Aims: Whether the multi-dimensional data of serially measured blood pressure contains information for predicting the future risk of death in elderly individuals in nursing homes is unclear. Methods: Of the elderly individuals staying in a nursing home, 19,740 and 40,055 individuals with serially measured blood pressure from day 1 to 365 (for AI-long) and 1 to 90 (for AI-short) along with the death information at day 366 to 730 and 91-365 were included. The neural network-based artificial intelligence (AI) was applied to find the relationship between BP time-series and the future risks of death in both populations. Results: AI-long found a significant relationship between the serially measured BP from day 1 to day 365 days and the risk of death occurring 366-730 days with c-statistics of 0.57 (95% CI: 0.51-0.63). AI-short also found a significant relationship between the serially measured BP from day 1 to day 90 and the rate of death occurring 91-365 days with c-statistics of 0.58 (95%CI: 0.52-0.63). Conclusion: Our results suggest that neural network-based AI could find the hidden subtle relationship between multi-dimensional data of serially measured BP and the future risk of death in apparently healthy elderly Japanese individuals under nursing care.

Recently, an increasing number of urban areas have established neighborhood committee organizations. We investigated organizations established in the 1970s and those formed from the 2000s in the Tokyo metropolitan area. Comparisons reveal that their performance and appraisal by local governments differ despite their almost identical organizational structure. If local governments expect them to serve as necessary planning units for urban management, then it is important that they have legal and political stability to local assemblies and community organizations. Local governments should have them participate in policy-making processes to enhance members' motivation. Furthermore, facilitative leadership encouraging social entrepreneurship is necessary to vitalize these committees.

要旨: 一側性難聴者4例を対象として, 聞こえの障害の実態と障害認識について, 自己評価尺度と半構造化面接を用いて検討し, 当事者の視点で後方視的に分析した。 自己評価尺度からは, 患耳側や騒音下の聴取, 音源定位などについて日常的に高い聞こえの困難度が指摘され, 心理社会的な課題には個人差を認めた。 叙述からは, 一側性難聴による聞こえの障害に関し, 4種のカテゴリと27種の概念が生成され, 難聴の自覚から社会生活上の課題に対処し受容していく個別の経緯と, 関連要因を検討した。 学童期から成人期までの一側性難聴による聞こえとコミュニケーションの障害についての心理的な受容と変容, 対処には共通性を認め, とくに, 思春期までの障害観の形成と発達課題の充実に, 養育者の心理状況や障害観等の影響が指摘された。 当事者の発達段階に応じた情報提供や同障者との交流など, 養育者と当事者に対する診断期からの長期的支援の視点の必要性が示唆された。

The system of lifetime employment was a feature of Japanese management style and it was said to be one factor of economic growth. It was a stable employment system that applied only to men. However, it has been difficult to continue the system. The idea that "man works outside the house, and woman in the house" was realistic because many men were under the lifetime employment system. It is not asked here whether the idea of the role of men and women is correct or not. However, when the lifetime employment system collapses, this idea also naturally collapses. Thus, the idea of work-life balance plan has arisen. This paper is to try to consider the possibility of such a plan.

KITAZAWA was born to the family of a retainer of the lord Sanada of the Matsushiro fief in Shinano Province. He studied under Zozan SAKUMA in his youth and was much influenced by his teacher. After the Meiji Restoration, he obtained a position in the central government, representing the Matsushiro fief. After that, he entered the services of the Historiographical Bureau of Tokyo University, the Department of Foreign Affairs and so forth.In 1879 he made utmost efforts to establish the Tokyo Geographical Society, and since then, on the one hand he did much for its management and development, and on the other hand, devoted himself to the studies on history and geography, especially on those of Mongolia and Korea, which adjoin to China. Many papers and books, containing outstanding results of his studies concerning the said fields, were given to the public during that time.He acted as a headman of Koishikawa Ward and also as a government official of Ogasawara Islands. He was a teacher of Japanese literature and Chinese classics at Takada Middle School in his last several years and died in 1901.
