It is well known that a submarine has the excellent propulsive performance in high speed. navigation in deeply submerged condition, but few papers have sufficiently shown economical feasibility of submarines for commercial use.The authors have studied about the feasibility of submarine tankers with a view to develop technical problems of commercial submarines. At first, the experiments on five submarine tanker models with the circular and rectangular hull sections were performed by method of three-component measurements at Mejiro No. 2 Tank.Examining the resistance, stability, floating draft and general arrangement of the submarine tankers, the authors chose the ST-2 type rectangular hull form as best for the submrine tanker. The lines of the ST-2 type hull form was shown in Fig. 3The ST-2 type submarine tankers with various dead weights and shaft horse powers were designed and their investment, operation cost, annual profit, freight rate per dead weight and capital recovery factor were calculated. The results of this study were summarized in Fig. 19 to 23. The best submarine tanker in the point of view of commercial feasibility was 40, 000 tons in dead weight and 20, 000 horse powers in shaft horse power under restrained condition within 14 meters in maximum floating draft. On the contrary to the previous opinions, was shown the fact that the submarine tankers would not be very fast than present conventional tankers. As shown in Fig. 23, the submarine tanker would not have commercial feasibility without drastically decreased cost of nuclear power plants.

44歳女性。2日間継続する発熱を主訴に近医受診し,尿路感染症の疑いで抗菌薬を投与されたが改善なく,翌日に全身筋肉痛と下痢を伴う40度の発熱とショックバイタルを呈し当院紹介となった。身体所見では結膜充血と顔面・四肢体幹にびまん性紅斑を認め,血液検査ではWBC 17,700 /μL,CRP 34 mg/dlと高度の炎症反応を認めた。身体所見及び,頸部〜骨盤部造影CTでは熱源となり得る有意な所見は指摘できなかった。月経期間中であったことや,以前からの生理用タンポンの使用歴からToxic shock syndrome (TSS) を疑い,多剤抗菌薬併用療法に加え,大量補液,昇圧薬による加療を開始した。血液培養は陰性であったが,腟細菌培養でmethicillin-sensitive Staphylococcus aureus (MSSA) が検出された。他の所見に加えて,第7病日には両手足の皮膚落屑を確認でき,TSSの診断を確定した。黄色ブドウ球菌が産生する毒素により引き起こされるTSSは敗血症性ショックを呈する疾患の中でも多臓器不全をきたし致死的となる可能性が高いが,疾患を想定した病歴聴取がなされなければ診断が困難な場合がある。近年日本でも生理用タンポンの使用率は増加傾向にあることより,月経関連TSSはさらに重要性が増すことが予想される。

症例1:55歳,男性。主訴,意識障害。既往歴,2型糖尿病。早朝に唸り声を上げているのを家人が発見,呼びかけても返答がないため救急要請。救急隊血糖測定で51mg/dL。当院救急外来搬入時血糖測定を施行し35mg/dL。ブドウ糖投与し,意識レベル改善,経過観察の後,独歩帰宅となった。症例2:74歳,男性。主訴,意識障害。既往歴,2型糖尿病。就寝後の排尿介助時に意識が朦朧としており,3時間後の排尿介助時は呼びかけに反応がなく,救急要請。救急隊血糖測定で61mg/dL。当院救急外来搬入時血糖測定を施行し36mg/dL。ブドウ糖を投与し,意識レベル改善,経過観察の後,独歩帰宅となった。今回経験した2例は低血糖発作症例であったが,病院前ブドウ糖投与プロトコール適応とならなかった。

アルコール摂取に伴う意識障害,ならびに迷惑行為は救急医療機関にとって避けられない問題である。今回,2012年1月から2013年9月までの21カ月間に,当院へ救急受診した急性アルコール中毒166例を対象に振り返り,これらについて性別,年齢,エタノール血中濃度,意識レベル,外傷の有無,暴言・暴力の有無について検討した。平均年齢は45.1±19.3歳,男性120人,女性46人であった。エタノール濃度が計測されている症例は129例あり,平均エタノール濃度は207.9±99.6mg/dLであった。約10%の16例で,医療従事者に対する暴言・暴力行為があり,うち4例が警察介入を要した。暴言・暴力などの迷惑行為のあった16例のうち14例は男性であり,平均年齢は36.2±17.4歳と若く,血中エタノール濃度は253.9±85.3mg/dLと高い傾向にあった。急性アルコール中毒は時として重大な転帰をたどることもあり,また,医療従事者にも被害を及ぼすことのある病態である。日常的に多く遭遇する症例であり,適切な対処を院内共通の認識とする必要がある。

症例は34歳の女性でNeurofibromatosis 1(以下,NF1)の既往があり,呼吸困難を主訴に救急搬入された。来院後間もなくショック状態となり,左椎骨動脈破裂の診断で緊急カテーテル塞栓術を施行した。その後,止血したが脳死とされ得る状態となった。若年症例であり家族の希望から臓器移植のドナー適応があるか検討を行った。NF1に合併する全身血管の奇形や脆弱性が問題となり,臓器提供には踏み切れないとの判断に至った。その後,第21病日に死亡退院となった。NF1はvon Recklinghausen 病として知られる先天性疾患であり,血管奇形・脆弱性から血管破裂・出血をきたし,しばしば重篤なショック状態に陥る。NF1に伴う血管奇形は国内でも多く報告される。本例では出血性ショックおよび血管攣縮で脳幹虚血・梗塞から脳死とされ得る状態になったと考えられた。臓器移植適応も検討されたが臓器提供には踏み切れなかった。

In some motor applications, it is very difficult to attach a position sensor to the motor in housing. One of the examples of such applications is the dental handpiece-motor. In those designs, it is necessary to drive highly efficiency at low speed and variable load condition without a position sensor. We developed a method to control a motor high-efficient and smoothly at low speed without a position sensor. In this paper, the method in which permanent magnet synchronous motor is controlled smoothly and high-efficient by using torque angle control in synchronized operation is shown. The usefulness is confirmed by experimental results. In conclusion, the proposed sensor-less control method has been achieved to be very efficiently and smoothly.

本稿では,フランスの社会政策を基礎づける規範的言説の歴史を検討し,二つの特徴を指摘する。第一は,フランス革命期の政治的秩序像によって,個人が伝統集団から析出され,国家と個人という二極構造が作られた点である。伝統集団から析出された個人の脆弱さが意識されてはじめて,工業化の下で生じる貧困が「社会問題」と認識された。第二に,フランスの社会政策は国家による公的扶助の拡張ではなく,「社会的」相互依存の中での個人の権利・義務関係から導かれた。そこでは社会の目的を問いなおす「自由検討の精神」の拡張と,産業社会への個人の統合という目的との緊張関係が内在していた。1980年代以降の改革では,「自由選択」の拡張という理念が浮上する。経済発展という目的に「社会的なもの」を従属させず,労働・家族・コミュニティ・文化・政治活動への参画の「自由選択」を保障するという点に,現代フランス社会政策の思想的な特徴が見いだせる。

Three-dimensional multi-layered optical storage is one of the most promising methods that break the limitation of recording density in optical storage. It stores the bit information onto the multilayered recording planes inside the thick recording medium and realizes the tremendous large recording capacity. This paper describes the recording/reading principles, the systems, and the materials suitable for three-dimensional multi-layered memory. We also present the experimental results that using photochromic organic materials and rhodamine-B-doped and Au(III)-doped PMMA for recording medium.

The purpose of this article is to examine the changes in the role played by the government in the market according to the theories of the French political economy from the 1780s to the 1830s. These theories are generally regarded as the precursors of “economics.” This article reveals that these theories attempt to use the market politically aiming to develop the people’s “well-being” or “happiness,” and to redefine the government’s role in the market. At the beginning of nineteenth century, J. B. Say and C. Dunoyer emphasized the political significance of a free industrial market. According to them, it enables the people’s “moeurs” to be independent and self-disciplined, so as to establish a post-revolutionary political order. Some contemporary political economists such as J. Droz and M. T. Duchâtel doubted the compatibility between the accumulation of wealth and the development of “happiness” of the people. They asserted the need for elementary education as it leads to the redistribution of “new wealth.” Moreover, social economists from the 1830s, such as A. de Villeneuve- Bargemont and E. Buret, emphatically discussed the perverse effect of industrialization, stating that the concentration of capital inevitably caused the pauperization of most of the people. They believed that the new role of the government should be the “moralization” of the poor through the organization of intermediate groups such as religious associations, saving associations, charity groups, mutual societies, and patriarchal families.
