Medical doctors actively participate in academic meetings and study intensively to achieve self-improvement in medical science and practice and thereby develop possibilities for future career success. It is important and valuable experience to learn techniques for questions and answers and discussions at academic meetings and to interact with people at social gatherings, along with reading academic books and journals. Participation or making academic presentations at academic societies is indispensable to becoming a specialist. Establishment of daycare nursery centers in academic meetings has been proposed as one of the positive actions to improve the environment to enable female doctors to continue to work actively. Here, we present information useful for the organizers of academic meetings at the time of setting up daycare nursery centers. The use of daycare centers differs depending on the academic society and is affected by factors such as whether the number of uses is small or conversely waiting for cancellation; the cost burden also varies. We find good examples of safety management, including cooperation with neighboring doctors. As the number of female doctors and the demand for academic daycare centers will increase, each academic society must secure nursery teachers, a place, and a budget to establish daycare nursery centers.

自閉症の医学的研究は、近年、次々と新しい事実・データを明らかにしており、それらの理論化が必要とされている。また近年の研究により、二十世紀フランスの哲学者ジル・ドゥルーズの思想の自閉症的側面が明らかにされつつある。本研究は両者を融合させ、人間についての新しい理論を作り出すことを目指している。哲学の研究者二人と、病院勤務の経験のある二人の医学研究者が共同で研究を行う点にこの研究課題の特徴がある。二〇世紀、哲学は精神分析学と強く結びついて発展した。それに対し、医学あるいは一般医療と哲学との交流は稀であったと思われる。本研究課題は、文医融合という研究の可能性を探るものでもある。

主動作筋の活動に先行して姿勢調節筋群が放電開始する現象は, 先行随伴性姿勢調節 (APAs) と呼ばれ, 補足運動野 (SMA) がその生成に関与すると考えられている。本研究では, SMAに対する経頭蓋直流電流陰極刺激 (陰極tDCS) が, APAsの機能に影響を及ぼすか否かを検討した。健常被験者11名が, 自己ペースで右上肢を急速拳上する課題を重心動揺計上で行い, この時の右三角筋と右大腿二頭筋の活動を表面筋電図で記録した。左側M1 (Cz上) とSMAに陰極tDCS (2 mAで15分間) 及び疑似刺激を行い, 刺激前後における両筋の放電開始時間差 (ΔEMG onset) の変化を比較した。陰極tDCS後におけるΔEMG onsetの有意な短縮はSMA刺激時にのみ認められた。APAsの生成にはSMAが重要な役割を果たし, また陰極tDCSはSMAの予測的姿勢調節機能を抑制し得ることが示唆された。

In this paper, I discuss the relationship between social organization and offspring-rearing methods, which are starting to be used as a novel indicator in interspecies societal comparisons. Additionally, I clarify the relationship between the intimacy of the mother–offspring relationship and the development of foraging in the offspring. I suggest that the establishment of a base camp in human evolution is important not only to facilitate effective hunting and gathering but also for immature individuals to reduce the cost of accompanying adults and engage in their own foraging activities. I also attempt to deconstruct weaning from the offspring’s perspective, which often focuses on the mother’s decision to stop breastfeeding. Specifically, I focus on the development of the offspring’s ability to acquire food independently and the offspring’s behavior away from its mother. I suggest that there is no significant difference between humans and African apes in the timing when the offspring significantly reduces its dependence on breast milk. This paper also depicts “early weaning” as a characteristic of the unique life history of humans, in which the mother conceives a second offspring at the same time as or even before the offspring becomes much less dependent on the mother’s milk.

【目的】本研究では、大腿骨近位部骨折患者において、リハビリテーション後に投与する栄養補助食品の効果の比較検討を行った。【対象と方法】大腿骨近位部骨折患者67例を対象とした (栄養補助食品群26例、オレンジジュース群41例) 。栄養補助食品群にビタミンD12.5μ g、BCAA2500mgを含んだ栄養補助食品をリハビリテーション後に投与した。アルブミン、末梢血リンパ球数、総コレステロール値から求めたタンパク代謝、免疫能、脂質代謝の指標であるCONUT値を栄養評価指標とし、入院時、手術1週後、4週後に比較検討を行った。【結果】CONUT値はオレンジジュース群では有意差を認めなかったが (入院時3.3、1週間5.4、4週間4.5) 、栄養補助食品群では入院後1週で悪化した値が、4週目では有意差をもって改善していることが確認された (入院時3.8、1週間5.7、4週間3.7、p<0.05) . 【結論】以上の結果から、ビタミンD、BCAAの強化に加え、カルシウム、脂質、炭水化物、微量元素等を含んだ栄養補助食品の投与は、手術的侵襲にて栄養状態が悪化する大腿骨近位部骨折患者に対して、栄養状態を改善することが確認された。

Hemangiopericytomas are soft tissue tumors of pericyte origin that line the capillaries and venous intima. It accounts for less than 1% of all brain tumors, but has a high rate of recurrence and metastasis even after complete resection. A 75-year-old male was admitted to our hospital for an examination of a right lung nodule following surgery for cerebral hemangiopericytoma. This case underwent open lung biopsy by video-assisted thoracic surgery, and metastatic hemangiopericytoma was revealed. No new pulmonary metastases have since been detected, and the patient is currently under a strict follow-up protocol.We report a surgical case of pulmonary metastasis, which is rare in this disease.

PURPOSE : The purpose of this study was to clarify the effects of prolonged expiration (PE) on respiratory and cardiovascular responses and autonomic nervous activity during the exercise.METHODS : Twenty-five healthy men (22±1years) were classified according to the breathing mode during the exercise : 2-second inspiration and 4-second expiration in 1 : 2 group, 3-second inspiration and 3-second expiration in 1 : 1 group and normal breathing in control group. The 6-minute exercise was performed at anaerobic threshold (AT) and 60%AT using a cycle ergometer as an exercise protocol. Respiratory rate (RR) and tidal volume (TV) were measured by the expired gas analysis. The power of low- (LF) and high-frequency components (HF) was analyzed from a Holter electrocardiogram to assess the heart rate variability. RESULTS : RR and LF/HF were significantly lower, TV and HF were significantly higher during the exercise of 60%AT and AT in the 1 : 1 and 1 : 2 groups than in the control group (P<0.05 or P<0.01). The increase of HR was significantly lower and that of HF was significantly higher during the exercise at 60%AT in the 1 : 2 group than in the 1 : 1 group (P<0.05). CONCLUSION : PE activated the parasympathetic nervous activity and consequently restrained an excessive increase of HR during the exercise at 60%AT.

近年スマート工場に関する研究が注目を浴びている.Industry 4.0に代表される第4次産業革命ではスマート工場が主役となっており,多くの研究論文が発表されている.しかしながら,その多くは自動化,知能化,IoT(Internet of Things), AI (Artificial Intelligence) に関するものであり,運営における同期化や柔軟性を考慮した設計に関する研究は見られない.本研究では,プロセスチーズ工場における乳化工程と充填工程を対象とし,設備償却費用,稼働費用,同期化費用,および柔軟性費用を考慮した設計問題を考える.スマート工場は必要なものを,必要な時に,必要な量だけ,効率よく製造することが必要条件であり,需要の変動など外部環境の変化や内部環境の変化に柔軟に対処できるように設計する必要がある.工場を設計するときに運用,同期化,柔軟性を考慮することが工場のスマート化に貢献できると考え,投資決定変数と運用決定変数を用いてモデル化を行う.乳化工程と充填工程には複数の設備を設置することができるとし,設備能力と台数の組み合わせを最適化することを目的とする.投資と運用を同時に考える場合,目的関数が非線形になる.そして,組み合わせ数が膨大になり組み合わせ爆発を起こすため,問題の特徴を用いて階層化し,効率的に最適解を求めるアルゴリズムを提案した.また,提案モデルは数値実験を通じてその妥当性を評価し,さらに事例研究を通じて既存工場の運営を評価すると同時に,非合理的な投資を合理的にするための分析を行った.

細胞/スキャフォールド複合体を利用した早期骨再生を目的に三次元フィブリンゲル内での長期骨芽細胞培養およびその石灰化誘導を行った。この材料をラット皮下に埋入したところ、あらかじめ石灰化誘導を行ったスキャフォールドにおいて、早期の異所性骨形成が確認された。

今年度は、マルティン・ハイデガーのヘルダーリン論にみられる狂気と詩作の関係についてラカン派の精神分析理論を用いながら研究し、その成果を研究会で発表した。また、ジル・ドゥルーズにおける狂気と創造性の関係を中心にラカン派の精神分析理論を用いながら研究しその成果を投稿できるようにまとめた。今年度の補助金は、主に上記の研究を遂行するための文献の購入費に当てられた。1. マルティン・ハイデガーのヘルダーリン論にみられる狂気と詩作の関係については、関西ハイデガー研究会第43回研究会にて発表した。なお、この研究を遂行するにあたって、補助金を用いて実存哲学に関する文献を多数購入した。2. 交付前の2016年6月26日に筑波大学にて開催された第63回日本病跡学会総会で発表した内容(ジル・ドゥルーズにおける狂気と創造性の関係については、彼の主眼が統合失調症ではなく自閉症スペクトラムにあったと考えることによって晩年の著作『批評と臨床』のパースペクティヴがより明瞭となること)の研究を深め、同内容に関する学術論文を準備した。その他に、ドゥルーズの「器官なき身体」という概念が、対立しているとしばしば目されるラカン派精神分析における言語の原初的形態である「ララング」に実際は近く、作家ジェイムズ・ジョイスの一部の作品がそのような言語によって構成される芸術であることを『早稲田文学』誌1022号で発表した。なお、この研究を遂行するにあたっては、補助金を用いて現代フランス哲学および精神分析と文学に関する文献を多数購入した。
