Purpose: The effect of our comprehensive strategy to reduce pain after minimally invasive mitral valve repair through a right mini-thoracotomy was assessed retrospectively.Methods: Our comprehensive strategy constituted the following: planned rib cutting to avoid rib injury, sufficient intercostal muscle division to mobilize the cut rib, limiting the number of intercostal ports, avoiding nerve entrapment, continuous extra-pleural intercostal nerve block, and regular use of oral non-steroidal anti-inflammatory drugs. We compared patients treated with this comprehensive strategy (Group S, n = 13) and patients before this strategy was implemented (Group C, n = 13). We used a numerical rating scale (NRS) as a pain scale during the first 3 days postoperatively.Results: The average NRS was significantly lower in Group S (0.82 ± 0.49) than in Group C (2.40 ± 1.46) (P <0.01). The maximum NRS was also significantly lower in Group S (3.23 ± 1.17) than in Group C (5.69 ± 2.43) (P <0.01). The number of patients using additional single-dose analgesic were significantly less in Group S (23.1%) than in Group C (84.6%) (P <0.01).Conclusion: Our comprehensive pain control strategy effectively reduced postoperative pain in minimally invasive mitral valve repair.

A patient with idiopathic neuralgia of the superior laryngeal nerve was cured by alcohol injection of the nerve. In this case, a biopsied segment of the nerve showed normal findings by the electro-microscopic examination.The results of examinations lead to the conclusion that idiopathic neuralgia may be caused by some difficulty of receptors of the sensory nerve in the mucous membrane of the larynx. In addition, authors reported that the vocal cords of the dog kept normal color and form after the internal branch of the superior laryngeal nerve was resected.

フィールドワークの教育にあたっては、フィールドワークが野外調査をもっぱらとし、言葉だけでは再現できないリアルな現場を調査対象とするため、現場から離れた教室的環境での講義や演習には、事例の提示や技法の紹介などで多くの困難が存在してきた。本研究は、マルチメディアを活用し、現場から離れた教室環境においても、リアリティのある現場の映像や音声情報を学生にインタラクティブに提示することによって、これまで教室講義では果たせなかったフィールドワーク教育のための新しい教育方法の開発を行った。本研究で開発した教材は、1.初学者(学部教育レベル)を対象とし、2.フィールドワークのための基本的な知識と技能の習得をめざし、3.マルチメディアされた素材(映像・音声・文字)によって現場の事例を擬似的現実として提示しながら、4.ステップを踏んでインタラクティブに学習を行うものである。教材の媒体は、教室講義のための補助的教材を想定し、CD-ROMとした。その後、さらに、たんに補助教材の制作だけでなく、それとメディアミックスする印刷教材と組み合わせて使用するよう教材の内容が練り上げられた。執筆者として、研究分担者を中心に7人の社会学・文化人類学者が1章づつを担当し、印刷教材の内容に対応する著者自身のフィールドワークの現場と調査経験を写真、映像、文字によって再現した。これらの研究成果をもとにして、CD-ROMが添付された大学教科書を出版する契約が出版社との間で交わされ、CD-ROM付きの教科書が2001年度秋に出版の予定である。

Lightmoor Village is a recent housing development site within the former Telford New Town area in Shropshire. It was constructed and managed by The Bournville Village Trust, a housing association which plays the central role in the management and on-going development of the Bournville Estate, a model village in suburban Birmingham.The village is located in a former coal mining area near the historical industrial complex of Ironbridge Gorge along the Severn river. After the Industrial Revolution, the area had been largely abandoned, but turned into farmland during the 20 th century. In 1968, the area was included into theplanning of the Telford New Town project. As the result of various twists in New Town policy-making, the site remained undeveloped until the turn of the 21st century.Meanwhile, the BVT, in celebration of its centennial, was planning to develop a "second Bournville" which would become another model village suitable for the new century. Ultimately, BVT applied its plan to the undeveloped site within Telford New Town.The master plan for the site was redrawn several times after the first version was presented in 1997, and ultimately the development of some 800 housing units was authorized in 1999. Actual construction on the site started in 2005, and the first 40 housing units were provided in 2006. The housing recession of 2007 caused a substantial delay in progress, and some revisions in the development scheme, but almost 400 housing units had been completed by 2013.Though Lightmoor Village was conceived as a "second Bournville", it is clearly different from Bournville in character. Lightmoor Village is designed as a pure residential community, with no industrial establishments or railway station included in the scheme. This is understood as a reflection of the development of motorization in the 20th century.
