Purpose: To assess the diagnostic performance of the tumor contact length (TCL) and apparent diffusion coefficient (ADC) for predicting extraprostatic extension (EPE) of prostate cancer with capsular abutment (CA).Methods: Ninety-three patients with biopsy-proven prostate cancer underwent 3-Tesla MRI, including diffusion-weighted imaging (b value = 0, 2000 s/mm2) and radical prostatectomy. Two experienced radiologists, blinded to the clinicopathological data, retrospectively assessed the presence of CA on T2-weighted imaging (T2WI). TCL on T2WI and ADC values were measured on detecting CA in prostate cancer. We used the receiver operating characteristic curves to assess the diagnostic performance of TCL and ADC values for predicting EPE.Results: CA was present in 58 prostate cancers among 93 patients. The cut-off value for TCL was 6.9 mm, which yielded an area under the curve (AUC) of 0.75. This corresponded to a sensitivity, specificity, and accuracy of 84.2%, 61.5%, and 69.0%, respectively. The cut-off value for ADC was 0.63 × 10–3 mm2/s, which yielded an AUC of 0.76. This, in turn, corresponded to a sensitivity, specificity, and accuracy of 84.2%, 59.0%, and 67.2%, respectively. The combined cut-off value of TCL and ADC yielded an AUC of 0.82. The specificity (84.6%) and accuracy (81.0%) of the combined value were superior to their individual values (P < 0.05).Conclusion: A combination of TCL and ADC values provided high specificity and accuracy for detecting EPE of prostatic cancer with CA.

Long-term use of nonsteroidal antiinflammatory drugs (NSAIDs) may cause serious side effects such as gastric mucosal damage. Resveratrol, a naturally dietary polyphenol, exhibited anti-inflammatory activity and a protective effect against gastric mucosa damage induced by NSAIDs. In this regard, we synthesized a series of resveratrol-based NSAIDs derivatives and evaluated their anti-inflammatory activity against nitric oxide (NO) overproduction in lipopolysaccharide (LPS)-stimulated RAW264.7 macrophages. We identified mono-substituted resveratrol–ibuprofen combination 21 as the most potent anti-inflammatory agent, which is more active than a physical mixture of ibuprofen and resveratrol, individual ibuprofen, or individual resveratrol. In addition, compound 21 exerted potent inhibitory effects on the LPS-induced expression of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β). Furthermore, compound 21 significantly increased the survival rate in an LPS-induced acute inflammatory model and produced markedly less gastric damage than ibuprofen. It was found that compound 21 may be a potent anti-inflammatory agent for the treatment of inflammation-related diseases.

Real-time monitoring of heart rate (HR) and body temperature (BT) is crucial for the prognosis and the diagnosis of cardiovascular disease and healthcare. Since current monitoring systems are too rigid and bulky, it is not easy to attach them to the human body. Also, their large current consumption limits the working time. In this paper, we develop a wireless sensor patch for HR and BT by integrating sensor chip, wireless communication chip, and electrodes on the flexible boards that is covered with non-toxic, but skin-friendly adhesive patch. Our experimental results reveal that the flexible wireless sensor patch can efficiently detect early diseases by monitoring the HR and BT in real time.

Visual display terminals (VDT) are standard equipment for many office workers. Their use, however, may increase the risk of developing adverse conditions related to vision, the musculoskeletal system, and mental health. We carried out a survey among 3070 workers aged 18 to 67 years (mean, 39.9 years) at a prefectural administrative office, in which 76% of subjects were visual display terminal (VDT) users. We examined the relationship between duration of daily VDT use and eyestrain, neck or upper extremity pain, back pain, and mental health, and estimated the effect of breaks and rest during VDT work on these symptoms. The 12-item General Health Questionnaire (GHQ-12: total scores ranged from 0 to 12) was used to identify potential poor mental health status, and subjects with 4 or more were considered to have symptoms of psychological distress. Seventeen percent of subjects reported eyestrain, 19.1% reported upper extremity pain, 11.6% reported back pain, and 17% of subjects had GHQ-12 scores of 4 or higher. Logistic regression analysis showed that duration of daily VDT use and lack of breaks and rest during VDT work were significantly associated with eyestrain, neck or upper extremity pain, back pain, and psychological distress. In order to protect users from the adverse effects associated with VDT work, reducing daily VDT exposure, taking breaks, and rest during VDT work are important.

Background:The second-generation cryoballoon (CB2) has demonstrated high procedural efficacy and convincing clinical success rates for pulmonary vein isolation (PVI). Nevertheless, data on the impact of different ablations protocols on durability are limited. The aim was to comparing the durability of PVI following 3 different ablation strategies in patients with recurrence of atrial fibrillation or atrial tachycardia undergoing repeat procedures.Methods and Results:In 192 patients, a total of 751 PVs were identified. All PVs were successfully isolated during index PVI. Thirty-one out of 192 (16%) patients were treated with a bonus-freeze protocol (group 1), 67/192 (35%) patients with a no bonus-freeze protocol (group 2), and 94/192 (49%) patients with a time-to-effect-guided protocol (group 3). Persistent PVI was documented in 419/751 (55.8%) PVs, and in 41/192 (21%) patients, all PVs were persistently isolated. The total rate of PV reconnection was not significantly different between the groups (P=0.134) and the comparison of individual PVs revealed no differences (P-values for RSPV: 0.424, RIPV: 0.541, LSPV: 0.788, LIPV: 0.346, LCPV: 0.865). The procedure times were significantly reduced by omitting the bonus-freeze and applying individualized application times (group 1: 123.4±31.5 min, group 2: 112.9±39.8 min, group 3: 86.67±28.4 min, P<0.001).Conclusions:Comparing 3 common ablation protocols, no differences for durable PVI were detected. Procedure times were significantly reduced by omitting the bonus-freeze cycle and by applying individualized application times.

The morphology of pelagic juveniles of Plectroglyphidodon johnstonianus (Perciformes, Pomacentridae) is reported on the basis of two specimens collected from the offshore region of Aguni Island, Okinawa, Japan. The juveniles were identified using morphometric and genetic evidence. The juveniles were characterized by their round body, elongated pectoral fin, and melanophore pigmentation pattern. In addition, fresh specimens had a large red spot on the lateral side of the body.

会議名: 言語資源活用ワークショップ2019, 開催地: 国立国語研究所, 会期: 2019年9月2日−4日, 主催: 国立国語研究所 コーパス開発センター本発表では、電子医療記録に含まれる実践医療用語の語構成を明らかにするために、独自に設計した語構成要素への分割とそれに対する意味ラベルの付与を行い、意味ラベルによる語構成のパターンを調査した。調査対象は、ComeJisyoSjis-1(111,664語)から、『分類語彙表 増補改訂版』に収録されている語を含む約7,000語から抽出した1,000語である。これらを短単位よりやや長めの語構成要素に分割し、意味ラベルを付与した。意味ラベルは、石井(2007)の複合名詞の語構造把握のための意味分類を参考にしたが、実践医療用語のために独自に設けたものも多い。分析結果から、以下のような点が明らかになった。(1)語構成要素数が2個と3個のものが全体の8割以上を占める。(2)意味ラベルは、「疾患」「身体部位」「状態」「症状」「医療行為」「時間」「生理」の7つで全体の約8割を占める。(3)意味ラベルは、語頭により多く出現するもの(「身体部位」「時間」)や語末により多く出現するもの(「医療行為」「症状」「障害」)などがあり、分布に偏りが見られる。

Nasal lymphoma (NL) is the most common nasal tumor in cats, and radiotherapy, chemotherapy, or a combination of these treatments have been described as the treatment for this disease. However, the previous studies included various machines and protocols of radiotherapy. Therefore, we aimed to retrospectively compare the prognosis among cases treated with palliative hypofractionated radiotherapy, chemotherapy, and a combination of them with united machine and protocol of radiotherapy. When compared overall survival and progression free survival, there was no significant difference among these three groups. The data of this study suggested that similar efficacy could be achieved by palliative hypofractionated radiotherapy, chemotherapy, or a combination of them.

There are many chemicals that have estrogen like activity in natural envrionment, and are well known as endocrine disrupting chemicals (EDCs). These chemicals disrupt the endocrine system of wildlife and induce the abnormality of reproductive phenomena. Recently, many scientists reported that EDCs in the aquatic environment interfere with the reproductive process of wild fish. However, there are few information about that in Asian countries. We conducted the survey of contamination by environmental estrogens (EEs) of coastal areas in Korea, People’s Republic of China, and Japan using the wild grey mullet. The grey mullet were collected from five sites (Ansan, Jeju, Yeosu, Tongyeong, and Busan) in Korea, three sites (Zhonshan, Luchaogang, and Pudong) in P.R. China, and three sites (Nagasaki, Omuta, and Fukuoka) in Japan. Contamination by EEs was determined by measuring vitellogenin (VTG) levels in serum and identifying gonadal abnormalities histologically (i.e., testis-ova). In four sites in Korea (Ansan, Yeosu, Tongyeong, and Busan), one site (Luchaogang) in P.R. China, and two sites in Japan (Nagasaki and Fukuoka), serum VTG in immature and male grey mullet was detected at levels greater than 1.0 μg/mL, which is considered to be an abnormal level. Testis-ova was detected in many sites of all countries, except for Yeosu and Jeju Island in Korea. These results suggest that the coastal areas of East China Sea are contaminated with EDCs.

In Part I of this series of studies, we demonstrated that the intensification rate of a numerically simulated tropical cyclone (TC) during the primary intensification stage is insensitive to surface drag coefficient. This leads to the question of what is the role of the boundary layer in determining the TC intensification rate given sea surface temperature and favorable environmental conditions. This part attempts to answer this question based on both a boundary layer model and a full-physics model as used in Part I. Results from a boundary layer model suggest that TCs with a smaller radius of maximum wind (RMW) or of lesser strength (i.e., more rapid radial decay of tangential wind outside the RMW) can induce stronger boundary-layer inflow and stronger upward motion at the top of the boundary layer. This leads to stronger condensational heating inside the RMW with higher inertial stability, and thus favorable for higher intensification rate. Results from full-physics model simulations show that the TC vortex initially with a smaller RMW or of lesser strength has a shorter initial spinup stage due to faster moistening of the inner core and intensifies more rapidly during the primary intensification stage. This is because the positive indirect effect of boundary layer dynamics depends strongly on vortex structure but the dissipation effect of surface friction depends little on vortex structure. As a result, the intensification rate of the simulated TC is very sensitive to the initial TC structure.

This study examines the role of boundary layer dynamics in tropical cyclone (TC) intensification by numerical simulations. The hypothesis is that although surface friction has a negative effect on TC intensification because of frictional dissipation (direct effect), it contributes positively to TC intensification by determining the amplitude and radial location of eyewall updrafts/convection (indirect effect). Results from a boundary layer model show that TCs with larger surface drag coefficient (CD) can induce stronger and more inwardly penetrated boundary-layer inflow and upward motion at the top of the boundary layer. This can lead to stronger and more inwardly located condensational heating inside the radius of maximum wind with higher inertial stability, and is favorable for more rapid intensification. Results from full-physics model simulations using the TC Model version 4 (TCM4) demonstrate that the intensification rate of a TC during the primary intensification stage is insensitive to CD if CD is changed in a reasonable range. This is because the increased/reduced positive contribution by the indirect effect of surface friction to TC intensification due to increased/reduced CD is roughly offset by the increased/reduced negative (direct) dissipation effect due to surface friction. However, greater surface friction can significantly shorten the initial spin-up period through stronger frictional moisture convergence and Ekman pumping and thus faster moistening of the inner core column of the TC vortex, but would lead to a weaker storm in the mature stage.

2006年にマスコミ等で大きく取り上げられた学校給食費の未納問題は、従来から学校が潜在的に抱える問題であった。その対策として、学校給食の教育における意義、必要性等の配慮から、就学援助制度等の整備も行われてきている。しかし、その対応については、給食費の徴収・管理を始め実に様々であることがわかった。一方で、食育基本法の制定に伴い、法の制定以来改正されてこなかった学校給食法の改正が行われ、教育における学校給食の位置づけも変わることとなった。これらのことを踏まえ、今後給食費の未納問題を未然に防ぐためには、学校給食の事前説明と公会計による給食費の管理が必要といえる。
