スキーは日常から離れた雪山で行うスポーツであり,一旦傷害が発生すると,環境・地理的条件のために救命救急が困難な状況が発生しうる.われわれはスキー場から救急隊を介さず直接当院に依頼を受け,当院の医療用ヘリコプター(医療ヘリ)を使用し,患者を搬送するシステム作りを試みている.箱館山スキー場(滋賀県高島市今津町)は滋賀県の北部に位置し,当院から直線距離約50km で,ヘリでは15分である.平成27年2月に,箱館山スキー場で実際に雪上搬送トレーニングを行った.問題点として,(1)着陸地点が限られる,(2)天候に左右されやすい,(3)着陸時の足場の問題,(4)病院からの交通,(5)費用等があり,救急車ほど安易に使用出来るものではないが,患者の利益のために活用が可能であれば,今後,他のスキー場,また様々なスポーツ現場に適応を広げていきたいと考えている.

Neurosurgical navigation systems using preoperative images have a big problem in their accuracy which is caused byBrain Shift. To this problem we have developed an image modification technique using brain mechanics. In this study a method is introduced in which the intraoperative brain shape is estimated through Finite Element Method. In the process the nearest model shape to intraoperative brain surface is firstly estimated by modifying an FEM result through adjusting mechanical property, then the residual between the result and the actual shape is obtained by a virtual load on the brain surface.The accuracy of our method is evaluated with phantom experiments.We prepare two kinds of phantom experiments for investigating the most adaptive conditions in terms with bindings, initial model parameters, etc. One is that using mathematical model whose mechanical property is elastic, and the other is that using soy bean curd whoes mechanical property is non-linear.

〔目的〕本研究の目的は,男性2型糖尿病者の筋量と血糖コントロールの関係を明らかにし,運動処方の一助とすることである.〔対象〕骨関節疾患および運動器疾患のない男性2型糖尿病者58名とした.〔方法〕血糖コントロール別に良好群14名,可群20名,不可群24名の3群に分類し,生体電気インピーダンス方式体組成計を用い上下肢・体幹筋量を測定した.〔結果〕上下肢・体幹の全てにおいて,不可群は良好群よりも有意な筋量の減少を認めた.さらに上肢,体幹においては,不可群は可群よりも有意な筋量の減少を認めた.〔結語〕上下肢・体幹筋量は,血糖コントロール不良者ほど減少していたことが明らかとなったことから,運動プログラム立案時には有酸素運動とレジスタンス運動を併用した運動を考慮する必要性が認められた.

ギ酸によるフェーリング液の還元を演示するために適切な実験条件を検討した。ギ酸とフェーリング液の混合溶液中でのギ酸と水酸化物イオンの初期濃度を変数として,赤色沈殿(Cu₂O)が析出する濃度領域を調べた結果,ギ酸とフェーリング液を混合した後のpHがおよそ8–10となる場合に,ギ酸がフェーリング液を明確に還元することがわかった。また,一定量のギ酸とフェーリング液を混合する方法が演示実験として簡便であることも再確認できた。Appropriate experimental conditions were studied to demonstrate the reduction of Fehling's solution by formic acid. The initial concentrations of formic acid and hydroxide ion in the mixed solution of formic acid and Fehling's solution were used as variables to investigate the concentration range where red precipitate (Cu₂O) formed. It was found that the reduction reaction occurred when the pH after mixing formic acid and Fehling's solution was about 8–10. In addition, it was reconfirmed that the method of mixing Fehling's solution and a certain amount of formic acid was convenient as a demonstration experiment.

Tendernesses and mouth opening pains at the region of temporomandibular joints and/or in masticatory muscles are very common on temporomandibular joint disorder patients.<BR>We usually give these patients anti-inframmatory analgesics or muscle relaxants until the treatment plan is decided, or when the splint therapy is ineffective for removing the tenderness or pain.<BR>Recently we have had an opportunity to use a new anti-inframmatory analgesic of arylacetate derivatives, Amfenac Sodium (AMF) to the patients who have temporomandibular joint disorders, and have evaluated the analgesic efficacy, side effect and usefulness of this analgesic.<BR>27 patients aged from 16 to 71 years (severity: mild 11, moderate 14 and severe 2) were given AFM (200 mg/day for 14 days) and evaluated symptoms were spontaneous pain, tenderness, mouth opening pain, interincisal opening range, clickin sound and crepitus at the 7th day and at the 14th day.<BR>As a result, total improvements were as follows: excellent 4 (14.8%), moderate 13 (48.1%), little 9 (33.3%), no change 1 (3.7%).<BR>6 side effects (22.2%) were reported, but those were mild (5 cases), or moderate (1 case). No severe side effect was found.<BR>Consequently, we concluded the usefulness of AMF as followed: very useful 4 (14.8%), luseful 12 (44.4%), relatively useful 10 (37.0%). Total usefulness including very useful, useful and relatively useful was 96.2%.

The purpose of this article is to clarify the base and moment for generating “subjectivity” in UEHARA Senroku’s “The Dead Person and the living person” theroy, by focussing the “imminency” and “responsibility” for “The Dead Person”. This study analyzed UEHARA’s works texts and clarified the following five points.1. UEHARA proposed the idea of “subjectivity of the living person as media of The Dead Person”. This idea rinks to criticism to religion, that implies to find out the human abilities which have been preserved in the form of religion in histroy.2. In the case of being imminet by message of “The Dead Person” who was killed unjustly, “the living person” is stimulated and roaded to must to check and direct own’s living styles and standpoints of valuing. According to this UEHARA’s theory context, “subjectivity of the living person” is founded by listening the wording of “The Dead Person” intently and correctly.3. This study examined to compare UEHARA’s “subjectivity” theory with Emmanuel Lévinas’s “subjectivity” theory(discussed by UCHIDA Tatsuru). Then this study indicated the following two common factors and two different factors in both theories. One factor of the common factors is the idea that wording of “The Dead Person” directs “the living person”. The other factor is the strong intention for recognizing something”(“etwas”) that is cannot be described by present academic methods, namely “extremly complex realties” and “the dynamics and chaos of human mind”.4. One factor of the different factors is following point. Lévinas used wordings of “withdrawing one’s previous statesments”. However UEHARA used wordings of “checking and bounding one’s previous statesments relatively”. The other factor is following point. Lévinas supposed the “subjectivity” to responsibility for “The Dead Person” in relation to absolute “God”. However UEHARA supposed the “subjectivity” to responsibility for “The Dead Person” in relation to historical recognition to the oath of Shakyamuni and Nichiren in Buddhism thoughts.5. On this discussion context, the momnet of UEHARA’s “subjectivity” is “responsibility” for “The Dead Person” as absolute “The Other Being”. Further, the base of UEHARA’s “subjectivity” is the one’s recognition and standpoints for attaching importance to “reverence to human life” in severe situation that all persons are fronted the risk of being killed and becoming assistants of killers.

2型糖尿病患者の骨格筋量の低下に影響を及ぼす因子を検討した.対象は2型糖尿病患者111名(男性58名,女性53名)で生体電気インピーダンス方式体組成計を用いて上下肢筋量を測定し,糖尿病に関連する因子との関係について男女別に検討した.上肢筋量および下肢筋量を従属変数としたステップワイズ重回帰分析の結果,男性の上肢ではHbA1c,下肢筋量では糖尿病神経障害とHbA1c,女性の下肢筋量では,糖尿病神経障害,LDLコレステロール/HDLコレステロール(L/H)比が有意な独立変数として選択された.本研究の結果から2型糖尿病患者の骨格筋量の低下には,男性では糖尿病神経障害とHbA1c,女性では糖尿病神経障害とL/H比が関与していた.2型糖尿病患者の骨格筋量低下に関連する因子は複数存在しており,さらにこれには性差が存在していることが明らかとなった.

閉塞型睡眠時無呼吸症候群患者の外来での運動継続率が糖尿病患者,肥満患者と比較し低値であった.そこで,運動の実施状況や継続ができない理由,運動指導の改善点などを明確にすることを目的にアンケート調査を実施した.その結果,運動する時間・意欲がないと回答した者が多かった.運動の効果や方法を科学的根拠に基づいて指導するだけでなく,どのようにして患者の行動を適切な方向に導くかといった行動変容アプローチも必要である.
