place, and treatment cost. Regarding knowledge about implant therapy, patients' most frequent reply was“ I do not know” when asked about postoperative pain, peri-implantitis, the importance of implant maintenance, and the relationship between general health and implant therapy. Fifty-nine percent of the patients recognized smoking as a risk factor for periodontal disease, of which 23% recognized smoking as a risk factor for implant therapy. Sixty-one percent of patients replied“ more than 20 years” when asked about the number of years that they believed the implants would be in place. This research identified the information required by patients in order to provide informed consent. Comprehensive information is required before informed consent is sought, as the majority of patients did not possess sufficient knowledge regarding implant therapy.

長期症例のインプラントに対して,しばしばインプラントの残存率が一つの成功基準として使用される.しかし,患者の実態を知るにはそれだけでは十分といえない.この研究の目的はインプラント治療を受けた患者の長期経過の実態を知ることである.我々は,インプラント治療後20年以上経過した患者に対しアンケート調査を行った.患者は九州インプラント研究会に所属する歯科医師によって治療された.アンケートは1,168名に送付し509名からの回答を得た(回答率44%).回答者の内,78%がインプラントに何も問題ないと答えた.また,歯の経過については68%が何もないと回答した.食事については84%が何でもよく噛めると回答した.また93%がインプラント治療に満足していると回答した.

目的:インプラント用従来型既成マグネットアタッチメントのキーパー吸着面はインプラント長軸と垂直に設定されている.したがって,もしインプラントが傾斜埋入された場合には,キーパー面は咬合平面と垂直とならない.このような場合にキーパーは大きなアンダーカットを生じることになり,維持力も減少してしまう.本試作キーパーはインプラント上のアングルアバットメントに接続するように設計開発された.また,角度補正型マグネットアタッチメントの吸引力を従来型のものと比較した.<br/>方法:マルチユニットアバットメント(Replace RP, Nobel Biocare, Sweden)に適合するように,試作キーパー(直径4.8mm,高さ3mm)をステンレス合金(72.0%Fe-26.0%Cr-2%others)より切削加工して製作した.試作キーパーは17度あるいは30度の角度付きマルチユニットアバットメントにスクリュー固定により接続した.比較試料としてPhysio Magnetキーパー#40(日立金属)を用いた.磁石構造体としてPhysio Magnet #40を両キーパーに装着し,デジタルフォースゲージ(Nidec-shimpo Corp)を装備した引張試験機に取り付けた.キーパーから磁石構造体が離脱するときの最大荷重量を吸引力(N)として測定した.測定値はt検定により危険率5%で統計解析を行った.<br/>結果:インプラントオーバーデンチャー用として,試作キーパーは容易に角度付きアバットメント上にスクリュー固定できた.試作キーパーと従来型キーパーとの吸引力を比較したところ,両者に有意差は認められなかった(<i>p</i>>0.05).<br/>結論:試作キーパーの吸引力は市販の従来型マグネットキーパーとほぼ同等であった.開発したキーパーは十分な維持力を有し,インプラントオーバーデンチャーのための適切な支台装置として使用できることが示唆された.

2010年にわが国の5学会の顎骨壊死検討委員会から出されたポジションペーパーが2016年に改訂され,骨吸収抑制薬関連顎骨壊死(ARONJ)の新しい診断基準が示された.ビスフォスフォネートやデノスマブのような骨吸収抑制薬は,骨転移を有する悪性腫瘍患者や骨粗鬆症に投与されており,ARONJは骨吸収抑制薬で治療を受ける患者におけるまれな合併症である.ポジションペーパーには,医師と歯科医師との間の相互連携と協力がARONJを管理するために重要,と提唱されている.まず,骨吸収抑制薬による治療開始までに,抜歯などの骨への侵襲を伴う歯科治療は終えておくことが望ましい.医師から歯科医師への依頼は少ないのが現状で,これを増やすことが急務である.そして,治療開始までに口腔管理が開始されたとしても,骨吸収抑制薬を投与中に歯性感染症を生じることはあり得る.抜歯はARONJ発症のリスク因子の1つとされるが,抜歯を避けることによる感染の持続もまたリスクであり,必要に応じて抜歯する.ARONJ予防のための骨吸収抑制薬の休薬の有効性は証明されていないため,医師と歯科医師との間で慎重に協議,検討する.

Purpose : This study was performed to investigate and compare the mechanical influence of attachment types on the implant and mucosal tissue under the posterior part of implant overdentures (IOD) with a combination of mechanical reaction analysis and structural analysis using the finite element method (FEM) .Methods : Implant overdentures with ball attachments (B-IOD), implant overdentures with locator attachments (L-IOD), and complete dentures (CD) were established in the experiment model. A 50 N load was applied onto the right first molar position of the overdenture, and the bending moment on each implant and the overdenture displacement were measured. Additionally, the FEM model was prepared based on DICOM data of the in vitro model. The interface between the denture base and mucosal tissue was assumed to be a contact interface, not a continuous interface. Displacement data measured by the experiment model analysis were inputted into the FEM model, and the stress distribution in the mucosal tissue under the posterior part of the denture was evaluated.Results : In the L-IOD, the bending moment in the loaded-side implant was greater than that in the nonloaded side. In the B-IOD, the stress on both the loaded and non-loaded sides was smaller and almost equal. Mucosal stress in the loaded posterior part using B-IOD was the highest among the B-IOD, L-OD, and CD. The mucosal stress in the L-IOD was lower than that in the B-IOD and CD on both the loaded and non-loaded sides.Conclusion : Stress in the mucosal tissue under the denture base was able to be evaluated using a combination of mechanical reaction analysis and FEM analysis. Differences in the mucosal stress distribution were detected among the CD, B-IOD, and L-IOD. The highest mucosal stress value was shown in the B-IOD.

In recent years, implants have been widely used in dental and oral fields. Many investigators have reported the indication of implants. Most of these cases progressed well, but the others did not. To investigate the patient's awareness of dental treatments for removal of dental implants, we mailed a questionnaire to 33 patients. 1. The rate of collected questionnaires was 81.8%. 2. 92.6% of the patients underwent treatments such as partial dentures after removal of the implants, but only 40% of the patients were satisfied with these treatments. 3. This investigation clarified that informed consent between the dentists and the patients was insufficient.

The purpose of this study was to examine the applicability of the newly-established method to analyze distribution of occlusal force among implant superstructure and natural teeth at different clench levels. Dental Prescale (Fuji Film Co. Tokyo) was used to record occlusal contacts and the force loaded on each of them. Four clench levels, which were 25, 50, 75, and 100%MVC, were regulated by visual biofeedback of masseter muscle EMG. This method was applied to five patients who received osseointegrated implants at the prosthodontic clinic of Kyushu University Dental Hospital attached to The Faculty of Dentistry and the applicability of the method was discussed. The results indicated that the Implant Support Ratio, which was defined in this study, can be used for the quantitative evaluation of the occlusal force distribution among implant superstructure and natural teeth at different clench levels. It was suggested that the distribution of occlusal force depends not only on the patterns of missing teeth but also the occlusal scheme and the conditions of the residual natural teeth.

In the severely atrophic maxilla, prosthetic rehabilitation is difficult. However, rehabilitation using osseointegrated implants with bone grafting can resolve this problem. Among the various methods of bone graft to the maxilla, sinus lift is commonly used. This report presents two cases of oral rehabilitation using osseointegrated implants with the sinus lift. In case 1, grafted bone was harvested from the mental region of the mandible, and implantation was performed at the same time as the autogenous corticocancellous bone block graft. In case 2, grafted bone was harvested from the alveolar bone in the anterior region of the mandible, and implantation was performed secondarily after the autogenous particulate bone graft. Bone biopsy for histological analysis was performed in the above two cases. In both cases, new trabecular bone was observed in the augmented sinus floor. However, the trabeculae were more dense on the alveolar side than in the central area. In the case with splintered bone, empty bone lacunae were seen in the grafted region.

In recent years, implants have been widely used in dental and oral fields. Many cases have been reported, and the indication of implants has been investigated. Many cases progressed well, but a few cases did not. The unfavorable cases of dental implant were reexamined by clinical observation. The results were as follows 1. The number of unfavorable cases of dental implant increased until 1992 but decreased after 1993. 2. The patients were 20 males and 29 females, and 83.7% of them were 40-69 years of age. 3. Pain was the most common chief complaint of the patients, followed by swelling. 4. Systemic diseases, including hypertension and diabetes mellitus, were noted in 63.3% of the patients. 5. Only 24.5% of the patients who visited our department were referred to us by the dentists who performed the implantations. 6. Right after implantation, 26.5% of the patients showed symptoms. 7. Treatment at our department was removal of most of the implants.

The bone augmentation method for dental implants has become a main means of controlling the prognosis of dental implants in clinical treatment. Platelet-rich plasma (PRP) has been accepted as a safe material which can be extracted from the blood of patients. Furthermore, PRP is used in many treatments to enhance bone formation. The aim of this study was to identify what kind of growth factor was affected by PRP on bone formation in an animal experiment. Six adult dogs were selected, and four implants were placed in the lower jaw of each dog; two implants were located on the right side and the other two on the left side, to obtain control and experimental sides respectively. After that, an artificial bony defect was made between two implants on both sides, and then the defect on the experimental side was filled up with PRP extracted from the same dog. Radiographic, histological and immunohistochemical methods were used for a comparative analysis. The results showed new bone formation due to PRP at the first week after the operation, and it was identified by both radiographic and histological methods. In addition, positive reactions of PDGF in immunohistochemical qualitative analysis and quantitative search were confirmed. From the data it was concluded that PDGF was the main growth factor for the bone formation when PRP was applied to the artificial bony defects of the studied dogs.

Successful prothodontic treatment is one of the important factors for long-term implantological success. In addressing the subject of implant prothodontics, one must consider not only the occlusal conditions but also the materials of the superstructures.<br/> Recently, castable ceramics has been introduced as biocompatible restorative materials. In particular, β-Ca(PO3)2, which is stable in H2O and acid, possesses similar physical features to tooth enamel. Thus, in the present study, color matching, strength, bone conditions of periimplant tissues, and influence on the oral environment when calcium phosphate derived castable ceramics crowns are used as a superstructures of implant were assessed. The results were as follows:<br/> 1. Color matching<br/> Color matching of castable ceramics to natural tooth is better than those of metal and thermosetting resin.<br/> 2. Crack<br/> No crack was found in the castable ceramics crowns during the observation period.<br/> 3. Influence on antagonist<br/> Antagonistic tooth did not have any clinical changes after constructing superstructures using castable ceramics.<br/> 4. Bone resorption<br/> No bone resorption was observed after cementing superstructures using castable ceramics.<br/> The results suggested that the calcium phosphate derived castable ceramics crown may be preferable to other materials as a superstructure of dental implant.<br/>
