健康な男子4名を被験者として, 1日の生活行動を規定した所要熱量の約30%に当る熱量をアルコールで日本酒にして約1l (アルコールとして121g, 858Calを含む) を毎日10日間にわたつて与え, 体重推移の上から熱量出納に基いてアルコールのエネルギー利用価を検討した。その結果を要約すると, 次の如くである。1. 一定の所要熱量下で糖質に替えアルコールを与えた1週間後には体重は平均約1.0kgの減少を来たし, その減少は投与アルコール量の約1/3に当る熱量を糖質で補うことによつて元の体重に復した。この結果を熱量出納に基いて計算するとアルコールのエネルギー利用価は約65%となつた。2. アルコールの1日尿中排泄量は投与アルコール量の1.1%であつた。3. 1日尿量は対照期の平均1740ccに対しアルコール投与期には2320ccに増量した。4. アルコール投与期間基礎代謝は平均約8%の上昇を示した。

外来診療における診断を目的とした医療面接は,問診票をみた段階で疾患を想起することから始まる.患者の話を頭のなかで映像化しながら,まずはopen questionで概要と問題点,患者ニーズを把握し,続いてclosed questionで想起した疾患仮説の妥当性を検証する.器質的疾患の見逃しを防ぐには精神疾患の除外が重要であり,代表的な精神疾患の特徴を理解し,積極的に診断する訓練が欠かせない.

Erdheim-Chester病(ECD)は異常に増殖した組織球系細胞が多臓器に浸潤する稀な疾患である.症例は67歳,男性.初診時から既に多臓器の障害があった.副腎皮質ステロイドの漸減中に痰の喀出困難や不整脈を生じ死亡した.剖検所見では,CD68陽性の組織球やリンパ球の浸潤および線維性結合織の増生を心臓,肺,腎臓等に認めた.特に右房における心筋の破壊は著しく,ECD病変による刺激伝導系の傷害が生命予後に影響を及ぼしたものと考えた.

日本の平安時代に僧侶の手によって編纂された漢字字書である高山寺本『篆隷万象名義』、天治本『新撰字鏡』、図書寮本『類聚名義抄』、観智院本『類聚名義抄』の四書は当時の日本人による漢字研究の優れた成果であり、人文学研究の基礎資料となっている。しかし、いずれも古写本であるため、崩し字や異体字が多く、解読に困難が多い。上記の四書のうち観智院本『類聚名義抄』は、収録の和訓が『日本国語大辞典』等に多数収録される重要資料であるが、これまでに全文の活字翻刻はなかった。本研究では観智院本『類聚名義抄』の全文翻刻と注釈を作成して、その内容をインターネット上に公開し、自在に検索できるようにすることを目指す。

わが国では,ほとんどの大気汚染物質は度重なる規則によって減少しているが,NOxについては増加傾向にある。このNOx問題の主な原因はディーゼル車の増加にあるといわれているが,ディーゼル車は経済優先政策により税制面(特に自動車用燃料に関わる税)においてガソリン車よりも優遇されている。したがって,汚染者負担原則に従うならば,NOx問題を解決するためには軽油税を引き上げるなどしてディーゼル車を減らすべきであると考えられる。そこで本研究では,軽油税の引き上げやディーゼル車の車齢制限策などのディーゼル車抑制策によるディーゼル車台数および大気汚染物質(窒素酸化物NOx,一酸化炭素CO,炭化水素HC,浮遊粒子状物質SPM)の排出動向を分析するためにコーホート型ディーゼル車普及率予測モデルを構築し,種々のディーゼル車抑制策の効果を検討した。分析の結果,NOxのみならず他の大気汚染物質(CO,HC,SPM)の削減効果もあり,また比較的人々の合意が得られ易いという観点から,軽油税(軽油価格)の引き上げが最も適当であるとした。

筆者らは、ミリ波帯を用いたスタジオ用のハイビジョンワイヤレスカメラ「ミリ波モバイルカメラ」の開発を進めている。このカメラには、高い回線信頼性を有した、高画質・低遅延でのハイビジョン映像の無線伝送が要求されている。今回、1フレーム以下の低遅延でハイビジョン映像を伝送することが可能な42GHz帯を用いたミリ波モバイルカメラを試作した。そして、2008年12月31日にNHKホールで開催された「第59回NHK紅白歌合戦」の番組撮影において、初めて使用したので報告する。

The effect of dietary cholesterol on the serum cholesterol level was examined with the adult and aged subject.In the first trial, a total of 35 healthy adults were divided into three groups given 5, 7 and 10 eggs daily for 10-15 days.The differences of mean value in serum cholesterol between the initial day and the final day were 7, 9 and 8mg/dl in each group of adults given 5, 7 and 10 eggs, respectively, indicating a tendency of slight elevation in serum cholesterol. However, it should be noted that there were some adult subjects who showed sensitive response to the serum cholesterol level by egg intake.In the next, nineteen healthy aged subjects were divided into two groups given 1 and 2 eggs every day for one month. Average increments of serum cholesterol in each group of aged persons were 3mg/dl for 1 egg and 12mg/dl for 2 eggs. The higher response seen in the aged persons given 2 eggs daily may be caused by their inactive life in comparison with the active adults.Anyway, the effect of egg cholesterol to the serum cholesterol level in human seems to be lower than that resulted from the dietary intake of saturated fatty acid such as butter, tallow and lard etc.

Squamous cell carcinoma arising in a dentigerous cyst is extremely rare. Diagnosis requires microscopic evidence of the transition from benign cystic epithelium to squamous cell carcinoma.We report a histopathologically documented case of squamous cell carcinoma showing malignant transformation within the wall of a dentigerous cyst. A 43-year-old woman was referred to our department for diffuse swelling of the left cheek associated with a small fistula of the buccal mucosa. Radiographic examination showed a round translucent image containing an impacted tooth in the left side of the maxilla. Squamous cell carcinoma was confirmed by excisional biopsy, and partial maxillectomy was performed. Histopathological examination of the resected specimens showed transition of the normal cystic epithelium to squamous cell carcinoma. The patient has been followed up for 4 years 5 months, with no evidence of recurrence.

福島県伊達市霊山町でかつて操業していた伊達永井鉱山の鉱石を用いて、地域の小学校高学年の児童を対象に、灰重石を分離する授業実践を行った。この実践は、児童が鉱石を破砕・粉砕し、鉱物を取り出し、希塩酸や紫外線ランプなどを用いて鉱物を同定するものである。実践の結果、児童は楽しみながら活動を行い、鉱石から灰重石を取り出すことができた。

Distigmine has reversible and persistent cholinesterase (ChE) antagonism, and is used for the dysuria due to low activity bladders such as the neurogenic bladder dysfunction widely postoperatively, but fatal cases were reported by cholinergic syndrome. Therefore a dose was limited to 5mg only for adaptation of “the dysuria due to hypotonic bladder such as after surgery and the neurogenic bladder dysfunction” in March, 2010. In the current study, we examined a ministerial policy in the package insert revision using Japanese Adverse Drug Event Report database (JADER). Using a side effect report registered with JADER from January, 2004 to June, 2016, we calculated Reporting Odds Ratio (ROR) which was the index of the safe signal of the medical supplies adverse event and we compared number of reports and ROR of the cholinergic syndrome by the distigmine in approximately the measure in March, 2010 and evaluated it. The number of reports of the cholinergic syndrome by the distigmine was 138 cases before March 2010 and 65 cases after March 2010. After a measure, the number of reports decreased. The possibility that the package insert revision of the distigmine contributed to a decrease in cholinergic syndrome onset was suggested. Whereas monitoring careful sequentially needs the onset of the cholinergic syndrome in constant frequency to be found.

NHK紅白歌合戦やゴルフ中継などに使用している42GHz帯ミリ波モバイルカメラの伝搬をより安定化,高信頼化させるため,送信,および受信アンテナの検討を行っている.今回,NHKホールにおける運用を考慮して,送信アンテナと受信アンテナについて,ビーム幅が異なるものを設計,試作し,伝搬実験を行うことで比較,評価した.その結果,NHKホールの環境においては,送信アンテナはビーム幅を広くし,受信アンテナはビーム幅を絞ることで伝搬特性が安定することを確認したので報告する.

ハイビジョン映像を低圧縮・低遅延で無線伝送する高性能なワイヤレスハイビジョンカメラ「ミリ波モバイルカメラ」は,ゴルフ中継や紅白歌合戦などの様々な番組撮影で使用されている.今回,ミリ波モバイルカメラの安定運用を目的として,復調装置から出力されるFFT演算結果とビタビ復号で訂正したビット数の情報を用いて,伝搬特性および伝送特性を解析するリアルタイム解析システムの開発を行った.本システムは,複数系統の受信信号に応じた受信CNRや遅延プロファイル特性に加え,MIMO多重伝送の伝送特性の把握に必要な送信相関・受信相関などの伝搬特性をリアルタイムで解析し,その解析結果をPCの一画面上で表示することができる.本稿では,本システムの詳細について説明するとともに,本システムを用いて測定したNHKホールの伝搬特性の測定結果について報告する.

Various plans for reforming and streamlining the system of secondary education in England carried out on the basis of the Butler Act since the forties must be evaluated within the framework of the British type of welfare state structure. Appraising from this viewpoint the entire educational process as tripartite, i. e. consisting of three continuous successive stages, it may be said that secondary education was opened to the people and an equitable distribution of educational opportunity was accomplished at a much higher rate than thirty and more years ago. But when one analyzes the present condition of “English Tripartitism”, in the sense that secondary education institutions were typified into three tracks, the consequences seem rather problematic.In a state which is planned as a welfare state, it is necessary that the potentialities of the social group are safeguarded in their variety and that within the relation, “planned-state - equal-society”, the individual realizes clearly the object of his loyalty. However in the real situation of the secondary school education system called “English Tripatitism”, the results in this respect are rather negative. By way of contrast it is thought that the comprehensive school planned and practised as a measure of critique on the “English Tripartitism”, aims at bringing about a balance between educational values properly speaking and social and economic values and as such possesses a much higher meaning.However the problem is not yet completely solved. “Streaming” remains as ever the internal principle of the comprehensive school and it does not look as if the true nature of ability in connection with excellence were clearly grasped. It appears as a proof for this statement that within the differentiated plan introduced in the comprehensive school, the “11-plus” had been simply exchanged for the “13-plus”.Fundamental problems in connection with an ideal of secondary education arise from reflection on the position of the individual determined by the comprehensive principle of a welfare state and from the search for a new concept of loyalty which renders possible both the social and economic function of the individual and the all around development of character. The various resulting problems concerning the secondary education reform in Great Britain seem to stem ultimately from the fact that the problems of the relationship between man and technology, between man and the system which forms the basis of the solution of these problems have not yet been sufficiently studied and solved.

The theme of progressive and continuous growth, one of five conspicuous in the educational legislation of England, was the first to be formulated in the development of that country's education. Both social and individualistic factors are to be found therein.This them blends cosiderations of social justice which underline the norms of natural law embodied in the educational legislation with insistence on positive activity to provide a firm base for the solution of all the problems raised in the educational philosophy of our times. Hence, this first theme with its characteristically English conception of education as progressive and continuous growth may, by reason of its historical background and its positive attitude torards modern questions, be acknowledged as qualified for consideration as a proposition with claims for universal recognition. It may likewise be considered as a expression in the field of education of social justice for all. Finally, this first theme is ultimately reducible to the fifth them, that on freedom.
