妄想的観念はその主題に基づいて分けることができる.負の感情価の主題を持つ観念は被害妄想的観念となり,正の感情価の主題を持つ観念は誇大妄想的観念となる.妄想的観念は主題の相違によりその発生メカニズムが異なると想定されている.そこで,本研究では,被害妄想的観念は「疎外」「被害」の主題を持つ観念で,誇大妄想的観念は「被好意」「庇護」「他者操作」「自己肯定」の主題を持つ観念で構成されると想定し,各6つの主題に対する感情と原因帰属の影響を検討した.対象は統合失調症患者48名であり,全ての対象に妄想的観念チェックリスト,原因帰属尺度,気分調査票短縮版による測定を実施した.その結果,正あるいは負の感情価の主題を持つ妄想的観念ごとに抑うつや怒りなど共通する影響要因が存在した一方で,各6つの主題ごとに異なる影響要因も存在した.以上のことから,妄想的観念の主題に応じた介入を展開することが望ましいと言える.

目的:統合失調症患者が地域生活において対処できない問題とその対処に向けた訪問看護師の支援を明らかにする.方法:A県内の精神科病院および訪問看護ステーションにおいて統合失調症患者を支援している訪問看護師を対象として面接調査を行い,Berelson,B.の内容分析を参考にして分析した.結果:訪問看護師が語った統合失調症患者が地域生活において対処できない問題は,【服薬自己管理ができないため薬を正しく飲めない】【入浴・洗髪・行為をできないため清潔が保てない】など11コアカテゴリが抽出された.統合失調症患者が地域生活において対処できない問題の対処に向けた訪問看護師の支援は,《服薬自己管理ができるように服薬確認や声かけを行う》など17コアカテゴリであった.結論:訪問看護師は,患者が地域生活において対処できない服薬,清潔,金銭管理,熱中症予防などの問題に対して,患者の状態に合わせて段階的に継続的な支援を行っていた.

本研究の目的は,統合失調症患者を対象としハンドマッサージを行い,主観的指標および客観的指標を用いてリラクセーション効果を明らかにすることである。対象者は,民間の精神科病院に長期入院をしている10名の統合失調症患者である。毎回のハンドマッサージ実施前後の脈拍,血圧,「心地よさ」の自己評価点について検討した。又,全ハンドマッサージの介入前後において総合評価尺度を用いて検討した。マッサージ中の対象者の言動については質的に検討した。その結果,リラクセーションの自己評価点については全対象者で有意に得点の上昇が確認され,脈拍については7名,血圧については5名で有意に低下していた。全対象者において,ハンドマッサージが心地よさをもたらす反応がみられ,また,対象者自らが自分の困りごとについて自発的に語りだす等の変化があり,看護師(研究者)と対象者の関係がより良いものになった。以上より,ハンドマッサージは統合失調症患者に効果があることが示唆された。

本研究は,2013年に足浴・フットマッサージの介入を行った,民間の精神病院に入院する6人の残遺型統合失調症患者が,6ヶ月を経過した時点においても,精神症状,精神症状による生活の質が維持されているのかを検討することが目的である。精神症状の評価はPositive and Negative Syndrome Scaleを用い,精神症状による生活上の支障の評価はQuality life Scaleを用い,足浴・フットマッサージ終了後と6ヶ月経過した時点の得点の差を比較した。また,足浴・フットマッサージを受けた患者の経験について質的記述的に検討した。その結果,Positive and Negative Syndrome Scale では,陽性症状尺度,陰性症状尺度,総合精神病理尺度のそれぞれの得点で有意差はなく精神症状は足浴・フットマッサージの終了後においても維持していた。また,足浴・フットマッサージ終了後に改善したQuality life Scale合計得点は有意に上昇し,足浴・フットマッサージ終了6か月後も維持されていた。足浴・フットマッサージを受けた患者の経験については,足浴・フットマッサージを受けたい思い,前向きな気持ちになり自信がでた,など6カテゴリーが得られた。足浴・フットマッサージ終了6か月後の変化については,社会に興味や関心がある,自分の事は自分でしたいなどカテゴリーが得られたことから,自閉的な生活に戻ることはなかった。以上より,足浴・フットマッサージ終了6か月後においても,足浴・フットマッサージの介入効果は持続していることが示唆された。\nSix patients with residual schizophrenia who were hospitalized at a private hospital were intervened in by foot bath and foot massage in 2013. The purpose of this research was to examine whether or not these patients maintained their psychological symptoms and quality of life even six months after the completion of foot bath and foot massage. The scores of psychological symptoms were measured using positive and negative syndrome scales (the positive syndrome scale, the negative syndrome scale and the general psychopathology scale) and those of quality of life were measured using a quality of life scale at the completion of foot bath and foot massage and then again six months after the completion of foot bath and foot massage, and differences between the scores were compared. The experiences and later lives of the patients were qualitatively and descriptively discussed. As a result, there was no significant difference between the scores of psychological symptoms measured at the time of the completion of foot bath and foot massage and six months after the completion. The scores were also maintained after the completion of foot bath and foot massage. There was significant improvement between total scores of quality of life measured at\nthe time of the completion of foot bath and foot massage and six months after the completion. As for the actual experience of foot bath and foot massage received by patients, six categories were obtained including: “want to receive foot bath and foot massage” and “have a positive emotion.” As for the changes in the lives of patients six month after the completion of foot bath and foot massage, were obtained including “have interest in and attention to society” and “take care of myself by myself,” so that nobody returned to autistic life. As above, it was suggested that the\neffects of intervention by foot bath and foot massage were maintained even six months after the completion of foot bath and foot massage.

多くの女性にとって化粧は,精神的満足や自尊感情を高めるための重要な行為だといわれるが,入院が長期化しがちな精神科病院においては,社会生活から遠ざかる中で,化粧に対する関心や欲求そのものが希薄になり,それがまた,医療者側の化粧への配慮を失わせて行くという負の循環を生み出してきた.しかしながら,化粧には,陰性症状,協調性,自発性等の改善効果があることや,「自・他の関心」や「女性としての生き方を考える」などの変化をもたらすという報告がある.そこで本研究では,統合失調症により精神科閉鎖病棟に入院する女性10名(希望者)を対象に化粧会を実施し,化粧による日常生活行動(ADL:activity of daily living)への影響について検討した.その結果,化粧は,ADL評価点を向上させ,さらにその効果は化粧会実施後も持続することが認められた.化粧には統合失調症を持つ女性の現実感覚を回復させ,ADLを改善する効果のあることが示唆された.(著者抄録)

The standard version of the Epic of Gilgamesh composed in Akkadian in the second millennium BC narrates the adventures of Gilgamesh, the tyrannical ruler of Uruk, and his heroic quest for immortality. After Gilgamesh grieved over the death of his beloved friend Enkidu, he became frightened by the inevitability of death and he set out on a long journey to visit Ut-napishtim, the wise, who had survived the flood in ancient times and had been endowed with immortality by the gods. Ut-napishtim, tells Gilgamesh, how he survived the flood with the help of Ea, the god of wisdom. Then he says to Gilgamesh, “But now, who will bring the gods to assembly for you, so you can find the life you search for? Come, for six days and seven nights do not sleep!” (XI 207-209) Gilgamesh fails this challenge and realizes it is impossible for him to obtain eternal life. On the return journey, Gilgamesh manages to find the herb of rejuvenation, which Ut-napishtim had told him about. But a snake chances upon the herb, eats and casts off its skin, depriving of Gilgamesh the rejuvenation he had hoped for.The present author does not agree with M. Eliade who argued that the Epic of Gilgamesh narrates the failed initiation of Gilgamesh due to his lack of wisdom. The editor of the Epic, possibly Sin-l?qi-unninni, must have intended to bring the immortal Ut-napishtim and the mortal Gilgamesh together by incorporating the flood myth into the Epic. The purpose of the editor seems to have been to declare that the times in which immortality could have been given to a human being were 128 long past.At the beginning of the Epic, the editor introduced Gilgamesh as the extremely wise man who “came a distant road and was weary but was granted rest.”Although Gilgamesh had returned to Uruk in vain, it is suggested that he became wise and overcame the fear of death. The Epic narrates, in my view, a story of a successful initiation which has been appealing to the people until today.

複数の包摂粒度を許容する漢字構造記述を内容アドレッシングを用いた分散データモデルの1つであるIPLDに基づいて実現する試みについて述べる.IPLDはMerkle DAGというデータ構造を用いている.これは暗号学的ハッシュを用いてラベル付けされた不変オブジェクトをノードとする有向非巡回グラフであり,オブジェクト間の関係は片方向しか表現できない.この制約の下で編集可能な文字知識を表現するために,不変性の高い基礎的オブジェクトへのリンクを含む複合的なオブジェクトによって可変性のある文字オブジェクトを表現するための形式を提案する.また,IPLDグラフの逆リンクや場所アドレッシングの実現手法について検討するとともに,既存のグラフストレージでの試験的実装についても述べる.

独立行政法人情報処理推進機構(IPA)では,行政で用いられる人名漢字等約6万文字の漢字を整備する「文字情報基盤整備事業」を推進しており,文字図形と文字情報の構築を行なっている.本稿では,これらの漢字文字情報の整備と,文字情報リソースを格納しオープンデータとして相応しい文字情報を提供する文字情報基盤データベースについて紹介するとともに,文字情報の整備と提供についての課題と今後に向けた取り組みについて報告する.
