The efficacy and safety of Kampo preparation Sho-seiryu-to were studied in a joint double-blind trial in comparison with a placebo. The study was carried out on 220 patients with perennial nasal allergy at 61 hospitals. Granules in a dose of 3 g were administered 3 times daily for 2 weeks. Moderate to high improvement was recorded in 44.6% of the treated patients and in 18.1% of those receiving placebo. The difference is significant (p <0.001). Side effects were noted in 6.5% of the treated patients and in 6.4% of the controls (not a significant deference). The side effects were mild and had no influence on the daily life of the patients.

Two cases of fish-bone foreign body were reported. Case 1 was a 75-year-old female who suffered from cervical pains after eating bonito. No foreign bodies were found by indirect laryngoscopical nor esophagoscopical examination. The foreign body was found embedded in the posterior wall of the pharyx on X-ray and CT scan. Case 2 was a 77-year-old female who had pains after eating bonito. Neither indirect laryngoscopical nor esophagoscopical examination revealed any foreign bodies in the pharyngoesophageal space. X-ray and CT scan revealed an extra-esophageal foreign body with a surrounding periesophageal abscess and cervical subcutaneous emphysema. By an extra cervical incision, the foreign body was found penetrating the esophagus and reaching carotid artery. These two cases suggest the importance of conducting X-rays and CT scans even if no foreign bodies are found under laryngo-esophagoscopical examination.

Eighteen ears of 14 patients developed sensorineural hearing loss associated with (A) pregnancy,(B) delivery,(C) puerperium,(D) menstruation and (E) administration of female hormones.Audiological examination revealed the following clinical aspects. 1) Hearing loss in (A),(B) and (C) occurred in the twenties and early in the thirties.2) Acute hearing loss of 9 ears showed unilateral, abrupt onset like sudden deafness. Sudden impairment of hearing occurred in pregnancy, immediately after delivery and in administration of progesterone and/or estrogen.3) Slowly progressed hearing loss of 4 ears developed bilaterally in puerperium or periodically in menstruation.4) Audiograms of (A),(B) and (C) showed high tone loss in most cases. Hearing impairment was slight or severe.5) Audiograms of (D) and (E) showed hearing loss of flat type. Hearing loss was moderate.6) Békésy audiogram showed type II configuration, SISI score revealed mostly positive.7) Pure tone audiograms of acute hearing loss were significantly improved, however restoration of slowly progressed impairment was poor.Effects of progesterone and estrogen on hearing loss were discussed in order to explain pathogenesis of these hearing impairments.It seemed unreasonable to regard the disease merely as side effects of female hormones.

当科言語治療室を訪れた言語聴覚障害児に対して実施した言語学習能力診断検査 (ITPA) の結果を集計し、分析した。対象は4歳1カ月-9歳9カ月の102名 (男児72名、女児30名)。そのうち88名は言語発達遅滞であり、その他の14名は構音障害や吃音といった何らかの言語聴覚障害を持っていた。分析は各対象児の評価点に対して行った。その結果、視覚-運動回路に比べて聴覚-音声回路の平均値が低く、その中でも特に「文の構成」と「ことばの類推」が低かった。各項目の評価点の相関を見ると同じ回路同士で高く、「数の記憶」はほかの項目との相関が低かった。

A huge cholesteatoma in a 50-year-old female involving the petrous apex was reported. The prominent features of the case were recurrent attacks of very slowly progressing facial paralysis with a 11-year interval and insidious destruction of labyrinthine function.<br>The cholesteatoma extended into all the air cell tracts up to the petrous apex, and to the retro- and infralabyrinthine-areas.<br>The cholestoma destroyed the otic capsule at the crus commune and resolved the dura mater to such an extent that the cerebellum could be seen through it. As she had experienced acute otitis media on only one occasion at the age of 4 and only had a small dry attic perforation, the possibility of a congenital cholesteatoma of the petrous apex was considered in addition to her clinical course.

喉頭全摘出術を受けた退院患者のQOLを検討するため, 近声会26人を対象にアンケート調査を行った.その結果を基にQOLに影響を及ぼす精神的, 身体的要因すなわち呼吸, 食事, 生活行動, 手術の満足度などについて検討した.無喉頭・気管呼吸者の愁訴の1位は嗅覚障害, 2位は「鼻がかみにくい」であった.両者共に鼻機能低下によるものであった.喉頭全摘出術を受けたことに対しての想いは, 回答者22人中21人が満足していた.その約半数以上が術後1年以内に満足を得ていた.現在の健康状態については回答者24人中3人が重複癌であった.喉摘後に胃癌(2人), 肺癌(1人)の手術を行っている.代用発声として食道発声が最も多く利用されていた.しかし, 咽頭全摘時年齢70歳以上の者は食道発声の習得できなかった.食道発声者の大多数は訓練し始めて1〜6ケ月の間に発声が可能となっていた.食道発声の利点については, 大多数が食道発声ができて, 自分自身に自身がもて外向的, 積極的になったと答えている.
