Decreases in saliva secretion compromise food mastication and swallowing, reduce mucosal immune function, and increase the risk for oral diseases like dental caries. Chlorella is a green alga that contains a variety of nutrients including amino acids, vitamins, and minerals. In our previous study, Chlorella-derived multicomponent supplementation did not affect salivary flow rates in healthy young individuals, but Chlorella-derived supplementation attenuated a decrease in saliva secretion that was observed during a kendo training camp. Hence, we hypothesized that Chlorella-derived supplementation increases saliva secretion in individuals with lower rates of saliva flow. Sixty-four subjects took Chlorella-derived tablets for four weeks. Before and after supplementation, saliva samples were collected by chewing cotton. In the complete study group, there was no difference in saliva production before and after supplementation (1.91 ± 0.11 ml/min before vs 2.01 ± 0.12 ml/min after). Analysis of subgroups based on saliva production before supplementation found an increase in saliva secretion in the lower saliva flow group (1.18 ± 0.06 vs 1.38 ± 0.08 ml/min), but no change in the higher saliva flow group (2.63 ± 0.11 vs 2.64 ± 0.15 ml/min). These results suggest that Chlorella-derived multicomponent supplementation increases saliva production in individuals with lower levels of saliva secretion.

Physiological qualities of F1 hybrid tomato seeds affected by the growing season; maturity stage at 40, 45, 50, 60, and 70 days after cross-pollination (DAP), and storage in plastic bags at 0°C for four months were studied. The characteristics of seeds from early harvested fruits (45 DAP) that subsequently ripened at ambient temperature (28 ± 2°C) for ten days (designated as “45+ripening”), were also investigated. Higher light intensity, and a wider range of night and day temperatures and relative humidity (RH), but lower night temperature and RH, were observed in the winter season compared to the rainy season. The winter climate delayed physiological maturity of the seeds by 10 days, indicated by the highest weight of dried seed, percentage germination and germination index which occurred at 60 DAP. On the other hand, in the rainy season physiological maturity occurred at 50 DAP. The proportions of irregularly shaped seeds, abnormal seedlings and weak seedlings were independent of fruit and seed maturity, while the amount of speckled seeds significantly increased with an increase in seed development on either the mother plant or the 45+ripening. Fresh (non-stored seeds) and stored seeds of the fruits harvested at 45+ripening and at 60 DAP showed the same quality. The number of fresh ungerminated seed in the seeds of 45 and 50 DAP was markedly reduced after four months storage at 0°C.

Pungency in peppers is due to the presence of the alkaloid capsaicin and its analogues, collectively known as capsaicinoids. These compounds are only produced in the Capsicum genus and function as deterrents to mammals from consuming the pepper fruits. Pungency in pepper is qualitatively controlled by the Pun1 locus, which encodes a putative acyltransferase enzyme. Mutations in the Pun1 gene result in a loss of pungency, and several Pun1 loss-of-function alleles have been identified in sweet peppers to date (pun11–3). However, variations in pun1 alleles have not been completely elucidated. In the present study, we report a new type of loss-of-function pun1 allele, named pun14, in a Japanese sweet pepper cultivar, ‘Nara Murasaki’ (C. annuum). Sequence analysis at the Pun1 locus revealed that this type of Pun1 allele is caused by a single adenine nucleotide insertion in the second exon region. This insertion is unique to ‘Nara Murasaki’ and is not present in wild-type Pun1. This insertion causes a frameshift mutation and a change in the amino acid sequence, resulting in a truncated protein. The results of gene expression analysis indicated that the expression of Pun1 in ‘Nara Murasaki’ was hardly detectable, while the transcripts of this gene were strongly expressed in a pungent cultivar. In a co-segregation test, the pun14 genotype perfectly co-segregated with non-pungency in 103 F2 population plants of a cross between ‘Nara Murasaki’ and a pungent cultivar. ‘Nara Murasaki’ and a DNA marker to distinguish the pun14 allele will be informative for understanding the domestication process of sweet peppers.

Background: The JAPAN-ACS (Japan Assessment of Pitavastatin and Atorvastatin in Acute Coronary Syndrome) trial showed that intensive statin therapy could induce significant coronary plaque regression in acute coronary syndrome (ACS). We evaluated the impact of metabolic syndrome (MetS) and its components on coronary plaque regression in the JAPAN-ACS patients. Methods and Results: Serial intravascular ultrasound measurements over 8–12 months were performed in 242 ACS patients receiving pitavastatin or atorvastatin. Patients were divided into groups according to the presence of MetS or the number of MetS components. Although the percent change in plaque volume (%PV) was not significantly different between the MetS (n=119) and non-MetS (n=123) groups (P=0.50), it was significantly associated with an increasing number of MetS components (component 0: −24.0%, n=7; components 1: −20.8%, n=31; components 2: −16.1%, n=69; components 3: −18.7%, n=83; components 4: −13.5%, n=52; P=0.037 for trend). The percent change in body mass index (%BMI) significantly correlated with %PV (r=0.15, P=0.021), especially in the MetS components 4 group (r=0.35, P=0.017). In addition, %BMI was an independent predictor of plaque regression after adjustment for the changes of low- and high-density lipoprotein cholesterol, triglycerides and HbA1c. Conclusions: The clustering of MetS components, but not the presence of MetS itself, could attenuate coronary plaque regression during intensive statin therapy in ACS patients. Therefore, to achieve a greater degree of plaque regression, it is necessary to treat to each MetS component and use lifestyle modification.  (Circ J 2012; 76: 2840–2847)

A series of 2,3-diphenylphenanthro[9,10-b]furans were prepared by the reactions of phenanthrene-9,10-dione with two equivalents of benzylidenetriphenylphosphoranes as a key step. The bis(4-bromophenyl) derivative can be readily lithiated by the action of n-butyllithium in diethyl ether. The corresponding 4-trimethylsilyl and 4-methylthio derivatives were obtained by quenching the dilithio derivative with trimethylsilyl chloride and dimethyl disulfide, respectively. The Buchwald–Hartwig amination of the bromide with diphenylamine afforded the diphenylamino derivative in a good yield. Crystallographic analysis of the parent diphenyl derivative reveals that the phenanthrofuran moiety has a slightly twisted helicene-like structure, and the compound forms a columnar stacking in the crystal. The phenanthrofurans display intense blue fluorescence both in CH2Cl2 and in the solid state. Their electrochemical properties obviously indicate high HOMO energy levels of the furans, as predicted by the density functional theory calculations.

Rotational atherectomy to an angulated calcified lesion is always challenging. The risk of catastrophic complications such as a burr becoming stuck or vessel perforation is greater when the calcified lesion is angulated. We describe the case of an 83-year-old female suffering from unstable angina. Diagnostic coronary angiography revealed an angulated calcified lesion in the proximal segment of the right coronary artery. We performed rotational atherectomy to the lesion, but intentionally did not advance the rotational atherectomy burr beyond the top of the angulation. We controlled the rotational atherectomy burr and stopped it just before the top of the angulation to avoid complications. Following rotational atherectomy, balloon dilatation with a non-compliant balloon was performed, and drug-eluting stents were successfully deployed. In this manuscript, we provide a review of the literature on this topic, and discuss how rotational atherectomy to an angulated calcified lesion should be performed.

Cavity growth occurring with crack extension and coal consumption during UCG processes directly influences the gasification efficiency and the estimated subsidence and gas leakage to the surface. This report presents an evaluation of the gas energy recovery, coal consumption, and gasification cavity estimation using a proposed stoichiometric method to analyze the coal gasification reaction process. We defined the evaluation parameters of rate of energy recovery and investigated the effects of different parameters using UCG trials conducted with coal blocks and coal seams, adopting different Linking-hole methods and operational parameters. Analyses of results obtained from laboratory experiments and small-scale field trials using V-shaped and L-shaped linking holes, and Coaxial-hole UCG models show that the gasification of Linking-hole models yielded average calorific values of product gas as high as 10.26, 11.11 MJ/m3 (lab.), and 14.39 MJ/m3 (field.). In contrast, the Coaxial-hole models under experimental conditions yielded average calorific values of product gas as: 7.38, 4.70 MJ/m3 (lab.) and 6.66 MJ/m3 (field.). The cavity volume obtained with Coaxial models was about half of the volume obtained from Linking-hole models. Results obtained for these UCG systems show that the feed gas and linking-hole types can influence coal consumption and product gas energy. Fissure ratios were also investigated. Results confirmed major factors underpinning gasification efficiency. Linking-hole types strongly influenced the development of the oxidization surface and fracture cracks for subsequent combustion in the gasification zone. Estimated gas energy recovery results support experimental observations within an acceptable error range of about 10%. Moreover, this stoichiometric approach is simple and useful for evaluating the underground cavity during UCG. Based on these results, we proposed a definition of the energy recovery rate, combined with the obtained volumes of gasification cavities that provide a definition of energy recovery and UCG effects. UCGにおいては,炭層内のき裂進展に伴う燃焼空洞の拡大と石炭の消費が重要であり,これがガス化効率や安全性 (地盤沈下,ガス漏洩等) に大きく影響する。本研究では,ガス化効率,回収エネルギーとガス化空洞の評価方法として,化学量論および化学平衡に基づく評価手法を検討した。生成ガス組成と求めたガス化反応式から,石炭の消費量,ガス生産量等を推定する方法である。また,エネルギー回収率を定義し,UCG室内モデル実験及び露天炭鉱の炭層で行った小規模現場実験の結果を評価し,リンキングの方式や注入ガス等のパラメータがガス化効率やガス化空洞の成長に与える影響を検討した。リンキングの方式として,L字,V字,同軸型のUCG実験を行い,ガス化効率の違いと,その原因を明らかにした。すなわち,リンキング型と同軸型モデルを比較すると,リンキング型UCGモデルの方が発熱量が高く,平均発熱量では,前者が10.26/11.11 MJ/m3 (室内) ,14.39 MJ/m3 (現場) であった。一方,同軸型モデル試験では,7.38/4.70 MJ/m3 (室内) と6.66 MJ/m3 (現場) と低い値であった。実験後の空洞体積の直接評価結果でも,リンキング型の方がガス化領域が拡大していることを確認した。リンキング方式の方が,炭層内にき裂を連続的に進展させやすいためと考えられる。また,エネルギー回収率の評価では,実験前後の供試体質量差から求めたエネルギー回収率と比較検討を行った。その結果,両者の誤差は約10%で,検討した手法によりエネルギー回収率や燃焼ガス化領域の石炭消費量を推定できることがわかった。以上の結果より,検討した化学量論法よる回収エネルギー評価手法は簡便で,実用的であることが明らかになった。

『太平記』に登場する正成は、智仁勇の三徳を兼備した武将で、天皇のために命を捧げた英雄として広く知られているが、近世期以降はその教訓的な側面が正成伝説の普及を支える肝要な要素となっていく。 『太平記』において正行の母は、父正成の戦死を悲しみ自ら命を絶とうとする正行を諌め、正成の遺訓の意味を再度教え諭す。このエピソードが端緒となり、その後正成の妻は良妻賢母として顕彰されていくことになる。 一方、近世前期に流行した「太平記読み」のテクストであった『理尽鈔』は、兵学中心の合戦談という性格から、正成の妻が正行に父の遺訓を教え諭す場面を省略し、その代わりに正成の首をめぐって足利直義と楠家の家臣間で繰り広げられた駆け引きに関するエピソードを挿入している。これは、合戦談の中では女性の存在が副次的にしか認識されないことから、『太平記』における正成の妻の情緒性豊かな描写が省略された結果と見られる。 このような『理尽鈔』における扱いとは別に、正成の妻は近世の早い時期から啓蒙目的の女訓書に登場している。仮名草子女訓書『本朝女鑑』では、『太平記』原典の正成の妻に関するエピソードが簡略な形で引かれており教訓を主眼とする女訓書の性質に即して、母として息子の誤りを戒める内容が中心になっている。 さらに、時代浄瑠璃においてはそれ以前とはやや異なる正成の妻のイメージが形成される。近松門左衛門の『吉野都女楠』において、正成の妻「菊水」は従来と同様に夫の遺志を継ぎ、息子を訓戒する良妻賢母として登場するが、その上に大力という性質をも兼ね備えた逞しい女性として描かれる。ここでは、男性のために自己を犠牲にする時代浄瑠璃の典型的な女性とは異なる、戦乱という苦難を生き抜く強い女性像が正成の妻に付与されていると言える。 一方、西沢一風・田中千柳の『南北軍問答』においては新しい趣向が設定され、正成の妻は女色に溺れる正行を訓戒する。正行の誤った行動を戒めるという点では、『太平記』と軌を一にするものの、正行が好色者として描かれる点に加えて、「泣男」杉本佐兵衛が正成の妻に代わって訓戒の内容を伝えるという点が新しい構想となっている。 このように、正成の妻は『太平記』から時代浄瑠璃に至るまで、良妻賢母としてのイメージを保ちながらも、その上に新たな趣向を取り入れつつ受容されていくことになる。

Objective We herein conducted a retrospective study to evaluate the long-term efficacy and safety of sitagliptin treatment in elderly patients with type 2 diabetes mellitus. Methods We analyzed the changes in glycemic control in 112 Japanese type 2 diabetes patients over 65 years of age treated with 50 mg/day sitagliptin. Hemoglobin A1c (HbA1c) levels, liver and kidney functions, and usage of hypoglycemic agents were recorded for 24 months. Results HbA1c levels were significantly decreased, and the significance of HbA1c reduction was maintained during the observation period [from 7.7±1.1% to 7.2±0.7% (p Conclusion Sitagliptin treatment continually decreases the HbA1c level for 24 months and is useful to reduce the dose of sulfonylurea in elderly patients with type 2 diabetes.

We report the findings of a 46-year-old man, who presented with fever and renal dysfunction while undergoing treatment for Crohn's disease with infliximab (IFX). Remittent fever and renal dysfunction with urinary casts developed and lasted for 3 weeks without deterioration of Crohn's disease. Renal biopsy revealed acute tubulointerstitial nephritis (ATIN). After the discontinuation of IFX, his fever and renal abnormalities resolved. We herein report the first known case of ATIN associated with IFX.

Intradialytic hypotension (IDH) is a very common complication during hemodialysis. IDH may lead to nausea, vomiting, or anxiety. In some severe cases, it may cause shock or death. Each hemodialysis treatment generally lasts for four to six hours and must be repeated two or three times a week. Currently, the hemodialysis procedure may lead to dangerously low blood pressure when blood pressure is not frequently assessed by medical staff. Therefore, the purpose of this work is to use the technologies of wireless sensor networks (WSN), global systems for mobile communications (GSM), and a MySQL database to develop a PC-based gateway and an automatic monitoring system to collect physiological data during hemodialysis and continuously monitor the IDH status of patients. The clinical definition of IDH during hemodialysis is a decrease in the systolic pressure of at least 20 mmHg and a decrease in the mean arterial pressure (MAP) of at least 10 mmHg. Our system uses a threshold-based algorithm to monitor and record the systolic and mean arterial blood pressure readings. The GSM module is utilized to send medical staff a warning message, and the message is displayed on the monitor if the readings reach a critical point. The experimental results show that this monitoring system is highly reliable.

Med., 1995, 175 (4), 285-288 - Activities of the elbow flexors during supination and pronation of the forearm in a normal human volunteer were studied by an electromyography (EMG). The volunteer performed the movement slowly or quickly holding elbow flexion at various angles with or without a load. The biceps brachii showed an increase and a decrease of EMG activities during supination and pronation, respectively. The brachialis and brachioradialis showed a reduction and an increment of EMU activities accompanied by an increase and a decrease of EMG activities in the biceps brachii, respectively. These findings seem to indicate that reciprocal contractions among the elbow flexors permit the biceps brachii to work for supination without an induction of elbow flexion.

The anti-slide packing is a packing of a number of three dimensional pieces of same size into a larger box such that none of them can slide in any direction. In this paper, we consider the problem of how to find a sparsest anti-slide packing. We give an IP formulation of this problem, and obtain the solutions for some small cases by using an IP solver. In addition, we give the upper and lower bounds on the ratio of the volume occupied by the pieces when the size of a box approaches infinity. For the case of piece size 2 × 2 × 1, we show that a sparsest anti-slide packing occupies at least 28.8% and at most 66.7% of the total volume.

Gynecomastia or benign proliferation of the male breast glandular tissue is not uncommon for adolescent males. Its pathogenesis has been attributed to a transient imbalance between estrogens and androgens. Ginseng is a popular herb with a long history of medicinal use. Oriental folk medicine describes it as both a tonic for restoring strength and a panacea. The term “ginseng” generally refers to a plant, Panax ginseng. Based on estrogen-like actions of Panax ginseng due to its structural similarity with estradiol, this agent could be speculated to cause gynecomastia. Here we report a 12-year-old Korean-Japanese boy with bilateral enlargement of the breasts with tenderness in the right breast, which was noticed about 1 month before his first visit to our outpatient clinic. He was diagnosed with gynecomastia based on physical, laboratory, and ultrasound examinations. Detailed questioning about his medications and supplements revealed that he had been given red ginseng extract daily to enhance his performance for 1 month before his clinical presentation. He wanted to make his body stronger as an athlete. He was recommended from his grandmother to take Panax ginseng for his purpose. After stopping this, there was no further growth of the masses and no pain when his right breast was pressed. In conclusion, physicians should consider ginseng in the investigation of gynecomastia.

[Purpose] The purpose of the present study is to examine the effects of abductor hallucis and gluteus maximus strengthening exercises on pronated feet. [Subjects and Methods] The present study was conducted with 18 adults without no history of surgery on the foot or ankle. One group performed both gluteus maximus strengthening exercises and abductor hallucis strengthening exercises, while the other group performed only abductor hallucis strengthening exercises five times per week for four weeks. [Results] The group that performed both gluteus maximus and abductor hallucis strengthening exercises showed smaller values in the height of navicular drop than the group that performed only abductor hallucis strengthening exercises. The muscle activity of the gluteus maximus and the vastus medialis increased during heel-strike in the group that added gluteus maximus exercises, and the muscle activity of the abductor hallucis significantly increased in both groups. [Conclusion] Given the results of the present study, it can be suggested that strengthening the gluteus maximus while also performing exercises to correct the pronated foot is an effective method for achieving normal gait.

Recent technical innovation has enabled chemical modifications of small materials and various kinds of nanoparticles have been created. In clinical settings, nanoparticle-mediated drug delivery systems have been used in the field of cancer care to deliver therapeutic agents specifically to cancer tissues and to enhance the efficacy of drugs by gradually releasing their contents. In addition, nanotechnology has enabled the visualization of various molecular processes by targeting proteinases or inflammation. Nanoparticles that consist of poly (lactic-co-glycolic) acid (PLGA) deliver therapeutic agents to monocytes/macrophages and function as anti-inflammatory nanoparticles in combination with statins, angiotensin receptor antagonists, or agonists of peroxisome proliferator-activated receptor-γ (PPARγ). PLGA nanoparticle-mediated delivery of pitavastatin has been shown to prevent inflammation and ameliorated features associated with plaque ruptures in hyperlipidemic mice. PLGA nanoparticles were also delivered to tissues with increased vascular permeability and nanoparticles incorporating pitavastatin, injected intramuscularly, were retained in ischemic tissues and induced therapeutic arteriogenesis. This resulted in attenuation of hind limb ischemia. Ex vivo treatment of vein grafts with imatinib nanoparticles before graft implantation has been demonstrated to inhibit lesion development. These results suggest that nanoparticle-mediated drug delivery system can be a promising strategy as a next generation therapy for atherosclerotic vascular diseases.

Clinical and histopathological characteristics of 16 dogs with nodal paracortical (T-zone) lymphoma (TZL) were evaluated. At initial examination, generalized lymphadenopathy was found in all dogs, and peripheral lymphocytosis was found in 10 of the 16 dogs. At initial diagnosis or during the disease course, 8 dogs (50%) were affected with demodicosis. Immunohistochemical analysis for CD3, CD20 and CD25 was performed for 6 dogs with TZL; the tumor cells were positive for CD3 and CD25 and negative for CD20. Median overall survival time was 938 days. A watchful waiting approach was adopted for 6 cases (38%), and 5 of the 6 dogs were still alive at the end of follow-up. The clinical course of TZL in dogs is generally indolent; however, many cases develop a variety of infectious and other neoplastic diseases after the diagnosis of TZL.
