Hypersensitivity to mosquito bites (HMB) is a rare disease characterized by transient intense skin reaction and systemic inflammation. Clinical presentation of HMB resembles other mosquito allergic responses, and it can also be difficult to clinically distinguish HMB from other severe allergic reactions. However, a distinctive pathophysiology underlies HMB.HMB belongs to a category of Epstein-Barr virus (EBV)-associated natural killer (NK) cell lymphoproliferative disorders (LPD). Hence, HMB may progress to systemic diseases, such as hemophagocytic lymphohistiocytosis, chronic active EBV disease, and EBV-associated malignancies.A triad of elevated serum IgE, NK lymphocytosis, and detection of EBV DNA in peripheral blood is commonly observed, and identification of EBV-infected NK cells usually facilitates the diagnosis. However, the effective treatment is limited, and its precise etiology remains unknown.Local CD4+ T cell proliferation triggered by mosquito bites appears to help induce EBV reactivation and EBV-infected NK-cell proliferation. These immunological interactions may explain the transient HMB signs and symptoms and the disease progression toward malignant LPD.Further research to elucidate the mechanism of HMB is warranted for better diagnosis and treatment of HMB and other forms of EBV-associated LPD.

Background: We previously reported the results of a questionnaire survey of 37 cardiac rehabilitation (CR) training facilities conducted during April 2020, in Japan.Methods and Results: We conducted a second questionnaire survey in 38 CR training facilities to explore the preventive measures against Coronavirus Disease 2019 (COVID-19) after a nationwide state of emergency was declared and to investigate differences between the 2 surveys. No significant differences were observed, except for the requirement for patients to wear surgical masks during CR (P=0.01) in the second survey. Thirty-four facilities (89%) continued CR with innovations, 61% revised their instruction manuals (vs. 46% in the first survey), and, in 39%, patients requested resumption of ambulatory CR and training videos.Conclusions: In the second survey, 74% of facilities were unable to continue conventional group ambulatory CR; however, patients maintained their physical activity and exercise regimens and managed their illnesses with the aid of telephones and mobile devices.

Active ingredients in the natural products have been considered to be used for alleviating the symptoms of ulcerative colitis, hence the effects of Lycium barbarum polysaccharides (LP) and capsaicin on dextran sulfate sodium (DSS)-induced colitis in rats were investigated. Rats were grouped into normal, DSS induced colitis, and colitis treated with 100 mg LP/kg body weight, 12 mg capsaicin/kg body weight, or combined 50 mg LP/kg body weight and 6 mg capsaicin/kg body weight. Treatment with LP or capsaicin was orally fed by gavage for 4 weeks, and 5% DSS was fed via drinking water for 6 days during week 3. Colon tissue and cecum content were collected for analysis. Treatments with LP and/or capsaicin ameliorated disease activity index scores, severity of colon distortion, and shrinkage of colon length. LP and capsaicin decreased colonic pro-inflammatory cytokine (IFN-γ, IL-17A, and IL-22) levels. Cecal microbiota in colitis rats were enriched with the genus Turicibacter and Lachnospira. The relative abundance of genus Ruminiclostridium_9 and Ruminoclostridium_1 was increased by LP and capsaicin treatment, respectively. Pretreatment with LP or capsaicin inhibits the severity of colonic damage in rats with DSS-induced colitis via anti-inflammation and modulation of colonic microbiota.

Summary: Anomalous aortic origin of a coronary artery from the opposite sinus of Valsalva is known as a cause of sudden cardiac death. However, it is often asymptomatic and incidentally diagnosed during evaluation for other cardiac diseases. We report a case of anomalous aortic origin of the right coronary artery (RCA) from the left sinus of Valsalva (ARCA) detected incidentally in a patient with degenerative mitral regurgitation (MR). A 47-year-old man, who had no history of myocardial ischemic symptoms on exertion, was admitted for sudden orthopnea. ECG revealed no ischemic changes and arrhythmias. Echocardiography demonstrated MR due to torn chordae of the posterior mitral leaflet. Computed tomography (CT) revealed the RCA arising from the left sinus of Valsalva at an acute angle and taking an interarterial course between the great arteries. The proximal RCA showed a circular shaped cross-section on CT, suggesting no presence of an intramural segment. Considering refractory heart failure and no history of myocardial ischemic symptoms on exertion as well as the findings of the CT angiography, urgent mitral valve repair was undertaken without surgical intervention for the anomalous RCA, and without evaluating myocardial ischemia. The patient recovered uneventfully. Postoperatively, myocardial perfusion scintigraphy demonstrated no exercise-induced myocardial ischemia. Patients with ARCA who are asymptomatic and whose coronary course is not intramural can be managed without surgical intervention for an anomalous coronary artery.

Formation of oxygen vacancy-rich conducting filament in the metal-oxide insulating layer due to applied electric field is studied using phase field modelling. We consider that the formation of the Frenkel defects plays a dominant role in the process of filament formation. The choice of any other type of defects have been neglected due to the consideration of charge neutrality. We find that during the initial stages of filament formation, intrinsic defect concentration plays a crucial role. However, during the late stages, when the filament formation approaches completion, because of the increased electric field, the generation of new Frenkel defects dominates the growth mechanism. Our observation confirms that the intrinsic defects are not sufficient for the completion of conducting filament in the insulating memresistive layer. Our numerical analysis helps us to better understand the formation mechanism of conducting filament and to determine the key material parameters that influence the operation of oxide based non-volatile random access memory (RAM) i.e. resistive RAM (ReRAM).

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) emerged in Wuhan, China, in December 2019. Despite the recent introduction of vaccines against SARS-CoV-2, more effective vaccines and antiviral drugs must be developed. Here, we isolated five SARS-CoV-2 strains from four patients with coronavirus disease (COVID-19) and an asymptomatic individual using pharyngeal swabs, nasopharyngeal swabs, and sputum samples. Cytopathic effects in inoculated Vero cells were observed between days 3 and 7. SARS-CoV-2 infection was confirmed by quantitative reverse-transcription polymerase chain reaction (RT-qPCR) and next-generation sequencing. Phylogenetic analyses of the whole genome sequences showed that the virus isolates from the clinical samples were belonged to the Wuhan and European lineages. These findings and isolated viruses may contribute to the development of diagnostic tools, vaccines, and antiviral drugs for COVID-19.

Two new species of the genus Tegecoelotes are described from Chubu and Chugoku Districts, Honshu, Japan, under the names, Tegecoelotes chikunii n. sp. and Tegecoelotes hibaensis n. sp., respectively. Tegecoelotes chikunii found in Nagano and Gifu Prefectures is characterized by having extremely elongated femur and patella, short and bifurcate conductor, and extended tegulum in the male palp. Tegecoelotes hibaensis is distributed in Hiroshima, Okayama, Tottori and Shimane Prefectures, and is characterized by having elongated patella of the male palp and linguiform epigynal teeth in the female genitalia.

Background: In 2016, two consecutive moderate magnitude earthquakes occurred in Ulsan, South Korea. Therefore, we aimed to investigate the impact of earthquakes on the mental health of residents in Ulsan.Method: We used data from the 2015–2017 Korean Health Insurance Review & Assessment Service National Patient Sample. We conducted an interrupted time series analysis using location-based controls. Changes in the number of antidepressants, benzodiazepines, and zolpidem prescriptions in Ulsan were compared to controls. Overall changes in weekly prescriptions one year after the first earthquake, compared to a non-earthquake scenario, were estimated.Results: In antidepressant prescriptions, the increase in trend after an earthquake was significantly higher than controls. However, the changes in benzodiazepines and zolpidem were not significant. Overall, the impact of the earthquake on weekly antidepressant prescriptions at one year was estimated as a 1.32 (95% CI: 1.18 - 1.56) rate ratio compared to the non-earthquake scenario. This corresponded to a 1,989.7 (95% CI: 1,202.1 to 3,063.0) increase in the number of prescriptions. Among subgroups, the increase was highest among males aged 20–39 years.Conclusion: The moderate earthquake in Ulsan was associated with an increase in antidepressant prescriptions. The increase in the male group aged 20–39 was the highest. The impact may vary according to the context of the population.
