Background:Atrial fibrillation, the most common cardiac arrhythmia, is associated with an elevated thromboembolic risk, including ischemic stroke. Guidelines recommend the stratification of individual stroke risk and tailored antithrombotic therapy. This study investigated the demographics, comorbidities, and prognosis of non-valvular AF (NVAF) in Korean patients.Methods and Results:We extracted data on 10,846 patients with newly diagnosed NVAF who were naïve to oral anticoagulants from the National Health Insurance Service-National Sample Cohort. CHADS2and CHA2DS2-VASc scores were calculated for each subject using claims data. The study endpoints were ischemic stroke, thromboembolism, and mortality. Mean age was 63.7 years, and 46.8% of the patients were women. Women were older and had higher CHADS2and CHA2DS2-VASc scores. During 30,138 person-years of follow-up, ischemic stroke occurred at a rate of 2.95/100 person-years. CHADS2and CHA2DS2-VASc scores showed good performance in risk prediction. CHA2DS2-VASc score performed better at discriminating stroke risk in patients with low-risk profiles. The presence of female sex and vascular disease added little improvement in risk prediction.Conclusions:Korean NVAF patients had high risk of stroke and mortality, and had multiple comorbidities. While both CHADS2and CHA2DS2-VASc schema had good performance in risk prediction, CHA2DS2-VASc score was superior in identifying truly low-risk patients. Given that Asian ethnicity is associated with bleeding events, individualized accurate risk prediction is necessary to improve patient outcomes.

[Purpose] Bicycle saddle height is a critical factor for cycling performance and injury prevention. The present study compared the variance in cadence frequency after exercise fatigue between saddle heights with 25° and 35° knee flexion. [Methods] Two saddle heights, which were determined by setting the pedal at the bottom dead point with 35° and 25° knee flexion, were used for testing. The relative variances of the cadence frequency were calculated at the end of a 5-minute warm-up period and 5 minutes after inducing exercise fatigue. Comparison of the absolute values of the cadence frequency under the two saddle heights revealed a difference in pedaling efficiency. [Results] Five minutes after inducing exercise fatigue, the relative variances of the cadence frequency for the saddle height with 35° knee flexion was higher than that for the saddle height with 25° knee flexion. [Conclusion] The current finding demonstrated that a saddle height with 25° knee flexion is more appropriate for cyclists than a saddle height with 35° knee flexion.

Objectives: This study investigated the performance enhancing effects of bicarbonate ingestion on soccer specific perfor mance. Design: A randomized and double-blinded study. Methods: Division III collegiate male soccer players (N = 7) participated in the study. Participants ingested either an alkaline treatment (.3 g/kg NaHCO3) or placebo (cornflower) 60 min prior to exercise on two separate testing sessions spaced 7 days apart. Distance to fatigue in the Yo-Yo Intermittent Recovery Test Level 2 was used to assess performance. Capillary blood was sampled pre-ingestion (PE), 60 min post ingestion (PI), exhaustion (EX), following 15 min of recovery (RC) and analyzed for lactate ([La–]) and pH. Results: No significant ( p > .05) differences between conditions existed for performance. A significant ( p < .05) main effect was found for [La–]: values at EX were significantly greater than PE, PI, and RC; however, no interaction ( p > .05) existed between time and condition. pH values at PE and PI were significantly greater than EX and RC in both conditions; however, pH was significantly ( p < .05) greater at PI than PE in treatment but not placebo. Conclusion: Bicarbonate ingestion increased extracellular buffering capacity but does not appear to enhance shuttle run per formance in Division III soccer players. mance.

A 43-year-old woman suffered clinical brain death after severe head injury. The patient met the criteria for the diagnosis of clinical brain death on Day 3. Aggressive hemodynamic and respiratory managements coupled with triple hormone therapy were performed at the family’s request, resulting in continued cardiac activity for a prolonged period. Spinal reflexes and automatisms were observed until cardiac arrest. Ventilatory support was discontinued on Day 168, when cardiac death was confirmed, and her kidneys and eyeballs were removed for transplantation. The patient survived for 165 days after the diagnosis of clinical brain death, which is an extremely prolonged period of somatic support for an adult patient after brain death. An extensive and informed discussion on the end-of-life treatment of clinically brain-dead patients is urgently required in Japan to establish treatment guidelines for such patients.

The recent application of microtomographic techniques to dental morphological studies has revealed an untapped source of biological information about extinct and extant human populations. In particular, this methodology has helped to characterize internal dental structures (enamel–dentine junction, pulp chamber, and radicular canals), maximizing the amount of information that can be extracted from a given specimen. In this study, we present a three-dimensional evaluation of the dental roots of the Sima del Elefante mandible, ATE9-1 (Atapuerca, Spain) by visual inspection, and by tomographic and microtomographic techniques. With 1.3 Myrs of age, this fossil represents the earliest hominin remains in Europe, and one of the very few human fossils for this period and region. Through this case study we aim to present a protocol for the description of the internal dental spaces, exemplify how the application of microtomographic techniques can significantly increase the amount of relevant and informative morphological features (even in the case of fragmentary/heavily worn teeth or teeth with hypercementosis), and explore some biological considerations about external and internal root morphology. There is neither a general nor straightforward correspondence between the external root morphology and the root canals. In cases where a high degree of hypercementosis is present, the external root anatomy can be highly confusing. Indeed the assessment of the internal root anatomy of ATE9-1 teeth has led us to the reclassification of the LC and the LP3 with respect to previous publications. The results of this study suggest that internal root anatomy could be used as a complementary source of biological information.

Background: Impact of immeasurable time bias (IMTB) is yet to be examined in self-controlled designs.Methods: We conducted case-crossover, case-time-control, and case-case-time-control analyses using Korea’s healthcare database. Two empirical examples among elderly patients were used: 1) benzodiazepines-hip fracture; 2) benzodiazepines-mortality. For cases, the date of hip fracture diagnosis or death was defined as the index date, and the inherited date of their matched cases for controls or future cases. Exposure was assessed in the 1–30 day (hazard) and 61–90 day (control) windows preceding the index date. A non-missing exposure setting included in- and outpatient prescriptions and the pseudo-outpatient setting included only the outpatients. Conditional logistic regression was done to estimate odds ratios (ORs) with 95% confidence intervals (CIs), where the relative difference in OR among the two settings was calculated to quantify the IMTB.Results: The IMTB had negligible impacts in the hip fracture example in the case-crossover (non-missing exposure setting OR 1.27; 95% CI, 1.12–1.44; pseudo-outpatient setting OR 1.21; 95% CI, 1.06–1.39; magnitude 0.05), case-time-control (OR 1.18; 95% CI, 0.98–1.44; OR 1.13; 95% CI, 0.92–1.38; 0.04, respectively), and case-case-time-control analyses (OR 0.99; 95% CI, 0.80–1.23; OR 0.94; 95% CI, 0.75–1.18; 0.05, respectively). In the mortality example, IMTB had significant impacts in the case-crossover (non-missing exposure setting OR 1.44; 95% CI, 1.36–1.52; pseudo-outpatient setting OR 0.72; 95% CI, 0.67–0.78; magnitude 1.00), case-time-control (OR 1.38; 95% CI, 1.26–1.51; OR 0.68; 95% CI, 0.61–0.76; 1.03, respectively), and case-case-time-control analyses (OR 1.27; 95% CI, 1.15–1.40; OR 0.62; 95% CI, 0.55–0.69; 1.05, respectively).Conclusion: Although IMTB had negligible impacts on the drug’s effect on acute events, as these are unlikely to be accompanied with hospitalizations, it negatively biased the drug’s effect on mortality, an outcome with prodromal phases, in the three self-controlled designs.

Background: Lower systolic blood pressure (SBP) is known to be associated with poor prognosis in heart failure (HF). We evaluated the efficacy and safety of sacubitril/valsartan according to baseline SBP tertiles in Japanese patients from the PARALLEL-HF study.Methods and Results: In all, 223 patients were stratified into tertiles according to baseline SBP (≤114 mmHg: n=75; >114 and ≤130 mmHg: n=76; and >130 mmHg: n=72). Patients with lower SBP (≤114 mmHg) had the highest median N-terminal pro B-type natriuretic peptide (NT-proBNP) concentrations at baseline (P=0.0184). No significant difference was observed between sacubitril/valsartan and enalapril for the composite outcome of cardiovascular death and HF hospitalization across SBP tertiles (P-interaction=0.2682). Although the P-interaction value was not significant (0.2106), a greater reduction in NT-proBNP with sacubitril/valsartan compared with enalapril was observed in patients with SBP >130 mmHg (P=0.0076). The incidence of hypotension-related events and reduction or discontinuation of treatment due to hypotension-related events was higher in the lower SBP subgroup, and these events were more frequent in the sacubitril/valsartan than enalapril group.Conclusions: The efficacy of sacubitril/valsartan compared with enalapril was consistent across baseline SBP tertiles in Japanese patients from the PARALLEL-HF study. Hypotension-related events were more common in patients treated with sacubitril/valsartan with lower SBP.

Cognitive decline is a well-known chronic side effect of multidisciplinary treatment of pineal region tumors, whereas epilepsy is an under-reported chronic consequence caused by multiple potential factors including radiotherapy, surgery, or chemotherapy. Some long-term survivors have suffered drug-resistant epilepsy after treatment, which impaired the quality of life. We report five consecutive patients with drug-resistant epilepsy after combined treatment of pineal region tumor (5 men, aged 21–42 years) among 1201 epilepsy patients who underwent comprehensive evaluation in our tertiary epilepsy center from 2011 to 2018. The comprehensive epilepsy evaluation included medical interview, long-term video electroencephalography (EEG) monitoring (VEM), and magnetic resonance (MR) imaging. The patients started to have seizures at 2–22 years after initial treatment for the tumor. Four of the five patients had focal impaired awareness seizures, whereas one patient had only visual aura. All patients had EEG seizures during VEM, which confirmed the diagnosis of focal epilepsy, but three patients had no interictal epileptiform discharges (IEDs). Two patients had diagnoses of focal epilepsy arising from the left occipital region based on ictal EEG findings. Both patients had MR imaging lesion in the left occipital lobe, radiation-induced cavernoma, or surgical injury. The remaining three patients showed poor localization of epileptogenic foci based on VEM and MR imaging. Drug-resistant epilepsy after multidisciplinary treatment of pineal region tumor is characterized by focal impaired awareness seizures with poorly localized EEG onset or rare interictal spikes.

We provide an updated taxonomic revision of the genus Marchantia L. in Japan. Seven taxa [Marchantia emarginata Reinw., Blume & Nees subsp. cuneiloba (Steph.) T.X.Zheng & M.Shimamura, M. paleacea Bertol. subsp. paleacea, M. paleacea subsp. diptera (Nees & Mont.) Inoue, M. papillata Raddi. subsp. grossibarba (Steph.) Bischl., M. pinnata Steph., M. polymorpha L. subsp. polymorpha and M. polymorpha subsp. ruderalis Bischl. & Boissel.-Dub.] were recognized based on the examination of types, newly collected specimens during intensive fieldwork and our recent molecular phylogenetic analyses. For each taxon, morphological descriptions, illustrations of important taxonomic features, distinguishing characters, distribution in Japan, habitat and detailed taxonomic notes were provided. Based on the remarkable morphological discontinuity, the elements of Marchantia subg. Preissia (Corda) D.G.Long, Crand.-Stotl., L.L.Forrest & J.C.Villarreal were reinstated as the genus Preissia Corda and the genus Bucegia Radian. We also discuss the redefinition of the genus Marchantia in anticipation of a comprehensive revision in the future.

Background: There is a strong demand for remote monitoring systems to gather health data. This study investigated the safety, usefulness, and patient satisfaction in outpatient care using telehealth with real-time electrocardiogram (ECG) monitoring after catheter ablation.Methods and Results: In all, 38 patients who underwent catheter ablation were followed up using telehealth. At the 3- and 6-month follow-up, a self-fitted Duranta ECG monitoring device was sent to the patient’s home before the online consultation. Patients attached the devices themselves, and the doctors viewed the patients by video chat and performed real-time ECG monitoring. The frequency of hospital visits and the ECG monitoring duration were compared with conventional in-person follow-up data (n=102). The completion rate for telehealth follow-up was 32 of 38 patients (84%). The number of hospital visits during the 6 months was significantly lower with telehealth follow-up than with conventional follow-up (median [interquartile range] 1 [1–1] vs. 5 [3–5]; P<0.0001). However, the ECG monitoring duration was approximately 4-fold longer for the telehealth follow-up (median [interquartile range] 89 [64–117] vs. 24 [0.1–24] h; P<0.0001). No major adverse events were observed during the telehealth follow-up. Patient surveys showed high satisfaction with telehealth follow-up due to reduced hospital visits.Conclusions: A combination of telehealth follow-up with real-time ECG monitoring increased the ECG monitoring duration and patient satisfaction without any adverse events.

Since the early phase of the coronavirus disease 2019 (COVID-19) pandemic, a number of research institutes have been sequencing and sharing high-quality severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) genomes to trace the route of infection in Japan. To provide insight into the spread of COVID-19, we developed a web platform named SARS-CoV-2 HaploGraph to visualize the emergence timing and geographical transmission of SARS-CoV-2 haplotypes. Using data from the GISAID EpiCoV database as of June 4, 2022, we created a haplotype naming system by determining the ancestral haplotype for each epidemic wave and showed prefecture- or region-specific haplotypes in each of four waves in Japan. The SARS-CoV-2 HaploGraph allows for interactive tracking of virus evolution and of geographical prevalence of haplotypes, and aids in developing effective public health control strategies during the global pandemic. With the increasing demand for active friction control, we newly proposed to use dielectrophoresis to change the flow of PG-droplet-containing PAO4 to reduce the friction coefficient. The friction result with a 1-mm roller shows 20% reduction in friction coefficient (from 0.065 to 0.052) at AC 100 V, and in situ observation exhibits that PG tracks are formed over the contact area. On the other hand, at a high bias of 1000 V, the friction coefficient increases to 0.065. In this situation, in situ observation exhibits that PG forms a horseshoe-shaped track covering only the roller edges. Controlled friction tests and FEM analysis using 5-mm rollers revealed a unique behavior; a balanced bias effectively attracts the PG to the roller surface, and surface forces can resist mild dielectrophoretic forces to spread the PG across the roller surface. The present study strongly suggests the importance that the bias strength should be controlled to achieve a balance between surface force and dielectrophoretic force in order to obtain excellent lubrication conditions.

Background: This study aimed to examine the association between risk of brain tumors and radiofrequency (RF) exposure from mobile phones among young people in Korea and Japan.Methods: This case-control study of brain tumors in young people was conducted in Korea and Japan under the framework of the international MOBI-Kids study. We included 118 patients diagnosed with brain tumors between 2011 and 2015 and 236 matched appendicitis controls aged 10–24 years. Information on mobile phone use was collected through face-to-face interviews. A detailed RF exposure algorithm, based on the MOBI-Kids algorithm and modified to account for the specificities of Japanese and Korean phones and networks, was used to calculate the odds ratios (ORs) for total cumulative specific energy using conditional logistic regression.Results: The adjusted ORs in the highest tertile of cumulative call time at 1 year before the reference date were 1.61 (95% confidence interval [CI], 0.72-3.60) for all brain tumors and 0.70 (95% CI, 0.16-3.03) for gliomas, with no indication of a trend with exposure. The ORs for glioma specifically, were below 1 in the lowest exposure category.Conclusions: This study provided no evidence of a causal association between mobile phone use and risk of brain tumors as a whole or glioma specifically. Further research will be required to evaluate the impact of newer technologies of communication in the future.

The temporal changes in the behavior of vegetative cells and cysts of the noxious red tide flagellates Chattonella (C. antiqua and C. marina) were investigated in Ago Bay, central Japan, mainly in 2013. In this study, in situ germination flux (cells m−2 day−1) was measured in May, June, and October 2013, using a ‘plankton emergence trap/chamber (PET chamber)’. Vegetative cells occurred in April and increased from May, forming a bloom in July with cyst production, and subsequently decreased thereafter. This change in the occurrence of the vegetative cells was considered to be controlled primarily by temperature and secondarily by nutrients and biotic factors. The PET chamber experiment showed that cyst germination in May contributed largely to forming the initial vegetative population to initiate blooming. An experiment to elucidate the effect of storage temperature on the germinability of cysts in the sediment showed that newly produced cysts (immature dormant cysts) mature (acquire germinability) under low storage temperature (10°C) within at least one month, indicating that cysts need to experience a cold season in the sediment for their maturation. The PET chamber experiment conducted in October before a cold season also confirmed that cysts scarcely germinated from the cyst population, containing many immature dormant cysts, in the in situ sediment. However, these cysts could act as seeds to form the initial population of the subsequent bloom in the next season, since the cysts will mature by then. Consequently, Chattonella cysts were concluded to play crucial roles in the population dynamics.

Background: The Impella®percutaneous left ventricular assist device has been available in Japan since 2017. This is the first large-scale registry study to analyze the efficacy and safety of Impella in Japanese patients with acute myocardial infarction with cardiogenic shock (AMICS).Methods and Results: The Japanese registry for Percutaneous Ventricular Assist Device (J-PVAD) has registered all consecutive Japanese patients treated with Impella. We extracted data for 593 AMICS patients from J-PVAD and analyzed 30-day survival and safety profiles. Overall 30-day survival was 63.1%. The 30-day survival of the Impella alone and Impella plus venoarterial extracorporeal membrane oxygenation (ECPELLA) groups was 80.9% and 45.7%, respectively. The Impella alone group was older and had a lower rate of cardiac arrest, milder consciousness disturbance, less inotrope use, lower serum lactate concentrations, higher B-type natriuretic peptide concentrations, and higher left ventricular ejection fraction (LVEF) than the ECPELLA group. Cox regression analysis revealed that older age and comorbid renal disturbance were common risk factors affecting 30-day mortality in both groups. Major adverse events were hemolysis (10.8%), hemorrhage/hematoma (7.6%), peripheral ischemia (4.4%), stroke (1.3%), and thrombosis (0.7%). LVEF improved in both groups during support.Conclusions: AMICS treatment with Impella showed favorable 30-day survival and safety profiles. The survival rate of patients treated with Impella alone was particularly high. Further studies are needed to improve outcomes of patients with ECPELLA support.

Background: Evidence from prospective cohort studies regarding the relationship between working hours and risk of cardiovascular disease is limited Methods and Results: The Japan Public Health Center-Based Prospective Study Cohort II involved 15,277 men aged 40–59 years at the baseline survey in 1993. Respondents were followed up until 2012. During the median 20 years of follow up (257,229 person-years), we observed 212 cases of acute myocardial infarction and 745 stroke events. Cox proportional hazards models adjusted for sociodemographic factors, cardiovascular risk factors, and occupation showed that multivariable-adjusted hazard ratios (HRs) associated with overtime work of ≥11h/day were: 1.63 (95% confidence interval [CI] 1.01–2.63) for acute myocardial infarction and 0.83 (95% CI 0.60–1.13) for total stroke, as compared with the reference group (working 7 to <9 h/day). In the multivariable model, increased risk of acute myocardial infarction associated with overtime work of ≥11 h/day was more evident among salaried employees (HR 2.11, 95% CI 1.03–4.35) and men aged 50–59 years (HR 2.60, 95% CI 1.42–4.77). Conclusions: Among middle-aged Japanese men, working overtime is associated with a higher risk of acute myocardial infarction.

HORYU-IV is a lean satellite weighing 10 kg or less designed at the Kyushu Institute of Technology. It is planned to be launched with the ASTRO-H in FY2015 as a piggyback on an H-IIA rocket. The main mission of HORYU-IV is to acquire on-orbit data of discharge phenomena occurring on high-voltage solar arrays. An electrical power subsystem (EPS) is one of the HORYU-IV bus subsystems. The function of the EPS is to provide uninterrupted power to all subsystems during the satellite's lifetime. High reliability, high efficiency and simplicity are the main requirements to be considered in the design of an EPS. Generally, an EPS consists of solar arrays, rechargeable batteries, and a power control and distribution unit (PCDU). A peak power tracking topology is preferred to extract the maximum power generated by the solar arrays. The extra energy is stored in nickel-metal hydride batteries. The simple design and usage of available commercial-off-the-shelf components are the main features of the PCDU which controls the battery charging and load voltage regulation. Functional tests under different operating conditions were carried out on the designed EPS to qualify its performance. The aim of this paper is to explain the design of the EPS, and to present and analyze the test results.
