症例は62歳,男性。近医にてC型慢性肝炎の診断のもとInterferon-β (IFN-β)で治療が開始された。治療開始後29ヶ月2週目にWBC 2,300/μl, Hb 7.2g/dl, PLT 4.7×104/μl, と汎血球減少を認めIFNを中止。しかし,その後も汎血球減少は進行,LDH 4,898IU/lと急激な上昇を認め,当科紹介入院となった。精査の結果,悪性貧血と診断,ビタミンB12投与により速やかな改善が得られた。ウイルス性肝炎に対してIFN治療が広く行われている。それに伴って様々な副作用が報告されているが,悪性貧血合併報告は稀である。IFN加療中に貧血を合併した場合,悪性貧血を鑑別する必要があると考えられ報告する。

The present study examined the recognition of numerical stimuli briefly presented in the peripheral and the central (foveal) visual fields of children with autism. The participants were 5 children with high-functioning Autism Spectrum Disorder (ASD) and 10 typically developing (TD) children of similar chronological age. The stimuli were number strings presented on a personal computer screen for 160 msec in the position of the fixation point (foveal condition) or in the peripheral visual field (retinal eccentricity of 16°) in one of the eight radial positions selected randomly (peripheral condition). The participants' task was to report the number of stimuli. The results showed that there were no significant differences in the mean response times between the TD and ASD groups. However, the mean percentage of correct answers in the ASD group was significantly higher than in the TD group for the peripheral condition. These results suggest that the effective visual field of the ASD group is expanded compared with the TD group. We discussed the relationships between perceptual characteristics and cognitive processes particular to ASD children.

ストループ課題および情動ストループ課題を用いて,統合失調症における情動的情報,非情動的情報に関する選択的注意機能について検討を行った.その結果,ストループ課題において,統合失調症者では不一致条件と中性条件の反応時間の違いは有意ではなかったが,統制群に比べて,不一致条件と中性条件の誤答率の違いは大きかったことから,無関連情報の処理を抑制する機能が低下していること,課題要求の保持が難しいことが示された.また,情動ストループ課題では,単語の種類による反応時間の違い,誤答率の違いは全体として有意ではなかったことから,情動的情報の認知機能障害は,情動的情報への注意バイアスによるものではなく,情動的情報に関する処理機能自体が低下しているためである可能性が示された.

【目的】骨粗鬆性椎体偽関節に対する経皮的骨セメント椎体形成術の治療報告. 【対象】症例は15例16椎体, 罹患高位は胸腰椎 (T11~L2) 15椎体, 腰椎 (L3) 1椎体. 男性2例, 女性13例, 平均年齢73歳. 経過観察期間は平均29カ月. 遅発性脊髄麻痺を7例に認めた. 【結果】手術時間は平均42分. 術中, 術後の全身合併症はない. 術直後, 全例腰背部痛は改善. 遅発性麻痺例も全例軽快 (MMT 1~2→3~5). 経過中, 隣接椎の圧潰を63%に認めた. 1年以上の経過観察例で, 注入椎は隣接椎体間で架橋形成を認めた. 【考察】本法は手術時間も短く, 疼痛も劇的に改善し有用である. しかし, 多くの症例で隣接椎の圧潰を認め問題である. また, 骨セメントに骨親和性がなくとも注入椎は周囲の架橋形成で安定化した. 遅発性麻痺例では椎体形成術のみで麻痺の改善が得られることから, 麻痺の発生には偽関節部の動的圧迫の要素が強いと考えられた.

The purpose of the present study was to investigate the ability of schizophrenic patients to take another person's perspective. Two perspective taking tasks were examined: perceptual and affective perspective taking. Three groups participated in the study: 22 schizophrenics, 11 transient psychotic disorder [TPD] patients, and 20 normal students. Both schizophrenic and TPD patients scored significantly lower in the both perspective taking tasks than the normal controls. The affective perspective taking scores of the two patients groups were compared with those of six year-old children. Although schizophrenic and TPD patients were thought to be able to infer another's affective state correctly, they sometimes made errors in the task, because they failed to control for their own affect. Thus, their response tendency was different from the normal children of the age. The two patients groups appeared to have a common specific difficulty in inferring another's emotions.

The propagation of curved detonation waves of gaseous explosives stabilized in rectangular-cross-section curved channels is investigated. Three types of stoichiometric test gases, C2H4 + 3O2, 2H2 + O2, and 2C2H2 + 5O2 + 7Ar, are evaluated. The ratio of the inner radius of the curved channel (ri) to the normal detonation cell width (λ) is an important factor in stabilizing curved detonation waves. The lower boundary of stabilization is around ri/λ = 23, regardless of the test gas. The stabilized curved detonation waves eventually attain a specific curved shape as they propagate through the curved channels. The specific curved shapes of stabilized curved detonation waves are approximately formulated, and the normal detonation velocity (Dn)−curvature (κ) relations are evaluated. The Dn nondimensionalized by the Chapman–Jouguet (CJ) detonation velocity (DCJ) is a function of the κ nondimensionalized by λ. The Dn/DCJ−λκ relation does not depend on the type of test gas. The propagation behavior of the stabilized curved detonation waves is controlled by the Dn/DCJ−λκ relation. Due to this propagation characteristic, the fully-developed, stabilized curved detonation waves propagate through the curved channels while maintaining a specific curved shape with a constant angular velocity. Self-similarity is seen in the front shock shapes of the stabilized curved detonation waves with the same ri/λ, regardless of the curved channel and test gas.
