Objective Periodontal disease may predispose individuals to cardiovascular disease (CVD). Diabetes mellitus, especially in patients with severe periodontitis, increases the risk of CVD mortality. However, the outcomes of periodontal therapy vary among the different treatment modalities. We aim to investigate whether periodontal treatment could influence the occurrence of CVD in patients with type 2 diabetes and periodontal problems. Methods A retrospective cohort study was conducted based on a dataset released by Taiwan National Health Insurance (NHI). The dataset was composed of randomly sampled, newly diagnosed diabetic patients who received insurance benefits from 1999 to 2001; patients who were younger than 18 years of age or who already had CVD before 1999 were excluded. The NHI code was used to identify the treatments, including subgingival curettage and flap operations. The patients' demographic variables were matched using a 1:4 propensity score. All of the subjects were followed up until the onset of CVD, or December 31, 2011. A Cox proportional hazards regression analysis was performed to evaluate the effects of periodontal treatment on the rates of myocardial infarction, heart failure and stroke. Results Three thousand thirty-nine and 12,156 diabetic subjects were classified into the advanced periodontal treatment group and the non-advanced periodontal treatment group, respectively. The Cox proportional hazards analysis revealed that although the overall incidence of CVD was not significantly improved (Hazard ratio, HR 0.95; 95% CI 0.90-1.01), advanced periodontal treatment reduced the rates of myocardial infarction (HR 0.92; 95% CI 0.85-0.99) and heart failure (HR 0.60; 95% CI 0.45-0.80). There was no significance difference in the incidence of stroke (HR 0.95; 95% CI 0.85-1.06). Conclusion Advanced periodontal therapy lowers the rate of CVD, especially myocardial infarction and heart failure. Dental management has a beneficial effect on the health of patients with type 2 diabetes.

The problem of beamforming and related beamspace high resolution direction-of-arrival (DOA) estimation is studied in this paper. All beamspace processing methods are based on the beam outputs and the beampattern design plays an important role in providing high quality beam output data for further processing. Three typical situations which are frequently encountered in practical sonar system working environment and the most widely studied MUSIC algorithm are considered herein. First, when isotropic noise is the dominant noise at sensors, conventional beamforming techniques provide the optimum performance in the sense that DOA estimate is the ML estimate. Good DOA estimates are obtainable by applying MUSIC to the beam outputs directly. Then, uncorrelated interferes with much higher strength than the wanted signals are assumed to be present in the sidelobe region, and low sidelobe Dolph-Chebyshev and adaptive MVDR beampatterns are designed to guarantee the performance of MUSIC. And finally, the robustness of conventional techniques is combined with the adaptivity of MVDR beamforming to deal with the situation when the interfere in the sidelobe region is strongly correlated with one of the wanted sources. Performance in all three situations is studied with numerical examples.

Background:The process of cardiomyocyte swelling involves changes of biomechanical properties and profiles of cellular genes. Although many genes have been proved to regulate cell edema of cardiomyocyte, the mechanisms involved in this event, as well as the biomechanical properties of swelling cell, remain unknown.Methods and Results:Whether histone deacetylase 1 (HDAC1) inhibition protects against hypoxia-induced H9c2 cardiomyocyte swelling is examined in this study. Hypoxia-induced changes in the biomechanical properties and cytoskeletal structure that are relevant to cell swelling were also determined. H9c2 cells were treated under a chemical hypoxia situation (cobalt chloride) with HDAC1 inhibition (chemical inhibitor or siRNA) for 5 h, followed by in vitro biological and mechanical characterization. The results showed that expression of HDAC1 instead of HDAC4 was upregulated by chemical hypoxia. HDAC1 inhibition protects H9c2 cells against chemical hypoxia-induced hypoxic injury and cell swelling. HDAC1 inhibition improved cell viability, decreased lactate dehydrogenase leakage, cell apoptosis, malondialdehyde concentration, cell volume, and particles on the cell surface, and increased superoxide dismutase activity. Moreover, chemical hypoxia induced a decrease of Young’s modulus, accompanied by alterations in the integrity of acetylated histone and organization of the cytoskeletal network. HDAC1 inhibition significantly reversed these processes.Conclusions:Based on the ideal physical model, HDAC1 inhibition protects against hypoxia-induced swelling in H9c2 cardiomyocytes through enhancing cell stiffness. Overall, HDAC1 is a potential therapeutic target for myocardial edema.

We studied the temporal and spatial characteristics of extreme typhoon rainfall in Taiwan using Central Weather Bureau hourly precipitation data from 21 surface stations during the past 51 years (1960-2010). Extreme rainfall is defined as 95th percentile intensity of total rain events, or equivalently, rain events greater than 9 mm hr<sup>-1</sup> which contribute 40% to the total rain amount in Taiwan. It was found that approximately 70% (20%) of extreme rain is in the typhoon season (Mei-Yu) from July to October (from May to June). There are significant variations of typhoon extreme rainfall over the annual and decadal time scales, with larger extreme rainfall values and events in the periods of 1960-1976 and 1994-2010, and less in the 1977-1993 period. The recent 1994-2009 period has the most extreme rainfall and events, as well as, inter-annual variability. In contrast, there are strong inter-annual variations of Mei-Yu extreme rainfall, but no significant decadal variations. The averaged typhoon rain intensity, however, is about the same, being 19 mm hr<sup>-1</sup> in all these three periods. Our analysis indicates that the typhoon extreme rainfall spatial pattern is phased locked with the Central Mountain Range, Taiwan. In general, the amount of extreme rainfall was related to the typhoon translation speed and duration time, but not typhoon intensity. Slower speeds and longer duration time lead to larger extreme rainfall values. Our analysis also indicate that the mean duration time of Taiwan landfall typhoons with northern tracks (tracks north of 23 degrees latitude) is about 3 hours longer than that of southern track typhoons in the last 51 years, and is more likely to produce three times as much extreme rainfall. The interactions of summer or winter monsoons with typhoons are also important factors that may contribute to the extreme rainfall in Taiwan. Examples of extreme rainfall due to typhoon circulation interaction with summer and winter monsoon flows are presented. Monsoon water vapor supply, typhoon slow translation speed, and mesoscale convection due to typhoon-monsoon flow interactions are the key factors in extreme precipitation events.

In our previous study, two synthetic thiophenes such as IMB-05 and IMB-15 were found as peroxisome proliferator-activated receptor gamma (PPARγ) agonists and exhibited beneficial effects on glucose tolerance of diabetic mice <i>in vivo</i>. In the present study, their effect on the transactivity of other nuclear receptors was further investigated. IMB-05 and IMB-15 could not only activated PPARγ but also efficiently activate PPARα in GAL4-hPPARα/γ (ligand binding domain (LBD)) chimeric receptor assay and PPAR response element (PPRE)-luc reporter gene assay with EC<sub>50</sub> values of 1.8—5.2 μ<small>M</small>, whereas no activity was observed in other nuclear receptor assays. In addition, the maximal efficacy of IMB-05 and IMB-15 in activating PPARα/γ was approximately 30% of that observed with Wy14643 and rosiglitazone. These data indicate that the two thiophene derivatives are novel class of partial PPARα/γ dual agonists, which may be the mechanism underlying their regulatory effects on glucose homeostasis.

1997年に人から分離されたH5N1インフルエンザウイルスの赤血球凝集素(HA)に対する単クローン性抗体を,DNA免疫法を利用して作製した.これらの抗体を用いて,H5型ウイルスのHA抗原性を解析したところ,1997年および2003年に人から分離されたH5N1ウイルスの間で抗原性がかなり異なることがわかった.

The objective of this study was to determine the genetic model of fruit color in the bitter gourd (Momordica charantia L.). Six generations (P1, P2, F1, F2, BCP1, and BCP2) in the hybrid set “MS 41 × MS 47” were used to estimate the genetic effects via generation mean analysis (GMA). The fruit color of the F1 population was green implying the green color was dominant over the white color. The fruit color of the F2 population showed a continuous distribution from white to dark green indicating this trait was controlled by quantitative genes. Genetic analyses using the Royal Horticultural Society scoring system (RHS score) fitted the three parameters model (χ23 = 1.59, P = 0.6607) and showed significant to highly significant additive and dominance effects, respectively. The GMA results of the six parameters model revealed a significant additive effect on chlorophyll b concentration. Chlorophyll a and total chlorophyll concentrations were nonsignificant in all parameters. The magnitudes of additive and additive × additive gene effects were larger than those of the dominance and dominance × dominance gene effects, which revealed that additive genes were more important in the contribution to chlorophyll concentrations. Broad-sense heritability estimates for color traits were 0.39, 0.611, 0.470, and 0.460 for concentrations of chlorophyll a, chlorophyll b, total chlorophyll, and RHS score, respectively. In contrast to previous findings, with the GMA analysis we believe that the genetic model of fruit color in bitter gourd is clearly a quantitative model.

We compared the effects of acupuncture and electroacupuncture on cell proliferation and neuroblast differentiation using specific markers, Ki67 and doublecortin (DCX), in the subgranular zone of the dentate gyrus (SZDG) in 13-week old Wistar rats. Acupuncture and electroacupuncture were applied simultaneously in the acu-points, ST36 (Zusanli) and GV20 (Baihui), once a day for 3 weeks. Acupuncture and electroacupuncture at these acu-points significantly increased the number of Ki67-positive cells and DCX-immunoreactive neuroblasts compared to the control or sham acupuncture group. Electroacupuncture treatment significantly increased the number of well-developed (tertiary) dendrites in the SZDG compared to acupuncture treatment. These results suggest that both acupuncture and electroacupuncture increase neurogenesis in the normal, but that electroacupuncture has greater effects on neuroblast plasticity than acupuncture in the dentate gyrus.

Berberine is one of the main alkaloids of Rhizoma coptidis which has been used as a folk medicine to treat diabetes mellitus for more than 1400 years in China. To investigate the chronic effect of berberine on diabetic hyperlipidemic rats, fasted rats were intraperitoneally injected 35 mg/kg streptozotocin. Diabetic rats were admitted after 2 weeks and given a high-carbohydrate/high-fat diet to induce hyperlipidemia. The rats were divided into 7 groups at the end of week 16: normal and diabetic rats received no drug, 5 treatment groups were administered with either 75, 150, 300 mg/kg berberine, 100 mg/kg fenofibrate or 4 mg/kg rosiglitazone per day for 16 weeks, respectively. The blood glucose, hemoglobin A1c, lipid metabolic parameters and hepatic glycogen and triglyceride were measured, and histopathology and peroxisome proliferator-activated receptors (PPARs) α/δ/γ expression of liver were determined by hematoxylin eosin and immunohistochemical staining. Berberine reduced diabetic rats' body weight, liver weight and liver to body weight ratio. Berberine restored the increased blood glucose, hemoglobin A1c, total cholesterol, triglyceride, low density lipoprotein-cholesterol, apolipoprotein B and the decreased high density lipoprotein-cholesterol, apolipoprotein AI levels in diabetic rats to near the control ones. Berberine alleviated the pathological progression of liver and reverted the increased hepatic glycogen and triglyceride to near the control levels. Berberine increased PPARα/δ expression and reduced PPARγ expression in liver of diabetic rat to near the control ones. Berberine improved glucolipid metabolism both in blood and liver in diabetic rats possibly through modulating the metabolic related PPARα/δ/γ protein expression in liver.

Clarifying the relationship of sex hormones to preclinical atherosclerosis could illuminate pathways by which androgens are associated with cardiovascular events and mortality. Our aim was to determine hormone profiles associated with carotid intima-media thickness (CIMT) and carotid atheroma, in men with and without known coronary artery disease (CAD). We included 492 community-based men aged 20-70 years (Group A) and 426 men with angiographically proven CAD aged <60 years (Group B). Fasting early morning sera were assayed for testosterone (T), dihydrotestosterone (DHT) and estradiol (E2) using mass spectrometry. CIMT and carotid plaque were assessed ultrasonographically. Mean (±SD) age was Group A: 53.8±12.6 and Group B: 49.6±5.1 years. Higher T was associated with reduced CIMT (-0.011 mm per 1-SD increase, p=0.042) and lower prevalence of carotid plaque (odds ratio [OR] per 1-SD increase, 0.68, p=0.012) in Group A, but not B. E2 was associated with increased CIMT in Group A (0.013 mm, p=0.011) but not B. Higher DHT and E2 were associated with reduced carotid plaque in Group B (DHT: OR=0.77, p=0.024; E2: OR=0.75, p=0.008), but not A. In community-dwelling men, higher T is associated with favourable CIMT and lower prevalence of carotid plaque, while higher E2 is associated with worse CIMT. In men with CAD, higher DHT or E2 are associated with less carotid plaque. T, DHT and E2 are differentially associated with preclinical carotid atherosclerosis in a cardiovascular phenotype-specific manner. Interventional studies are needed to examine effects of exogenous T and its metabolites DHT and E2, on atherogenesis.

[Purpose] Nonspecific neck pain is a common musculoskeletal disease. Therapeutic exercise has been shown to improve pain and disability in short-term and midterm follow-ups. This study performed a literature review of the long-term effects of therapeutic exercise on subjects with nonspecific chronic neck pain. [Subjects and Methods] The databases of the CINAHL, MEDLINE, PEDro and PubMed were used. Randomized controlled trials (RCT) published from January 2000 to January 2014 and explicitly including a one-year follow-up were identified. [Results] Only six articles were included in this review. They had scores of 5 to 8 points on the PEDro scale, and the level of evidence was grade I. The study results show that the main exercises used were cervical strengthening and endurance training exercise. Short-term exercises (10 to 12 weeks) helped to improve the body function, structure, activity and participation immediately after the intervention, but not at the long-term follow-up. On the other hand, long-term interventions (1 year) resulted in improvements in body function and structure at the 3 year follow-up. [Conclusion] The results of the six high-quality studies suggest that long-term exercise have long-term benefits for patients with nonspecific neck pain in terms of body function and structure.

Medical comorbidities were considered as confounding factors. Multiple logistic regression models were used to evaluate the potential risk of zolpidem exposure on glaucoma with/without adjustment for the effects of confounding variables.<BR><b>Results: </b>The exposure rate of zolpidem use in the glaucoma group was significantly higher than that of the control group (2.8% vs. 2.0%, <i>P</i> < 0.0001). The adjusted odds ratio (OR) of the risk of glaucoma for those with zolpidem use vs. those without was 1.19 (95% confidence interval [CI], 1.02–1.38). Compared to non-zolpidem users, zolpidem users with an average dose of more than 200 mg/year had significantly increased risk of glaucoma (OR 1.31, 95% CI 1.03–1.68).<BR><b>Conclusions: </b>This study suggests that the use of zolpidem might increase the risk of subsequent glaucoma. Further confirmatory studies are recommended to clarify this important issue.
