Paclitaxel, a taxane, is frequently administered intravenously as an anticancer agent. When a peripheral intravenous catheter is used for paclitaxel infusion, clinical nurses often observe signs such as slight swelling at the catheter placement site, lack of blood return, and difficulty in continuing the infusion. However, the cause(s) of such phenomena at the puncture site has not yet been elucidated. The aim of this study was to obtain ultrasonography images of subcutaneous tissues and veins of patients undergoing paclitaxel and carboplatin chemotherapy and compare ultrasonography images taken immediately before catheter removal with those of patients receiving other types of taxanes. We studied 24 patients receiving chemotherapy, including seven receiving paclitaxel and carboplatin chemotherapy, through a peripheral intravenous catheter in a chemotherapy unit for outpatients of a university hospital in Japan. Ultrasonography images of venipuncture sites were obtained before catheter insertion and immediately before catheter removal. We observed subcutaneous edema in the absence of visible manifestations at the puncture sites of all patients undergoing paclitaxel and carboplatin chemotherapy, but not in any patients receiving other types of taxanes. When vesicant agents and vehicles have caused subclinical subcutaneous edema, clinical nurses may detect early slight extravasation by using ultrasonography.

Cre/loxP is a site-specific recombination system extensively used to enable the conditional deletion or activation of target genes in a spatial- and/or temporal-specific manner. A number of pancreatic-specific Cre driver mouse lines have been broadly established for studying the development, function and pathology of pancreatic cells. However, only a few models are currently available for glucagon-producing α-cells. Disagreement exists over the role of the MAFB transcription factor in glucagon expression during postnatal life, which might be due to the lack of α-cell-specific Cre driver mice. In the present study, we established a novel Gcg-Cre knock-in mouse line with the Cre transgene expressed under the control of the preproglucagon (Gcg) promoter without disrupting the endogenous Gcg gene expression. Then, we applied this newly developed Gcg-Cre mouse line to generate a new α-cell-specific Mafb conditional knockout mouse model (MafbΔGcg). Not only α-cell number but also glucagon production were significantly decreased in MafbΔGcg mice compared to control littermates, suggesting an indispensable role of MAFB in both α-cell development and function. Taken together, our newly developed Gcg-Cre mouse line, which was successfully utilized to uncover the role of MAFB in α-cells, is a useful tool for genetic manipulation in pancreatic α-cells, providing a new platform for future studies in this field.

This study assessed the effects of retroperitoneal carbon dioxide (CO2) insufflation on cardiopulmonary variables and intra-abdominal pressure (IAP) in mechanically ventilated dogs in sternal recumbency with the abdomen unsupported, following placement of a positioning kit and towels under the pectoral and pelvic regions. General anesthesia was induced in eight healthy adult male Beagles. A Swan-Ganz catheter was placed in the pulmonary artery via the jugular vein for cardiac output measurements. A Foley urethral catheter was placed to monitor transvesical IAP. A 10 mm balloon blunt-tip trocar was inserted into the retroperitoneal space. With a fixed respiratory rate and tidal volume by mechanical ventilation, insufflation pressure was sequentially increased from 0 to 10 mmHg in 5 mmHg increments, followed by desufflation. All variables were measured before insufflation, 5 min after the establishment of each insufflation pressure, and after desufflation. At 10 mmHg, the IAP was nearly equal to insufflation pressure. Cardiopulmonary function was not compromised at any point, although the cardiac index (CI), heart rate, mean arterial pressure (MAP), and mean pulmonary arterial pressure increased within normal ranges. End-tidal CO2 concentration, arterial CO2 partial pressure, and oxygen delivery index (DO2I) increased, whereas pH decreased, at 10 mmHg. CI, MAP, and DO2I did not recover to baseline after decompression. Thus, retroperitoneal CO2 insufflation up to 10 mmHg is well tolerated by mechanically ventilated dogs positioned in sternal recumbency with the abdomen unsupported, although sympathetic changes may occur with an insufflation pressure increase.

Background:The safety of discontinuing oral anticoagulant (OAC) after ablation for atrial fibrillation (AF) in Japanese patients has not been clarified.Methods and Results:A study based on the Atrial Fibrillation registry to Follow the long-teRm Outcomes and use of aNTIcoagulants aftER Ablation (AF Frontier Ablation Registry) was conducted. Data were collected from 3,451 consecutive patients (74.1% men; age, 63.3±10.3 years) who had undergone AF ablation at any of 24 cardiovascular centers in Japan between August 2011 and July 2017. During a 20.7-month follow-up period, OAC therapy was discontinued in 1,836 (53.2%) patients; 51 patients (1.5%) suffered a stroke/transient ischemic attack (TIA), 71 (2.1%) suffered major bleeding, and 36 (1.0%) died. Patients in whom OAC therapy was discontinued were significantly younger than those in whom OACs were continued, and their CHA2DS2-VASc scores were significantly lower. The incidences of stroke/TIA, major bleeding, and death were significantly lower among these patients. Upon multivariate adjustment, stroke events were independently associated with relatively high baseline CHA2DS2-VASc scores but not with OAC status.Conclusions:Although the incidences of stroke/TIA, major bleeding, and death were relatively low among patients for whom OAC therapy was discontinued, stroke/TIA occurrence was strongly associated with a high baseline stroke risk rather than with OAC status. Thus, discontinuation of OAC therapy requires careful consideration, especially in patients with a high baseline stroke risk.

Both nutritive and non-nutritive sweeteners may influence energy and glucose metabolism differently. The hypothesis that sucrose, fructose, aspartame, and sucralose intake differently modulate energy and glucose metabolism was tested in an estrogen-deficient animal model. At 30 min after giving aspartame and sucralose (10 mg/kg body weight), an oral glucose tolerance test (OGTT) was conducted with glucose, sucrose, and fructose in ovariectomized (OVX) rats. After OGTT, they were continuously fed high fat diets including either 10% corn starch (Control), 10% sucrose (Sucrose), 10% fructose (Fructose), 0.05% aspartame + 9.95% starch (Aspartame) or 0.05% sucralose + 9.95% starch (Sucralose) for 8 week. During 30 min after acute administration of aspartame and sucralose, serum glucose concentrations increased despite slightly increased serum insulin levels before glucose infusion. However, glucose tolerance was not significantly different among the groups. In chronic study, serum glucose concentrations were lowest and insulin highest at the overnight-fasted state in Aspartame and Sucralose. Postprandial serum glucagon-like peptide-1 (GLP-1) and insulin levels were higher in Aspartame and Sucralose than Control. Hepatic insulin signaling (pAkt → pGSK-3β) and phosphoenolpyruvate carboxykinase (PEPCK) expression were lower in Sucralose and Aspartame than the Fructose. Serum acetate levels produced by gut microbiota were higher were lower in the fructose group than Aspartame and Sucralose groups. In conclusion, aspartame and sucralose with a meal might be preferable sweeteners to fructose and sucrose in estrogen deficient rats, and possibly post-menopausal women; however, this needs to be confirmed in human studies.

7年間にわたり毎日植物(雑草)を食べては嘔吐することを繰り返していた11歳去勢雄ミニチュアプードル犬が来患した。犬は散歩に行くたびに雑草を食べ、その後嘔吐することを繰り返していた。医学的検査では異常は認められなかった。行動学的検査と稟告を通して、我々はこの犬が植物を摂食する何らかの理由を有していると診断した。我々は犬が餌に欠乏しているものを補充するため、あるいは消化を助けるために植物を摂取していると推測したので、飼い主に現在の餌を与えることを中止し、高繊維食餌を与えるよう指示した。その結果、餌を高繊維食餌に変えて3日後より植物摂食と嘔吐が無くなったことを飼い主から聞いた。餌を高繊維食餌に変えて13ケ月間、犬に臨床検査上問題は無かった。以上のことは、特に繊維欠乏食が犬の植物摂食行動に関係している事を示唆している。

[Purpose] This study applied variable plank exercises on a stable surface consisting of a mat, on an unstable surface consisting of a dynamic cushion placed under the upper extremities and lower extremities. [Subjects] Forty-three healthy male and female subjects in their 20s voluntarily consented to participate in this study. [Methods] The subjects performed the plank exercises on the three support surfaces. Ultrasound imaging was conducted while they carried out plank exercises. [Results] The changes in the thicknesses of both the transversus abdominis and internal abdominal obliques were statistically more significant for LEDCP than for MatP. In addition, the changes in the thickness of the transversus abdominis were statistically more significant for LEDCP than for UEDCP. [Conclusion] Therefore, applying an unstable condition to the lower limbs using a dynamic cushion is considered more effective during plank exercises.

多読プログラムは多くの語学授業において用いられている。多読の効果はすでに実証されているが,日本で人気のあるTOEICのリーディングセクションのスコア増加との関連性についての研究では,様々な結果が報告されている。本研究では,学習者の学習状況が追跡できるウェブサイトとして広く利用されているMReaderのクイズデータを用い,読んだ本の単語総数,多読の頻度,内容理解という点から多読とTOEICの関連性について分析する。過去4年間でリーディングの授業を受けた総数105名の学生のMReaderデータを,多読プログラム開始10か月後のTOEICリーディングセクションのスコアと突き合わせた。わずかではあるが,単語数,クイズの平均点,TOEICのリーディングスコア増加の間に統計的に有意な関連性が確認できた。これらの結果は,多読がTOEICのリーディングスコア増加に繋がるという効果を裏付けるものではあるが,よりさらなる研究が必要である。

現在福島第一原発の廃炉活動が進められているが、その廃炉工程には高放射線環境での作業が必要であり、ロボットの投入による速やかな廃炉活動が求められている。しかし通常ロボットの頭脳であるシリコン半導体集積回路は、放射線耐性が低く、高い放射線環境下では破損する。そのためシリコンとは違う半導体による耐放射線集積回路の構築が求められている。本研究では4H-SiC半導体による放射線耐性に優れたCMOS集積回路のための研究を行い、デバイス・小規模回路の研究、デバイス高性能化の研究、極限環境応用のための研究を国際共同研究として推進した。

We present a microRNA (miRNA) detection method that achieves enhanced sensitivity by means of a power-free microfluidic chip without the requirement of an external power source. The miRNA detection is completed by sandwich hybridization between probe DNAs and target miRNA with small sample volume (0.5 μL) within 20 min. Fluorescence signals after hybridization were amplified by laminar flow-assisted dendritic amplification (LFDA) using fluorescein isothiocyanate (FITC)-labeled streptavidin (F-SA) and biotinylated anti-streptavidin (B-anti-SA) as amplification reagents. To enhance the sensitivity of on-chip miRNA detection, the hybridization buffer solution was newly optimized with three main components—sodium dodecyl sulfate (SDS), formamide and dextran sulfate—that are known to strongly influence hybridization. An on-chip miRNA detection test in the newly optimized hybridization buffer (0.2% SDS, 5% formamide and 1% dextran sulfate) revealed dramatic increases in both the LFDA signal in the sample channel and the signal-to-background ratio (S/B ratio). Moreover, the LFDA signals in a blank reference channel remained low due to the suppression of non-specific bindings and hybridizations. By changing the hybridization buffer, we obtained an improved limit of detection (LOD) that was 0.045 pM (miRNA-196a) and 0.45 pM (miRNA-331), which are around 30- and 10-fold better than that of when control hybridization buffer was used. The improved performance of our miRNA detection system with short running time and high sensitivity could contribute to future research, including point-of-care diagnostic systems.

In this study, we document a case of phenobarbital-induced anticonvulsant hypersensitivity syndrome (AHS), which has been rarely reported in veterinary medicine. A 2-year-old, 5.4 kg, neutered male Russian Blue cat was diagnosed with idiopathic epilepsy and started on phenobarbital treatment. Eight days after initiation of phenobarbital treatment, the cat showed tachypnea and hyperthermia. CBC and serum biochemistry were unremarkable. However, the patient showed high serum amyloid A (SAA). On abdominal ultrasonography, generalized enlargement of abdominal lymph nodes and splenic multiple hypo-echoic nodules, which were consistent with reactive lymphadenopathy were found. The cat was diagnosed with AHS, and phenobarbital was discontinued. After 10 days of cessation, the patient had normal SAA, and clinical signs were resolved.

Long-term use of nonsteroidal antiinflammatory drugs (NSAIDs) may cause serious side effects such as gastric mucosal damage. Resveratrol, a naturally dietary polyphenol, exhibited anti-inflammatory activity and a protective effect against gastric mucosa damage induced by NSAIDs. In this regard, we synthesized a series of resveratrol-based NSAIDs derivatives and evaluated their anti-inflammatory activity against NO overproduction in LPS-stimulated RAW264.7 macrophages. We identified mono-substituted resveratrol–ibuprofen combination 21 as the most potent anti-inflammatory agent, which is more active than a physical mixture of ibuprofen and resveratrol, individual ibuprofen, or individual resveratrol. In addition, compound 21 exerted potent inhibitory effects on the LPS-induced expression of TNF-α and IL-1β. Furthermore, compound 21 significantly increased the survival rate in an LPS-induced acute inflammatory model and produced markedly less gastric damage than ibuprofen. It was found that compound 21 may be a potent anti-inflammatory agent for the treatment of inflammation-related diseases.
