[背景]エホバの証人信者は宗教的信条に基づき輸血治療を受け入れない.エホバの証人信者に心臓血管手術を行うにあたり,当院では絶対的無輸血の立場をとっている.[目的]エホバの証人信者に対して当院で行った心臓血管手術における治療戦略は妥当であったか後方視的に検討した.[方法]2013年1月から2020年12月までの間に,7例のエホバの証人信者に対して心臓血管手術を行った.年齢は64±10(49~78)歳であった.術前Hb値が13 g/dl以上になることを目標に,赤血球造血刺激因子製剤と鉄剤の投与を行った.初回外来受診時と術前のHb値はそれぞれ12.3±2.2(7.5~14.5)g/dl,14.1±1.4(11.1~15.2) g/dlであった.術中は入念な外科的止血を重視し,加えて希釈式自己血輸血と回収式自己血輸血を施行した.術後は必要に応じて赤血球造血刺激因子製剤と鉄剤の投与を行った.[結果]全例,無輸血で入院死亡は認めなかった.手術時間は307±103(157~478)分,体外循環時間は137±28(115~178)分,大動脈遮断時間は90±27(68~136)分であった.術後に心筋梗塞,脳血管障害,出血再開胸,急性腎障害,手術部位感染は認めなかった.追跡期間は3.7±2.6(0.3~7.4)年であり,全例生存を確認できた.[結論]エホバの証人信者に対する心臓血管手術において,絶対的無輸血の立場をとる当院での治療戦略は許容できるものと考えられた.

【要旨】 抑肝散は古くから小児の癇癪、夜泣きや不眠症に対する効果が認められてきた生薬である。近年、認知症に伴う精神・行動障害に対する有効性に関する報告が増えている。我々は、境界性人格障害および遅発性ジスキネジアの症状改善にも有効であることや、治療抵抗性の統合失調症の増強療法にも有用であることを報告した。本稿ではこれらの臨床試験を総説するとともに様々な精神疾患への応用についての考察を加えたい。

研究期間の入院患者総数は132人のうちDSM-IVに基づく診断基準で統合失調症と診断された27人、うち入院加療中にアカシジアが発現した患者は4人である。研究期間の統合失調症患者総数55人(平均年齢37歳)のうちアカシジアの発現した患者総数は11人でアカシジアの発現率は20. 0%である。アカシジアが改善しなかった患者3人に抑肝散(7. 5g/日、分3食前)を投与したが、3例ともアカシジアは改善しなかった。終夜睡眠ポリグラフ検査実施例は24例(13歳~75歳、平均年齢36. 7歳±11. 7歳)であり、うちビデオカメラ撮影可能であったのは3例である。従って抽出できた「覚醒時」ミオクローヌス患者は3例のみである。当初の計画及び目的に対する達成度は遅れてしまった理由として、研究期間前に全ての対象患者が多種類の抗精神病薬で既に治療開始されていた症例であったためアカシジアの発現率と「覚醒時ミオクローヌス」の有症率とを単純には比較できなかったことによる。

塩酸ミノサイクリンの抗精神病薬効果に関する研究塩酸ミノサイクリンは、テトラサイクリン系の抗生物質であるが、強力な神経保護作用を有することも明らかになりつつある。そのため、本研究では、統合失調症の治療薬としての塩酸ミノサイクリンの有用性とその治療効果を明らかにすることを目的とした。統合失調症患者に塩酸ミノサイクリンと抗精神病薬を用いて二重盲検試験を行い、また、統合失調症モデル動物でも同様に薬物を投与して検討を行った。その結果、ミノサイクリンが抗精神病薬様作用を有し、統合失調症の治療における増強療法の一つの手段となりうることが明らかになった。また、その効果機序に脳内における抗炎症作用の機序が想定される。

文章生成は非言語的な世界を言語記号の世界に対応付ける過程とみなされるが,この非言語的な世界をどのレベルで捉え,どのように表現するかということが問題になる.筆者らは,イソップ物語の「きつねとぶどう」を素材とし,物語・背景となっている主人公の認識・思考・行動過程を問題解決手法で捉え,それに沿って物語を生成しようと試みている.本研究の目的は,この主人公の複雑で多岐に渡る心の動きをどの様に表現し,その中から文章生成のための言語情報をいかにして抽出するかについて考察し,実際に文章生成システムを構成することにより機械処理の立場から心理的活動と自然言語の結びつきを明らかにしようとすることである.本稿では,研究の背景を概観するとともに,テキスト構造を考慮した物語情報の抽出法について述べる.

This paper focuses on faith−based medical neglect in order to analyze the problem, how should social majority, which manages legal system, respond to disobedience of religious minorities. As an object of case−study I take up an incident, in which a seven−month−old baby boy died of serious atopic dermatitis. In this case parents of the dead baby were arrested and prosecuted for Abandonment Causing Death by Person Responsible for Protection. Police and public prosecutor claimed, “These parents neglected to take their baby to the hospital and gave him no appropriate medical cure.” However parents did “cure” by means of “Therapeutic Touch” (placing their hands near a patient), based upon their religious belief. Their behavior was regarded as “medical neglect” by reason that they did not cure their baby based on Modern Western Medicine. In this paper I would show relativity and limitation of Modern Western Medicine (that Modern Western Medicine is one of various curing methods, therefore we could never say that Modern Western Medicine is the one and only appropriate medical cure) with the help of studies on “Complementary and Alternative Medicine” (CAM), History and Philosophy of Science, Medical Anthropology. In the end this paper comes to conclusion (1) that curing method based on Modern Western Medicine cannot be superior to other methods, for example, “Therapeutic Touch” on religious doctrine (both of them are in principle placed on an equal plane), (2) that “We” namely social majority should manage legal system and state power in moderate and reflective posture. In the discussion on moderation and reflectiveness it made reference to studies on “crime of conscience” in German legal philosophy, especially to Gustav Radbruch’s argument about “Überzeugungstäter.” Radbruch suggested that person (“Überzeugungstäter”), which commits to offense for political / religious reasons, should receive imprisonment without hard labor in order to save her / his honor.

Power retail companies have the risk of paying the penalty due to the imbalance between planned and actual power generation values. And the risk is supposed to increase in the future. Batteries and electrolyzers are a possible solution to reduce the imbalance. To mitigate the high installation cost of electrolyzers, it is important to increase the capacity factor of electrolyzers. In this study, a mathematical model was developed to simulate the operation of a power retail company using a battery and an electrolyzer. Then we proposed a new strategy to produce hydrogen using electricity from JEPX, when the wholesale electricity price is lower than a certain value. The model was formulated as a mixed integer linear programming problem, and the effect of the strategy was evaluated by solving this problem. As a result, this strategy achieved high capacity factor of the electrolyzer, around 60%. In order to reduce the risk of JEPX price fluctuation, we also proposed an electrolyzer’s operation strategy using the prediction error distribution of JEPX price. The effect was quantified through Monte-Carlo simulation. Although the deviation of the yearly revenue was not much reduced by this strategy, the high deviation was notably reduced in the daily revenue.

Effective demand creation due to the public investment is important factor for the activation of regional economy. This effect is called the flow effect of the project. The purpose of this paper is in analyzing how much effective demand can be created by the public investment project in the region. For this purpose, I improve the regional I/O model classified by road construction project, sewer and park construction project and agricultural and forestry development project. And I calculate the amount of production and the number of workers caused by the execution of each project. The results are as follows.At first, as shown in the inverse matrix coefficients, if we ignored the land and compensation fee rate and the regional distributive conditions of the budget, the effect of each public investment project becomes almost same level. But by the kind of project, there are some differences in land and compensation fee rate, and in regional distributive conditions of the budget. Due to these differences, the flow effect varies in the kind of project and the region. For example, this effect of sewer and park construction project is remarkable in the urban prefecture, and this effect of agricultural and forestry development project is biggest in the rural prefecture. The road construction project has little effect in both of the urban and the rural prefecture, compared with other projects.Secondly, in all kinds of public investment project, not only the local project induces the production inthe rural prefecture but also it induces the production in the urban prefecture, through out the industrial structure. In agricultural and forestry development project, the amount of production that flows from the rural prefecture to the urban prefecture is more about 10% of the whole. Other most parts influence the rural economy.

The Fu xi Liu shi si gua fang wei tu, which is continuously digested the transition of Gua, was associated with binary system by G. W. Leibniz. And we cleared the mechanism by which Liu shi si gua is generated from Kun, which is considered to be the root of all things, was systematized by the binary system, in addition, the process that a new Gua is generated by the transition of Yi was systematized by symbolic notation. Here, although the xue Liu shi si gua have discussed by the use of binary system, it is not clear why the xue Liu shi si gua is discussed by binary system. In this paper, in order to examine the meaning of discussing the xue Liu shi si gua by binary system, we examine how to discuss it by Boolean algebra, and furthermore, discussed Boolean function formed from the Yi xue Liu shi si gua. binary system, it is not clear why the xue Liu shi si gua is discussed by binary system. This paper is about Arthur Kaufmann's legal philosophy. Especially it focuses upon his theories of “the right to cheap resistance, ” “the principle of tolerance, ” and “relational person, ” which Kaufmann took into much account in his later years. These theories are related to his basic thinking on ontology of law (right), legal hermeneutics, legal methodology. I will try to describe Kaufmann's design for “an ideal society” by my own evolutional interpretation of his legal philosophy. In this description I pay attention to his insistence that statutory law (positive law) should be moderate or last resort (ultima ratio). The ideal society in Kaufmann's legal thought is, I think, a society through which voices of people (minorities) raising objections to the status quo can be circulated surely, and in which other people pay respect to the voices and can be stimulated to break away from their present viewpoints by “another vision” of the society in the voices.

Recently, a personal ICT (Information and Communication Technology) system is useful for everyday life. In the field of education, research and social contribution activities at higher education institutions, the small-scale ICT systems have also been widely used. Here, we introduce an easy-to-use system built by ourselves, and summarize the practices to the fields of the system. In particular, we focus on the following usage. ・Usage as LMS (Learning Management System) in tuition ・Usage for construction of co-editable site ・Usage for student advising in laboratory ・Usage for work-sharing The system has been built in a cloud server (Amazon EC2 (Elastic Compute Cloud)1) provided by Amazon.com) and a CMS (Content Management System) is being used on it. As CMS, we adopted WordPress 3) on Amazon Linux OS provided by "WordPress powered by AMIMOTO< 2). According to W3Techs 4) and Gigazine 5), the share of WordPress reached 25.0% of websites all over the world. There are cases where it is possible to effectively utilize WordPress built on Amazon EC2 in the field of education, research and social contribution activities. If you think that working efficiency in such fields improves by building the small-scale ICT system in this article, we would like to recommend the aggressive introduction to building the system.
