【目的】H19年からH21年に行ったゆうきプログラムでは,統計学的に有意な改善は見られなかったことを第120回西日本整形外科学会で報告した.今回,九州臨床研究支援センターに同じデータの分析を依頼し異なった結果を得たので報告する.【方法】H19年10月からH21年11月まで8の字ゆらしとCKC中殿筋調整訓練によるゆうきプログラムを行った片側性変形性股関節症99例99関節を対象とした.男性17例(67±8歳),女性82例(63±12歳)であった.治療開始時と3ヵ月後でのJOAスコアー,NRSを比較した.前回は,Wilcoxonの符号順位検定を行い,両側P<0.05を有意とした.今回,九州臨床研究支援センターに当時と同じエクセルデータの分析を依頼した.【結果】今回は,1標本t-検定での分析が行われJOAスコアー,NRSともに統計学的に有意な改善が見られていた.【考察】診療機関ではデータの入力のみを行い,データの抽出・解析は,専門の第3機関に依頼する必要があると思われた.

過去5年間に当科で治療を行った特発性後骨間神経麻痺9例(男性4例,女性5例,平均年齢62.1歳,平均経過観察期間11.7か月)の治療成績を報告する.3例は保存的治療で軽快し,手術的治療を行ったのは6例であった.3例に対し神経線維束間剥離のみを行い,残りの3例は神経線維束間剥離に加え,一期的に腱移行術を行った.神経線維束間剥離のみを行った3例はいずれも発症から約10か月で手術が行われ,全例MMT4以上に改善を認めた.一期的に腱移行を行った3例のうち,2例はMMT4以上に改善したが,糖尿病,頚髄症を合併した1例は回復が悪かった.麻痺に先行する疼痛を伴った症例は33%であった.後骨間神経の所見でくびれを認めた症例はなかったが,神経の色調の変化を3例に認めた.発症後約10か月の経過観察が妥当であるが,合併症がある症例や高齢など回復が期待できない場合は一期的な再建も考慮してよいと思われた.

A man, aged 26 years, was treated malaria in May, 1942, by spinal injection of “Bagnon” (Quinine preparat). Directly after injection he was attached by severe nervous symptoms, from which he recovered completely after a half year, and returned to normal works.From October, 1950, he was caught by reffered low back pain and paresis of legs. By the laminectomy of X and XI thoracal spines, severe adhesions of spinal cord with arachinoidea and dura was detected, which were detached gentlely between arachinoidea and dura, especially in ythe regions of nerve roots. After one month from operation., he was free from pain, but paresis of legs remained unchanged.It is interesting to notice that the nervous symptoms caused by adhesive spinal meningitis occur after interval of several years, and worth while to indicate that the detachment of adhesions in the regions of nerve roots is important to relieve pains.

踵骨骨嚢腫3例の手術治療を経験したので若干の文献的考察を加えて報告する.症例は7歳男児が2名,12歳女児が1名であり3症例とも踵部痛で来院した.画像検査で踵骨骨嚢腫と診断した.運動時痛があり,嚢腫のサイズも大きかったため全例に病巣掻爬,人工骨移植(β-TCP)を行った.平均経過観察期間は23ヵ月(13-36ヵ月)であった.経過観察期間中,単純X線で嚢腫の再発は認めなかった.諸家の報告と同様に踵骨骨嚢腫に対する病巣掻爬,人工骨移植は有用な治療法と考えられた.

当院では前十字靭帯(以下ACL)損傷に対して解剖学的二重束再建を行っており,大腿骨孔はtranstibial法で作成している.H22年4月からH25年3月までに施行した解剖学的二重束ACL再建術を施行した16例中,術中に起きた合併症3例について報告する.症例1.大腿骨骨孔作成時にガイドピンの先端が大腿骨内で折損した.ガイドピンをハンマーで叩いて刺入したため髄内釘になってしまい,大腿骨内で折損した.症例2.術後のX線像にて大腿骨の前内側骨孔外に金属粉と思われる陰影を認めた.ガイドピンが弯曲したままドリルした事が原因と考えられた.症例3.脛骨の後外側骨孔を作成時,骨孔作成ガイドを倒しすぎたため顆間隆起を損傷した.【まとめ】解剖学的二重束ACL再建術において術中合併症を生じた3例を報告した.transtibial法で大腿骨孔を作成する場合は,ガイドピンが適切に挿入されていることと脛骨骨孔作成時は関節面の軟骨損傷を防ぐため骨孔刺入角度に注意する必要がある.

鎖骨遠位端骨折に対するフックプレート(HP)と非フックプレート(n-HP)の術後経過を比較した.HP群は14例(男性12例,女性2例,平均年齢47.8歳,術後平均観察期間9.7カ月)で全例LCP clavicle hook plateを使用した.n-HP群は12例(男性10例,女性2例,平均年齢41.2歳,術後平均観察期間9.9カ月)でClavicle Wiring plate 6例,Tension band wiring, Scorpion plate, Now J, Distal clavicle locking plate, LCPクラビクルプレートラテラルエクステンション,髄内釘をそれぞれに使用した.最終観察時の平均JOA ROMスコア(30満点)はHP群22点,非HP群28点で,n-HP群で良好であった.全例で骨癒合を認め,HP群で肩峰下のびらんを13例,偽関節を1例に認めた.肩峰を跨がないHP以外の内固定では術後肩関節可動域が良好な傾向であった.

【はじめに】上腕骨近位端骨折に対する骨接合術後の合併症である上腕骨頭壊死の検討を行ったので報告する.【対象および方法】対象は術後上腕骨頭壊死10肩,性別は男性3肩,女性7肩.手術時年齢は平均68歳であった.骨折型はNeer分類:3-part:2肩,3-part脱臼:1肩,4-part:6肩,4-part脱臼:1肩,平均経過観察期間は32ヵ月であり,術後肩関節可動域(屈曲,外旋),X線分類(Cruess分類),追加手術について調査した.【結果】屈曲は平均88°,外旋は平均36°であった.Cruess分類はstage 2:1肩,stage 3:1肩,stage 4:6肩,stage 5:2肩であった.追加手術として,2肩に人工骨頭置換術,1肩にスクリューの抜釘を行った.【まとめ】70歳未満では壊死後の可動域は比較的良好で,壊死のリスクが高い骨折型でも骨接合は選択肢の一つと考えられた.

A rare case of intratendinous ossifications of the bilateral patellar tendons was reported. A fourteen-year-old boy, who was a superior short runner, visited our office, complaining of pain in both knees after running. Physical examination revealed hard masses in bilateral patellar tendons and tenderness of those. No other abnormalities were noted. Roentgenograms of both knees showed ossification in the area of the patellar tendon. Of course, those were extra-articular, arthrographically. Differential diagnosis was made from patella biprtita, Osgood-Schlatter disease and others. Conservative therapies consisting of anti-inflamatory drugs, local injection and others were effective, but the symptom arose repeatedly because he trained hard. We are planning to resect the ossifications after the running seasen.

経過中に左右の環軸椎回旋位固定を生じたダウン症児の1例を経験したので報告する.斜頚,頚部痛が出現した2週後に当科を受診した.頚椎単純CTで環軸椎回旋位固定を認めたため,Glisson牽引を開始した.入院時は右回旋,左斜頚であったが,牽引7日後には左回旋,右斜頚と逆方向に回旋した.さらにGlisson牽引を3週間追加したが改善しなかったため,全身麻酔にて可及的整復後に中間位でhalo vest固定した.固定1週後の頚椎単純CTで左回旋の残存および後頭環椎関節での右回旋を認めたため,やや右に過矯正し固定した.固定3週後の頚椎単純CTで右回旋を認めたため,中間位に再固定した.固定8週後の頚椎単純CTで軽度右回旋が残存していたが,本人の不耐性のため9週でhalo vestを除去した.発症後6ヵ月の時点で,頚部痛,斜頚は改善し,頚椎の回旋可動域は正常である.

傍骨性骨軟骨異形増生は,手足の短管骨に好発し傍骨性腫瘤を形成する疾患である.今回我々は,脛骨骨幹部に発生した傍骨性骨軟骨異形増生の稀な1例を経験したので報告する.症例は12歳,男児.1年前,サッカー中に右下腿前面を打撲した後より同部位に腫瘤を自覚した.腫瘤が徐々に増大し運動後の疼痛を認めるようになったため,近医を受診した.脛骨前面に発生した骨腫瘍を疑われ,精査目的に当科へ紹介された.右下腿中央内側に5 cm大の骨性隆起を認め,単純X線像で脛骨近位骨幹端から骨幹部にかけて辺縁整で台地状に隆起した骨腫瘍を認めた.MRIでは,T1強調像で低信号,T2強調像では腫瘍基部で低信号,隆起部では高信号を示す二層性の変化を認めた.腫瘍隆起部に造影効果を認めた.切開生検術を行い,傍骨性骨軟骨異形増生と診断された.腫瘍基部を含む切除術を施行し,現在術後1年2ヵ月で再発を認めず,切除部に骨形成を認める.

We Reported two cases of pyogenic arthritis of the knee which were obliged to undergo arthrodesis. Both cases had past histories of repeated steroid injection into the knee joints. Initially they were treated with closed irrigation-suction therapy, but did not improve at all, and finally underwent arthrodesis.Second case was a deep sepsis after total knee arthroplasty. In both cases bacterial isolation from specimens revealed pseudomonas pseudomonas aeruginosa, but bone union was obtained favorably by external fixation.

初回術後多発肺転移と複数回局所再発を来し治療に難渋した右肘頭骨巨細胞腫にdenosumabが著効した1例を報告する.【症例】18歳,女性.右肘頭骨腫瘍にて当科紹介され,単純X線像,切開生検にて骨巨細胞腫の診断に至った.骨腫瘍掻爬,アルコール処理,自家腸骨移植施行後4ヵ月で肺転移を認め化学療法施行,zoledronateを投与した.術後9ヵ月で局所再発し再手術施行するも,計3度再発を来した.転移性肺腫瘍は徐々に増加,増大し初回手術後4年2ヵ月より肺炎,血胸を来すようになりdenosumabを投与開始した所,転移性病変は縮小し症状改善した.10ヵ月投与後に一旦休薬したが,再度転移性病変が増大したため投与を再開した.以降転移性病変は縮小傾向であり,現在術後6年2ヵ月経過している.

Most reports on the results of treatment of tears of the rotator cuff have included surgical results, and only a few results of long-term therapy in non-surgically treated patients have been reported. We followed up non-surgically treated patients whose course had been monitored, and performed questionnaire surveys and direct examinations about 2, 6 and 19 years after the start of follow-up.The subjects were 14 conservatively treated patients diagnosed with the complete tear of the rotator cuff by shoulder arthorgraphy in our department during the 3-year period between 1986 and 1989 and whose course had been known at the time of the survey in 1990 and 1994 and 1998. They could be followed up by questionnaire and direct examination.Their mean age at the time of the initial examination was 61.6 years, and the mean follow-up period was 10.2 years. Patients with only slight pain after initial conservative treatment and who did not want to undergo surgery, were treated conservatively.The evaluation was based on the scoring of JOA. All but 2 had favorable long term courses.

We analysed possible pathophysiology of cubitus varus deformity after supracondylar fracture of the humerus using clinical materials and experimental models. In addition to varus, distal portion of the humerus usually rotates internaly and extends posteriorly. As a result, pronation of the forearm, external rotation of the shoulder and elbow flexion are limited in the patients with cubitus varus deformity.

Ten patients with acute lateral ligament injuries were treated by early non-bracing mobilization which involved early weight bearing and dynamic joint control training. No functional orthosis was needed using this method. A paired Student's t test was used to compare side-to-side difference on stress radiograph. Significant decrease occurred for both mean talar tilt angle from 10.9° to 1.0° (p<0.0001) and mean anterior drawer distance from 4.3mm to 0.3mm (p<0.0005). This early motion seems to have resulted in a number of benefits. It mechanically stimulated the injured ligaments to heal, and enabled immediate neuromuscular coordination retraining and financial savings compared to other methods. Early mobilization and weight bearing is considered as the method of choice and provided quick recovery in ankle mobility.

We report a case of pustulotic arthro-osteitis. Arthro-osteitis is a rare condition in palmoplantar pustulosis (PPP). A 60-year-old woman was admitted to our hospital with persistent low back pain and limitation of the lumbar flexion and extension. Diagnosed with PPP, surgical treatment by anterior spinal fusion was performed. She recovered well, and bone union is good today.

咳嗽により発症した腹直筋血腫の1症例を経験したので報告する.症例は77歳男性,糖尿病の既往歴があった.肺炎による激しい咳嗽後に右上腹部の疼痛と腫脹が出現した.超音波検査にて腹壁膿瘍が疑われたが,CTにより腹直筋血腫と診断し得た.保存的加療により症状は軽快した.3カ月後のCTでは血腫は著明に縮小しており,経過は良好である.

Socioeconomical and psychological influences to the operated patients with low back pain were studied from 1976 to 1977 by means of questionnaires.Operated patients had been heavily suffered from economical loss for average six to eight months of work-off time which was statistically correlated with the duration from admission to returning to work and the operated method. However, the loss was not related to the postoperative medical evaluation and the patient's satisfaction to the operation as of psychological reaction.The fear of the recurrence was statistically forced their jobs into lighter work.Posterior appooach for aged female was revealed to be made carefully for psychological problem.

Injuries to the thoracic and lumbar spine may result in severe instability with progressive neurologic defecit. Attention must be directed not only to restoring or improving neurologic function where possible, but also to promoting bony stability.We have carried out one stage decompression-stabilization procedure consisted of Harrington and anterior spinal fusion with anterior decompression in a total of 6 patients. All 6 of these with thoraco-lumbar spinal injuries had a neurogic defecit.The results have shown to be beneficial in restoring spinal alignment, allowing early mobilization of the patients, shortening rehabilitation, preventing the late complications of increasing deformity in all patients and recovering neurologic function in 4 cases with incomplete neurologic defecit.

Carpal boss is a bony prominence involving the carpometacarpal (CM) joint of the index and long fingers.Two symptomatic cases of carpal boss were reported. These cases were successfully treated with excision of localized bony prominence, together with degenerated joint surface.
