Lymphomatoid granulomatosis (LYG) is a rare lung disorder diagnosed by radiological imaging of multiple pulmonary nodules and occasionally induced by methotrexate (MTX) use. To date, the treatment of LYG has not been standardized. We herein report the case of a patient with grade 3 MTX-related LYG who presented a bulky lung mass. Importantly, the disease condition only improved after the discontinuation of MTX and remained stable for more than 1 year. Chest physicians should be aware that LYG can develop as a single lung mass and spontaneously regress, even without aggressive chemotherapy, following the cessation of MTX.

By means of studying articles published by contemporary Japanese architects, this report aims to illustrate their ideas on public architecture. At first, the content of each article was studied, and some comments were extracted from each theory. Then, they were abstracted into two different levels: the level of the subject and the level of the representation. Next, the meaning of these comments was analyzed by applying the method created by Jiro Kawakita. Those theories were then distracted on the basis of certain ideas. The subject was correlated with the representation, and their relationship was studied. Consequently, a definite meaning was found. In this particular study, we have pointed out the design ideas of contemporary Japanese architects about public architecture after the Second World War.

患者は64歳,女性.自宅玄関で椅子から転落し右側胸部を強打した.近医で肋骨骨折が疑われ,当院を紹介受診した.胸部X線・CTで第5,6肋骨骨折を認めた.入院にて疼痛コントロールを行い,退院予定であったが,受傷の11日後に急激な右胸部痛が出現した.胸部CTで多量の胸水貯留,胸腔ドレナージで新鮮血の排液を認めたため,胸腔鏡補助下に緊急開胸手術を施行した.胸腔内には多量の凝血塊が存在し,未診断であった第7肋骨の鋭利な骨折端が胸腔内に突出し,近傍の横隔膜に損傷を認めたことから,同部が遅発性血胸の原因と考えた.血腫除去,第7肋骨を部分切除し,横隔膜を縫合後,閉胸した.術後経過は良好で,術後22日目に退院した.結論:中下位肋骨骨折時には,横隔膜損傷による遅発性血胸を合併し得ることを念頭に置いた対応が必要である.

A patient with a trochanteric fracture of the left femur presented at our hospital. Initially, muscle strengthening of the left lower leg was attempted. This led to subsequent improvement in muscle weakness, and the patient could walk by herself with a cane. However, when she tried walking without a cane, the movement of her first step seemed unstable. We realized that the major impairment was instability at the first step of walking. To resolve this issue, we aimed at practical improvement of the instability. We focused on alignment of the pelvis and trunk, and on the center of pressure at the first step of walking. Efficient, smooth walking is possible due to a reverse reaction phenomenon derived from the standing position in a healthy person. However, in the present case, the movements of the center of pressure and the patient’s standing position at the first step were different from the general pattern. We concentrated on improving her posture and control in the standing upright position. Thereafter, the patient’s standing upright position and the center of pressure at the first step changed remarkably and she acquired smooth walking following the first step.
